1.Effect of Combined Rehabilitative Training on Stroke in Convalescent Stage

ObjectiveTo observe the effect of combined rehabilitative training on stroke patients in convalescent stage.Methods52 stroke patients with hemiplegia were divided into rehabilitation group (n=26, receiving combined rehabilitative training) and control group (n=26, not receiving combined rehabilitative training). Scores of functional independence measure (FIM) and Fugl-Meyer assessment (FMA) of all patients were assessed when admission and discharge.ResultsFor patients of rehabilitation group, FMA scores when discharge was not significant difference compared with admission, but FIM scores was significantly higher than that of control group (P<0.01).ConclusionCombined rehabilitative training can improve function and living quality of stroke patients in convalescent stage.

3.Effects of botulinum toxin on spasticity in the ankle plantar flexors of children with cerebral palsy:A randomized,controlled trial

Objective To compare the effect of botulinum toxin A(BTX-A)applied according to experi- ence with its effect when the application is guided by electrical stimulation on spasticity in the ankle plantarflexors of children with cerebral palsy(CP).Methods Forty-five children with CP were randomly assigned into 2 groups to receive injections of BTX-A guided by electrical stimulation,or injections of BTX-A guided by experience.All chil- dren received a local injection in the ankle plantar flexors.Physiotherapy and ankle-foot orthoses were applied by a physical therapist 3 days after the BTX-A injections.After the first 10 days,the therapy was administered by the patient's family.Clinical assessments included the patient's passive range of movement(PROM),scoring on the Ash- worth scale(MAS),the composite spasticity scale(CSS),and the D and E dimensions of the gross motor function measure(GMFM),and walking velocity(WV).Assessments were performed before treatment and at 3 days,2 weeks,1,2,and 3 months following the injection with BTX-A.Results All children showed significant decrease in spasticity(PROM,MAS and CSS)after 3 days.The improvement was maintained at 3 months.When compared with the results before the injection,the improvements in standing and walking(GMFM)and in walking velocity were statistically significant after 2 weeks of treatment for both groups,and were maintained at 3 months.The differences in PROM and CSS scores at 3 days,2 weeks,1,2,and 3 months following the injection were statistically significant between the 2 groups.Significant differences were also found between the 2 groups in MAS scores at 3 days,2 and 3 months after treatment,and in GMFM and WV at 2 and 3 months after treatment.Conclusions A BTX-A injec- tion,whether guided by electrical stimulation or experience,in combination with physiotherapy,can reduce spasticity in the ankle plantarflexors of ambulant children with CP and improve their functional performance.BTX-A injection guided by electrical stimulation was more effective than an injection guided by experience.

Objective: To assess potential mechanisms responsible for the antitumor activities of the streptococcal preparation OK 432.Methods: C57BL/6 mice were transplanted subcutaneously with 5?10 5 B16 melanoma cells and treated with OK 432 (1KE/week?3) or saline (control) intraperitoneally 3 days after the inoculation and tumor size was measured biweekly. The effects of OK 432 on splenocyte cytokines (IL 2, IL 4, IL 6, IL 10, IL 12, IFN ?) production of tumor bearing mice were investigated both in vitro and in vivo. Brain injury is one of the most serious diseases in neonatal period,which can cause cerebral palsy,motor development delay,cognitive dysfunction and learning difficulties and other sequelae,and severely affects the health development and quality of life of the newborn.Neonatal brain injury (NBI) is a wide range of diseases caused by a variety of causes,its clinical manifestations lack specificity,clinically,it is difficult to judge the severity,duration and the time of prenatal injury,and it has been paid much attention to by scientific researchers and clinicians.At present,imaging method is a major means of NBI diagnosis,but imaging examination is usually a lag and limitations.Levels of humoral biomarkers change early after brain injury,and early brain injury can be predicted by detecting their changes.In recent years,a variety of sensitive brain damage biomarkers have been detected in various body fluids of newborns,mainly including neuron-specific enolase (NSE),ubiquitin carboxyl hydrolase L1 (UCH L1),S100B protein,tau protein,myelin basic protein (MBP),glial fibrillary acidic protein (GFAP) and activin A and so on.the application and research progress of these commonly used biomarkers in NBI are reviewed in this paper.
7.Effect of Therapy Combination with Rehabilitation Approach and Acupuncture on Cerebral Trauma during Recovery Stage

Objective To observe the effect of therapy combined with rehabilitation approach and acupuncture on cerebral trauma during the recovery stage. Methods 48 cases were divided into two proups: 24 cases in treatment group who applied the rehabilitation combined with acupuncture, the other 24 cases in control group who applied acupuncture alone. They were evaluated with Functional Independent Measure (FIM) 3 months after treatment. Results The scores of FIM were significantly difference between these two groups (P<0.05). Conclusion The therapy combined with rehabilitation approach and acupuncture can improve the recovery of cerebral trauma.
8.Study on Immunotolerance of Kidney Graft Induced by Tolerant Dendritic Cells

Objective To study the immune tolerance effect after renal transplantation,an animal model of tolerant dentritic cell adaptive transfer renal transplantation was constructed.Methods Tolerant Dentritic cells from SD rats were separated and purification in vitro,after a series of procedures,the Dentritic cells were administrated in the portal vein of Wistar rats,then conduct the operation of renal transplantation for Wistar rats.Renal tissue pathology and splenic lymphocyte reaction were test in 3、5、7、9、11、和13d after renal transplantation.Results After the renal transplantation,time of acute rejection happened in control group and experimental group is 4.69?1.26d and 10.69?1.63d,respectively.The mean survive time between two group has significant difference.Conclusion Renal transplantation immune tolerance could be induced by the tolerant dentritic cell of donor,by the mean of administrating the cell into host portal vein.The immune tolerance has specificity.
9.Clinical feature of severe hand, foot and mouth disease with acute pulmonary edema in pediatric patients

ObjectiveTo explore the clinical feature of severe hand, foot and mouth disease (HFMD) in pediatric patients, and to observe the hemodynamic changes in those with acute pulmonary edema.Methods A prospective observation study was conducted. Thirty-five severe HFMD pediatric patients with acute pulmonary edema admitted to the intensive care unit (ICU) and Department of Pediatric of First People's Hospital of Foshan from May 2008 to September 2014 were enrolled. The clinical features were thoroughly investigated. Hemodynamic data were monitored by pulse-indicated continuous cardiac output (PiCCO) in 5 cases, and the changes in PiCCO parameters were observed at ICU admission (0 hour), and 24, 48, 96 hours after treatment.Results Thirty-five patients who met the diagnostic standard of severe HFMD were enrolled, including 22 male and 13 female, aged from 7 months to 4 years. Six patients were younger than 1 year, 13 1-2 years, 12 2-3 years, and 4 patients 3-4 years old. The most common time of occurrence of pulmonary edema was 3-4 days after the onset of the disease. Fever and central nervous system symptoms were found in all the patients, and examination of the cerebral spinal fluid (CSF) revealed non-bacterial inflammatory changes. PiCCO results showed a tendency of lowering of heart rate (HR), systemic vascular resistance index (SVRI), and extravascular lung water index (EVLWI) after the treatment, and the values obtained at 96 hours were significantly lower than those at 0 hour [HR (bpm): 119.0±14.7 vs. 200.8±19.7, SVRI (kPa·s·L-1·m-2):148.9±14.6 vs. 209.6±58.7, EVLWI (mL/kg): 10.5±1.9 vs. 34.8±10.8,P< 0.05 orP< 0.01], global end-diastolic volume index (GEDVI) was also gradually decreased without significant differences among all the time points, together with a tendency of increase in stroke volume index (SI) and cardiac index (CI). The values of the parameters at 96 hours were significantly higher than those at 0 hour [SI (mL/m2): 38.5±6.5 vs. 17.4±2.8, CI (mL·s-1·m-2): 75.0±8.0 vs. 55.5±8.5, bothP< 0.01]. Left atrium was found to be enlarged, and left ventricular systolic function decreased in two patients by cardiac ultrasonic. Four out of 35 patients died, and functional disability of extremities was found in 1 patient. Other patients were cured and discharged without any sequelae.Conclusions Severe HFMD complicated by acute pulmonary edema is a perilous condition in children, accompanied commonly by pathologic changes in central nervous system and systolic dysfunction of left ventricle. According to the results with PiCCO monitoring, HFMD patients suffering from acute pulmonary edema may be of cardiac origin in addition to neurogenic origin.
10.Clinical significance of CD4+CD25hiCD127low regulatory T cells in the peripheral blood of patients with cardiac cancer

Objective To analyse the dysfunction of immunity and clinical significance in patients with cardiac cancer.Methods The level of CD4+ CD25hi CD127low Treg cells were detected by flow cytometry (FCM),and serum IL-10 and TGF-β1 levels were determined by enzyme linked immunosorbent assay (ELISA) kit in 56 patients with cardiac cancer.15 healthy volunteers were tested as normal controls.The clinical data of each patient were collected and analyzed. Results There was a significantly higher percentage of CD4+ CD25hi CD127low Treg cells in patients with cardiac cancer (5.73±1.56)% than that (4.45±1.06)% of healthy volunteers (P<0.01).The IL-10 and TGF-β1 levels in the serum of patients with cardiac cancer were also significantly higher than that of healthy volunteers (P<0.05).There was a positive correlation between levels of IL-10.TGF-β1 and the levels of CD4+ CD25hi CD127low Treg cells.The number of CD4+ CD25hi CD127low regulatory T cells in the peripheral blood of cardiac cancer patients were significantly correlated with clinical stages and metastasis lymph node.Conclusion The CD4+ CD25hi CD127low Treg cells in the peripheral blood of cardiac cancer patients is significantly increased in comparison with that in healthy volunteers,and was also correlated with different stages.The abnormal levels of CD4+ CD25hi CD127low Treg cells may be related to tumor progression in patients with cardiac cancer.
