5.Application of percutaneous transluminal angioplasty in hemodialytic patients with arteriovenous fistula stenosis

Arteriovenous fistula stenosis developed in 32 patients who received hemodialysis from April 2014 to April 2016, including 24 cases of autologous arteriovenous fistula ( AVF) and 8 cases of artificial vessel arteriovenous fistula ( AVG).In AVF patients, there were 20 cases with blood flow decreasing and 4 cases of venous hypertension , while in AVG patients there were no cases with blood flow decreasing and 8 cases with venous pressure increasing (χ2 =17.77, P<0.001).All patients underwent percutaneous transluminal angioplasty ( PTA ) . In all patients the vascular diameter was increased significantly after PTA treatment [(3.6 ±0.2) vs.(1.2 ±0.1) mm, t=18.52, P<0.001].The blood flow during hemodialysis was increased significantly after PTA treatment in patients with anastomoti stenosis [(265.4 ±21.5) vs.( 150.5 ±23.1 ) ml/min, t =21.81, P <0.001 ]; however, there were no significant changes in venous pressure [(68.6 ±8.7) vs.(69.3 ±6.1) mmHg(1 mmHg=0.133 kPa), P>0.05].The blood pressure was decreased after PTA treatment [( 71.2 ±7.9 ) vs.( 170.4 ± 15.7) mmHg, t=34.62, P<0.001] in patients with venous outflowtract stenosis , however, there were no significant changes in the blood flow [(264.1 ±15.2) vs.(260.7 ±17.3) ml/min, t=1.04, P>0.05].
6.Pay attention to the problem of the overtreatment of thyroid nodules

Thyroid nodules are common lesions which are over-treated in Mainland China. This article encourages using standard diagnostic procedures to differentiate malignant nodules from benign nodules especially by ifne needle aspiration (FNA) cytology. The proper treatment should be applied according to different FNA results to avoid overtreatment.
7.The application of color Doppler flow imaging examination in diagnosis of liver tumor

Objective:To investigate the diagnostic value of color Doppler flow imaging(CDFI) in the diagnosis of liver tumors.Methods: UE, CEUS, and CDFI were performed in 104 patients with liver cancer who were admitted to our hospital from January 2013 to December 2014, in order to analyze the diagnostic value of three methods for liver tumor compared with the pathological results.Results: The 115 masses of the group included 97 malignant masses(84.34%) and 18 benign masses(15.66%). The diagnostic accuracy of CDFI for benign and malignant liver tumors was 55.56% and 74.23%. There was no significant difference between UE and CDFI in the diagnosis of benign liver tumors and CEUS was higher than that of CDFI and UE in the diagnosis of benign and malignant liver tumors. There was no statistical difference in the diagnostic accuracy of CDFI combined with UE and CEUS(x2=2.20, P=0.138).Conclusion: CDFI is a basic method for the diagnosis of liver tumors. The CEUS or UE can be used to improve the accuracy of diagnosis for the liver which is not typical or small lesions.
8.Clinical study on the characteristics and treatment of critical limb ischemia caused by limb arteriosclerosis obliterans in elderly patients ≥ 70 years

Objective To investigate the characteristic and optimal therapy of critical limb ischemia (CLI) caused by limb arteriosclerosis obliterans (ASO) in elderly patients ≥70 years. Methods The clinical and follow-up data of 122 elderly patients (≥ 70 years) with CLI caused by ASO were analyzed. The differences were compared between 70-79 years patients (67 cases) and 80- 91 years patients (55 cases). Results The time from onset to treatment in 80-91 years patients was significant longer than that in 70-79 years patients:(45 ± 15) d vs. (26 ± 10) d, the ratios Rutherford class 4-6 and Trans-Atlantic Inter-Society Consensus (TASC) Ⅱclass C-D lesions in 80-91 years patients were significantly higher than those in 70-79 years patients: 89%(49/55) vs. 67%(45/67) and 91%(61/67) vs. 100%(55/55), and there were statistical differences (P<0.01 or<0.05). The ankle arm index (ABI) after endovascular treatment in 70-79 years patients (37 cases) and 80- 91 years patients (22 cases) were significantly higher than those before treatment: 0.86 ± 0.05 vs. 0.40 ± 0.05 and 0.85 ± 0.05 vs. 0.39 ± 0.08, and there were statistical differences (P<0.05). There were no statistical differences in ABI of conservative treatment patients before and after treatment (P>0.05). The conservative treatment mortality in 80- 91 years patients was significantly higher than that in 70- 79 years patients: 29%(7/24) vs. 9%(2/23), and there was statistical difference (P<0.05). Ninety-five patients were followed up for 1-24 months, and the median was 15 months. Nine patients (7%) died within 30 d. In the first year of follow-up, 8 cases underwent amputation, 6 cases died, and 6 cases had stent thrombosis. In the second year of follow-up, 12 cases underwent amputation, and 18 cases died. The median survival time was 19 months. The mortality in conservative treatment patients was significantly higher than that in endovascular treatment patients:53%(19/36) vs. 24%(14/59), and there was statistical difference (P<0.05). The survival time in amputation patients had increasing trend, but there was no statistical difference (P>0.05). Conclusions The≥70 years elderly patients with CLI caused by ASO have more complications and longer time from onset to treatment, and the lesions are mainly Rutherford class 4-6 and TASC classⅡC-D. Endovascular treatment is difficult, but the effect is better than conservative treatment.
9.Refractory Gastroesophageal Reflux Symptoms Not Necessarily Denotes Gastroesophageal Reflux Disease

The symptoms of gastroesophageal reflux may be induced by acid reflux, nonacidic or weakly acidic reflux, and also can occur with hypersensitive esophagus, esophageal motility disorders, or other organic/anatomical abnormalities of esophagus.Refractory gastroesophageal reflux disease (GERD) denotes the symptoms of GERD could not be remitted with the standard dosage proton pump inhibitor (PPI) therapy.In most patients with refractory symptoms the etiology is of non-reflux-related causes, such as eosinophilic esophagitis, esophageal motility disorders, gastroparesis, or concomitant functional gastrointestinal disorders.A careful and detailed understanding of patient's medical history, examination with endoscopy and biopsy pathology, and esophageal pH-impedance monitoring are very important for identifying the potential cause.
10.Effects of c-fos antisense oligoneuleotide and p21 genetic transfection on the intimal proliferation of venous autografts in rabbits

AIM: To investigate the effects of c-fos antisense oligoneuleotide and p21 genetic transfection on the intimal proliferation of venous autografts. METHODS: The external jugule veins were autografted into common carotid arteries in the same side in 20 New Zealand rabbits, which were divided evenly into experimental and control group randomly. The transplanted veins of experimental group were immersed in the adenovirus - mediated p21 gene solution for 15 minutes just before anastomosis and coated with c-fos antisense oligoneucleotide glue gel just after anastomosis, while the control was only treated with empty vector. The transplanted vascular sample were taken at 2 weeks after operation. The intimal thickness (IT), degree of restenosis (DR), expression of proliferating cell nuclear antigen (PCNA), quantity of VSMC were determined by immunohischemistry. RESULTS: The IT , DR and expression of PCNA, VSMC were decreased, compared to control group. CONCLUSION: Transfection of c-fos antisense oligoneuleotide and p21 gene inhibits the intimal proliferation of venous antografs. [
