2.Clinical study of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens implantation

AIM: To analyze the clinical value of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens (IOL) implantation.
METHODS: We selected 100 patients ( 102 eyes ) undergoing vitrectomy combined with phacoemulsification and IOL implantation in our hospital from October 31 2013 to October 31 2015. They were divided into routine operation group, 50 cases (52 eyes), and femtosecond laser group, 50 cases (50 eyes). The parameters were compared between the two groups, including total operation time, postoperative complications ( elevated intraocular pressure, uveitis, vitreous hemorrhage, and retinal detachment ), postoperative intraocular pressure and postoperative half of the visual acuity ( near visual acuity, visual acuity, uncorrected visual acuity, corrected visual acuity).
RESULTS: The difference on total operation time of the two groups was statistically significant ( P < 0. 05). The differences between the two groups on postoperative intraocular pressure, uveitis, vitreous hemorrhage, retinal detachment were not statistically significant ( P >0. 05). There were no significant differences on internal pressure, near visual acuity, far visual acuity, uncorrected visual acuity, corrected visual acuity at 6mo after surgery (P>0. 05).
CONCLUSION: Femtosecond laser in vitrectomy combined with phacoemulsification and IOL implantation has a high safety and efficacy with the advantages as accurate, efficient.
3.One-stage reconstruction of anterior/posterior cruciate ligaments using the allogenic achilles tendon in 3 cases

To explore the effect of allogenic achilles tendon on one-stage reconstruction of antedor/postedor cruciate ligaments,3 cases with ruptured anterior/posterior cruciate ligaments and received allogenic achilles tendon reconstruction,at Department of Orthopaedics,General Hospital of Chinese People's Armed Police Forces,from June 207 to April 2008,were collected.A more than 6-month follow-up showed that all cases could walk at 12 weeks postoperatively.Anterior andposterior drawing tests,as well as inversion and extroversion stress test were negative,with 100° movement ranges.In addition,no serious complication occurred.The Lysholm knee fuction scores were 87,83 and 85,respectively.The results suggest that the allogenic achilles tendon seems to be a satisfactory treatment option for one-stage anterior/posterior cruciate ligaments reconstruction,which has merits of easy operation,small trauma and good short-term results.However,further research is needed to determine the role of allogenic achilles tendon in knees with multi-ligament injury.

6.Local infiltration analgesia and femoral nerve block for pain control after total knee arthroplasty:a meta-analysis

BACKGROUND:Both local infiltration analgesia and femoral nerve block are used for the pain management after total knee arthroplasty. Controversy stil remains regarding the optimal technique for pain relief. OBJECTIVE:To systematical y evaluate analgesic effects of local infiltration analgesia and femoral nerve block after total knee arthroplasty. METHODS:Databases including PubMed, EMBASE, the Cochrane Library, Web of Science and CBM, were comprehensively searched to identify randomized control ed studies comparing local infiltration analgesia with femoral nerve block. Two reviewers independently selected trials, included literatures, extracted data, and assessed the methodological qualities of included studies according to Cochrane Handbook for Systematic Reviews of Interventions Version 5.1.0. When there were different opinions, it was decided by the third author. Data were analyzed by RevMan 5.3 software. RESULTS AND CONCLUSION:(1) Eleven randomized control ed trials involving 566 patients were included. (2) At 24 and 48 hours (h) after surgery, there were no significant differences between the local infiltration analgesia and femoral nerve block groups, in Visual Analog Scale scores [MD24 h=0.15, 95%CI (-0.26, 1.28), P>0.05;MD48 h=0.19, 95%CI (-0.06, 0.44), P>0.05] in the resting state, and [MD24 h=-0.01, 95%CI (-0.51, 0.48), P>0.05;MD48h=0.18, 95%CI (-0.45, 0.82), P>0.05] in the active state, amount of analgesic drug use [MD24 h=-2.23, 95%CI (-5.63, 1.16), P>0.05;MD48 h=2.44, 95%CI (-1.08, 5.95), P>0.05], hospital stay [MD=0.05, 95%CI (-0.40, 0.50), P>0.05], postoperative nausea and vomiting [OR=1.09, 95%CI (0.39, 3.04), P>0.05] and postoperative infection [OR=0.99, 95%CI (0.44, 2.59), P>0.05]. (3) These results indicated that the analgesic effect of local infiltration analgesia was identical to that of femoral nerve block after total knee arthroplasty. Due to its simple operation, local infiltration analgesia can be used as a standard analgesia method after total knee arthroplasty.
7.Research progress in colorectal cancer stem cells

BACKGROUND: Colorectal cancer is a common malignant tumor in digestive tract. Theory of tumor stem cells (TSCs) offers a new approach to understand the development mechanism of colorectal cancer. OBJECTIVE: To review the progress in the existence, origin and identification of colorectal cancer stem cells (Co-CSCs). RETRIEVAL STRATEGY: Pubmed was undertaken to identify relevant articles published from January 1990 to July 2007 with the of "colorectal cancer,stem cell,tumor stem cells" in English.At the same time, Vip database was undertaken to identify the relevant articles published between January 1990 and July 2007 with the key words of "colorectal cancer, stem cell, tumor stem cells" in Chinese. The data were selected primarily, and then quotations of each article were checked. Inclusive criterion: the articles related to the advances of the existence, origin and identification of Co-CSCs were selected. Exclusive criterion: the articles with repetitive research were excluded. Totally 63 relevant articles were selected and 33 of them met the inclusive criterion. The 30 excluded articles were of old or repetitive content. LITERATURE EVALUATION: Of the 33 inclusive articles, 28 were on the existence and origin of Co-CSCs, 4 were about the isolation and identification of Co-CSCs, and 1 was about the problems and prospect. DATA SYNTHESIS: TSCs is a rare population of cells in tumor tissues with infinitely proliferative and self-renewal potential that drive the formation and growth of tumors. It may become a new target to eradicate malignant tumor. TSCs have been discovered in leukemia and some solid tumors. Intestine stem cells are poorly differentiated. Its phenotype is difficult to identify. There are many reports on the quantity, location, origin, function and relation with intestinal disease. Co-CSCs have been isolated and identified as EpCAMhigh/CD44+ cells recently, which accelerate to understand the mechanism of colorectal cancer. CONCLUSION: The discovery of Co-CSCs is one of the advances in researching of malignant tumor. TSCs may become a new target to eradicate malignant tumor, and brings hope to cure it.
8.Stady on Quality Control of Shuxiong Soft Capsule

Objective To establish the quality control standard for Shuxiong soft capsule. Method Radix et Rhizoma Notoginseng, Rhizoma Chuanxiong were identified by TLC, and the content of Ginsenoside Rg1 were detdrmined by HPLC. Result The TLC sports developed was fairly clear. The HPLC method showed good repeatability, and the average recovery of quercitrin was 97.21%, RSD=0.65%. Conclusion The method is simple, accurate and can effectively control the quality of Shuxiong soft capsule.
9.Ureteroscopic holmium laser lithotripsy for ureteral calculus: Report of 72 cases

Objective To investigate the clinical effect of ureteroscopic holmium laser lithotripsy for the treatment of ureteral calculus.Methods A total of 72 cases of ureteral calculus was treated with stone fragmentation from April to November 2004,by using the Lumenis VersaPulse PowerSuite 100W Holmium Laser system and the Circon ACMI Micro Ureteroscope.Under ureteroscopic visualization,the laser lithotrite was introduced to the stones,and holmium laser intraluminal stone fragmentation was carried out. Results The operation time was 14~83 min(mean,28 min).Three cases of fragmentation failure were due to upper ureteral stones migrating to the renal pelvis.The success rate of fragmentation on one session was 95.8%(69/72),which was 92.1%(35/38) for upper segment calculi and 100%(34/34) for middle-to-lower segment calculi.Intraoperative complications included 3 cases of ureteral perforation(4.2%),9 cases of stone migration(12.5%),and 72 cases of hematuria(100%).Postoperative complications included 45 cases of lumbago(62.5%),72 cases of hematuria(100%),and 5 cases of urinary tract infection(6.9%), without ureteral stricture.The kidney,ureter,and bladder(KUB) X-ray radiograph on the first postoperative day showed a stone-free rate of 86.1%(62/72),and the remaining residual stones were thoroughly expelled within following 2 weeks.The length of postoperative hospital stay was 2~5 d(mean,3 d).Re-examinations with B-ultrasonography or intravenous urography(IVU) at 3 months after operation in 46 cases revealed no ureteral stricture.Hydronephrosis subsided by 1.4?0.5 cm in 28 cases and completely disappeared in 21 cases.Conclusions Ureteroscopic holmium laser lithotripsy for treating ureteral calculus offers satisfactory clinical effects and low complication rates.
10.A randomized controlled trail about whether to use antibiotics in tension-free inguinal hernia re-pair

Objective To explore whether antibiotic prophylaxis is beneficial in tension-free in-guinal hernia repair Method A perspectiverandomized controlled study was conducted which included 1 28 patients who underwent tension-free inguinal hernia repair from July 201 1 to August 201 3.In the anti-biotic prophylaxis group,antibiotic was intravenously administrated within 30 minutes before surgery.In the control group,no preoperative antibiotic wasadministrated.The incidence of incision infection was com-pared between groups.Result Incision infectiondeveloped in onepatient of the antibiotic prophylaxis group and two patientsin the placebo group,indicating no significant difference between the groups(P >0.05).Conclusion Before tension-free repair,there is no real need to apply prophylactic antibiotics reg-ularlyfor inguinal hernia without high risk factors.
