1.Clinical and laboratory study of early nerve foot lesion after collagenase chemonacleolysis

[Objective]To investigate the etioloty of the early nerve root lesion(RNRL) after collagenase chemonucleolyoio(CC) and its prevention and treatment.[Method]Twelve-hundred and eighty case of lumbar disc herniation undergoint cc were reviewed and analyzed.From January,2000 to December,2004 with follow-up time of 4~30 months,in which 12 cases(0.94%)had obvious ENRL after CC.Eighteen rabbits were randomly divided into three groups of six each:Group A,posterior longitudinal ligament intepated+intradisca injection;group B,parterior longitudinal ligamen+booteen+intradiscal injection,and group C,extradiscae injection.The disc and the nerve roots at the inject side were removed,fixed and examined by microscope,and the changes of the discs end nerve roots were analyzed in different inject side and different time after CC.[Result]As the collagenase dose was large in groups of intradiscal+intradiscal injections case of ENRL in this group were more than that in other groups.In the 12 ENRL cases,ten cases of ENRL appeared in about 7 days after CC,8 cases without nerve root compression and all cases were improved in different degree,but no one reeovered completely at follow-up with excellent and good rate of efficiency in the cases operated in 3 days than that in cases operated after 8 days.In the experiment rabbits,there lowed obvious lyeis of nucleus.Pulpus in group A and group B,with severe damage of nerve roots in group B,while in group C,there was no obvious lipis of nucleuss pulpus and damage of nerve roots[Conclusion](1)As the larger collagenas dose in the cases with intradiscal and extradiscal injections can raise.The incidence of ENRL,it is suggested that CC used in cases with broken posterior longitudenal ligament of and brous anmulus and,with nucleus pulpuses herniated into spinal canal,CC should be prudent to apply.(2)The prognoseo of the ENRL after CC is poor and is related to operation time.(3)The incidence of ENRL after CC can be lowered with absolute bed rest and changing body position after CC.
2.The significance of detection of Twist,p53 and E-cadherin in breast cancer

Objective To observe the expression and correlation of Twist,p53 and E-cadherin mRNA in breast cancer.Methods Thirty breast cancer tissue and adjacent noncancerous tissue in patients underwent modified radical surgery,and the normal breast tissue in 10 patients with benign breast lesions,respectively,were detected the expression of Twist,p53 and E-cadherin mRNA by RT-PCR and the relationship with clinicl pathological factors and their interrelationships.Results There was significant correlation between the expression of Twist mRNA and the degree of differentiation[high and middle differentiation 93.33%(14/15),1ow differentiation 53.33%(8/15)],lymphatic invasion[yes 93.75%(15/16),no 50.00%(7/14)]and TNM staging[Ⅰ-Ⅱ?stage 46.15%(6/13),III-Ⅳ?stage 94.12%(16/17)]in breast cancer tissue(P<0.05),while there was no obvious correlation between the expression of Twist mRNA and the age,tumor diameter,adventitial infiltration,estrogen receptor,progesterone receptor(P>0.05).The expression of Twist and p53 mRNA in breast cancer tissue were significantly higher than those in adjacent noncancerous tissue and normal breast tissue[(112.36±12.53)% vs.(101.37±9.12)% and(98.03±9.04)%,(101.73±13.32)% vs.(91.43±7.43)% and(90.83±6.58)%,P<0.01],while the expression of E-cadherin mRNA in breast cancer tissue was significantly lower than that in adjacent noncancerous tissue and normal breast tissue[(55.43±12.49)% vs.(69.39±5.63)% and(72.73±7.93)%,P<0.01],there was no significant difference between adjacent noncancerous tissue and normal breast tissue(P>0.05).The expression of Twist mRNA in breast cancer tissue was positively correlated to the expression of p53 mRNA(r=0.526,P=0.004),and negatively correlated to the expression of E-cadherin mRNA(r=-0.500,P=0.000).Conclusions Twist and p53 in breast cancer tissue show higher expression,while E-cadherin shows lower expression.Twist can promote invasion,metastasis of breast cancer by up expression of p53 and down expression of E-cadherin,and it plays an important role.

6.A method based on cumulative toxicity for evaluating pharmacokinetics of oral traditional Chinese medicine in animals

AIM: To improve the acute cumulative death rates method (ACD method) in animal by oral administration. METHODS: A mathematic model was established to evaluate the dose-toxic effect relationship by twice oral administration and the experimental steps were improved too. The methodological quality was explored using the simulation data from computer program and the real experimental date from the reference paper. RESULTS: The results showed that the experimental data could be fitted to its theoretical data from LD_5/2 to LD_ 95/2. Concentration-time curve after po ordinary powder of Semen Strychni in mice were fitted to a one-compartment with T_ 1/2(ka)= 1.136 h,T_ 1/2(ke)= 7.100 h,and T_ max= 3.576 h. CONCLUSION: The improved ACD method can be used in the pharmacokinetics of TCM by oral administration.
7.Accuracy of renal blood flow assessment by transesophageal echocardiography during carbon dioxide pneumoperitoneum

Objective To determine the accuracy of renal blood flow assessment by transesophageal echocardiography (TEE) during carbon dioxide (CO2) pneumoperitoneum.Methods The left renal arterial diameter (RAD) and the Doppler velocity time integral (VTI) were measured by TEE before peumoperitoneum (T0, baseline), at 1, 5, 10, 15, 20 and 30 min of pneumoperitoneum and 1 and 5 min after deflation in 35 patients undergoing elective laparoscopic cholecystectomy. The left renal blood flow (LRBF) and the left renal blood perfusion resistance (LRPR) were calculated according to the following formulae: LRBF = 1/4π x RAD2 x VTI x HR, LRPR = MAP/LRBF.Three months later, the TEE images of 10 cases were randomly selected and reviewed by the same and another research team member to check the repeatability and consistency of the LRBF determination during operation, respectively. The quality of the TEE images was evaluated by another specialist.Results Before pneumoperitoneum, 94% of the TEE images were rated as satisfactory. There was no significant difference between the qualities of the TEE images obtained before and during pneumoperitoneum. The variabilities between the RADs measured by TEE during and 3 months after operation were 9.28% by the same team member and 8.71% by another team member. The variabilities between the VTIs measured by TEE during and 3 months after operation were 5.61% by the same team member and 6.25% by another team member. The linear regression analysis of the LRBF showed that the slope and the intercept were 1.05 and 31.4 ml/min respectively by the same member and 0.92 and 47.3 ml/min respectively by another member. The LRBF was decreased during pneumoperitoneum and the LRPR was increased.Conclusion TEE can be used to accurately monitor the changes in renal blood flow during CO2 pneumoperitoneum.
8.Preoperative self-efficacy of patients with lumbar disc herniation and its influential factors

Objective To study the preoperative self-efficacy of patients with lumbar disc herniation(LDH) and its influential factors. Methods Seventy-six patients with LDH underwent operation from June 2008 to March 2009 were enrolled in the current study. The patients were investigated with GSES,SAS,SDS and self-made general characteristics before operation. The data were analyzed with SPSS 11.5. Results The score of preoperative self-efficacy was 25. 84 ±6. 76. The course of disease, education level, patients' hobbits could affect self-efficacy (F = 15. 371,P<0.01;F = 18.078,P<0.01;t =2. 368,P<0. 05). Preoperative self-efficacy correlated negatively with the course of disease (r = - 0.541, P < 0. 01). And preoperative self-efficacy correlated with education level and hobbits(r = 0. 589, P < 0. 01; r = 0. 226, P < 0.05) . The score of preoperative dysphoria and depression was significantly higher than norm (P < 0. 01) . Preoperative self-efficacy correlated negatively with dysphoria and depression (r = -0. 494, P < 0.01; r = - 0. 443, P < 0. 01). Conclusions The score of preoperative self-efficacy with LDH is low; the score of preoperative dysphoria and depression is high. Preoperative self-efficacy correlated negatively with the course of disease. And preoperative self-efficacy correlated with education level and hobbits; Preoperative self-efficacy correlated negatively with preoperative dysphoria and depression.
