1.Interventianal therapy of acute myocardial infarction complicated with ventricular septal rapture

Objective To evaluate the feasibility and safety of the treatment for ventricular septal rupture (VSR) by transcatheter interventional therapy coordinated with percutaneous coronary intervention (PCI) or percutaneous transluminal coronary angioplasty (PTCA).Methods Six patients who suffered from cardiogenic shock due to acute myocardial infarction (AMI) and VSR were given intra-aortic balloon pump (IABP) and mechanical ventilation support. After three weeks, they underwent transcatheter closure of ventricular septal rupture with Amplatzer ventricular septal occluder, and at the same time PCI and PTCA were performed. Results All the patients successfully underwent ventricular septal defect occlusion, including two cases of residual shunt. Segmental stenosis of the middle of anterior descending branch and circumflex artery were noted in three patients, and then stent implantation was used. The other three cases had multi-vessel lesion but were without stent implantation. All the patients with stent implantation survived and discharged. Conclusions It is safe and feasible to apply transcatheter closure of ventricular septal rupture with Amplatzer ventricular septal occluder, coordinated with PCI and PTCA for patients with acute myocardial infarction complicated with ventricular septal rupture.
2.Significance of serum amyloid A expression in adipose tissues of patients with gestational diabetes mellitus

Objective To investigate the expression of serum amyloid A (SAA) in patients adipose tissue with gestational diabetes mellitus (GDM) and the correlations between SAA and insulin resistance (IR) and body mass index (BMI).Methods A total of 60 single full-term pregnant women underwent cesarean section from June 2013 to December 2013 was enrolled in this study (GDM group,n =30;control group,n =30);serum SAA level was detected with Enzyme-Linked Immunosorbent Assay (ELISA);and mRNA expression of SAA1 in adipose tissue was determined by reverse transcription PCR (RT-PCR);SPSS software was used to compare these markers,and the correlations between SAA and HOMA-IR,BMI were analyzed with Pearson correlation method.Results SAA,mRNA expressions in omental and subcutaneous fat in GDM group (0.447 ± 0.069,0.291 ± 0.067) were significantly higher than those in control group (0.194 ± 0.070,0.231 ± 0.068,P < 0.01).Serum SAA levels [(21.038 ± 6.648) mg/L] and homeostasis model assessment of insulin resistance(HOMA-IR) (4.168± 2.416) in GDM group were significantly higher than those in control group [(14.384 ± 12.770) mg/L,2.045 ± 1.008,P < 0.05];SAA1 mRNA expression levels in omental and subcutaneous fat were positively correlated with serum SAA (r =0.353,0.342,P < 0.01).SAA1 mRNA expression levels in omental were positively correlated to pregestational BMI,late gestational BMI,weight gain in pregnancy and HOMA-IR (r =0.543,0.644,0.340,0.473,P < 0.01),and SAA1 mRNA expression levels in subcutaneous fat were positively correlated to pregestational BMI,late gestational BMI,and HOMA-IR (r =0.788,0.693,0.504,P < 0.01),but was no correlation with weight gain in pregnancy(r =0.013,P > 0.05).Conclusions SAA mRNA expressions in omental and subcutaneous fat in GDM group and serum SAA levels increase,which is positively correlated with BMI and the degree of insulin resistance,SAA may participate in the formation of GDM by increasing insulin resistance.SAA may be used as a new monitor of GDM.

AIM: To determine the mycoplasma infection and the drug resistance in outpatients with NGU. METHODS: Mycoplasma culture, identification and drug sensitivity assay were carried out with samples of 472 NGU patients by using one complex mycoplasma kit. 5.Comparative study between CT features and pathologic changes of inflammatory and tumorous diseases in peritoneum

Objective To investigate the CT manifestations of inflammatory and tumorous diseases in peritoneum, and to improve the abilities of diagnosis and differential diagnosis. Methods Conventional non-enhanced and enhanced CT were performed in 63 patients. The mean value of peritoneal structures in normal persons and patients were measured and calculated. The CT features of abnormal peritoneum were divided into 5 classes that were collated with their pathological changes. The numbers of each CT class appeared in inflammatory and tumorous diseases were countered. The data were treated with statistical methods for comparing the differences.Results (1) In normal persons, the non-enhanced and enhanced mean CT values were (-88.60?18.30) and (-78.73?16.90) HU, respectively, while in patients, they were(-38.63?15.64) and (-1.42?14.42) HU, respectively. CT value of abnormal peritoneum was significantly higher than that of normal group. (2) There were 183 places of abnormal peritoneum in five classes: firstly, fine and blur attenuation in 65 places (56 inflammations and 9 tumors); secondly, linear or strip thickening of peritoneum in 25 places (12 inflammations and 13 tumors); thirdly, attenuation of disorganized strip and piece or nodus in 50 places (8 inflammations and 42 tumors); fourthly, soft tissue mass in 28 places (5 inflammations and 23 tumors); and fifthly, cake peritoneum in 15 places ((2 inflammations) and 13 tumors). OBJECTIVE: To study protection effect of trasylol on cerebral injury resulting from cerebral ischemia-reperfusion in the interference of immunological response.METHODS: 105 SD rats were randomly assigned into sham operation group,model group and trasylol group (n=35).Focal ischemia-reperfusion model were established by occluding middle cerebral artery.24 h before ischemia and at the 0 h,24 h,4 d,6 d,8 d,10 d,12 d and 14 d of reperfusion,trasylol group were given trasylol via tail vein and other two groups were given normal saline.The infiltration of CD4+ and CD8+ T lymphocytes was observed using immunohistochemistry at different stages of cerebral ischemia-reperfusion.RESULTS: As compared with sham operation group,CD4+and CD8+ T lymphocytes count in model group were increased significantly after cerebral ischemia-reperfusion injury and obtained maximum value at 10th day.CD4+ and CD8+ T lymphocytes count in trasylol group were lower than in model group after cerebral ischemia-reperfusion injury and obtained maximum value at 8th day (P

ObjectiveTo summarize X-ray and CT findings of soft tissue and bone infections secondary to acquired immunodeficiency syndrome (AIDS).MethodsThe data of X-ray and CT findings of soft tissue and bone infections in 18 patients with AIDS were retrospectively collected and analyzed.ResultsOf 18 patients with AIDS,the CT features of soft tissue demonstrated that subcutaneous patchy high density in 1 case which considered as cellulitis,round low density lesions with ring enhancement in 6 cases which considered as soft tissue abscesses,heterogeneous density lesions with peripheral enhancement in 1 case which considered as pyomyositis.Of 18 patients with AIDS,septic arthritis was found in 4 cases involving knee lesion in 3 cases and hip lesion.In the 4 case,the X-ray films showed bony destruction in 2 cases and the CT showed bone destruction in 3 cases and arthroedema in 4 cases.Of 18 patients with AIDS,osteomyelitis was found in 9 cases of which tuberculosis was considered in 8 cases and vertebral involvement in 6 cases.In the 9 cases,the X-ray films and CT displayed bony destruction,hyperostosis,small sequestra,and intervertebral space narrowing.Of 18 patients with AIDS,costal lesions were found in 3 cases in which the CT showed expandable bony destruction.Of 18 patients with AIDS,ilium and cacroihac joint lesions were found in 1 case in which the X-ray films and CT showed bony destruction,sequestra,and joint widening.Of 18 patients with AIDS,chronic pyogenic osteomyelitis of femur was found in 1 case in which the X-ray films showed bony destruction,hyperostosis osteosclerosis,and periosteal reaction.Conclusion The X-ray and CT features of soft tissue and bone infections secondary to AIDS are characterized.The X-ray and CT are useful tools to early diagnose soft tissue and bone infections secondary to AIDS.

Objective To study the abdominal CT findings of the penicilliesis marneffei in patients with AIDS.Methods Abdominal CT imaging features of the the penicilliesis mameffei in 35 patients with acquired immunodeficiency syndrome (AIDS) were retrospectively analyzed.Results Abdominal lymph adenopathy was observed in 27 (77.1%) cases.Hepatosplenomegaly was observed in 18 (51.4%) cases.Lesions in the liver and spleen were found in 11 (31.4%) cases which included 3 cases of low density nodules in the liver (8.6%) and 5 cases of low density nodules in the spleen (14.3%).5 cases of diffuse low density in the liver (14.3%) and 5 cases of heterogeneous enhancement in the liver (14.3%) .Thickening of the intestinal wall was observed in 3 cases (8.6%). Ascites was observed in 4 cases (11.4%).Pancreatitis was found in 1 case(2.9%).Conclusions Penicilliosis marneffei in patients with AIDS usually involves multiple organs in the abdomen.Abdominal lymphadenopathy, hepatosplenomegaly and parenchymai lesions in the liver and spleen are the most common CT findings in the abdomen.
9.CT Findings of Pulmonary Tuberculosis in the Patients with HIV Infection and AIDS

Objective To evaluate CT features of pulmonary tuberculosis in the patients with HIV infection and AIDS(HIV/AIDS).Methods CT findings of pulmonary tuberculosis were retrospectively analyzed in 48 patients with HIV/AIDS.Results The CT features of pulmonary tuberculosis in HIV/AIDS included: secondary pulmonary tuberculosis in 19 cases(39.6%), mostly showing patchings shadows,10 cases with large confluent consolidation; hematogenous pulmonary tuberculosis in 17 cases(35.4%), including 7 cases of acute miliary tuberculosis(showing miliary lesions evenly distributed in both lungs), and 10 cases of sub-acute disseminated tuberculosis(showing nodular patterns diffusely distributed in both lungs, asymmetrical in size and distribution); complex lesions in 10 cases(20.8%, showing patchy, nodular and miliary lesions). Among all the patients, 36 (75.0%) showed enlarged mediastinal lymphnodes, 13(27.1%) pleural effussion, 10(20.8%) pericardial effusion.Conclusion The characteristics of pulmonary tuberculosis in the patients with HIV/AIDS were mostly diffuse distribution.
10.Effect of Shenfu injection on Nrf2 signaling pathway in rats with cerebral ischemia reperfusion-induced brain injury

Objective:To study the neuro-protective effects of Shenfu injection on Nrf 2 signaling pathway affected by cerebral ischemia reperfusion.Methods: A total of 68 adult male Sprague-Dawley rats were randomly divided into sham group , cerebral ischemia-reperfusion group,and(8 mg/kg)Shenfu injection treatment group.Shenfu injection was injected intraperitoneally in the rats after MCAO.Neurologic deficit was evaluated after 24 hours of reperfusion.All the rats were sacrificed after 24 h of ischemia-reoerfusion for biochemical analysis or Nissl staining.Results:Shenfu injection treatment significantly increased the expression of Nrf 2,HO-1 and NQO-1(P<0.05).Furthermore,Shenfu injection treatment significantly reduced the expression of cleaved -caspase-3 and attenuated neurological deficits after cerebral ischemia reperfusion ( P<0.05 ).Lastly, Shenfu injection could evidently alleviate the severity of neuronal degeneration ( P<0.05 ).Conclusion:Shenfu injection could confer neuroprotection after cerebral ischemia reperfusion through modulating the Nrf2 signaling pathway.
