2.Oncoplasty in breast conservative surgery

ObjectiveTo evaluate oncoplastic techniques on breast conservative surgery.MethodsClinical data of 50 breast cancer patients who received oncoplastic breast conservative surgery from October 2009 to August 2011 were retrospectively analyzed.The indication,methods,outcome and morbility of oncoplastic surgery were assessed. ResultsAll patients had solitary tumors.Twelve patients received neoadjuvant therapy and thirty-eight received upfront surgical therapy.Different oncoplastic surgery techniques were applied based on the location of tumor.There were 20 tumors located in upper outer quadrant receiving lateral radial or horizontal excision; Four lower inner and seven lower outer tumors received V and J shape excision respectively; Three low pole and seven upper pole tumors received inverted T and batwing excision respectively; Six tumors located periareolar area and three tumors located retroareolar area were removed using round block and J shape excision.Two patients experienced postoperative hematoma and three patients had incisional dehiscence followed by secondary infection. Another developed skin ischemia and necrosis. Thirty-five patients have finished the postoperative adjuvant radiotherapy. The appearances of the breasts were satisfactory in all patients except one having deviation of nipple and two patients suffering from dimpling in incisional region.Patients gave high scores about their breast appearance.After 4- 27 months follow-up,two patients had local recurrence and received mastectomy.Other patients were free of recurrence.ConclusionsBreast oncoplastic surgery technique benefits those whose tumors located in special area of the breast resulting in a satisfactory cosmetical appearance of the breast.
3.Effect of photon on blood biologic components in photochemical treatment

Objective To explore the effect of photon on blood biologic components in blood photochemical treatment. Methods After the blood sample was adjusted to an appropriate density, it was treated with 0.1 nmol/ml 8-MOP (8-methoxypsoralen), 0.1 nmol/ml TFO (triple helix-forming oligonuletide) and UVA (ultraviolet A radiation) at the intensity of 1 800 μW/cm2 for 3~20 min. The changes of biologic activities of major components in blood were measured with automatic blood gas analyzer, platelet aggregation analyzer, blood coagulation analyzer, micropipette aspiration system and assay of poly-lysine adsorption. Results The oxygen content in blood was increased gradually. The resilience of erythrocyte was enhanced ,but its adhesiveness was decreased. The parameters related to blood coagulation had some changes but all remained within the normal ranges. Conclusion Under the definite condition of blood virus being inactivated effectively, the nonspecific effect of photosensitive response may improve blood oxygen content, enhance the transfiguring ability of erythrocyte and decrease the blood viscosity, but having no obvious change on blood coagulation.
4.The significance of routine exposure of recurrent laryngeal nerve in thyroidectomy

Objective To investigate the significance of routine exposure of recurrent laryngeal nerve (RLN)in thyroidectomy.Methods The data of consecutive 232 patients with thyroidectomies during 2009-2010 were retrospectively reviewed.All operations,either lobectomy or total thyroidectomy were performed by the same surgeons.The RLN was identified routinely during the operation.Results Totally 280 RLNs were exposed in 181 thyroid lobectomies and 51 total thyroidectomies(98.9%).Ten patients experienced post-operative hoarseness(3.6%).Seven patients were diagnosed with paralysed vocal cord post-operatively,although the RLN were identified intact during the surgeries,and their voice became normal in 2 months.Another 3 patients'RLNs were transected injured and re-anastomosed during the same procedures and their voice became normal in 4 months.Conclusions Exposing the recurrent laryngeal nerve in the thyroidectomy is an effective way to prevent permanent injury to RLN.
5.Protective effects and mechanism of heat shock response on cardiovascular system in rats after heat exposure

AIM: To study protective effects and mechanism of heat shock response (HSR) on cardiovascular system in rats after heat exposure. METHODS: The study was divided into 2 experiments: ① Protective effects of HSR on cardiovascular system in rats after heat exposure. SD rats randomly allocated into 2 groups: heat shock group (HS group), sham control group (SC group). HS group were treated with heat shock, but SC group weren't. After recovering for 20 h at room temperature, two groups exposed to death in thermal environment, and blood pressure and electrocardiogram were measured continuously. Through Chart software mean arterial pressure(MAP), existent time etc were acquired. ② SD male rats randomly allocated into 3 groups: HS group, SC group and normal temperature control group (NC group). NC group weren't treated. The treatment in HS and SC group was identical with in the first experiment, but it would be terminated at 73 min after heat exposure, meanwhile content of MDA of myocardium were measured. RESULTS: ① Existent time in HS group was longer than that in SC group and shock arrived later; ② During earlier period after heat exposure MAP had no significant changes between HS and SC group, but after 60 mins MAP in HS group were higher than that in SC group; ③ Compared with NC group, content of MDA in myocardium in SC group was higher significantly at 73 min after heat exposure. Howerer, content of MDA in HS group was lower than in SC group, and had no significant changes with NC group. CONCLUSION:Through decreasing production of MDA in myocardium, HSR has a protective effect on cardiovascular system in rats after heat exposure.
9.Clinical research of post-stroke motor aphasia treated with acupoint application of jieyu plaster combined with acupuncture.

OBJECTIVETo compare the difference in clinical efficacy on post-stroke motor aphasia among the combined therapy of acupoint application of jieyu plaster and acupuncture, simple acupuncture and simple acupoint application of jieyu plaster.
METHODSEighty-six patients of post-stroke motor aphasia were randomized into an acupuncture group (28 cases) , an acupoint application gruop (29 cases) and the combined therapy group (29 cases). In the acupuncture group, acupuncture was applied at the Speech No. 1 area and three-tongue needling points, once a day, 6 times a week. In the acupoint application group, jieyue plaster was applied to Yongquan (KI 1) and Laogong (PC 8), once a day, and the bilateral acupoints were selected alternatively. In the combined therapy group, the therapeutic methods of the first two groups were used in combination. The treatment lasted for 4 weeks in the three groups. The speech function score was observed and compared before and after treatment in the three groups and the efficacy was compared among the three groups.
RESULTSThe total effective rate was 86.2% (25/29) in the combined therapy group, which was better than 67.9% (19/28) in the acupuncture group and 69.0% (20/29) in the acupoint application group (both P < 0.05). After treatment, the scores of spontaneous conversation, restatement and nomenclature in the speech function were all improved as compared with those before treatment in the three groups (all P < 0.01). The results in the combined therapy group were apparently better than those in the acupuncture group and the acupoint application group (all P < 0.01).
CONCLUSIONThe combined therapy of acupoint application of jieyu plaster and acupuncture apparently improves the speech function in the treatment of post-stroke motor aphasia and the efficacy is better than that of simple acupuncture or simple acupoint application.

10.Meta - analysis of the risk factor for endophthalmitis in patients after cataract surgery

AIM: To explore the main risk factors related to the incidence of endophthalmitis in patients after cataract surgery in China and to provide evidence for prevention.METHODS: The results of 5 studies on the main risk factors of endophthalmitis in patients after cataract surgery were analyzed by Meta-analysis method.
RESULTS: The pooled odds ratio values and 95% CI of age(≥70), diabetes, vitreous overflow, operative time ( ≥ 10min ), common operating room and control of using time of topical anesthetic were 1. 81(95% CI: 1. 43-1. 69),3. 66 (95% CI: 1. 64 - 8. 16),2. 21 (95% CI: 1. 46 -3. 32),3. 54 (95% CI: 2. 47 - 5. 06),2. 77 (95% CI: 2. 07 -3. 72),2. 09(95% CI: 1. 53-2. 86).
CONCLUSION: The main risk factors of endophthalmitis were the age(≥70), diabetes, vitreous overflow, operative time(≥10min), common operating room and control of using time of topical anesthetic.
