1.Comparative study of two patterns of medical rescue for the international disaster relief

Objective To study how to enhance the effectiveness of international disaster relief in order to explore the features and applications of two different patterns of medical rescue.Methods A retrospective analysis of the experiences of the China International Relief Team,including the constitution of the team,working hours on the disaster area,and the numbers of patients rescued per doctor per day,during the 12 occasions of international mission over the past 10 years since the team founded.Results Since the first time for international rescue operation acted in Algeria in 2003,the China International Search and Rescue (CISAR) team has taken part in 12 humanitarian relief operations which can be categorized into two patterns of medical rescue:the first pattern is integrated by search,rescue and initial medical service on site,the second one is exclusively for medical relief (the first pattern consisting of 15.4% medical staff and 61.4% medical staff in the second pattern).According to the former pattern,CISAR team should arrive at the stricken area of earthquake less than 3 days and withdraw after 7 - 10 days of work with about 10.8average working days.Four survivors trapped in collapsed buildings were rescued during the six international rescue operations,and 662 sick and casualties on average were treated each time.The latter pattern often carried out by CISAR team after the work of the former pattern (over 10 days after the disaster) had been done with average working time of 17.8 days.There were 6812 patients on average treated in each time.Comparison between the two patterns showed that the latter one needed more medical staff and longer working days.The data of these two patterns varies significantly. Conclusions The search and rescue pattern is different from the pattern of exclusively medical rescue,and the urban search and rescue team is integrated the search and rescue with medical service,which was proposed by the United States,because of its versatility in the stricken areas of earthquake.It gives lot of useful information to the developing countries for setting up modern relief teams.The time and mode of sending a search and rescue team or a medical team to the stricken area of disaster depend on the assessment of disaster and the requirements of the local government.The capability of the search and rescue team can be extended beyond the ruins,but the capability of the medical team can hardly extend deep enough beneath the ruins,so that the first pattern is much superior to the second one.To set up a local relief team should meet the requirement of the local government to deal with different kinds of disaster.

Objective To investigate the clinical effect of maternal pat with Chinese medicine in episiotomy wound care. Methods One thousand one hundred cases of maternal with full-term,vaginal delivery, episiotomy in the Beijing Tongren Hospital Affiliated to Capital Medical University from Jun. 2013 to Jun. 2014 were randomly divided into test group and control group. Patients in test group were given routine nursing care combined with Chinese medicine maternal pat to nurse perineal wound,while in the control group were given routine nursing combined with common maternal pat. The data of perineal wound swelling,pain and healing were recorded and compared in two groups. The incidence of comfort and satisfaction in test group was 90. 18%,significantly higher than that of control group(73. 09%;Z= -5. 614,-7. 345;P ﹤0. 05). Conclusion Nursing lateral episiotomy wound using Chinese medicine maternal pat can reduce the swelling, pain and promote wound healing with convenient and comfortable. Therefore,it is worth of the clinical popularization and application.
3.Preparation of Paeonol-HP-β-cyclodextrin Inclusion Compound and Its Prescription Technology Optimiza-tion

OBJECTIVE:To prepare paeonol-HP-β-cyclodextrin (PAE-HP-β-CD) inclusion compound and to optimize its pre-scription technology. METHODS:PAE-HP-β-CD was prepared by freeze drying method and validated. Using inclusion rate as in-dex,main drug-accessory ratio,inclusion time,inclusion temperature and stirring speed as factors,the preparation technology was optimized by central composite design-response surface methodology. RESULTS:Prepared PAE-HP-β-CD underwent phase transfor-mation. The optimal inclusion technology was as follows as main drug-accessory ratio of 3.39∶1,inclusion temperature of 50 ℃, inclusion time of 3.2 h, stirring speed of 350 r/min. Relative error between measured value (87.46%) and predicted value (89.12%) of inclusion rate was 1.86%(n=6). CONCLUSIONS:PAE-HP-β-CD inclusion compound is prepared successfully, and its prescription technology is stable and feasible.

Objective To explore the effects of mulitinational united medical transport of emergency and critical patients in Indonesia after earthquake tsunami disaster.Method Various factors,which affected mulitinational united medical transport in Indonesia after earthquake tsunami disaster were analyzed.Results All of the 217 emergency and critical patients were successfully transported to the destination.Conclusion Mulitinational united medical rescue had advantages on integrating medical resources and improving efficiency.During mulitinational united medical transport,the tasks should be distributed rationally,and self-protection of medical workers was the basis of accomplishing medical rescue.
5.Effects of Acarbose Combined with Metformin on Related Indexes of Patients with Type 2 Diabetes

OBJECTIVE:To investigate the effects of acarbose combined with metformin on related indexes of patients with type 2 diabetes mellitus.METHODS:A total of 100 patients with type 2 diabetes mellitus were randomly divided into control group (50 cases) and observation group (50 cases).Control group was given Acrbose tablet 50 mg orally,3 times a day.Observation group was additionally given Metformin hydrochloride tablet 0.5 g orally,3 times a day,on the basis of control group.Both groups were treated for 12 weeks.The levels of fasting plasma glucose (FPG),fasting plasma insulin (FINS),glycosylated hemoglobin (HbA1c),2 h postprandial plasma glucose (2 hPG),postprandial 2 h insulin (2 hFINS),insulin resistance index (HOMA-IR),the proportion of Th cell subsets in CD4+ T cells,IL-22,IL-17A and IFN-γ,mRNA expression of IL-22,IL-17A and IFN-γ were observed in 2 groups before and after treatment.The occurrence of ADR was recorded.RESULTS:After treatment,FPG,FINS,HbA1c,2 hPG,2 hFINS,HOMA-IR,the proportion of Th cell subsets in CD4+ T cells,the concentrations of IL-22,IL-17A and IFN-γ,expression of IL-22 mRNA,IL-17A mRNA and IFN-γ mRNA in 2 groups were significantly lower than before treatment,with statistical significance (P<0.05);there was no significant difference in the proportion of Th1 in T cells between 2 groups (P>0.05).Other indexes of observation group were significantly lower than those of control group,with statistical significance (P<0.05).There was no statistical significance in the incidence of ADR between 2 groups (P>0.05).CONCLUSIONS:For patients with type 2 diabetes mellitus,acarbose combined with metformin can effectively control the level of blood glucose,improve insulin resistance,balance Th cell subsets,reduce inflammatory factors levels but don't increase the occurrence of ADR.

AIM:To determine the biological feature of circulating endothelial cells (CECs) in acute promye-locytic leukemia ( APL) patients before and after treatment , and to analyze the relationship between CECs and the clinical characteristics .METHODS: The CECs were sorted from peripheral blood by magnetic-activated cell sorting and then counted by 3-color flow cytometry.The cells were identified by immunofluorescence staining for the expression of CD 146, CD31, CD144, VEGFR-2, CD45 and CD133.The CECs were cultured in vitro, and the tube formation and colony-forming rate were determined .RESULTS:Increased quantity of CECs was observed in CD 34 positive group and group with WBC >10 ×109/L (P<0.05).The quantity of CECs had a significant difference among low risk , medium risk and high risk groups (P<0.05).The positive rate of CD133 and quantity of CECs significantly reduced in 32 APL patients when they gain complete remission after treatment (P<0.05).The amount of tube formation and colony-forming rate were significant-ly reduced after treatment (P<0.05).The ratio of CECs quantity from APL patients after treatment to that before treatment had a negative correlation with arsenic concentration in urine on day 7 during As2O3 treatment (P<0.05).CONCLU-SION:Accurately counting CECs may be helpful for evaluating prognosis and designing treatment strategy .
7.Value of multislice spiral CT on the effect of conversion therapy for gastric cancer patients with positive exfoliative cytology

Objective:To explore the exfoliative value of multi-slice CT (MSCT) on conversion therapy of gastric cancer patients with positive evaluation cytology (P 0CY 1) . Methods:A total of 36 P 0CY 1 gastric cancer patients receiving conversion therapy in a prospective, single-center, phase Ⅱ clinical trial were enrolled. MSCT examinations were performed before and after conversion therapy. Its solid tumor efficacy evaluation criteria (response evaluation criteria in solid tumors, Recist) 1.1 score and tumor volume reduction rate were evaluated. The Spearman correlation test was used to analyze the correlation between Recist 1.1 score and tumor volume reduction rate and the results of conversion therapy. The ROC curve was used to determine the defined value of the volume reduction rate to identify the effectiveness of conversion therapy, and formulate new grading standards. Results:According to the conversion of free cancer cells in the abdominal cavity , 15 of 36 patients had successful conversion therapy and 21 had failed. The rate of tumor volume reduction in the successful and failed conversion groups was 44.38%±37.86% and -54.96%±156.92%, respectively( P=0.016). The Recist 1.1 score was moderate correlated with the results of conversion therapy ( R=0.540, P=0.001), and the rate of tumor volume reduction was significantly correlated with the results of conversion therapy ( R=0.657, P<0.001). When the tumor volume reduction rate of 26.27% was used as the effective threshold for evaluating conversion therapy, the AUC under the ROC curve was the largest, and the sensitivity and specificity were 80.0% and 85.7%, respectively. Conclusion:Both the MSCT-measured Recist 1.1 score and the tumor volume reduction rate can be used to evaluate the efficacy of conversion therapy in patients with pure exfoliated cytology-positive gastric cancer, and CT tumor volume measurement significantly correlates with conversion therapy results.
8.Evaluation of multislices helieal CT for preoperative treatment effect and prognosis of patients with locally advanced gastric stromal tumor

Objective:To evaluate multislices helical CT (MSCT) on the efficacy and prognosis of preoperative treatment for locally advanced gastric stromal tumors(GIST).Methods:From Oct 2012 to Oct 2014 at the Fourth Hospital of Hebei Medical University 30 patients received MSCT before and after preoperative imatinib treatment to measure the changes of the GIST longest diameter, CT value and tumor volume of the primary lesion. The correlation of Choi score, tumor volume reduction rate and histological efficacy evaluation were analyzed. ROC curve was drew. Kaplan-Meier method was used to draw survival curves, and the overall survival rates under the new classification were calculated.Results:The median time for preoperative treatment was 8 (4 to 14) months. Postoperative pathology showed 4 cases (13%) with mild effects and 3 cases (10%) with low effects. Seventeen cases (57%) with moderate effect and 6 cases (20%) with high effect. Choi score was moderately correlated with histological efficacy evaluation ( R=0.512, P<0.05), and tumor volume reduction rate was strongly correlated with histological efficacy evaluation results ( R=0.620, P<0.05). When the tumor volume reduction rate of 45.83% was used as the effective threshold, the AUC under the ROC curve was the largest, and the sensitivity and specificity were 87.0% and 85.7%, respectively. The 5-year overall survival rate of 30 patients was 87%. According to the new volume grading standard, the 5-year survival rates of the effective group and the ineffective group were 95% and 67% ( P<0.05) , respectively . Conclusion:MSCT measurement of Choi score and tumor volume reduction rate can evaluate the efficacy of preoperative treatment in patients with locally advanced GIST, and tumor volume measurement standards also have certain value in prognosis perdiction.
9.Effects of phentolamine on cardiac function, myocardial injury index, and hemodynamics in septic patients with myocardial injury

Objective:To investigate the efficacy of phenolamine in the treatment of sepsis-induced myocardial dysfunction and its effect on cardiac function, myocardial injury index, and hemodynamics in patients.Methods:The clinical data of 79 patients with sepsis-induced myocardial dysfunction who received treatment in Huangshi Central Hospital, Edong Healthcare Group from February 2017 to February 2020 were retrospectively analyzed. These patients were divided into a control group (without phenolamine treatment, n = 41) and an observation group (with phenolamine treatment, n = 38) according to whether they received phenolamine treatment or not. Clinical efficacy, cardiac function, myocardial injury index, and hemodynamic index pre- and post-treatment were compared between the two groups. Results:There was no significant difference in 28-day mortality rate between the two groups ( P > 0.05). Intensive care unit length of stay and mechanical ventilation duration in the observation group were (9.33 ± 3.52) days and 83.00 (28.50, 138.00) hours, which were significantly shorter than (12.17 ± 4.15) days and 111.00 (47.50, 169.00) hours in the control group ( t = 3.26, Z = -2.27, both P < 0.05). The response rate in the observation group was significantly higher than that in the control group [81.58% (31/38) vs. 60.98% (25/41), χ2 = 4.05, P < 0.05]. After 7 days of treatment, the left ventricular ejection fraction in each group was significantly increased, and the left ventricular end-diastolic diameter and left ventricular end-systolic diameter in each group were significantly decreased compared with before treatment (all P < 0.05). After 7 days of treatment, the left ventricular ejection fraction in the observation group was significantly higher than that in the control group ( t = 3.29, P < 0.05), and left ventricular end-diastolic diameter and left ventricular end-systolic diameter were significantly lower than those in the control group ( t = 5.94, 11.21, both P < 0.05). N-terminal pro-brain natriuretic peptide and cardiac troponin I levels in each group were significantly decreased with time (both P < 0.05). At 24 and 72 hours and 7 days after treatment, N-terminal pro-brain natriuretic peptide and cardiac troponin I levels in the observation group were significantly lower than those in the control group (both P < 0.05). After 7 days of treatment, heart rate in each group decreased significantly compared with that before treatment (both P < 0.05), mean arterial pressure, cardiac index, and stroke output index in each group increased significantly compared with those before treatment (all P < 0.05). After 7 days of treatment, heart rate in the observation group was significantly lower than that in the control group ( t = 4.90, P < 0.05), and mean arterial pressure, cardiac index, and stroke output index in the observation group were significantly higher than those in the control group ( t = 4.37, 3.23, 6.01, all P < 0.05). Conclusion:Phentolamine can improve hemodynamics, reduce myocardial injury and improve cardiac function in patients with sepsis-induced myocardial dysfunction.
