2.Analysis of the monitor results of Marmota sibirica plague foci from 2004 to 2008

Objective To master of the characteristics of development and variation in plague natural foci of Marmota sibirica, and to analyze the plague monitoring results from 2004 to 2008. Methods From May to September every year, we monitored plague in Manzhouli, Chenbaerhuqi, Xinbaerhuzuoqi, Xinbaerhuyouqi,Ewenkeqi and Yakeshi city. The monitored area was 20 000 - 40 000 hm2 in every county. The density of Marmota,ground squirrel and moonlighting rats was surveyed by path method, one-day bow-clip method and a clamp was placed every five meters, respectively. The classification and identification of ectoparasite fleas were done by using low power lens. The fleas in the ground squirrel hole were collected by flannel rubber stick, then classified and identified. According to National Standard of Plague Diagnostic Criteria(GB 15991-1995), the collected rats and fleas were detected by isolation and cultured Yersinia pestis, the serums of collected rats were tested by indirect hemagglutination test. Results In five years, the density of Marmota and ground squirrel was 0.010/hm2 and 0.602/hm2, respectively. The capture rate of moonlighting rats was 2.69% (258/9600). The flea infection rate of Marmota was 17.54%(10/57) and the fleas index was 2.54. The fleas infection rate of ground squirrel was 28.40%(213/750) and the fleas index was 1.01. Flea infection rat of ground squirrel hole was 5.60%(46/822) and the fleas index was 0.17. The total number of various hosts of Yersinia pestis detected was 1351, the groups number of the variety of cultured fleas was 127, and the pathogen test results were negative. The number of serum tested was 1064, positive number was 43, and the detection rate was 4.04%(43/1064). The highest positive titer was 1 : 1280. Other than 2006, the remaining four years were found positive for blood clotting material; positive serum was found in a total of three regions, they were Manzhouli, Xinbaerhuzuoqi, and Xinbaerhuyouqi; and 30, 12 and 1 copies of hemagglutination-positive sera were detected. Conclusions The epidemic of plague natural foci of Marmota sibirica is in a active state, and gradually expands the scope. We must continue to strengthen the inspection of the bacteria, bearing in mind the replacement of the region's main host, make every effort to prevent and control of human plague.
3.X-ray analysis of 30 cases of pulmonary tuberculosis in plateau section

Objective To improve the understanding of tuberculosis X-ray performance at Linzhi region in the east of Tibet,in order to better guide clinical early diagnosis.Methods The X -ray image characteristics of 30 patients with pulmonary tuberculosis from Linzhi region in the east of Tibet were retrospectively analyed,Original they all took chest plain films at initial diagnosis.Results The original hairstyle tuberculosis was not found;Blood type disseminated tuberculosis in 5 cases (16%).Composition of mixed density shadow image performance,calcification and satellite stove were founded;Invasion tuberculosis were in 8 cases (26%),images showed two pulmonary diffuse distribution of nodular shadows;Hollow fiber tuberculosis were in 17 cases (58%),images showed empty,fibrosis, endobronchial spread of three characteristics,and lesions was wide,especially with effusion infiltration priority,which were distributed in the lung base and at the front.Conclusion Plateau pulmonary tuberculosis with invasion and fiber cavity is given priority to,and have a specific image characteristics,it can be used for diagnosis.And its X -ray per-formance are significantly different from that of mainland cities.
4.Experimental hematology bridging the gap between laboratory and clinic: hope of hematology.

This article summarizes the progress of hematology in the recent tens years to show that experimental hematology used to pick up the 'hints' from clinical problems as the renewal of research directions and targets in experimental studies continuously. As the feedback, the results from lab investigations inserted into clinical practice and eventually made a quick modernization of hematology, which was actually a good model for the "translational research". The past few decades have witnessed tremendous advances in our understanding of normal hematopoiesis where genes dictate, epigenetics regulate, transcription factors mediate, and stem cells self-renew and differentiate. Dissection of disease pathogenesis not only elucidates molecular basis of disorders including hemoglobinopathy, aplastic anemia, hemophilia, hematopoietic malignancies such as leukemia and myeloproliferative disorders, but also provides therapeutic targets for drug development. Introduction of targeted therapies and combinatory targeting therapies greatly benefits hundreds of thousands of patients, and even turns acute promyelocytic leukemia from highly fatal to highly curable. In the 21st century the experimental hematology is entering the era of genomics and system biomedicine, and the pace of progress extrapolates to a prediction of hematologic neoplasms control in this century.

5.Basic and clinical studies of the gene product-targeting therapy based on leukemogenesis--editorial.

In the last twenty years, using all-trans retinoic acid (ATRA) as a differentiation inducer, Shanghai Institute of Hematology has achieved an important breakthrough in the treatment of acute promyelocytic leukemia (APL), which realized the theory of reversing phenotype of cells and provided a successful model of differentiation therapy in cancers. Our group first discovered in the world the variant chromosome translocation t(11;17)(q23;q21) of APL, and cloned the PML-RAR alpha, PLZF-RAR alpha and NPM-RAR alpha fusion genes corresponding to the characterized chromosome translocations t(15;17); t(11;17) and t(5;17) in APL. Moreover, establishment of transgenic mice model of APL proved their effects on leukemogenesis. The ability of ATRA to modify the recruitment of nuclear receptor co-repressor with PML-RAR alpha but not PLZF-RAR alpha caused by the variant chromosome translocation elucidated the therapeutic mechanism of ATRA from the molecular level and provides new insight into transcription-modulating therapy. Since 1994, our group has successfully applied arsenic trioxide (As(2)O(3)) in treating relapsed APL patients, with the complete remission rate of 70% - 80%. The molecular mechanism study revealed that As(2)O(3) exerts a dose-dependent dual effect on APL. Low-dose As(2)O(3) induced partial differentiation of APL cells, while the higher dose induced apoptosis. As(2)O(3) binds ubiquitin like SUMO-1 through the lysine 160 of PML, resulting in the degradation of PML-RAR alpha. Taken together, ATRA and As(2)O(3) target the transcription factor PML-RAR alpha, the former by retinoic acid receptor and the latter by PML sumolization, both induce PML-RAR alpha degradation and APL cells differentiation and apoptosis. Because of the different acting pathways, ATRA and As(2)O(3) have no cross-resistance and can be used as combination therapy. Clinical trial in newly diagnosed APL patients showed that ATRA/As(2)O(3) in combination yields a longer disease-free survival time. With the median survival of 18 months, none of the 20 cases in combination treatment relapsed, whereas 7 relapsed in 37 cases in mono-treatment. This is the best clinical effect achieved in treating adult acute leukemia to this day, possibly making APL the first adult curable leukemia. Based on the great success of the pathogenetic gene target therapy in APL, this strategy may extend to other leukemias. Combination of Gleevec and arsenic agents in treating chronic myeloid leukemia has already make a figure both in clinical and laboratory research, aiming at counteracting the abnormal tyrosine kinase activity of ABL and the degradating BCR-ABL fusion protein. In acute myeloid leukemia M(2b), using new target therapy degradating AML1-ETO fusion protein and reducing the abnormal tyrosine kinase activity of c-kit will also lead to new therapeutic management in acute leukemias.

6.Anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung:characteristic findings on chest radiography and spiral CT

Objective To summarize the characteristic findings on chest radiography and spiral CT of anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung.Methods Five patients with anomalous systemic arterial supply to normal basal segments of the left lower lobe of the lung were retrospectively reviewed.Chest radiography,and contrast-enhanced spiral CT scan were analyzed. Results On posteroanterior chest radiographs,the findings in 5 cases of this anomaly were all shown as a retrocardiac mass,narrowing lower lobar pulmonary artery shadow,and without normal lower lobar pulmonary artery branches but there were dilated abnormal markings in left lower lung zone.On contrast- enhanced spiral CT scans,the involved left lower lobe of the lung had mild volume loss but normal bronchial trees in all 5 cases and areas of ground-glass opacity in 2 cases.The absence of normal left lower lobar pulmonary artery distal to the origin of superior segmental artery,anomalous systemic artery originating from the descending aorta,diffuse dilatation of the systemic arterial branches distributed in the basal segments of the left lower lobe were also found in all the patients.One patient who underwent angiography had similar vessel features with spiral CT.Conclusion There are some characteristic findings on chest radiography in this anomaly.the contrast-enhanced spiral CT scans are important in diagnosing and avoiding invasive angiography and life-threatening lung biopsy.
7.Operative strategy of congenital atlantoaxial dislocation-induced Chiari malformation and (or)syringomyelia

Objective To study the operative strategy of congenital atlantoaxial dislocation(CAAD)-induced Chiari malformation and (or) syringomyelia. Methods The operation in reported 23cases of CAAD-induced Chiari malformation and (or) syringomyelia was composed with the transoral resection of odontoid process to achieve anterior decompression at first stage and occipito-cervical bone grafting fusion at second stage. Results MRI examination revealed the tonsils ascent and (or) syrinx reduction in 19 cases after first-stage operation. Compared with their preoperative manifestations, 14cases were obviously improved and 5 improved to some extent after operation, while 4 were unchanged. Conclusions CAAD is the main cause of tonsils descent and (or) syringomyelia in the series of patients. After anterior decompression by transoral resection of odontoid process, most patients will get recovered in tonsils descent and (or) syringomyelia reduction. The main aim of posterior operation is to reconstruct the stability of craniovertebral junction.
8.Analysis of clinical effects of iliolumbar fixation in treating sacrum fracture of Denis type II.

OBJECTIVETo evaluate the clinical effects of iliolumbar fixation for the sacrum fractures of Denis type II.
METHODSThe clinical data of 86 patients with sacrum fracture of Denis type II treated by iliolumbar fixation from January 2008 to January 2012 were retrospectively analyzed. There were 55 males and 31 females, aged from 17 to 55 years old with an average of 39.1 years. Among them, 73 cases complicated with pelvis fracture and 13 cases with acetabular fracture; 37 cases with sacral neurological symptoms and 49 cases without sacral neurological symptoms. Fracture healing time, nerve function, clinical function and complications were observed in the patients.
RESULTSIn 86 cases, 6 cases were out of followed-up and 80 cases were followed up from 24 to 71 months with an average of 36 months. The mean fracture healing time was 13 weeks (ranged, 10 to 38 weeks). According to Gibbons scoring to evaluate the neurological function, preoperative nerve rehabilitation, lower limbs feeling, lower limbs activity,bladder and rectum function,total score respectively were 0.62 +/- 0.04, 1.54 +/- 0.35, 1.12 +/- 0.18, 0.23 +/- 0.01, 3.46 +/- 0.47 and postoperative respectively were 0.82 +/- 0.12, 0.36 +/- 0.04, 0.05 +/- 0.01, 0.03 +/- 0.01, 1.25 +/- 0.22, there were statistically significant differences between preoperative and postoperative (P < 0.05). According to Majeed scoring to evaluate the clinical function, postoperative pain, standing, sitting, sexual life, work ability, total score respectively were 22.54 +/- 4.02, 27.93 +/- 5.46, 8.47 +/- 3.61, 2.54 +/- 1.33, 16.46 +/- 4.34, 81.32 +/- 8.73, 60 cases got excellent results, 17 good, 3 fair. The main complications including fracture nonunion of 5 cases,deep incision infection of 1 case, and screw prominence resulting uncomfortable of 8 cases.
CONCLUSIONIliolumbar fixation has the advantages of stable fixation, satisfactory functional rehabilitation, less complications, and is a good method in treating sacrum fracture of Denis type II.

9.Design of BP neural network based on multi-parametes for VF detection

Objective To develop a BP neural network to differentiate between ventricular fibrillation( VF) and non-VF rhythms.Methods Eighteen metrics were extracted from the ECG signals.Each of these metrics respectively characterized each aspect of the signals, such as morphology, gaussianity, spectra, variability, and complexity.These metrics were regarded as the input vector of the BP neural network.After training, a classifier used for VF and non-VF rhythm classification was obtained.Results and Conclusion The constructed BP neural network was tested with the databases of VFDB and CUDB, and the accuracy was 98.61%and 95.37%, respectively.
10.Three-Column classification for fracture of tibial plateau and the related reliability assessment

Objective To investigate the reliability of the Three-Column classification for fracture of tibial plateau and compared with the conventional Schatzker classification.Methods From December 2004 to March 2007,304 patients with tibial plateau fracture were treated in our department.The CT scans and 3D reconstruction were performed in all patients before operation.All the patients were classified by the Three-Column classification and then treated with open reduction and internal fixation via the corresponding approaches based on the Three-Column classification.To validate the Three-Column classification,four doctors classified 50 patients which were chosen randomly from 323 consecutive fracture cases.Schatzker classification was also classified at the same time.Inter-observer reliabilities of the Three-Column classification and the Schatzker classification were figured out respectively in the form of Kappa value.Results According to the Three-Column classification,there were 4 pure compression fractures (1.2%),181 one-column fractures(56.0%),108 two-column fractures(33.4%) and 30 three-column fractures (9.3%) in all 323 cases.In one-column fracture cases,the numbers of lateral,medial,and posterior column fractures were 114 (63.0%),53 (29.3%),and 14 (7.7%),respectively.Among two-column fracture cases,the lateral-medial column fractures (33.3%) were 36.While the number of medial-posterior column fractures and lateral-posterior column fractures were 34 (31.5%) and 38 (35.2%),respectively.There was a moderate inter-observer reliability when using Schatzker classification (Kappa=0.567; range,0.513-0.589).There was a substantial interobserver reliability when using the Three-Column classification (Kappa=0.766; range,0.706-0.890).Conclusion The Three-Column classification based on CT and 3D reconstruction is simple and more reliable for fracture of tibial plateau.It is suggested that the new classification system,the Three-Column classification,can be used and popularized in the orthopaedic surgery.
