3.Dynamic Changes of Chlorogenic Acid and Luteoloside from Donghai County of Honeysuckle at Different Blossoming Stages

This study was aimed to acquire the dynamic changes of chlorogenic acid and luteoloside of honeysuckle at different collecting periods in order to decide the best harvesting time of honeysuckle in Donghai County, Jiangsu Province. The content determination method used in the detection of chlorogenic acid and luteoloside of honeysuckle was from the 2010 edition of the Chinese Pharmacopoeia. The skills of HPLC fingerprint characteristic features, and the yield of pressed flowers were combined in the evaluation of honeysuckle at the three-green period, two-white pe-riod, great-white period, silver-flower period and the golden-flower period. The results showed that the content of honeysuckle at different blossoming stages had obvious changes in content of chlorogenic acid and luteoloside, as well as the pressed flower quality and yield of the flower. It was concluded that the optimal harvest time of honey-suckle was for the two-white period and the great-white period, which was consistent with the real origin.
4.Analysis of influencing factors of early poor prognosis in elderly patients with transient ischemic attack

Objective:To analyze the clinical characteristics that affect the early outcomes of the elderly patients with transient ischemic attack(TIA) and provide the guidance for the prediction of disease recurrence.Methods:From January 2016 to December 2019, 315 TIA patients over 60 years old based on the time-clinical definition who were admitted to the department of neurology in Beijing Shijitan Hospital were divided into recurrence cerebrovascular events group(RCVEs group) and non-recurrence cerebrovascular events group(non-RCVEs group) according to the recurrence at 1 month after onset. The clinical characteristics of the two groups were compared by Logsitic regression analysis.Results:There were 29 cases in the RCVEs group and 286 cases in the non-RCVEs group. Compared to the non-RCVEs group, patients in RCVEs group were more likely have higher prevalence of hypertension history(14/15, 83/203), moderate or severe intracranial artery stenosis(21/8, 132/154), carotid stenosis(22/7, 107/179), DWI positive, motor symptoms, carotid artery stenting and higher times of TIA episodes(2(2, 3), 2(1, 2)), fasting blood glucose and glycosylated hemoglobin levels(8.86(8.17, 9.56)mmol/L, 5.82(5.27, 6.33)mmol/L). Logistic regression analysis showed that patients in RCVEs group had a DWI positive( B=1.634, OR=5.124, 95% CI: 2.898-9.059, P=0.000) and higher prevalence of moderate to severe stenosis of carotid artery( B=0.572, OR=1.773, 95% CI: 1.050-2.993, P=0.032). Conclusion:DWI positive and moderate to severe carotid stenosis may influence the early poor prognosis in patients with TIA.
5.Clinical features of mtDNA A3243G mutation in adult patients

Objective To analysis the clinical manifestations of mtDNA A3243G mutation in adulthood.Methods The clinical features were investigated in 36 cases (28 patients from 5 families with the mutation and 8 sporadic cases),in whom mtDNA A3243G mutation was confirmed genetically in 23 cases (15 cases from 5 mutation families and 8 sporadic cases).Cranium radiology was performed in 14 cases.Muscal biopsies were performed in l0 cases.Results Among 28 cases in the 5 family,there were 9 cases (32.1%) with stroke like episodes,17 cases (60.7%) with diabetic mellitus and 16 cases (57.1%) with deafness.Such symptoms usually combined with each other and rarely existed alone. Cardiomyopathy and renal failure were uncommon.In the 23 cases with mtDNA A3243G mutation,14 cases (61.0%) had mitochondria] myopathy,encephalopathy,lactic acidosis,and stroke-like episodes (MELAS),mostly presenting cognitive abnormalities,dysarthria or aphasia and headache,3 cases (13.0%) were asymptomatic carriers,2 cases (8.7%) had autonomic dysfunction,2 cases (8.7%) had diabetic mellitus with or without nerve deafness,1 case (4.3%) had diabetic mellitus with infertilitas and cardiomyopathy,respectively.Cranial radiological images revealed the changes more commonly in the temporal and occipital lobes and less frequently in the frontal lobes.Ragged red fibers were confirmed in 9 of 10 cases with muscle biopsies.The proportion of mutant mtDNA A3243C was not significantly different between MEALS (28.75%?13.69%) and non-MELAS (25.08%?11.54%).Conclusions mtDNA A3243G mutation mainly results in the lesions in the central nerve system,pancreatic island and acoustic nerve in adulthood.Heart and kidney are less frequently involved.Cognitive abnormalities,aphasia and headache are the major symptoms of adult MELAS.Families have with more than 1 patient with diabetic mellitus and deafness,indicating that the mutation is other than MELAS mutation.We should pay more attention to the non-MELAS symptoms in the families with mtDNA A3243G mutation.
6.Axonal lesion in chronic inflammatory demyelinating polyneuropathy

Objective To investigate the axonal lesion in chronic inflammatory demyelinating polyneuropathy(CIDP).Methods Eighteen patients had undergone sural nerve biopsy.The clinical and electrophysiological distinction based on the different pathological changes were analyzed.Results Five patients with demyelination predominance which presented myelinated fiber with thin myelin.Three of them showed also mild axonal degeneration.Eight patients with axonal lesion predominance which presented Wallerian degeneration and regeneration of myelinated fibers.Three patients with mixed myelin and axon lesion of myelinated fibers and two with mild lesion.There was no significant difference between CIDP predominantly with axonal lesion and demyelination.Electrophysiological examination shows both axonal lesion and demyelination feature in some of the 2 types patients at the same time.Conclusions Axonal lesion is a common pathological change in CIDP and should not be considered as an exclusive criterion in diagnosis of the disease.Infiltration of macrophages is a common change.
7.Evaluation of Cognitive-communication Ability for Chinese Youngsters Aged 20-30 Years

Objective To investigate the cognitive-communication ability of people aged 20-30 years in China. Methods 139 normal participants took part in the Cookie Theft test. 8 different indexes were used to analyze the language samples. These indexes were total words number, the percentage of incorrect statement, dysfluencies, providing structure support, repetition, content units, elaborations of content units and irrelevancies. Results The rater consistence reliability was 1, 0.89, 0.85, 0.93, 0.92, 0.98, 0.98, 0.62, respectively. Conclusion The norms have acceptable reliability.
8.A three-dimensional finite element study of the temporomandibular joint with different curve of Spee.

OBJECTIVETo analyze the stress distribution in temporomandibular joint (TMJ) with different depth of curve of Spee (CS) by three-dimensional finite element model (3D-FEM).
METHODSFEM with different depth of CS were established. The depth was 2.50, 0 and 4.50 mm (model 0, 1, 2). The stress distribution were analyzed by software Abaqus. The change of stress were analyzed by paried t test.
RESULTSThe stress distribution patterns in TMJ with different depth of CS were similar. The Von Mises on the anterior surface of condyle and the Von Mises on the intermedial zone of the disc was higher. The S. Max. Principal of the condyle was mainly tensile stress. The S. Min. Principal of the condyle was mainly compressive stress. The S. Max. Principal of the disc was mainly tensile stress. The S.Min. Principal of the disc was mainly compressive stress.In model 0, the eviations of the Von Mises, the S.Max. Principal and the S.Min. Principal on the anterior surface of condyle were 0.1057 - 0.2298 MPa, -0.2243 - -0.3738 MPa and -0.3494 - -0.1165 MPa. The eviations of the Von Mises, the S.Max. Principal and the S. Min. Principal on the intermedial zone of the disc were 0.1099 - 0.6880 MPa, -0.3217 - 0 MPa and -1.2000 - -0.1000 MPa. The D-values of of Von Mises of condyle and disk between different models were extremely significant (P < 0.01).
CONCLUSIONSWith the same plane of occlusion, the stress distribution patterns of TMJ with different depth of CS is similar to that of normal occlusion. The final results also validate the anterior surface of condyle and intermediate zone of articular disc are the weight-bearing areas in TMJ. The changes of depth of CS change the value of the stress.

10.Cushing's ulcer in traumatic brain injury.

Traumatic brain injury (TBI) remains a complicated and urgent disease in our modernized cities. It becomes now a public health disease. We have got more and more patients in Neurosurgery Intensive Care Unit following motor vehicle accidents and others causes. TBI brings multiple disorders, from the primary injury to secondary injury. The body received the disturbances in the brain, in the hypothalamo-pituitary-adrenocortical (HPA) axis, in the gastric mucosa, in the immune and neuroendocrine systems. The mortality of TBI is more than 50 000 deaths / year, the third of the mortality of all injuries. Cushing ulcer is one of the severe complications of TBI and its mortality rate is more than 50%. Many studies have improved the management of TBI and the associated complications to give patients a better outcome. Furthers studies need to be done based on the similar methodology to clarify the different steps of the HPA axis and the neuroendocrine change associated. The aim of the present review is to assess the clinical and endocrinal features of hypopituitarism and stress ulcer following TBI.
