Objective To investigate the changes of the serum level of highly sensitive C-reactive protein (hs-CRP)in patients with cerebrovascular disease and its clinical significance. Methods The serum level of hs-CRP in 60 patients with acute cerebral infarction and 60 patients with acute cerebral hemorrhage and 40 healthy persons were determined.All the patients were scored by clinic neurological deficit scale(NDS),and were compared in groups. Results The serum level of hs-CRP in patients with acute cerebral infarction and with acute cerebral hemorrhage were significantly higher than that in normal controls(t=19.712 13.217 P <0.01).There was no obvious difference between the patients with acute cerebral infarction and with acute cerebral hemorrhage. Conclusion The serum level of hs-CRP was closely correlated with the severity of cerebrovascular disease.

Objective To analyze the clinical presentation and the effects on both mothers and fetuses with HELLP syndrome induced hyregtensive disorder complicating pregrancy and to investigate the effective methods for diagnosis and treatment. Methods 15 cases of HELLP syndroms were retrospectively analyzed on material and perinatal outcomes. Results In 15case of women with HELLP syndrome, all of them presented antepartum HELLP. Mean gestational age at delivery was 34 weeks. Severe maternal complications included placental abruption,DIC ,postpartum hemorrhage,fetal distress, et al. All of them underwent cesaresn section and required transfusions with blood or blood products. 1 case of perinatal death was related to placental abruption. Conclusion HELLP syndrome is a severe obstetric complication. Better prognosis may be achiveved by early diagnosis, synthetic therapy and optimal termination of pregnancy.

Objective To construct an eukaryotic expression plasmid containing gene coding for the hemagglutinin-neuraminidase(HN)of newcastle disease virus(NDV)oncolytic strain Italien,and then to express the protein in eukaryotic cell.Method The HN cDNA was synthesized from viral RNA by RT-PCR,and the eukaryotic expression vector of HN gene(named pcDNA3.1-HN)was constructed.The vector pcDNA3.1-HN was transfected into CHO-K1 cell by liposome,and G418 was used to select stable clones expressing HN gene.The expression of HN protein was visualized by Western blot and Immunofluorescence microscopy.Results Restriction analysis and DNA sequencing proved that HN gene was correctly cloned into expression vector.Western blot analysis and immunofluorescence showed that the HN was expressed in CHO-K1 cells.Conclusion The HN cDNA of NDV was successfully cloned into eukaryotic vector which showed good expression of HN protein in CHO-K1 cells.

Objective To explore the bioactivity and stability of the antifungal substance produced by Streptomyces NG-715 as well as to establish the assay for biological activity detection. Method Take the antifungal substance as experimental materials, and test its minimum inhibitory concentration and minimum bactericidal concentration on four fungi strains including Saccharomyces cerevisiae, Penicillium citrinum, Aspergillus niger, Rhizopus sp. Aspergillus niger was used as indicator strain to measure the biological activity and stability of the antifungal substance. Results The results showed that the MIC of the antifungal substance on four fungi strains including Saccharomyces cerevisiae,Penicillium citrinum,Aspergillus niger,Rhizopus sp were 1.25, 2.5, 3.75 and 3.75μg/mL, respectively. The MBC of the antifungal substance on four fungi strains were 2.5, 10, 17.5 and 17.5μg/mL, respectively. Linearity regress equation of the antifungal substance in Aspergillus niger was y=26.963 x-27.6,R 2=0.9991. The antifungal substance was pH-stable, heat-stable but ultravio1 et-sensible. Conclusion The results from this study will porvide useful information for the further extraction and analysis on the bioactive compound.

Objective To evaluate the efficacy of the parathyroidectomy (PTX) in the treatment of severe secondary hyperparathyroidism (SHPT) with Sagliker syndrome (SS). Methods A retrospective review was undertaken among 212 SS patients underwent PTX in our hospital and with more than 3 years' follow up. The definitions of the efficacy were based on the postoperative intact parathyroid hormone level (iPTH). Cure showed that the iPTH was ＜ 150 ng/L; marked effectiveness was 150-300 ng/L; effectiveness was 301-500 ng/L;ineffectiveness was ＞500 ng/L. The status was defined as persistent SHPT if iPTH was ＞ 150 ng/L after surgery. The status was considered as SHPT recurrence if iPTH was ＜ 100 ng/L in the first week after surgery, and gradually increased and ＞ 150 ng/L with the follow-up. Results ( 1) Ten patients were involved and the average dialysis time was 142 months [male/female: 4/6; age 30-54 (39. 3 ± 10. 4) years]. All patients had severe bone and joint pain, accompanied with progressive facial increases, chicken breast, kyphosis, hip bone deformities, and body height shortening. (2) Preoperative tests: the median of iPTH 2000(1800-2863) ng/L; serum calcium (2. 45 ±0. 21) mmol/L, phosphorus (2. 19 ±0. 51) mmol/L, alkaline phosphatase ( ALP) (1189. 8 ± 780. 0) IU/L. Two to four enlarged parathyroid glands were confirmed by ultrasound and 99Tcm-MIBI parathyroid scintigraphy. ( 3 ) Surgical procedures: local or general anesthesia for PTX. Supplement with calcium and calcitriol implemented low serum calcium after PTX. (4) Follow-up: symptoms, including bone pain, muscle weakness, skin itching, and insomnia, were significantly improved after surgery. Transient hoarseness occurred in 2 cases. The iPTHs of all patients were decreased significantly after surgery. The median of iPTH was 55.5 ( 10-967) ng/L at 1 month post PTX, and was significantly less than prior to PTX (P＜0. 001). Eight patients were cure , 1 marked effectiveness ,and 1 ineffectiveness. Two patients were persistent SHPT, and 1 died of heart failure in the 4th year after PTX. The development of bone deformities was stopped and malnutrition was improved in long-time follow up. The level of iPTH 135(28-390)ng/L(P＜0. 001 ) , serum calcium, phosphorus, and ALP showed normal in the third year. The SHPT recurrence was appeared in the 2nd and 3rd year in 2 out of 8 patients, respectively. Conclusions Total PTX can effectively treat SS by SHPT. It can improve prognosis for patients, such as bone pain disappearing, bone deformities stopping and malnutrition improving, etc. The level of iPTH may rise again in some patients in the future. Therefore, more attentions should be paid to monitoring.

Objective To investigate the expression of periostin in the kidneys of rats with obstructive nephropathy and its relevance to renal interstitial fibrosis.Methods Eighteen male adult SD rats were randomly divided into sham group,model group and benazepril group (6 in each group).Unilateral ureteral obstruction (UUO) model was induced by ligating the left ureter of rats.RT-PCR was used to detect the mRNA expressions of periostin and TGF-β1,and ELISA to detect the protein expression of periostin,Ang Ⅱ,and TGF-β1 in kidney tissue.Pathological changes of renal tissue were observed by HE and Masson staining.The protein expression of collagen Ⅰ (Col Ⅰ ) in kidney tissue was examined by immunohistochemical staining.Results The mRNA expressions of periostin and TGF-β1 in model group increased markedly as compared with sham group (all P＜0.05),and benazepril could decrease these mRNA expressions (all P＜0.05).The protein expressions of periostin,Ang Ⅱ and TGF-β1 in kidney tissue were significantly increased in model group as compared with sham group (all P＜0.05),and benazepril could decrease these protein expressions (all P＜0.05).The expression of periostin in kidney tissue was positively correlated with the expressions of Ang Ⅱ,TGF-β1 and Col Ⅰ,as well as the degree of renal interstitial fibrosis (r=0.652,0.781,0.776 and 0.825 respectively,all P ＜0.05).Conclusion Periostin expression is up-regulated in kidney tissue of rats with obstructive nephropathy,which is associated to the over-deposition of extracellular matrix (ECM) in the kidneys of UUO rats.

Objective To investigate the effects of diabetic duration on liver fat content (LFC) in patients with type 2 diabetes,and to explore its relationship with the outcome of liver disease.Methods A total of 435hospitalized patients with type 2 diabetes were recruited.The history data,results of laboratory tests,and hepatic 1 H-MRS were collected,and nonalcoholic fatty liver disease (NAFLD) fibrosis score (NFS) was calculated.Results The prevalence of NAFLD in newly-diagnosed type 2 diabetes mellitus (NT2DM) group was higher than that in predousb-diagnosed type 2 diabetes mellitus (PT2DM) group (92.7％ vs 82.2％,P＜0.05),with higher LFC [(27.97 ± 16.88 vs 19.44± 15.54) ％,P＜0.01].The LFC was reduced with prolonged duration of diabetes.Partial correlation analysis showed that LFC was negatively correlated with duration of diabetes (rs =-0.233,P＜0.01) after adjustment for gender,age,body mass index (BMI),oral anti-diabetic drugs,lipid-lowering drugs,and insulin treatment.Multiple linear regression analysis showed that LFC was positively correlated with BMI,albumin,and alanine aminotransferase while negatively correlated with duration of diabetes.The proportion of patients without advanced fibrosis (NFS＜-1.455) was significantly higher in NT2DM group than that in PT2DM group (26.3％ vs 15.5％,P＜0.05),and the proportion of PT2DM in patients with advanced fibrosis (NFS＞0.676) was significantly higher than that of NT2DM (79.2％ vs 20.8％,P＜0.05).NFS was positively correlated with the duration of diabetes (rs =0.236,P＜0.01).The liver fat content in patients with advanced liver fibrosis decreased significantly,and the LFC was negatively correlated with NFS (rs =-0.164,P＜0.01).Conclusions The duration of diabetes is an independent influencing factor of LFC.With the extension of the duration of diabetes,the decreased LFC in type 2diabetic patients with NAFLD is related to the development of advanced fibrosis.The decrease in LFC in type 2diabetic patient is associated with poor outcome of NAFLD.

Objective To analyze the association of fat content,enzymes,and fibrosis in liver with iron overload in patients with type 2 diabetes,and to explore the relationship between iron overload and severity of nonalcoholic fatty liver disease (NAFLD) in these patients.Methods Five hundred and thirty hospitalized patients with type 2 diabetes and 18 patients with abnormal glucose metabolism undergoing liver biopsy were recruited.History data,results of laboratory tests,liver ultrasound,hepatic 1 H-MRS were collected and serum ferritin level was determined.Results The serum ferritin level was significantly higher in patients with NAFLD than that without NAFLD [(328.7±252.2 vs 239.9 ± 171.8) μg/L,P＜0.01].Serum ferritin was an independent risk factor for NAFLD (P＜0.05).Multiple linear regression analysis showed that serum ferritin was positively correlated with liver fat content after adjustment for sex,age,and duration of diabetes.The serum ferritin level in NAFLD with elevated liver enzymes was significantly higher than that in simple steatosis [(429.9 ± 287.4 vs 293.4 ± 233.3) μg/L,P＜0.01].Serum ferritin was an independent risk factor for elevated liver enzymes in patients with NAFLD (P ＜0.05).Serum ferritin level in patients with advanced fibrosis was significantly lower than that in patients without advanced fibrosis [(246.8 ± 191.2 vs 382.5 ± 253.7) μg/L,P＜0.01].In 18 patients with NAFLD proven by biopsy,serum ferritin level was slightly higher in NASH group than that in simple steatosis group,but there was no statistically significant difference.Serum ferritin levels were comparable between patients with and without advanced fibrosis.Conclusion The iron overload in type 2 diabetic patients seems to be an independent risk factor for the development of NAFLD and elevated liver enzymes.Iron load in patients with advanced fibrosis is significantly decreased.

Objective To investigate the effects of trichostatin A(TSA)on the mice model of collagen induced arthritis(CIA).Methods Mice model of rheumatoid arthritis(RA)was induced in DBA/1 mice with type Ⅱ collagen.Paws were scored for histological severity of arthritis.The severity of inflammation of mouse joint was evaluated by histological examination.Real-time PGR was used to determine the cytokine mRNA expression.Cytokine production was measured by ELISA from serum,spleen cell culture or dendritic cell and T cell co-culture supematant.T cell proliferation was examined by MTT method.Results TSA can significantly suppress the severity of the arthritis in CIA.IFN-γ was elevated in CIA mice,but was inhibited significantly by TSA introduced either at the same time with immunization or at the onset of manifestation of arthritis.Collagen specific T cell proliferation was significantly suppressed by introduction of TSA.Increased level of IL-4 by T cells was observed in TSA treated group compared to that of control group.Conclusion IL-4 level was increased and played a critical role in the protective effects of TSA in CIA.TSA suppresses the progress of CIA by regulates the balance of Th1/Th2 differentiation.
