OBJECTIVETo observe the effect of anisodamine (Ani) injection on the survival rate and histologic result of flaps with ischemia-reperfusion injury, so as to demonstrate the protective effect of Ani on the flap survival.

METHODSA total of 48 healthy male Wistar rats were randomly divided into model control, normal saline(NS) and anisodamine groups, with 16 rats in each group. An 3 cm x 6 cm axial flap was formed at the right lower abdomen with abdominal superficial blood vessel as the pedicle. 0.5 cm x 0.5 cm skin tissue was taken from the middle part of flaps in each group, immediately after operation, 12, 18, 24 h after operation. The superoxide dismutase (SOD), nitric oxide (NO), nuclear factor-kappaB contents in the specimens were detected. The histologic study was also performed. The flap survival rate was recorded 7 days after operation.

RESULTSFlap survival rate was (78.6 +/- 7.3) % in Anisodamine group. 12, 18, 24 h after reperfusion injury, the SOD was (103.3 +/- 3.9), (82.6 +/- 3.8), (67.5 +/- 4.6) U/mg; the NO was (5.33 +/- 2.05), (4.75 +/- 1.68), (4.15 +/-1.59) nmol/mg; the NF-kappaB was 0.211 +/- 0.039, 0.313 +/- 0.033, 0.096 +/- 0.028. The contents of SOD, NO and NF-kappaB had the statistical difference of at different time. The skin pathological changes in Anisodamine group was obviously better than those in NS group. Flap survival rate in Anisodamine group was significantly higher than that in NS group.

CONCLUSIONSIn the flap with ischemia-reperfusion injury, Anisodamine can reduce the damage of free radical, increase the blood flow, reduce the production of NF-KB, decrease inflammatory reaction. So Anisodamine can increase the survival rate of flaps with ischemia reperfusion injury.

Objective To evaluate the diagnostic value of hysterosalpingography in infertility caused by salpingian obstraction.Methods 892 cases of femiles infertility were reported in the group,ranged in age from 20 to 36 years and the mean age was 26 years.The hysterosalpingography was taken under fluoroscopic guidance during third to 7th day after menstruction,the radiographes were taken and analysed.The different segments and shapes of salpingian obstruction were correspondently divided into 3 levels and 3 degrees respectively according to the different therapeatic methods for infertility.Results There were total 776 cases with salpingian obstructional infertility,of them,both side involved in 582 cases,and one side in 194 cases,the total of abnormal fallopian were 1358 tubes including 1 st level 423,2nd level 561;3rd level 374,1 st degree 263,2nd degree 553 and 3rd degree 542.Conclusion Hysterosalpingography plays an important role in the therapy and diagnosis of the inferility caused by salpingian obstraction.

BACKGROUND: As a convenient and useful physical activity measurement tool,pedometer is widely used in research and intervention about physical activity of population. OBJECTIVE: To assess validity and reliability of the pedometer in measuring daily physical activity. DESIGN,TIME AND SETTING: The experiment was performed at the Department of Kinesiology,Nanjing Institute of Physical Education and Sports from April to June 2007. PARTICIPANTS: Thirty college students,15 majoring in Kinesiology and 15 majoring in social and physical education,15 females and 15 males,with an average age of (20?1) years,were selected from Nanjing Institute of Physical Education and Sports. METHODS: Participants wore a Digi-Walker200 for one week to record results of pedometer measurement,and filled in Bouchard Physical Activity Diary every day to note activity in 24 hours and calculate energy consumption. All participants completed International Physical Activity Questionnaire including large and moderate intensity activity,walking and sitting in a week. MAIN OUTCOME MEASURES: Daily steps,energy expenditure,sedentary time,waking time,exercise time,and weekly physical activity condition. RESULTS: Step counts were positively correlated with daily energy expenditure,walking time,and exercise time,and negatively related with sedentary time. Step counts of different physical activity levels were significantly different: more step counts could better reflect physical activity levels. Furthermore,exercise or continuous walking (more than 15 minutes) could significantly increase step counts. Daily physical activity including 30-minute moderate intensity activity could achieve or exceed the target steps of 10 000. CONCLUSION: Pedometer provides a useful and objective indicator of daily step counts assessing daily physical activity,but it is difficult to reflect activity intensity and patterns.

Background Clinical effectiveness of intavitreal injection of glucocorticosteroid for macular edema has been verified,especially triamcinolone acetonide(TA).However,the efficacy and safety of combination of TA with macular laser grid photocoagulation for macular edema is concerned. Objective This clinical trial was to evaluate the efficacy and safety of intravitreal injection of TA combined with macular laser grid photocoagulation in the treatment of macular edema. Methods A case-cohort study was designed.One hundred and twenty eyes of 120 patients with macular edema from diabetes or retinal vein occlusion were included in this study.The patients were randomized into trial group and control group,with the matched age,course,visual acuity,intraocular pressure (IOP).The patients of the trial group received intravitreal injection of TA combined with macular laser grid photocoagulation,and those of the control group were managed with macular laser grid photocoagulation only.Best corrected visual acuity ( BCVA),optical coherence tomography(OCT),fundus fluorescein angiography(FFA) and IOP were examined before TA injection and 1 week,1 month,3 months,6 months after treatment and compared among different time points between two groups.Written informed consent was obtained from each patient prior to entering this trial. Results Compared with TA injection before,the BCVA was significantly elevated in the trial group 1 week,1 month,3 months and 6 months after TA injection( all P=0.000),however,no obvious improvement of visual acuity was found in the control group before and after treatment at any time point (P＞0.05 ).At various time points,the visual acuity was significantly improved in the trial group than the control group (P =0.037,0.000,0.002,0.046 ).Macular thickness was significantly decreased at various time points after TA injection in comparison with before TA injection in the trial group(all P=0.000),but no significant change in macular thickness in the control group between before and after treatment at any time point( P＞0.05 ).Macular thickness was lower in the trial group compared with the control group at various time points after treatment ( P＜0.05 ).Recurrence of macular edema was seen in 7 eyes ( 1 1.67％ ) 4-6 months,and the IOP raise( ＞21 mmHg)was found in 11 eyes( 14.1％ )after TA injection in the trial group.Conclusions Intravitreal injection of TA combined with macular laser grid photocoagulation can be an effective method in the treatment of macular edema.However,recurrence of macular edema or increase of IOP may occur in a few patients within 6 months after TA injection.A long-term follow-up should be performed for the evaluation of efficacy and safety after intravitreous injection of TA.

OBJECTIVE: To study the influence of stress-relaxation plate on disorganization and repair of the cortex beneath the plate. METHODS: A washer made of viscoelastic polyethylene was placed between the screw and the screw hole of conventional stainless rigid plate (RP) to produce a stress-relaxation plate (SRP). Both SRP and RP were applied to osteotomized tibia in 48 New Zealand rabbits. Healing process of the fracture with either SRP or RP fixation (control) was comparatively studied with polarized light microscopy, in situ hybridization of collagen mRNA and immunohistochemical technique from 2 to 36 weeks postoperatively. RESULTS: The study of plated bone remodeling showed that the degree of cortex osteoporosis beneath the plate was similar between the SRP and RP group within 12 weeks postoperatively. In comparison, the disorganization of bone structure in SRP group happened later and milder than that of RP group, and the repair process began at 12 weeks after implantation. As a consequence, the absorption cavities became smaller and the structure of collagen fibers became well oriented along with these changes by polarized light microscopy. In addition to these, the in situ hybridization analysis of collagen genes and the immunohistochemical study of type I, III collagen showed that the osteoblasts lying on the surface of absorption cavities expressed and synthesized type I collagen at 8 to 12 weeks after implantation. From this time on, the changes above became more evident significantly before most of cavities were repaired by 36 weeks. In contrast to the changes in the SRP group, no expression and synthesis of any kind of collagen could be observed during 12 to 36 weeks after implantation in RP group. CONCLUSIONS: Without removal of the bone plate, the SRP fixation not only reduces the degree of plated bone osteoporosis, but also makes the disorganized bone structure restored to normal in terms of the expression and synthesis of type I collagen mRNA of osteoblasts lying on the surface of absorption cavities.

Objective To explore the correlation among the concentration of peripheral blood histone deacetylase 2 (HDAC2),C -reactive protein (CRP),proealeitonin (PCT)and lung function among patients with acute exacerbation period of chronic obstructive pulmonary disease (AECOPD).Methods A total of 60 patients with AECOPD were selected as the test group,and 40 healthy subjects in the same medical examination center were selected as the control group.The HDAC2,CRP,PCT,lung function testing and blood gas analysis were measured on Day 0,3,7,1 0.Results Along with the process of AECOPD patients,in the treatment of 0 d,3 d,7 d,1 0 d,the average level of CRP and PCT in peripheral blood of patients was decreased gradually,while the average level of HDAC2 was gradually increased (P<0. 05 ).In patients treated with 0 d,3 d,7 d,1 0 d,HDAC2 levels were negatively correlated with CRP and PCT at the same time (rCRP was-0. 31 6,-0. 435,-0. 495,-0. 547,respectively,and rPCT was -0. 41 4,-0. 332,-0. 481 ,-0. 523,respectively, P<0. 01 ).The level of HDAC2 was positively correlated with lung function in the same period (rFEV1 was 0. 594,and rFEV1/Expected value was 0. 561 ,P<0. 01 ).Conclusion HDAC2 may be a negative regulator of the inflammatory response among the patients with AECOPD,and is associated with the degree of airflow obstruction of the patients.

ObjectiveTo investigate whether fatty liver disease is a risk factor for lacunar infarction. MethodsA retrospective analysis was performed for the clinical data of 457 patients with lacunar infarction (lacunar infarction group) and 120 control patients, who were hospitalized in our hospital from 2007 to 2017, to analyze whether fatty liver disease is a risk factor for lacunar infarction. The chi-square test was used for comparison of categorical data between groups, and the t-test was used for comparison of continuous data between groups. Univariate and multivariate regression analyses were used to screen out the risk factors for lacunar infarction. ResultsThe lacunar infarction group had a significantly higher incidence rate of fatty liver disease than the control group (60.39% vs 39.17%, χ2=17.379, P＜0.001). The multivariate regression analysis showed that after adjustment for age, sex, smoking, drinking, hypertension, and diabetes, fatty liver disease was an independent risk factor for lacunar infarction (odds ratio ［OR］=1.96, 95% confidence interval ［CI］: 1.21-3.02, P=0.003). There was a significant interaction between obesity and fatty liver disease (P=0.001). Non-obese fatty liver disease was an independent risk factor for lacunar infarction (OR=3.29, 95% CI: 1.55-7.23, P＜0.001); however, in the obese subgroup, obese fatty liver disease was not an independent risk factor for lacunar infarction(P=0532)，Age(OR=6.67,95%CI:1.98~121.56,P＜0001), hypertension (OR=638,95%CI:512~12.06,P＜0001) were independent risk factors for lacunar infarction. ConclusionNon-obese fatty liver disease is an independent risk factor for lacunar infarction.

Objective To explore the association of ATP-binding cassette (ABC) efflux pump gene Rv1217c-Rv1218c and the drug resistance of Mycobacterium tuberculosis.Methods A total of 34 Mycobacterium tuberculosis clinical isolates including 24 drug-resistance isolates which were resistant to riffampicin,isoniazid,streptomycin or ethambutol and resistant to at least one second-line antituberculosis drug,and 10 drug-sensitive isolates were involved in this study.The RNA of isolated strains was extracted and then reverse transcribed. Gene expression was performed by real-time polymerase chain reaction (PCR) and data was analyzed by t test and Logistic regression analysis.Results The expressions of Rv1217c in rifampicin-resistant group (2.13 ± 1.89,t =3.44,P＜0.01),isoniazid-resistant group ( 1.84 ± 1.86,t =3.16,P＜ 0.05),streptomycin-resistant group ( 1.86 ±1.96,t=2.78,P＜0.05) and ethambutol-resistant groups (3.36±2.35,t=3.04,P＜0.05) were all higher than sensitive isolates (0.42 ± 0.31).The expressions of Rv1218c in rifampicin-resistant group (2.54±1.84,t=3.82,P＜0.01),isoniazid-resistant group (2.34± 1.84,t=3.72,P＜0.01),streptomycin-resistant group (2.15±1.86,t=3.01,P＜0.01) and ethambutol-resistant groups (3.78± 1.78,t=4.22,P＜0.01 ) were all higher than sensitive isolates (0.65 ± 0.42).The expressions of Rv1217c and Rv1218c in multidrug resistant group were 2.74±2.07 and 3.33± 1.77,respectively,which were both higher than polydrug resistant group (0.79 ± 0.47 and 1.03 ± 0.79,respectively; t =2.91,P＜0.05 ; t =3.84,P＜0.01,respectively).Logistic regression analysis found that high Rv1217c expression was positively correlated with rifampicin resistance and negatively correlated with isoniazid resistance (both P＜ 0.01 ),while high Rv1218c expression was negatively correlated with rifampicin resistance and positively correlated with isoniazid resistance (both P＜0.01 ).High expressions of two genes were both positively correlated with ethambutol resistance and multidrug resistance (both P＜0.01 ) and not statistically correlated with streptomycin resistance (P＞0.05).Conclusions The expressions of ABC efflux pump gene,Rv1217c-Rv1218c,are correlated with multiple drug resistance.The overexpression may contribute to the multidrug resistance of Mycobacterium tuberculosis.

Objective: To understand the health education demands of primary and middle school students in Guiyang. Methods: A multi-stage stratified cluster random sampling was used to conduct a questionnaire survey for 5 540 students in grades 3-9 in Guiyang. Results: The report rate of dental caries decreases with the increase of school age(P<0.05), and the reporting rate of myopia and the appearance of sexual development characteristics increases with the increase of school age(P<0.05). Proportion of middle and primary school students who could get enough sleep every day(17.6% and 22.0%) and one hour of outdoor activities every day(13.9% and 20.9%) was relatively low.The proportion of graduating students with running away from home and suicide/self-injury ideas/plans and behaviors was higher than other grades in the same school period(P<0.05).The proportion of middle school female students who have thoughts/plans of running away from home, suicidal/self-injury plans and behaviors was higher than that of male students.Students with greater academic pressure,fewer friends,and lack of communication with their parents were more likely to have thoughts/plans or behaviors of running away from home and suicide/self-harm(P<0.05). There were 4 600 students(83.3%) who expressed the demands for health education.School was the primary source of health knowledge for students,with the class meetings accounted for the most practices.Health education on healthy behaviors and lifestyles were highly expected. Conclusion The health education demands of primary and middle school students in Guiyang show age and sex specific characteristics.Health education should follow the physical and psychological development characteristics of students and adapt to the needs of students.

Human epidermal growth factor receptor 2 (HER2) belongs to the transmembrane glycoprotein receptor family. Overexpression of HER2 could directly lead to tumorigenesis and metastasis. This phenomenon could be observed in the breast cancer, ovarian cancer, gastric cancer, lung cancer and prostate cancer. Compared with the conventional chemotherapy, the targeted treatment of antibody is more specific and has lower side effects. This review describes the current status of monotherapy and combination therapies of anti-HER2 antibodies, trastuzumab and pertuzumab, with chemotherapeutic drugs. The development trends of new formats of anti-HER2 antibody drugs such as bispecific antibody, immunotoxin are also discussed.
