1.Emotional problems among newly diagnosed HIV-positive men with homosexual sex behaviors

Objective:To know about the prevalence of depression,anxiety,and suicidal behaviors among newly diagnosed HIV-positive men who have sex with men.Methods:A cross-sectional study with a consecutive sample was conducted in the HIV/AIDS Voluntary Counseling and Testing Clinic of the Center for Disease Control and Prevention.A standard set of questionnaires,namely,the Patient Health Questionnaire Depression Scale (PHQ9),Generalized Anxiety Disorder Scale (GAD-7),and a self-designed suicidal behavior questionnaire were used to assess the subjects'emotional problems.Results:Among 321 newly diagnosed HIV-positive men who had sex with men,the positive rate of depression and anxiety were 41.1% (132/321) and 31.5% (101/321),respectively.The rate suicidal ideation after HIV diagnosis was 27.7% (89/321).Individuals who were living with others,had HIV related clinical symptoms,higher HIV/AIDS related stress,and lower social support were more likely to have positive depressive symptoms.Individuals who were unmarried,had lower HIV/AIDS related stress,and lower social support were more likely to have positive anxiety.The prevalence of suicidal ideation after HIV diagnosis was higher among individuals who were unwilling to join in HIV/AIDS support groups,had HIV related clinical symptoms,higher HIV/AIDS related stress,lower social support,and both positive depression and anxiety symptoms.Conclusion:The prevalence of emotional problems is high among newly diagnosed HIV-positive men who have sex with men.It is warranted to promote mental health service for this vulnerable group.
2.Effects of sunscreens on delayed type hypersensitivity in mice

Objective To study the influence of sunscreens with different efficacy on delayed type hypersensitivity (DTH) and their immunoprotective effect in mice.Methods A cohort of mice were randomly divided into 5 groups with 10 mice in each group:group 1 as the positive control without irradiation,group 2 receiving solar-simulated radiation (SSR) only,group 3 receiving SSR and protected by sunscreen l with sun protection factor 15(SPF15)and persistent pigment darkening(PPD)12,group 4 receiving SSR and protected by sunscreen 2 with SPF 50 and PPD 28,and group 5 as the negative contml receiving SSR only.SSR was carried out on the back of mice with the UVA dose being 1.4 J/cm2 and UVB dose being 100 mJ/cm2 for 10 days.After a 5-day irradiation,the groups 1 to 4 were immunized by intraperitoneal injection with 100 μl(107 cells/ml) of Candida albicans suspension.On the 10th day both sides of the posterior foot pad were measured;then the foot pads were injected with additional 50 μl of the Candida albicans suspension.Twenty-four hours after the injection,the thickness of each foot pad was measured,and immunosuppression rate was calculated.Finally,the mice were sacrificed and skin samples were obtained from the back of these mice followed by the examination of CDla, CD80 and CD86 expression by Western blot.Resets The thickness of edema in foot pads was 0.41±0.38 mm,0.21±0.23 mm and 0.30 ± 0.25 mm in group 1,3 and 4,respectively,significantly higher than in group 5 and 2(0.04±0.03 mm,0.14±0.12 mm,respectively,all P<0.05),while no significant difference was observed between the group 3 and 4(P>0.05).Significant differences were observed in the immunosuppression rate between group 2,3 and 4(73.0%±11.3%,54.1%±6.4%,29.7%±7.5%,respectively,all P<0.01).Western blot revealed a significant increment in the expression of CDla protein in group 1 compared with group 2 as well as in the expression of CD86 protein in group 1 and group 3 compamd with group 2 and group 5(all P<0.05),but no statistical difference was observed between the other groups in the expression level of CDla,CD80 or CD86(P>0.05).Conclusions The exposure to sub-erythema dose of UV can induce DTH,and sunscreens have an immunoprotective effect in this process.Epidermal Langerhans cells are not essential for UV-induced immunosuppression.
3.Observation about the treatment effect with home-made suction drainage and algoplaque on severe pressure sore

Objective Observation and discussion about the treatment effect with home-made vacuum drainage and Algoplaque on severe pressure ulcers. Methods The patients with severe pressure ulcers selected from Yuebei Peole′s Hospital of Shaoguan city from January to December 2016 were randomly divided into three groups:experimental group, control group A and group B (30 cases in each). The patients were treated with home-made vacuum drainage and Algoplaque in the experimental group, meanwhile a simple use of Algoplaque treatment in control group A and VSD technology in control group B only. Observed and compared the effect in the three groups on the time of dressing change, wound healing time and the total cost. Results The wound healing time of experimental group.contral group A and group B were (24.10 ± 2.12), (33.26 ± 1.71), (27.87 ± 1.95) days, the times of dressing change were 5.52 ± 0.96, 35.84 ± 1.81, 7.23 ± 1.09, dressing costs were (629.95 ± 28.10) yuan, (1354.29 ± 301.63) yuan, (10825.38±1678.21) yuan, and the differences were statistically significant (F=175.961, 5017.527, 1029.377, all P<0.01), and the wound healing time, dressing times, dressing costs were lower than the control group A and B in the observation group. Conclusion The treatment of home-made vacuum drainage and Algoplaque on patients with severe pressure ulcers can obviously reduce the time of dressing change and wound recovery, and the cost also. It is worthy to promote in clinic.
4.Effect of Shenfu injection on Nrf2 signaling pathway in rats with cerebral ischemia reperfusion-induced brain injury

Objective:To study the neuro-protective effects of Shenfu injection on Nrf 2 signaling pathway affected by cerebral ischemia reperfusion.Methods: A total of 68 adult male Sprague-Dawley rats were randomly divided into sham group , cerebral ischemia-reperfusion group,and(8 mg/kg)Shenfu injection treatment group.Shenfu injection was injected intraperitoneally in the rats after MCAO.Neurologic deficit was evaluated after 24 hours of reperfusion.All the rats were sacrificed after 24 h of ischemia-reoerfusion for biochemical analysis or Nissl staining.Results:Shenfu injection treatment significantly increased the expression of Nrf 2,HO-1 and NQO-1(P<0.05).Furthermore,Shenfu injection treatment significantly reduced the expression of cleaved -caspase-3 and attenuated neurological deficits after cerebral ischemia reperfusion ( P<0.05 ).Lastly, Shenfu injection could evidently alleviate the severity of neuronal degeneration ( P<0.05 ).Conclusion:Shenfu injection could confer neuroprotection after cerebral ischemia reperfusion through modulating the Nrf2 signaling pathway.
5.Effect of urinary proteins and advanced glycosylation end products on ly-sosomes in renal tubular epithelial cells

[ ABSTRACT] AIM:To investigate the effects of pathological products, urinary proteins and advanced glycosyla-tion end products ( AGE) produced in the progression of chronic kidney disease ( CKD) , on the structure and function of lysosomes in renal tubular epithelial cells ( TECs ) , and try to find a novel approach for preventing or delaying CKD. METHODS:The renal specimens of the untreated patients with minimal change nephrotic syndrome (MCNS), diabetic nephropathy (DN) or normal kidney were collected.The expression of lysosomal-associated membrane protein 1 (LAMP1) and cathepsin B ( CB) was studied in TECs by indirect immunofluorescent staining.Human renal tubular epithelial cell line HK-2 was incubated with 8 g/L urinary proteins or 100 mg/L AGE.The expression of LAMP1 and CB was investigated by indirect immunofluorescence and the activity of CB and cathepsin L ( CL) was measured by biochemical and enzymatic as-says.The degradation of DQ-ovalbumin was also determined.RESULTS: The lysosomal membrane permeabilization oc-curred in the TECs of MCNS and DN patients.After treatment with urinary proteins or AGE-BSA, the lysosomal membrane permeabilization of the HK-2 cells was increased.The activity of CB and CL and degradation of DQ-ovalbumin were de-creased as compared with normal control group.CONCLUSION:The digestive function of lysosome was decreased and ly-sosomal membrane permeabilization occurred in the TECs exposed to urinary proteins and AGE, which might be a key factor to induce the tubulointerstitial fibrosis.
6.Analysis on the value of cytological and histological noninvasive methods in endometrial cancer screening

Objective To explore the value of cytological and histological noninvasive methods in the screening of endometrial cancer(EC).Methods Seventy-six cases of suspected EC high risk patients diagnosed in the gynecological outpatient department of the People Hospital of Longhua from January 2015 to May 2017 were selected as the research object,the endometrial cells and tissues were collected for cytological and histological examination,the diagnosis of curettage or hysteroscopy performed as a "gold standard" to determine the effect of cytology,histology and combined examination on the ensitivity,specificity,positive coincidence rate of EC diagnosis.Results 76 patients with suspected EC high risk factors were diagnosed as EC 48 cases(63.16%)via "gold standard".The sensitivity,specificity and positive coincidence rate of cytology were(75.00%,89.29%,80.26%,χ2=1.0105,P=0.6033),histology were(79.17%,92.86%,84.21%,χ2=0.3117,P=0.8557)and combined examination in EC were(83.33%,92.86%,86.84%,χ2 =1.2260,P=0.5417).Conclusion Cytological and histological noninvasive screening has high sensitivity,specificity and positive coincidence rate in EC and has a high diagnostic value for individual or combined exams and can be used as a routine screening for EC in clinical practice.
7.Umbilical cord blood mesenchymal stem cells transplantion for polymyositis/dermatomyositis:variation of Th cytokines

BACKGROUND:In recent years, the application of stem cel s to treat autoimmune diseases has become a hot spot. But, studies on umbilical cord blood mesenchymal stem cel s transplantation for the treatment of polymyositis/dermatomyositis are rarely reported. OBJECTIVE:To explore the immunologic mechanism of Th cytokines on the occurrence and development of polymyositis/dermatomyositis by observing the changes in serum interferon-γ, interleukin-4 and interleukin-17 in patients after umbilical cord blood mesenchymal stem cel s transplantation. METHODS:Eighty-one polymyositis/dermatomyositis patients were selected and divided into conventional therapy group (n=44) undergoing glucocorticoid and immunosuppressants therapy and cel transplantation group (n=37) undergoing intravenous infusion of umbilical cord blood mesenchymal stem cel s at a density of (3.5-5.2 )×107 . Dosing regimen was same in the two groups. After fol ow-up of 1, 3, 6 months, the changes of creatine kinase and myodynamia were evaluated;after fol ow-up of 3 and 6 months, lung imaging was evaluated;in the cel transplantation group, interferon-γ, interleukin-4 and interleukin-17 levels were detected before treatment and at 3 and 6 months after treatment. RESULTS AND CONCLUSION:At 1, 3, 6 months after treatment, the creatine kinase level was significantly decreased, and the muscle force grade was significantly increased in both groups (both P<0.001). Compared with the conventional therapy group, the creatine kinase level was lower and the muscle force grade was higher in the cel transplantation group (both P<0.001). Results from lung imaging test showed a remarkable improvement after cel transplantation, and it indicated that umbilical cord blood mesenchymal stem cel s transplantation had good stability. At 6 months after transplantation, the level of interferon-γwas significantly increased, while the interleukin-4 level was decreased significantly (both P<0.01);at 3, 6 months after cel transplantation, the levels of interleukin-17 were significantly decreased (P<0.01). Levels of interleukin-4 and interleukin-17 were positively correlated with the level of creatine kinase at 6 months after cel transplantation (r=0.467, 0.488, both P<0.05), but there was no obvious correlation between the levels of interferon-γand creatine kinase (r=0.213, P>0.05). These findings indicate umbilical cord blood mesenchymal stem cel s transplantation combined with glucocorticoid and immunosuppressants therapy can adjust immune network effects and improve the immune tolerance in polymyositis/dermatomyositis patients, which is safe and effective.
8.Reliability and validity for Chinese version of the 9-item Shared Decision Making Questionnaire.

To translate the English version of the 9-item Shared Decision Making Questionnaire (SDM-Q-9) into Chinese, and to test its reliability and validity in Chinese version.
Methods: A total of 720 inpatients were recruited randomly from 6 hospitals in Changsha and were investigated using the Chinese version of SDM-Q-9. SPSS 23.0 and AMOS 23.0 were used to test the reliability and validity.
Results: There were 660 participants completed and returned valid questionnaires (valid return rate was 91.7%). An analysis of internal consistency yielded a Cronbach's α at 0.945 and the correlation of test-retest reliability was 0.319 for whole instrument. The correlations between the items and total scale ranged from 0.790 to 0.879 (P<0.001). A single factor was extracted by exploratory factor analysis and it could explain 69.824% of the total variance. The confirmatory factor analysis revealed a good model fit. The goodness-of-fit index (GFI), adjusted goodness-of-fit index (AGFI), comparative fit index (CFI), normal fit index (NFI), and root mean square residual (RMR) were 0.870, 0.784, 0.926, 0.921, and 0.054, respectively.
Conclusion: The Chinese version of SDM-Q-9 is proved to be reliable and eligible except the correlation of test-retest reliability is relatively low. It can be used to assess the patient's perspective in the process of shared decision making in clinical situation.

9.Clinical and prognostic analysis of 21 cases of primary breast lymphoma.

OBJECTIVETo analyze the clinical features, therapeutic methods and prognosis of primary breast lymphoma (PBL).
METHODSTwenty-one PBL patients treated in Ruijin Hospital from January 2003 to December 2013 were included in this study, with 17 diffuse large cell lymphoma (DLBCL), 1 mucosa-associated lymphoid tumor (MALT), 1 follicular lymphoma (FL), 1 Burkitt lymphoma and 1 subcutaneous peniculitis T-cell lymphoma according to the WHO 2008 classification. Of 21 patients, only one patient with MALT has bulged tumor mass (>7 cm), other patients had tumor mass <5 cm. Six patients had core needle biopsy of tumor, 2 modified radical operation, and others tumor excision for diagnosis. All the patients received chemotherapy. The impacts of surgery, rituximab and prophylaxis with lumbar puncture on the outcomes of patients were analyzed. Survival was estimated using Kaplan-Meier method and compared by log-rank test. All the results were analyzed by SPSS 10.0.
RESULTSAmong 21 PBL patients, 19 achieved complete remission (CR), 1 partial remission (PR) and 1 disease progression (PD). Followed-up till July 2014, with median follow-up of 14 months (6-75 months), only one patient died, with 3-year survival of 92.3%. Compared with chemotherapy alone, the progression-free survival (PFS) of combination therapy (surgery plus chemotherapy) was significant longer (P=0.015), but without statistic differences of CR rate and overall survival (OS) between two groups. Among the 20 patients with CD20-positive tumor cells, 17 received Rituximab. PFS and OS, as well as CR rate of PBL had no difference between the treatment with and without Rituximab. The incidence of central nervous system (CNS) infiltration had no difference between patients with and without CNS prophylaxis through lumber-puncture and intrathecal injection.
CONCLUSIONWith the common subtype of DLBCL, PBL patients had good outcome. Surgery, as a method to obtain tumor samples for diagnosis, could not prolong OS of patients. Therefore, radical operation shouldn't be recommended. PBL was reported to have high risk of CNS events, but prophylaxis with lumber puncture and intrathecal injection couldn't decrease the incidence of CNS infiltration.
