Chinese Journal of Experimental Ophthalmology 2013;(3):270-274
Background Increase of lens thickness at incipient cataract is a key factor of onset of primary angle-closure glaucoma (PACG).Phacoemulsification (Phaco) or phacotrabeculectomy (Phacotrabe) have been documented to be effective for the patients of PACG associated with cataract. However, which surgery is more effective and safe is lack of evidence. Objective This study was to assess and compare the clinical effectiveness of Phaco versus Phacotrabe for PACG with cataract. Methods The relevant literature was searched electronically from the PubMed (1966 to June 2011),EMB Reviews (1966 to June 2011) and Cochrane Library (Issue 1,2011).The manually searching of relevant conference proceedings was used as the supplement. The articles of randomized controlled trial (RCT) about the clinical effectiveness of Phaco versus Phacotrabe for PACG with cataract were included. The methodology quality of included literature was graded. The analysis indexes included intraocular pressure (IOP)-lowing range, postoperative administration of glaucoma drugs, incidence of positive complication, postoperative best corrective visual acuity (BCVA) and perimetry damage. The RevMan4.2 software from Cochrane Collaboration was used for the Meta analyses. Results Three RCTs about phaco versus Phacotrabe for PACG with cataract were selected in this study with the 164 eyes of 164 cases.Meta analysis showed that the IOP-lowing range was larger in the Phacotrabe group compared to only Phaco group with the WMD of 1.17 and 95% CI of 0.06-2.27 (P =0.040),and the drug dosage of anti-glaucoma was less in the Phacotrabe group in comparison with the Phaco group with the WMD of 0.5 and 95% CI of 0.24-0.77 (P =0.000).However,the incidence of postoperative complication was higher in the Phacotrabe group than that of the Phaco group with the RR of 0.08 and 95% CI of 0.02-0.33 (P =0.000).No significant difference was found in the BCVA (WMD =0,95% CI:-0.13-0.13,P=1.00) andperimetry (WMD =1.01,95%CI:0.56-1.82,P=0.98).Conclusions Compared with Phaco,Phacotrab has a better IOP-lowing effectiveness and slightly worse safety.Phaco and Phacotrab have a fairly influencc in the postoperative BCVA and perimetry. As the sample sizes of the included trials are relatively small, more welldesigned large-scale RCTs are needed.

BACKGROUND:Which an ideal method to treat type Rockwood Ⅲ acromioclavicular joint dislocation is,traditional or surgical therapy,is still controversial.OBJECTIVE:To investigate the clinical effect of clavicular hook plate implantation on type Rockwood Ⅲ acromioclayicular joint dislocation.METHODS:A total of 56 patients with type Rockwood Ⅲ acromioclavicular joint dislocation were selected from Department of Orthopaedics of Yiling Hospital between December 2005 and June 2008.There were 42 males and 14 females, aged 16-65 years and mean age of 32 years. All patients were treated with clavicular hook plate implantation.Indicators including visual analogue scale (VAS),the United States shoulder and elbow surgeon score (ASES),and Constant and Murley scoring system were tested pre-operatively,one year postoperatively, before internal fixation, and 3 months after internal fixation;additionally, functional changes of the shoulder joint and complications were analyzed before and after clavicular hook plate implantation.RESULTS AND CONCLUSION:The surgery and following-up were successfully achieved in 56 cases. The following-up lasted for 15-30 months, with the mean time of 20 months. The internal fixation was taken out at about 1 year after implantation.Coracoclavicular ligament was repaired in 32 cases but not in the 24 cases. Two patients with acromioclavicular joint dislocation recurrence were excluded, and coracoclavicular ligament therapy was not performed.Shoulder pain, foreign body sensation, and internal fixation failure occurred in two cases. One year after operation,VAS was decreased compared with that before operation, but ASES and Constant and Murley score were significantly increased (P<0.01).At three months after internal fixation,VAS was decreased compared with that at 1 year after operation, but ASES and Constant and Murley score were increased (P<0.05).The results demonstrated that clavicular hook plate implantation for treating type Rockwood Ⅲ acromioclavicular joint dislocation is simple and less invasive, thus it is an ideal internal fixation.

Acta Pharmaceutica Sinica 2003;38(12):893-896
AIMTo investigate the stereoselectivity in absorption of trans tramadol (trans T) in rat intestine.
METHODSThe duodenum, jejunum and ileum were separately perfusated in situ with trans T dissolved in Krebs-Ringer buffer. Trans T enantiomers in the perfusate were analyzed with a high performance capillary electrophoresis (HPCE) method.
RESULTSThe absorbed fractions of trans T enantiomers were similar among the different segments of the rat intestine. The absorbed fraction of (+)-trans T was lower than that of (-)-trans T when the concentration of trans T was not higher than 40 mumol.L-1. As the concentration of trans T increased, the absorbed fractions of trans T enantiomers were reduced and the difference in absorbed fractions between trans T enantiomers became not significant.
CONCLUSIONTrans T enantiomers can be absorbed in different parts of the rat intestine. The intestinal absorption of trans T was stereoselective, (-)-trans T being preferentially absorbed.

Objective To assess the correlation between polymorphisms in the coagulation factor Ⅶ (F Ⅶ) gene hypervariable region 4 (HVR4) site and risk related to coronary heart disease (CHD) in different ethnic populations, especially the Asian populations. Methods Publications up to April 2013,from CBM,CNKI,Wanfang Database,VIP,PubMed,Cochrane Library and Embase were searched to collect data from case-control studies related to F Ⅶ gene HVR4 site and CHD in populations from different ethnicities.Quality of studies was evaluated, available data extracted and both RevMan 5.1 and Stata 11.0 softwares were used for Meta-analysis.Results Fifteen case-control studies were included, involving 3167 cases with CHD group and 3168 cases in the control group. Results on this Meta-analysis showed that:a) polymorphism of the FⅦ gene HVR4 site H7/H6 +H5and CHD,b) H7H7/H6H6 + H7H6 and CHD were both slightly correlated between people with different ethnic backgrounds. However, the H6 allele versus H7 + H5 allele and CHD showed different results-a high correlation seen in different ethnic groups.H5 allele versus H6 + H7 allele and CHD did not appear significant difference(OR=l.20,95%CI:0.76-1.90,P=0.43).Conclusion Both FⅦgene HVR4 polymorphisms H7 allele and the H7H7 genotype might have served as protective factors for CHD in different ethnic groups,H6 allele might serve as a risk factor for CHD,but H5 allele was likely not to be associated with CHD in different ethnic groups.

Objective:
To illustrate the surgical strategy of sequential correction in degenerative lumbar scoliosis (DLS) with type C coronal imbalance, and to evaluate the clinical outcomes and advances of sequential correction technique.
Methods:
Twelve patients (2 males and 10 females) applying sequential correction technique from January 2015 to August 2017 were retrospectively reviewed. The ages of the cohort ranged 48-74 years and the average value was 52.3±8.4 years. The sequential correction technique was mainly applied in 3 steps: correction of local kyphoscoliosis with satellite rod on convex side of lumbar spine; correction of lumbosacral curve with L 4-S1 Intervertebral fusion and satellite rod on convex side of lumbosacral spine; correction of global deformity with bilateral long rods. The coronal parameters including Cobb angle and distance between C7 plumb line and center sacral vertical line (C7PL-CSVL), and the sagittal parameters including global kyphosis (GK) and sagittal vertical axis (SVA) were assessed at pre-operation, post-operationand last follow-up. The quality of life was evaluated using SF-36 questionnaire, and paired t test was used for the statistical analysis.
Results:
The average follow-up period was 16.7±4.8 months. The Cobb angles at pre-operation and post-operation were 59.6°±18.7° and 25.6°±12.4° (

Objective To analyze the epidemiological characteristics of malaria in Leshan City,so as to provide the evi-dence for formulating the strategy and measures for consolidating the achievements of malaria elimination. Methods The data of epidemiological characteristics of malaria in Leshan City from 1950 to 2015 were collected and analyzed with the descriptive epidemiology method. Results There were four larger scale epidemics of malaria in Leshan City from 1950 to 2015. The order of malaria in infectious diseases dropped from the first to the twentieth. The peak season for malaria epidemics ceased. The Plas-modium species, patient age, occupation and sex distribution of malaria were different in the different periods. In November 2016,the whole city achieved the national standard for malaria elimination. Conclusions After years of prevention and con-trol,the epidemiological characteristics of malaria have changed significantly in Leshan City, and the effect of anti-malarial mea-sures is significant. In the future,we should strengthen the monitoring, and prevention and control of imported malaria.

Objective:
To investigate the efficacy and safety of rapamycin in children with tuberous sclerosis complex (TSC) associated renal disease.
Methods:
A prospective self-control study was conducted. The clinical data of 92 children diagnosed with tuberous sclerosis complex associated kidney disease at the People′s Liberation Army General Hospital from January 2011 to January 2019 were collected. The long-term rapamycin treatment for all patients initiated at 1 mg/(m2·d), which was gradually adjusted to reach a blood concentration of 5-10 μg/L. The changes of the maximum diameter of renal lesions in children after rapamycin treatment were observed and analyzed with Wilcoxon test.
Results:
Ninety-two children, including 52 males and 40 females, who met the criteria were analyzed. Sixty patients had only renal angiomyolipoma(RAML), while 24 patients had only multiple renal cysts(MRC), and 8 patients had both lesions. The age of TSC diagnosis was 16.0 (7.0, 42.0) months, and the age of initial treatment with rapamycin was 63.5 (21.0, 103.0) months. The follow-up lasted for 12.0 (4.0, 23.0) months. Sequencing of TSC1 and TSC2 genes was performed in 54 children with TSC, including 3 patients (6%) with mutations in TSC1 gene and 51 patients (94%) with mutations in TSC2 gene. The maximum RAML diameter before treatment was 7.0 (4.0, 9.0) mm. The best effect reached at 3 months of treatment, with the diameter of 4.0 (0,7.0) mm. The maximum diameters at 6 months, 1 year and 1-2 years were 5.0 (0,9.8) mm, 5.0 (1.5, 8.5) mm, 5.5 (3.0, 9.0) mm, respectively, and were significantly different from the baseline (
