2.Expression of NFkappaB p65 and its target genes in gastric cancer and precancerous lesions.

OBJECTIVETo study the expression of NFkappaB p65 and its target genes in intestinal metaplasia (IM), dysplasia (Dys), gastric cancer (GC) infected with Helicobacter pylori (Hp) and explore the mechanism of infection by cytotoxin-associated antigen A expressing Hp (CagA(+)Hp) in the development of gastric cancer.
METHODSCagA antibody in blood sample of 289 patients was determined by ELISA. Hp was detected by rapid urease test and Warthin starry staining. Expression of NFkappaB p65 and its target genes in IM, Dys and GC was examined by immunohistochemistry.
RESULTSIn IMI approximately II, IMIII, DysI, DysII approximately III and GC, the expression of NFkappaB p65 was significantly higher in patients with CagA(+)Hp infection than those without CagA Hp infection. In IMIII and DysII approximately III, the expression of NFkappaB p65, c-myc, CyclinD(1) and bcl-xl was significantly higher in patients with CagA Hp infection than those without CagA Hp infection. In gastric cancer infected with CagA(+)Hp, the expression of NFkappaB p65, c-myc, CyclinD(1) and bcl-xl was significantly higher in intestinal type than in diffuse type.
CONCLUSIONThere are different mechanisms in intestinal type and diffuse type in the development of gastric cancer. The occurrence of intestinal type gastric cancer is associated with CagA(+)Hp infection which by NFkappaB p65 upregulating the expression of c-myc, CyclinD(1),bcl-xl in patients with IMIII, DysII approximately III. It may be an effective method to prevent gastric cancer by inhibiting NFkappaB p65.

3.Analysis of the main components of coptis-evodia herb couple by HPLC-DAD-MS.

A comprehensive HPLC-DAD-MS method was developed to study the chemical components of semi-bionic extract of Coptis-Evodia herb couple. The extract was isolated on a Hypersil BDS C18 column (4.6 mm x 200 mm, 5 microm) using acetonitrile-ammonium formic buffer as mobile phase by gradient elution. Detection was performed on DAD and MS equipped with an electrospray ionization (ESI) source by full scan and product full scan on positive mode. The chromatogram of Coptis-Evodia showed seventeen main peaks, eight of which were from Evodia while the others were from Coptis. By comparison of the retention time, the on-line UV spectra and MS spectra, four peaks were identified as jatrorrhizine, hydroxevodiamine, palmatine and berberine, and three peaks were deduced as epiberberine, columbamine and coptisine. In addition, berberine and palmatine were quantitatively determined. No new component was created in the semi-bionic extract of the herb couple, yet the solubilities of berberine and palmatine decreased.

4.Preparation of soluble microneedles of Aconitum brachypodum alkaloids

AIM To prepare the soluble microneedles of Aconitum brachypodum Diels alkaloids.METHODS Centrifugal molding method was adopted in the preparation of soluble microneedles.With chondroitin sulfate consumption,PVP K120 consumption and 40%ethanol consumption as influencing factors,piercing rate as an evaluation index,the formulation was optimized by Box-Behnken response surface method,after which the morphology,piercing performance,drug content and in vitro transdermal performance were investigated.RESULTS The optimal formulation was determined to be 123 mg for chondroitin sulfate consumption,298 mg for PVP K120 consumption,and 2.4 mL for 40%ethanol consumption,the piercing rate was 98.3%.The soluble microneedles were yellow and square patch with conoid needle,which could pierce aluminum foil and rat skin,along with the drug content of(0.94±0.025)mg.The soluble microneedle group demonstrated the accumulative permeability rate of 91.4%within 24 h,which was higher than that in the gel ointment group,and the permeability accorded with Higuchi equation.CONCLUSION The soluble microneedles of A.brachypodum alkaloids exhibit good mechanical strength,which can achieve effective transdermal delivery of drugs.
5.1H-MRS study of auditory cortex in patients with presbycusis

Objective To study the metabolic changes of auditory cortex in patients with presbycusis by using proton magnetic resonance spectroscopy(1H-MRS). Methods Ten normal hearing volunteers(youth group),10 normal hearing of elderly (aged group) and 8 patients with presbycusis (presbycusis group ) were checked with proton magnetic resonance spectroscopy. N-acetylaspartic acid ( NAA),creatine ( Cr),choline ( Cho),γ-aminobutyric acid (GABA),glutamic acid ( Glu ) compound were measured.The differences between the groups were semi-quantitatively analyzed.Results When compared with youth group,reduced NAA/Cr,increased Cho/Cr were found in the aged group and presbycusis group (P < 0.05 ). GABA/Cr ratio and Glu/Cr ratio were significant difference between presbycusis group and youth group(P < 0.05 ). There were no significant difference in the GABA/Cr and Glu/Cr ratios in the bilateral auditory cortex between the youth group and the aged group(P > 0.05 ).When compared with aged group,the metabolic changes of auditory cortex in patients with presbycusis were remarkable ( P < 0.05 ).Conclusions 1H-MRS is a noninvasive technique that can provide useful information concerning the metabolic changes of auditory cortex in human. In comparison to the aged group and the youth group,the changes of NAA,GABA,Cho and Glu is found in auditory cortex in patients with presbycusis.
6.Induction of UGT1A1 expression by praeruptorin A and praeruptorin C through hCAR pathway.

This study is purposed to investigate the effects of praeruptorin A (PA) and praeruptorin C (PC) on UGT1A1 in HepG2 cells through hCAR pathway. PA and PC were incubated with HepG2 cells for 24 h and 48 h, mRNA and protein expressions of UGT1A1 were determined by real-time PCR and Western blotting assays. Additionally, effects of PA and PC on UGT1A1 mRNA and protein expressions were also measured after transient transfection of a specific CAR siRNA for 72 h in HepG2 cells. UGT1A1 mRNA and protein expression levels were significantly increased by PA and PC after incubation for 48 h. Moreover, the mRNA and protein up-regulations of UGT1A1 were attenuated by transient transfection of a specific CAR siRNA, suggesting the induction was mediated by CAR. The results suggest that PA and PC can significantly up-regulate UGT1A1 expression partially via the CAR-mediated pathway.

8.Clinical efficacy and safety of peramivir trihydrate sodium chloride injection in the treatment of acute simple uncomplicated influenza

Objective To evaluate the clinical efficacy and safety of peramivir trihydrate sodium chloride injection in the treatment of acute simple uncomplicated influenza.Methods Accepting the 60 cases of in-fected patients as subjects.All patients were given a single intravenous infusion of peramivir sodium chloride injection 100 mL/0.3 g.Before treatment, 3 days and 7 days after treatment, all patients were inter-viewed 3 times and the subjects ’ symptoms and adverse events were eva-luated.Results Fever response rate was 100%, alleviate time was (23.40 ±13.43 ) h, other symptoms remission rate was 75% to 93%.Adverse events were given priority to the anomalies of laboratory tests , including elevated triglycerides in 11 cases, elevated alanine aminotrans in 1 case, QTc prolongation in 1 case, most of them returned to normal.No serious adverse events appeared.Conclusion Application of perami-vir sodium chloride injection in the treatment of acute simple uncompli-cated influenza is relatively safe and effective.
9.Significance of the double mutations of C1673T/C1799G in HBV C promoter

Objective To explore the biological and clinical significance of the double mutations of C1673T/C1799G in HBV C promoter (CP).Methods Totally 136 patients were enrolled,including 25 asymptomatic carriers (AsC),38 patients with chronic hepatitis B (CHB),24 patients with chronic severe hepatitis B (CSHB),36 cases with liver cirrhosis (LC) and 13 cases with hepatocellular carcinoma (HCC).HBV subgenotypes and mutations in CP of all samples were determined by nested-PCR and direct nucleotide sequence analysis.The C to T mutation at nucleotide 1673 and C to G at nucleotide 1799 were analyzed in different subgenotypes,and the relationships of C1673T/C1799G double mutations with HBV replication,the expression of HBeAg,and with the severity of liver disease after chronic HBV infection were studied.Results Of the 136 patients,110 were subgenotype Ba,1 was Bj,7 were C1,and 18 were C2.C1673T/C1799G double mutations in Ba were determined in 106 (96.4%) samples,which was significantly higher than in C1 (14.3%) and C2 (12.5%) subgenotype (P < 0.0001).In contrast to nonmutation group,HBV DNA content in mutation group had no significant difference (P > 0.05).The prevalence of the mutation was lower in HBeAg positive patients (71.4%) than in HBeAg negative patients (87.5 %) (P < 0.05).The frequencies of the double mutations were not significantly different among ASC,CHB,CSHB,LC and HCC groups (P > 0.05).Conclusion In Ba subgenotype,double mutations of C1673T/C1799G is much popular than in C1 and C2; the mutation has no effect on HBV replication,and may not be associated with the outcome of chronic HBV infection.
10.Psychological status in 1083 hospitalized patients with coronary artery disease

Objective To explore the prevalence and associated factors of anxiety and depression symptoms in hospitalized Chinese patients with coronary artery disease(CAD). Methods From June 2007 to May 2009, 1083 hospitalized patients with confirmed coronary artery disease were recruited in this study.The ZUNG Self-rating Anxiety Scale(SAS)and the ZUNG Self-rating Depression Scale(SDS)were used for the psychological assessment. Economic status, living condition and the environment of both living and working places were evaluated by epidemiological questionnaires. Results The prevalence of pure anxiety,pure depression symptoms and the combination of anxiety and depression symptoms were 7. 9%, 28.3% and 14. 3% respectively. Incidence of anxiety and depression symptoms was significantly higher in female patients compared with in male patients(P = 0. 003, 0. 012 respectively)and in aged patients than in middle-aged patients(P = 0. 001). The elderly, less than 9 years of education and poor sleep quality increased the risk of anxiety symptom with ORs of 1.63(95% CI: 1. 21 - 2. 21), 1.54(95% CI: 1.15 -2. 07)and 1.62(95% CI: 1.34 - 1.96), respectively, while workplace noise, history of chronic disease and poor sleep quality increased the risk of depression symptom with ORs of 1.52(95% CI:1. 18 - 1.98),1.36(95%CI:1.06-1.75)and 1.27(95%CI:1.08-1.50), respectively. Female(OR=1.91, 95%CI:1. 22 -2.98), aged patient(OR = 1.84, 95% CI:1. 23 -2. 76), workplace noise(OR = 1.61, 95%CI:1. 07 -2. 42), history of chronic disease(OR = 1.84, 95% CI: 1.24 - 2. 71)and poor sleep quality (OR = 1.73, 95% CI: 1.35 -2. 21)were significantly correlated with the combined incidence of anxiety and depression symptoms. Conclusion Around half of the Chinese hospitalized CAD patients were complicated with various degrees of anxiety and/or depression symptoms. Female and aged patients were at higher risk for anxiety and depression symptoms. Sleep quality, workplace noise, years of education and history of chronic disease were independent risk factors for anxiety or depression symptoms.
