1.Pathogenic analysis of rat infected hantavirus in Henan province, 2014-2016

Objective To analyze the genotyping of hantavirus and investigate the pathogenic features of local rats in Henan province. Methods A total of 600 rats captured in Queshan county, Zhumadian city from 2014-2016 were chosen to find out the major species and density. Rat lung specimens were detected by RT-PCR using partial M and S segment primers, then sequencing and phylogenetic analysis based on M segment (2003-2302 nt) were performed to analyze gene subtype and evolution. Results In the field of Queshan county, major species were sewer rats and apodemus agrarius, and the average density of rats was 1.33%-1.83%. Sewer rats, mus musculus and apodemus agrarius were major species in the residential area, and the average density of rats was 1.36%-1.97%. Hantaviruses were detected by RT-PCR in three captured rats in 2014, and the species were mus musculus, cricetulus triton and sewer rats. Nucleotide homology similarity based M and S segment of three positive products was 100%. Phylogenetic analysis indicated the virus was belonged to S4 subgenotype of Seoul virus, which was similar with the strains in Korea and Hubei province, China. Conclusion The virus from rats in Queshan county, Henan province is seoul virus, S4 subgenotype. It is necessary to take the relevant prevention and control measures to prevent hemorrhagic fever of renal syndrome because of wide host range.
2.Ecological study on the relationship between meteorological factors and incidence of fever, thrombocytopenia and leukopenia syndrome (FTLS)

To explore the relationship between meteorological factors and incidences of fever, thrombocytopenia and leukopenia syndrome (FTLS), incidence data of FTLS in Xinyang City were collected and described. The relationship between FTLS and monthly meteorological factors such as average atmospheric pressure, average temperature, average relative humidity, average wind speed and precipitation was analyzed with simple correlation and multi-stepwise linear regression. The results showed that there was a significant negative correlation between incidence of FTLS and monthly average atmospheric pressure (P<0.01), and there were significant positive correlations between monthly average temperature, monthly average relative humidity, monthly precipitation and incidence of FTLS (P<0.05). The multi-stepwise regression showed that the regression equation was Y (monthly cases) =-12.70 + 0.28X2 (monthly average temperature), and the coefficient of determination was 0.76. The incidences of FTLS are correlated with temperature.
3.Investigation of animals infected with novel bunyavirus in Xinyang City,Henan Province,China

To investigate the animals infection situation of novel bunyavirus in Xinyang City ,Henan Province ,China , animal serum samples such as cattle ,dog ,swine ,mice were collected in Shangcheng County and Guangshan County in Xinyang City .All the serum samples were detected by novel bunyavirus ELISA and real time RT-PCR method .A total of 292 animal serum samples were collected including 5 kinds of animals .The result of all the animal serum samples were negative by using real time RT-PCR ,and the positive rate was 45 .19% (141/312) by ELISA method .Of the 5 animal serum samples including mice ,cattle ,goats ,swine and dogs ,the positive rate were detected to be 1 .06% ,100 .00% ,76 .27% ,3 .57% ,and 75 .00%respectively .There was significant difference in results among 5 kind of animal serum antibodies .Animals such as cattle and dog may be the host of novel bunyavirus which were detected novel bunyavirus antibodies in cattle and dog in Xinyang City , Henan Province ,China .
4.Development and application of molecular diagnostic method of hemophilia A

Objective To develop and validate a method for detecting factor 8 gene (F8) mutations in hemophilia A patients by Ion Torrent semiconductor sequencing .Methods Intron 22 and intron 1 inversions of F8 gene were identified by long distance PCR (LD-PCR), other mutations in the F8 gene were identified by Ion Torrent sequencing.Candidate variants were validated by Sanger sequencing .Sanger sequencing was applied to screen HA carriers from 11 female family members in the 8 pedigrees.One pregnant woman was offered prenatal diagnosis via analyzing the fetal DNA obtained through amniocentesis . Results Four missense mutations ( c.1331A >C, 1648C >T, c.6506G >A, c.6544C >T), two frameshift mutations ( c.2393 _2394insT, c.6320delG), one splicing mutation ( IVS5 +5G >A), one nonsense mutation (c.43C >T) and one Inv22 mutation were identified in all nine probands respectively . Among 11 female family members, 10 females were identified to be HA carriers, and one didn′t carry the maternal pathogenic mutation.Prenatal diagnosis result showed that the fetus inherited the wild -type maternal allele and was predicted to be unaffected by HA .Conclusion The targeted Ion Torrent sequencing is a reliable and efficient method to detect F8 mutations in patients with Hemophilia A disease .
5.Efficacy and safety of telbivudine alone and combined with adefovir for the treatment of nucleos(t)ide-naive chronic hepatitis B in patients with high-level hepatitis B virus load.

OBJECTIVETo compare the efficacy of telbivudine monotherapy and telbivudine combination therapy with adefovir in patients with nucleos(t)ide-naive chronic hepatitis B, high-level hepatitis B virus (HBV) load and hepatitis B e antigen (HBeAg)-positivity, and to explore the relationship between treatment regimen adherence and treatment outcomes.
METODSA retrospective study was performed with 123 HBeAg-positive, high-level viral load (HBV DNA≥10(7) copies/ml), nucleos(t)ide-naive chronic hepatitis B patients. Fifty-three of the patients received combination therapy with telbivudine and adefovir dipivoxil,while 70 patients received the telbivudine monotherapy. All patients were tested for rates of conversion to HBV DNA-negative status,alanine aminotransferase (ALT) normalization, HBeAg seroconversion, drug resistance, and side effects at treatment weeks 12, 24, and 48. Treatment regimen adherence was assessed through self-reporting, and interviews were used to explore the relationships to treatment outcomes. The chisquare test, t test and Fisher's exact test were used for statistical analyses.
RESULTSThe rates of HBV DNA negative conversion in the combination group at treatment weeks 12, 24 and 48 were 62.3% (33/53), 88.7% (47/53) and 94.3% (50/53) and were significantly different from those in the monotherapy group at weeks 12 and 24.The rates of ALT normalization were significantly different between the two groups at treatment week 12 (94.3% vs. 77.1%). The rate of HBeAg seroconversion in the combination group at treatment week 48 was 39.6%, and significantly different than that of the monotherapy group. The rates of drug-resistance in the combination and monotherapy groups at treatment week 48 were 3.8% and 11.4%, and the proportion of non-adherence to the treatment regimen was 53.3%, which significantly affected treatment outcome. No side effects occurred in either treatment group.
CONCLUSIONTelbivudine combination treatment with adefovir was more effective than telbivudine monotherapy and elicited a low drug resistance rate in nucleos(t)idenaive chronic hepatitis B patients with high-level HBV load and HBeAg-positivity. Adherence to the therapy regimen was a key factor influencing treatment outcomes.

6. Laboratory diagnosis and molecular tracing of dengue bordline cases in Henan Province, 2017

Objective:
To confirm the laboratory diagnosis of dengue bordline cases reported in Henan Province and trace its origin from molecular level in 2017.
Methods:
The study samples were blood samples (3-5 ml), which came from 8 suspected cases of dengue fever reported in the 2017 direct reporting system of Henan provincial infectious disease monitoring network. Meanwhile, case investigation was conducted according to National dengue fever surveillance programme. Serum were separated from blood samples and tested for Dengue NS1 antigen, IgM & IgG antibodies, and dengue RNA. According to dengue diagnosis criteria, confirmed cases were identified by testing results. Samples carried dengue RNA performed for real-time PCR genotyping and amplification of E gene. Then, the amplicons were sequenced and homological and phylogenetic analyses were constructed.
Results:
8 serum samples of suspected dengue cases were collected in Henan Province, 2017. Six of them were diagnosed as dengue confirmed cases. All the dengue confirmed cases belonged to outside imported cases, 5 of them were positive by dengue RNA testing. Genotyping results showed there were 1 DENV1 case, 2 DENV2 cases and 2 DENV3 cases. A DENV2 case and a DENV3 case of this study were traced its origin successfully. The sequence of Pakistan imported DENV2 case belongs to cosmopolitan genotype, which was the most consistent with Pakistan's DENV2 KJ010186 in 2013 (identity 99.0%). The sequence of Malaysia imported DENV3 case belongs to genotype I, which was the most consistent with Singapore's DENV3 KX224276 in 2014(identity 99.0%).
Conclusion
The laboratory diagnosis and molecular traceability of dengue cases in Henan Province in 2017 confirmed that all cases were imported and did not cause local epidemics.
7.Evaluation on the effects of multidisciplinary cooperative nutrition management model in acute stroke patients with dysphagia

Objective:To analyze the effects of multidisciplinary cooperative nutrition management model in acute stroke patients with dysphagia.Methods:From February 2019 to February 2020, 69 acute stroke patients with dysphagia were enrolled in this study. After exclusion of those unable to complete the trial, patients were randomized into control group ( n=30) and experimental group ( n=30). Patients in the control group were given routine nutrition management, while patients in the experimental group were treated under multidisciplinary cooperative nutrition management model. Nutritional indicators were compared between the two groups on Day 1, 7 and 14 after admission, including levels of albumin (ALB), pre-albumin (PALB), hemoglobin (HB), triceps skin-fold (TSF) thickness on the uninjured side, upper arm muscle circumference etc. Incidence of gastrointestinal complications and infectious complications was also recorded. Results:There was no difference between two groups in the levels of HB, TSF thickness and upper arm muscle circumference on the uninjured side (all P>0.05). However, the serum levels of ALB and PALB on Day 7 and 14 in the experimental group were higher than that in the control group(all P<0.05), The incidence of gastrointestinal complications ( P=0.015)and infectious complications ( P=0.016) in the experimental group was lower than that in the control group. Conclusion:Multidisciplinary collaborative nutrition management improved nutritional indicators, reduced the incidence of gastrointestinal complications and infectious complications in acute stroke patients with dysphagia, making multidisciplinary collaborative nutrition management model worthy of clinical promotion and application.
8.Curcumin inhibits HeLa cell invasion and migration by decreasing inducible nitric oxide synthase.

OBJECTIVETo investigate the inhibitory effects of curcumin against HeLa cell invasion and migration and explore the underlying mechanisms.
METHODSHeLa cells were exposed to curcumin treatment at the concentrations of 0, 10, 25, 50, 100, 150 and 200 µmol/L for 24 h. MTT and TUNEL assays were used to assess the cell proliferation inhibition and apoptosis, respectively. Transwell assay was used to evaluate the invasiveness and migration of the treated cells, and RT-PCR and Western blotting were employed to detect the changes in the expression of inducible nitric oxide synthase (iNOS), and MMP-9 and E-cad, the 2 markers of cell invasion and migration, were detected by Western blotting. The capacity of NO production in HeLa cells was measured by Griess method.
RESULTSCurcumin inhibited the proliferation of HeLa cells by inducing cell apoptosis in a concentration-dependent manner. Curcumin inhibited the invasion and migration of HeLa cells by increasing E-cad expression and decreasing MMP-9 expression, and also decreased the expression level of iNOS and NO production in the cells.
CONCLUSIONCurcumin inhibits the invasion and migration of HeLa cells by decreasing the expression of iNOS.

9.Surveillance for viral diarrhea in sentinel hospitals in Henan province, 2013-2015

Objective To investigate the infection status of human rotavirus,calicivirus,astrovirus and enteric adenovirus in children aged <5 years in disease surveillance areas in Henan province from 2013 to 2015.Methods A total of 880 stool samples were collected from four sentinel hospitals and group A rotavirus was detected by ELISA and group A rotavirus G/P genotyping was performed with nested multiplex RT-PCR,while rotavirus (group B,C),calicivirus and astrovirus were detected by two-step multiplex RT-PCR and adenovirus were detected by PCR.The epidemiological data of positive cases were statistically analyzed.Results A total of 594 positive samples were detected,including 24 mixed infection samples,370 rotavirus positive samples (42.0%);162 calicivirus positive samples (18.4%);69 astrovirus positive samples (7.8%) and 17 enteric adenovirus positive samples (1.9%).The overall positive rate of four viruses was significantly higher in urban area than in rural area,but the positive rate of rotavirus was higher in males than in females and in younger age group than in older age group.G9P [8] was the major genotype of group A rotavirus,there were two seasonal infection peaks in autumn and spring.Norovirus Ⅱ was the predominant type of calicivirus and the infection peak was in spring.Viral diarrhea cases were distributed in different age groups,mainly in age groups 0-12 months (rotavirus) and 3-5 years (calicivirus).The main clinical symptoms included fever,diarrhea and vomiting.The etiological characteristics differed with gender and area.Conclusions The infection rate of diarrheal viruses was higher in young children <5 years old in disease surveillance areas.The epidemiological and clinical features varied with the type of pathogen.
10.Distribution and molecular type of Salmonella from external environment in Henan province

Objective To understand the distribution of Salmonella in external environment in Henan province,and explore the distribution of different serotypes of the Salmonella and their homology.Methods A total of 4 488 samples were collected form animal dung,meat products and kitchen utensils,and identified by biochemical tests and serotyped by serum agglutination reaction.The predominant serotypes were further typed by PFGE.Results A total of 324 Salmonella strains were detected in these samples,the detection rate was 7.21%.The 324 Salmonella isolates belonged to 39 serotypes,S.enteritidis (24.07%,78/324) and S.derby (20.37%,66/324) were predominant.Forty six strains ofS.enteritidis and 30 strains of S.derby were divided into 12 and 17 molecular patterns by digestion with Xba Ⅰ,while chicken and swine were the predominant animal hosts.Conclusions Serotyping of external environment Salmonella were phenotypically diverse and the serotype of Salmonella from different sources were different.The same clone was prevalent in same area.It is necessary to strengthen supervision and surveillance to ensure food safety.
