1.Intensional definition of research-oriented hospitals and research on their features

Proceeding from such perspectives as the development of medical science and technology,the prevention and treatment of diseases,and the needs of hospitals for self-development,the paper discusses the significance of building research-oriented hospitals.It also expounds the concept and intensional features of research-oriented hospitals in light of the successful experience of specialty hospitals that have developed through science and education,the development strategies and measures of research-oriented enterprises and universities,and the unique features of hospitals.At the same time the paper puts forward suggestions for appropriately handling certain relationships in the building of research-oriented hospitals.

4.A comparative study on three devices in treatment of dislocation of acromioclavicular joint

Objective To compare the results of 3 methods to treat complete dislocation of acromioclavicular joint. Methods Three different surgical techniques were employed: Dewars procedure (in Group A of 32 cases),tension band wiring (in Group A of 44 cases), and Wolter plate fixation (in Group A of 20 cases). There were 81 males and 15 females, or totally 85 cases of acute dislocation and 11 chronic dislocation. Results 96 cases were followed up with an average period of 50 months. Acording to karlssons standards, in group A, 26 patients were assessed as good, 5 as fair and 1 as poor. In group B, 20 patients were assessed as good, 13 as fair and 11 as poor. In group C, 15 patients were assessed as good, 4 as fair and 1 as poor. There were differences between A and B, C and B. But there werent statistic differences between A and C. Conclusion The Dawers procedure is believed to be a good, simple and safe method to treat acromioclavicular joint dislocation, with no need of a second operation to remove the implant and with less complications.
5.Adopting the system of overall responsibility by the attending physician in large general hospitals

By analyzing the situation that large general hospitals face at present in their development and the background of reform in the system of overall responsibility by the attending physician in the hospital, the authors offer an account of the guiding ideology for large general hospitals to adopt the system of overall responsibility by the attending physician, the specific steps taken and the initial results attained. They also discuss the impact of the system of overall responsibility by the attending physician on the hospital from the perspectives of human resources, technological innovation, managerial system, and information system. The positive significance of the system of overall responsibility by the attending physician in the management of the hospital reform is also identified.

The identification and classification of Thermus to Thermales is revie wed. Up to now, the order Thermales is consist of only one family Thermaceae, w h ich includes 4 genera with 14 valid species about 50 strains, and with 4 unvalid spcecies and more than 50 strains unidentified. The morphology, physiology, G+ C mol%, DND hybridization, fatty acid components, 16S rRNA alignment and secondar y structure analyses have been tools for the taxonomy of Thermales.
7.Significance of Resistance Index of Intrarenal Artery in the Diagnosis of Acute Upper Ureteral Obstruction

CDFI was performed in 20 patients with acute upper ureteral obstruction secondary to ureteral stone. Intrarenal flow of unilateral obstructed kidneys and contralateral nonobstructed kidneys was observed. Resistive index (RI) of intrarenal arteries was measured. The results showed that RI of intrarenal arteries in unilateral obstruted kidneys was elevated during acute upper ureteral obstruction with renal colic. BACKGROUND: Insulin resistance and impaired insulin secretion are the key complications of polycystic ovary syndrome (PCOS). The incidences of insulin resistance syndrome (IRS), impaired glucose tolerance (IGT) and diabetes mellitus have been clearly shown to be race-related, thus the association between polycystic ovary syndrome and insulin resistance was race-related.OBJECTIVE: To analyze the correlation of insulin resistance with body mass index (BMI), blood lipids and plasma glucose in Tibetan patients with PCOS.DESIGN: A prospective study based on Tibetan population.SETTING: Department of Cardiology, General Hospital of Chinese PLA; Second Department of Internal Medicine,General Hospital of Tibetan Military Area Command of Chinese PLA.PARTICIPANTS: Thirty-six Tibetan PCOS patients, aged 25-42 years with a mean age of (32±5) years were selected from the Second Department of Internal Medicine and Department of Gynecology, General Hospital of Tibetan Military Area Command of Chinese PLA. Another 36 female inpatients were selected as the controls (control group), aged 24-35 years with a mean age of (30±5) years.METHODS: The disease histories were collected and general conditions were recorded. Oral glucose tolerance test (OGTT, 75 g) was conducted for all the subjects. All the subjects werefasted for 12 hours to collect venous blood samples to detect the levels of hormone, fasting insulin, total cholesterol, triglyceride, high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C). Ovary was detected with echocardiography. The insulin resistance index was calculated by the homeostasis model assessment (HOMA-IR) was calculated by the formula:fasting insulin (mlU/L)×FPG (mmol/L)/22.5. The blood pressure, BMI and levels of blood lipids, plasma glucose and insulin were compared between the PCOS group and control group.MAIN OUTCOME MEASURES: Blood pressure, BMI and levels of blood lipids, plasma glucose and insulin were observed in both groups.RESULTS: All the 36 PCOS patients and 36 controls were involved in the analysis of final results. ① The BMI, systolic blood pressure and diastolic blood pressure in the PCOS group were higher than those in the control group (t =3.426 5,2.448 4, 2.212 5, P < 0.05-0.01). ② The levels of total cholesterol, triglyceride and LDL-C in the PCOS group were higher than those in the control group (t=2.104 9, t =2.304 7, 2.400 0, P< 0.05-0.01), whereas the level of HDL-C was lower that in the control group (t =4.800 0, P< 0.01). ③ The levels of fasting insulin and 2-hour plasma glucose and HOM in the PCOS group were higher than those in the control group (t =7.809 5, 12.365 0, 2.789 9, P< 0.01).CONCLUSION: Tibetan PCOS patients present a clustering of atherosclerotic risk factors, including obesity, adverse lipid profile, hypertension, hyperglycemia, etc.

Based on hospital development cycle and the core rigidities theory,the authors made a strategic analysis on the building of research-oriented hospital of the Eastern Hepatobiliary Surgery Hospital.They put forward that building a research-oriented hospital is key to exceeding the core rigidities of the hospital and promoting core competence.In their practice,they developed the model featuring innovation culture as the leader,hospital-institute in one as the pattern,composite talents as the core,integrated innovation as the mechanism,and high-level medical cooperation as promoter.
10.Application of ultrasound counter currentextraction in patent of traditional Chinese medicine.

The patent information of ultrasound countercurrent extraction used in traditional Chinese medicine was analyzed in this paper by the samples from Derwent World Patent Database (DWPI) and the Chinese Patent Abstracts Database (CNABS). The application of ultrasound countercurrent was discussed with the patent applicant,the amount of the annual distribution, and the pharmaceutical raw materials and other aspects. While the technical parameters published in the patent was deeply analyzed, such as material crushing, extraction solvent, extraction time and temperature, extraction equipment and ultrasonic frequency. Thought above research, various technical parameters of ultrasound countercurrent extraction used in traditional Chinese was summarize. The analysis conclusion of the paper can be used in discovering the technical advantages, optimizing extraction conditions, and providing a reference to extraction technological innovation of traditional Chinese medicine.
