1.Clinical application of ultrasound-guided transversus abdominis plane block for postoperative analgesia in patients received lower abdominal surgery

Objective To observe the postoperative analgesia effect of ultrasound-guided trans-versus abdominis plane block (TAP)in patients received lower abdominal surgery.Methods Ninety patients undergoing lower abdominal surgery were randomly divided into group A (received TAP), group B (received patient controlled intravenous analgesia with sulfentanil),and group C (received patient controlled epidural analgesia with sulfentanil and ropivacaine)for postoperative analgeda.All patients received combined spinal and epidural anesthesia.VAS score at postoperative 2,4,8,16 and 24 h,as well as the side effects related to postoperative analgesia,were observed and recorded for each patient.Results Comparing with group A,VAS scores of group B at postoperative 2,4,8,16 and 24 h were obviously higher (P< 0.05);Comparing with group A,VAS score of group C at post-operative 24 h was obviously lower (P< 0.05).Comparing with group A,nausea and vomiting,pru-ritus,and somnolence were more obvious in group B (P < 0.05);nausea and vomiting,sensory and motor abnormality of lower limbs and urinary retention occurred more frequently in group C (P <0.05).Conclusion Ultrasound-guided TAP can effectively relieve postoperative pain 24 hours,and has no obvious side effect in patients received lower abdominal surgery.
2.The Advantages of Chinese Medicine in the Treatment of Postoperative Thyroid Carcinoma

Thyroid cancer is one of common malignant tumor in neck, which can occur in any age. Females are more likely to have this disease than males. Most patients get a nodule on their thyroid, which typically does not cause any symptoms. Surgical resection is an effective method to cure thyroid cancer. In recent years, more and more evidence support that traditional Chinese medicine plays an important role in the recovery of postoperative thyroid carcinoma.
3.Changes of DNA content in the fibroblasts of narrow pedicled subdermal vascular network flap during its surviving

Objective To study the dynamic changes of DNA content of the fibroblasts in narrow pedicle subdermal vascular network flap (SVNP) during their surviving. Methods Eighteen pieces of surviving skin in pig back with size of 16 cm?8 cm and pedicle wideness of 4 cm were stained with acridine orange, and their DNA contents were measured with the adherent cell analysis and sorting cytometer 570. The tissue samples were taken at 3, 4 ,5 ,and 7 days after operation from different parts of the flap.Results The DNA content of fibroblast in the proximal part of the flap was slightly changed, those in the middle portion increased from 3rd to 5th day and became normal at 7th day, and in the distal portion progressively increased after operation. The results suggested that the transitional part of the flap and skin graft in the middle portion was most active in function and metabolism, and well survived, but the distal portion of the graft was poor in surviving and had a long repairing period. These indicated that local tissue hypoxia stimulated proliferation of fibroblasts and synthesis of the DNA in a short time, but as time wert by, the proliferative speed and DNA synthesis in the individual cell were heterogeneous, probably because of the degeneration or necrosis of some epidermal cells in the graft. Conclusion The wideness and length of the pedicle as well as its vascular anastomoses should be readjusted to promote the blood circulation of the distal skin graft tissues.
4.The role of cardiac renin-angiotensin system in the development of volume-overload myocardial hypertrophy in rats

The present study was performed to determine whether or not that cardiacrenin-angiotensin system (RAS) plays an important role in volume-overload myocardialhypertrophy. In order to control the increase of circulating RAS, aortacava fistula withnephrectomy (ACF+NT) rats were used in the experiment. The ventricular angiotensin Ⅰ,Ⅱ (Ang Ⅰ,Ⅱ) contents and angiotensin-converting enzyme (ACE) activities were measuredby means of radioimmunoassay and biochemical method. It was shown that in ACF+NTrats, the Ang Ⅱ contents and ACE activities in both left and right ventricles increasedsignificantly despite the plasma Ang Ⅰ, Ⅱ and renin activity remained at a lower level.These results indicate that: (1) Cardiac RAS as a relatively independent system, plays aregulatory role in the development of volume-overload cardiac hypertrophy; (2) The in-crease of Ang Ⅱ content was partly caused by the enhancement of ACE activities; (3) Atleast at the early stage of volume-overload cardiac hypertrophy, the circulating RAS maynot be a necessary promoter.
5.Inhibition effect of bisoprolol combined with trimetazidine on myocardial remodeling in hypertensive myocardial hypertrophy

Objective To investigate the myocardial remodeling inhibition effect of bisoprolol combined with trimetazidine in the treatment of hypertensive myocardial hypertrophy.Methods 86 cases with hypertension and cardiac hypertrophy in Lingwu People’s Hospital of Ningxia from June 2015 to June 2016 were selected and randomly divided into two groups,43 cases in the control group received routine clinical treatment,43 cases in the experiment group received more with bisoprolol combined with trimetazidine treatment.Blood pressure,ultrasonic cardiogram,levels of serum TNF-α, MMP3 and CRP were compared before and after the treatment.Results Compared with before treatment,levels of blood pressure,IVST,LVPWT,LVMI, MMP3,TNF-αand serum CRP significantly decreased after treatment,and compared with the control group,levels of blood pressure,IVST,LVPWT, LVMI,MMP3,TNF-αand serum CRP in the experiment group were lower,the differences were statistically significant (P<0.05),and the effective rate in the experiment group was higher than the control group ( P<0.05 ) .Conclusion Bisoprolol combined with trimetazidine can effectively inhibit the remodeling of hypertension in patients with myocardial hypertrophy,has good clinical efficacy.
6.Anti-hyperuricemia effect and mechanism of polydatin in mice.

Hyperuricemia mice model was established with uricase inhibitor (potassium oxonate) and uric acids in serum were observed. Polydatin (5, 10, 20 mg · kg(-1)) and benzbromarone (16.7 mg · kg(-1)) were given ig for 7 d in mice. Kidney tissues were used to detect gene contents ofurate anion transporter 1 (URAT1), organic anion transporter 1 (OAT1) and organic anion transporter 3 (OAT3) by real-time-PCR. The results showed that polydatin and benzbromarone can significantly reduce uric acid in blood of hyperuricemia mice (P < 0.05), compared with the model group. URAT1, OAT1 and OAT3 contents of the kidney in hyperuricemia mice changed significantly (P < 0.05), compared with the blank group. Polydatin can significantly inhibit the changing trends in these genes induced by potassium oxonate in a dose-dependent manner, the difference was significant (P < 0.05), compared with the model group. Those indicated that polysatin could reduce the level of the serum uric acid through promoting uric acid excretion.

7.The comparison of two different detection methods for anti-gp210 antibody

Objective To attempt using the synthetic poly-peptides in the carboxyl terminal of gp210 antigen as the substrate for anti-gp210 antibody detection and search for a simple assay method of detecting anti-gp210 antibody. Methods The enzyme linked immunosorbent assay (ELISA) method for anti-gp210 antibody detection was set-up by chessboard test. Anti-gp210 antibody was tested in the serum of both patients with primary biliary cirrhosis (PBC) and the control group by ELISA and immuno-blotting. Results The working concentration of gp210's antigen was 5 μg/ml. The optical density higher than 0.61 (x+3s) was designated as the positive cutoff of anti-gp210 antibody. There was no statistical difference between the two assays (P=0.617). And there was very strongly positive correlation between them (P=0.000, r=0.868). Conclusion The sensitivity and specificity are basically consistent between the assays using synthetic poly-peptides of gp210 antigen and native antigen as the detecting substrates. The former substrate is preferred to be used for the testing of anti-gp210 antibody in clinical laboratories.
8.Gold nanoparticles in tumor sensitize radiotherapy:how to maximize the effect of gold nanorods?

BACKGROUND:Gold nanorods have unique optical properties, good histocompatibility, easily control ed surface modification, which have a broad application prospect in the field of biology and medicine. OBJECTIVE:To summarize the optical and thermal properties of gold nanoparticles, and to review the mechanism and research advances of gold nanoparticles, especial y gold nanorods in tumor sensitize radiotherapy. METHODS:A computer-based search of PubMed and CNKI databases was undertaken with the keywords of“gold nanoparticles, radiotherapy sensitivity”in English and Chinese, respectively. The included articles were related to nanoparticles used in vitro and in vivo for clinic study as wel as gold nanoparticles-related tumor sensitize radiotherapy. RESULTS AND CONCLUSION:Gold nanoparticles exhibit unique properties of surface plasmon resonance properties. Compared with the traditional spherical gold nanoparticles, gold nanorods are capsule-like gold nanoparticles containing two absorption peaks that are a transverse surface plasmon resonance absorption peak and a longitudinal surface plasmon resonance absorption peak corresponding to the dimensional characteristics of their horizontal axis and vertical axis, respectively. By regulation of the ratio of horizontal axis and vertical axis, we can realize the manipulation of the longitudinal surface plasmon resonance absorption peak location. Thus, the longitudinal surface plasmon resonance absorption peak can be changed from visible region to the near-infrared region which is the most ideal optical transmission through for al of biological tissues. As a kind of novel radiotherapy sensitization agent, gold nanorods have the properties of low toxicity, high efficiency, high yield. Its sensitization effect is not only related to the exposure dose, own concentration and size, surface modified compounds, but also related to the types of radiation and celllines. However, for radiotherapy sensitization of gold nanorods how to avoid the limits of radioactive ray species, choose appropriate trim and maximize the use of gold nanorods stil need a further research.
9.Anatomical base of radial nervetransfer operation

[Objective]Through the anatomical observation and measurement on middle and distal humerus and radial nerve,To investigete a new radial nerve transfer operation.[Method]By means of anatomical methods,radial nerve's stretching branch,the relationship between radial nerve and deep brachial vessel in 30 upper climbs were observed and measured on 15 adults fixed in 10% formalin.The anatomical structures of humerus of one hundred specimens were also observed.This data were made by designing and performing the radial transfer operation after statistical analyzing.[Result](1)Length of humerus: The left side was (30.60?1.46)cm and the right side(31.37?1.23)cm.The length of sulcus for radial nerve: the left side was(5.65?1.01)cm and the right side(6.57?0.58)cm.The stretch angle of radial nerve: the left was 13.00??1.08?,the right13.86??0.97?.(2)The stretch angle of radial nerve: the radial nerve in the sulcus for radial nerve went from superomidial region to lateral lower region and the angle open to the inner part was formed.The angle is obvious in 29 sides,that was 96.67%,and not obvious only in 1 side,that was 3.33%.(3)The position of the radial nerve superficial and deep branches: The radial nerve in 17 sides branches were above interepicondylar line,which was 76.67%.Below the line were 10 sides,cover 33.33% and on the line 3 sides,cover 10.00%.(4)The curve distance between the exit of radial nerve and the brarnches was(14.26?1.01)cm.(5)The relationship between radial nerve and deep brachial vessel: the radial nerve lies inside of the deep brachial vessel were 12 sides,lies the outside of the deep brachial vessel were 10 sides and cross the deep brachial vessel were 8 sides.[Conclusion]The design of radial nerve transfer operation is reasonable,it conforms to the radial nerve's anatomy and physiology and provides much wider operation space for the performing of all kinds of internal fixation on fractures of middle and distal humerus.It reduces the injury of the radial nerve in the course of the operation and alter the operation while taking the internal fixation substance out.It doesn't increase the difficulty and injury.
10.Investigation on the clinical practice of transcatheter embolization for acute gastrointestinal hemorrhage

Objective To study the tactics, methods and relevant factors of transcatheter embolization for acute gastrointestinal hemorrhage.Methods Fifteen patients with acute gastrointestinal hemorrhage were embolized by one of the methods of Polyvinyl Alcohol (PVA), gelfoam or metal coils. Four of the fifteen patients were upper gastrointestinal hemorrhage, the other cases were lower gastrointestinal hemorrhage which were embolized using coaxial microcatheter. Results Fourteen of the fifteen patients were treated successfully by these methods. There were total 17 times of embolization for 16 parts, the success rate reached 94.1%. The other one reveaded an infarction of intestine after the embolization and was cured by resection. One of the fifteen patients appeared a recurrent hemorrhage 3 months later, and confirmed to be a hemangiolymphangioma. Two patients with malignant tumor were operated upon selectively. The other patient of intestinal hemorrhage was embolized successfully by using a metal coil after shock. Leiomyoma complicated with large area of bleeding was finally proven by operation.Conclusions Transcatheter embolization for acute massive gastrointestinal hemorrhage is safe and efficient under different choice of methods. The key of success is the right selection of embolized target artery and dosage of emboli.
