1.Two Laboratory Detections of Epstein-Barr Virus:A Comparative Analysis

OBJECTIVE To compare the effectiveness of two laboratory detections methods of Epstein-Barr virns (EBV). METHODS Immuno-fluorescence technique was applied to detect the serum EBV-specific antibody and EBV-DNA was identified by fluorescence quantitative polymerase chain reaction (FQ-PCR) in 124 children infected with EBV. RESULTS The positive rates of VCA-IgM,VCA-IgG,EA-IgG,EBNA-1-IgG and EB-DNA in various samples of 124 cases were 0,89.5%,23.4%,49.2% and 30.3%,respectively,with 21 cases (16.9%) showing simultaneously positive in EB-DNA and EBNA-1-IgG. CONCLUSIONS The two laboratory detections of EBV share their advantages and disadvantages. FQ-PCR is a method for time-saving,accurate and sensitive detection of EBV-DNA,showing clinical significance in the diagnosis of EBV-associated diseases. It's worthy of clinic application.
2.Clinical application of continuous rapid tissue expansion

Objective To study a simple surgical method for continuous rapid tissue expansion and its clinical application. Methods The operation was accomplished in two-stage procedures. First, expanders were placed in the subcutaneous pocket through interlesional perpendicular short incisions. Continuous expansion by trace syringe pump began from the second day after expander placement. The speed of water injection was set according to patient's algesthesia reaction and vascularity of skin. The second operation was carried out at the third to fifth day after the expansion completed.Results The method was put into clinical practice in 22 cases (16 cases of scar, 2 cases of capillary angioma, and 4 cases of black nevus), and 35 expanders were placed. The speed of water injection was set in 0.2~0.8ml/hr/100ml.The mean time of water injection was 15.3 days. In the process of expansion, all cases were satisfactory except 1 case, in which the expander exposed in the incision. After the second operation, all flaps survived except one showing distal skin necrosis. The result was satisfactory after 3 months to 1 year follow-up. Conclusion The procedure consists of expander placement through interlesional perpendicular short incisions and continuous expansion in advance, which maximally shortens the time of expansion and hospitalization. This expansion method is a simple, safe, reliable, and ideal rapid tissue expansion technique.
3.Expression of Ang-1, Ang-2 and Tie-2 receptor mRNAs and their implications in hemangioma by RT-PCR

Objective To detect the expression level of Ang-1, Ang -2 and Tie-2 receptor mRNAs in the hemangioma tissuses and to explore the effects of Ang-1, Ang-2, and Tie-2 receptor on the angio-genesis in the hemangioma. Methods By use of semiquantinative reverse transcription polymerase chain reaction, we detected the expression level of Ang-1 mRNA, Ang-2 mRNA and Tie-2 mRNA in 30 cases of hemangioma (17 specimens of proliferating hemangioma, and 13 involuting hemangioma )together with 10 cases of normal skin as control, and the expression level of Ki-67 protein was also detected by an im-munohistochemica method. Results The expression of Ang-2 mRNA and Tie-2 mRNA in the prolifera-ting hemangiomas was higher than that in involuting hemangiomas (P<0.01) . The expression of Ang-1 mRNA in hemangiomas was negative. Conclusion The unbalance of Ang-Tie-2 system is probably impor-tant for the pathogenesis and development in hemangiomas.
4.Biological evaluation within a risk management process.

Bio-evaluation within the medical device quality/risk management system is a risk analyzing and assessing process. On the basis of data from characterization of materials, scientific literatures, application history, bio-toxicology testing and so on, weighing the benefit and the risk, bio-evaluation does a conclusion to "take" or "quit" the product design. There is no "zero risk" though "no toxicity" always is the most desirable conclusion in a testing report. The application history data is the most comprehensive among the information available, since no testing system can "clone" the human body. In addition, the capital cost has to be taken into account when bringing the sophisticated testing technologies into the evaluating system. Investigating the #G95-1 of FDA CDRH and the changes of ISO 10993-1, the trend to integrate bio-evaluation into a quality/risk management process can be figured out.

5.Effect of astragalus polysaccharides on the cellular immune function in rats with sciatic nerve Wallerian degeneration

Objective:To explore the effect of astragalus polysaccharides on the cellular immune functions in rats with sciatic nerve Wallerian degeneration.Methods:10 female Wister rats were established as the sciatic nerve injury model,which were randomly divided into astragalus polysaccharides group and control group.Then 20 mg/kg of astragalus polysaccharides were peritoneal injected every postoperative day in experimental group for 7 days and the same volume of saline for the control group.The content of IL-1? in serum and in supernatants of spleenocytes and macrophages was measured by Sandwich ELISA.Results:The proliferation ability of splenic T cells and macrophages in astragalus polysaccharides group was higher than that of control group(P0.05).Conclusion:Astragalus polysaccharides induces the cellular immuno-regulation in sciatic nerve Wallerian degeneration rats and by this way to promote nerve regeneration.
6.The application of medpor and split-thickness skin graft in formation of cranioauricular sulcus during auricular reconstruction with Nagata method.

OBJECTIVETo investigate the application of medpor and split-thickness skin graft in formation of cranioauricular sulcus during auricular reconstruction with Nagata method.
METHODSThe first stage operation was fulfilled according to the Nagata two-stage method which involves fabrication and grafting of the costal cartilage framework. The second-stage ear elevation operation was undertaken 6 months later to form the cranioauricular sulcus. Split-thickness skin was taken from temporal and accipital area. After releasing the auricular framework and transplanting C shaped medpor at the rear side of framework, the temporaparietal fascia flap was transferred to cover postauricular medpor and framework. Then the split-thickness skin graft was implant on the fascia surface.
RESULTSFrom July 2010 to August 2012, 20 cases (22 ears) were treated. Partial necrosis of temporaparietal fascia flap and framework exposure happened in 1 case. Successful ear reconstruction was achieved in other cases with satisfactory cranioauricular sulcus during the follow-up period of 6-18 months (average, 13 months).
CONCLUSIONSThe application of medpor and split-thickness skin graft in the ear elevation of Nagata method for auricular reconstruction for microtia can achieve satisfactory results. It not only avoids the obvious scar in the donor site due to harvesting full-thickness and intermediate-thickness skin, but also reduces chest trauma due to harvesting costal cartilage.

7.Erlotinib versus vinorelbine therapy in chemotherapy-naive elderly patients with advanced non-small-cell lung cancer and EGFR mutation: a randomized, controlled trial

Objective To compare the efficacy and adverse effects of erlotinib versus vinorelbine naive patients with advanced non-small cell lung cancer (NSCLC) and epidermal growth factor receptor (EGFR) mutation.Methods Totally 46 elderly patients with histologically confirmed advanced NSCLC and EGFR mutations (exon 19 dclction or L858R point mutation) were enrolled.Patients were randomly divided into two groups:erlotinib group (43 cases,150mg per day until disease progression or unacceptable toxicities) and control group (21 cases,vinorelbine-based chemotherapy,single vinorelbine chemotherapy or vinorelbine-based double chemotherapy).Results Response rates and disease control rates were significantly improved with erlotinib compared with vinorelbine (78.6% and 88.1% vs.38.1% and 61.9%,respectively,P< 0.05).There was a significant difference in median progression-free survival (11.6 months vs.5.6 months,P<0.05),while no statistical difference in median overall survival with erlotinib compared with vinorelbine (19.0months vs.16.5 months,P=0.193).The most frequent adverse effects were grade Ⅰ or Ⅱ and no patients stopped treatment due to adverse effects and no drug-relatcd death.The primary adverse effects were skin rash (71.4%),diarrhea (31.0%)and liver dysfunction (23.8%) in the erlotinib group and neutropenia (66.7%),nausea or vomit (47.6%),anemia (42.9%),platelet decline (33.3%),constipation (33.3%) and peripheral neuritis (23.8%) in the vinorelbine group.Vinorelbine group versus erlotinib group have more 3-4 level adverse reactions (15/21 vs.7/42)(x2=1.69,P=0.193).Conclusions Erlotinib treatment has advances in PFS,ORR and DCR and tolerability compared with vinorelbine-based chemotherapy in elderly patients with advanced NSCLC and EGFR mutation,while overall survival is in no difference.Erlotinib may be a reasonable first-line treatment option for elderly patients with advanced NSCLC and sensitive EGFR mutation.
8.Gefitinib in first-fine therapy for 34 cases of advanced non-small cell lung cancer

Background and purpose: It has been proven that gefitinib can be safely and efficiently used to treat advanced non-small cell lung cancer (NSCLC) as a molecule targeted drug. This research was aimed to investigate the efficacy and toxicity of gefitinib as the first-line therapy for advanced NSCLC. Methods: A total of 34 pathologically-confirmed NSCLC patients who were not willing to receive or tolerate traditional cytotoxic drug chemotherapy were enrolled into the study. Gefitinib was orally administered 250 mg daily until disease progression or the occurrence of intolerable toxicity. Results: The objective response rate of gefitinib was 29.4%. The disease control rate was 61.8%. The rate of symptom relief was 47.1%. The median progression-free survival was 3.0 months. The median overall survival was 10.2 months. One-year survival rate was 35.3%. The objective response rate of nun-smoker was higher than smoker (P=0.023). The disease control rate for the patients with rash toxicity after administration of the drug were higher than those without rash (P=0.005). Logistic regression showed that rash was an independent disease control factor (P=0.003). The most common drug-related adverse events were rash and diarrhea. Conclusion: Gefitinib provided another choice to patients who are unwilling or unable to be treated by chemotherapy.
