1.One case report of double hand allograft

Objective To evalute the feasibility of a double hand allogroft to reconstuct the limbs'function. Methods A brain dead donor was chosen under the test of blood group in ABO and Rh,HLA match,PRA and the mixed leukocyte culture.The level of the transplantated hands was 5 cm proximal to the wrists The procedure of the allotransplantation was similar to that of the autologous replantation.The immunosuppressants were taken two days before the operation,in the operation and after the operation. The circulation,the vital signs and the rejection were under the supervision. Results The progress was satisfactory. No rejection was found postoperation.After 10 weeks the callus in both ends of radius and ulna was obvious.After 5 months sensation recovered.After 7 months the internal fixation(plates and screws) was removed and tenolysis was done. Now the recipient could use engrafted hands to wash face,make the bed,wear clothes,brush teeth,peel oranges,eat with a spoon,tie the shoelace slowly,pick up a telephoneor and use the conroller of Television. Conclusions With the immunosuppressive agents,the double hands allograft can survive.The growth rate of bone and nerve is faster than that of autologous replantation.

3.Observation of pulmonary function in patients with diabetes mellitus

BACKGROUND: The morbidity of diabetes mellitus increased recently.There are few reports about the pulmonary function status of the patients with diabetes mellitus.OBJECTIVE: To comparably observe the difference of the pulmonary function status between patients with diabetes mellitus and normal persons,and to probe into the relationship between the course of disease of diabetes mellitus and the functional change of pulmonary function.DESIGN: Case controlled observation SETTING: Renmin Hospital, Wuhan University PARTICIPANTS: Totally 90 patients with diabetes mellitus hospitalized in the Renmin Hospital, Wuhan University between January 2000 and De cember 2003 were recruited. All the patients participated in the experiment voluntarily. Totally 20 cases were of insulin dependent diabetes mellitns (IDDM) and 70 cases were of non-insulin dependent diabetes mellitus (NIDDM). Another 90 cases were divided into 2 groups with the boundary line of 10 years: group with disease course <10 years (n=40) and group with disease course >10 years (n=50). And 30 normal persons with healthy pulmonary function was set as healthy control group, all the testees were voluntary participants in the experiment.METHODS: Pulmonary function index of the participants of each group was measured: vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1%), peak expiratory flow rate(PEFR)maximal voluntary ventilation (MVV) and comparison was performed between them.Comparison of the pulmonary function status of the patients between different levels of diabetes mellitus.pulmonary function between different groups : Compared with health control group, VC , FVC and MVV of the patients with diabetes mellitus was significantly decreased (t=1.999-2.301, P < 0.05).There was no significant difference in the VC, FVC and MVV between patients with monary function between patients with different diabetes mellitus: the pulmonary function status in the group with disease course >10 years was improved less than that in the group with disease course <10 years [ VC (L):2.62±0.65 vs 3.25±1.25;FVC(L): 2.40±0.52 vs 3.21±0.98;FEV1%:75.31±2.31 vs 80.63±5.56;PEFR (L/s):4.33±0.68 vs 5.98±1.02;MVV (L/s): 76.22±6.98 vs 89.21±5.69 (t=2.012-2.350, P < 0.05)].CONCLUSION: Pulmonary function of the patients with diabetes mellitus has reduced tendency compared with the normal persons, and the pulmonary function of the patients with long course of disease decreases significantly. More attention should be paid to diabetes patients' pulmonary function changes.
4.The role of SIRT1 in the treatment of diabetic nephropathy.

Diabetic nephropathy presents an increasing trend worldwide. It has been an attractive area to find novel targets for the treatment of diabetic nephropathy. SIRT1 (Sirtuin 1), a member of deacetylation enzymes, regulates cell senescence, metabolism, and apoptosis. In last ten years, lots of studies showed that SIRT1 exerts a protective effect in the progression of the diabetic nephropathy by promoting reconstruction of energy homeostasis, modulating cell redox state, resisting cell apoptosis, inhibiting inflammation and ameliorating renal fibrosis. SIRT1 has become a potential new target for the treatment of diabetic nephropathy.

5.Effect of Weide'an Tablet on Gastric Function and the Expressions of EGF and EGFR in Gastric Tissue of Model Rats with Stress-induced Gastric Injury

OBJECTIVE:To study the effects Weide'an tablet on gastric function and the expressions of epidermal growth fac-tor(EGF)and its receptor(EGFR)in gastric tissue of model rats with stress-induced gastric injury,and investigate its mechanism in the treatment of gastric ulcer. METHODS:Rats were randomly divided into normal group (normal saline),model group (nor-mal saline),positive group (ranitidine hydrochloride,0.015 mg/kg) and Weide'an tablet high-dose,medium-dose,low-dose groups (1.7,0.87,0.43 g/kg),10 in each group. Except for normal group,rats in other groups were given fasting swimming in cold water to induce gastric ulcer model. After modeling,all rats were intragastrically administrated once a day,for 2 weeks. The body mass of rats in 0,7,14 d of administration was recorded. After 12 h of last administration,gastric juice secretion,gastric juice pH,gastric ulcer area and gastric mucosal damage index of rats were detected,and the expressions of EGF and EGFR in gas-tric tissue were detected. RESULTS:Compared with normal group,body mass and gastric juice pH in 7,14 d in model group were declined;gastric juice secretion and gastric ulcer area were increased,gastric mucosal damage index was increased;and the expressions of EGF and EGFR in gastric tissue were enhanced,with statistical significances(P<0.05 or P<0.01). Compared with model group,except that the body mass,gastric juice secretion,gastric juice pH and gastric ulcer area in Weide'an medium-dose group in 7 d,and body mass,gastric juice secretion,gastric juice pH,gastric ulcer area and the expression of EGF in gastric tis-sue in Weide'an low-dose group in 7,14 d were not significantly improved,above indexes were significantly improved in other ad-ministration groups(P<0.05 or P<0.01);and the effect of Weide'an tablet showed a certain dose-effect relationship. CONCLU-SIONS:Weide'an tablet can significantly improve the gastric function of model rats with stress-induced gastric injury;and the mechanism may be related with enhancing the expressions of EGF and EGFR and promoting ulcer healing.
6.Cloning and Prokaryotic Expression of Transcriptional Co-activator Gene of Clonorchis sinensis and Functional Analysis of the Expressed Protein

Objective To construct prokaryotic recombinant plasmids of transcriptional co-activator (TC) gene of Clonorchis sinensis, express and purify the recombinant protein and analyze its biological function. Methods A pair of primers was designed according to the known sequence of TC gene. The TC gene fragment was amplified by PCR. After purification and digestion with BamHⅠ and SalⅠ , the TC gene was connected to the prokaryotic expression vectors, pGEX-4T-1 and pET30a(+). By cloning target gene into these vectors, pGEX-4T-1 and pET30a(+), prokaryotic recombinant plasmids of TC gene were constructed and transferred into E.coli BL21. The positive expressed recombinants were detected by SDS-PAGE and Western blotting. Immobilized metal (Ni 2+ ) chelation affinity chromatography was used to purify His-TC produced by the expression of the recombinant protein pET30a(+)-TC. Results The recombinant plasmids, pGEX-4T-1-TC and pET30a(+)-TC, were constructed successfully. SDS-PAGE testified that the molecular weight of the recombinant protein was correct. Western blot analysis of GST-TC recombinant protein testified that the recombinant protein could be recognized by immunized rabbit serum, which means the protein is GST-immune active and the clone can express recombinant Clonorchis sinensis antigen. After affinity chromatography of the pET-TC protein, there was only one protein band with expected size on the SDS-PAGE gel. Conclusion The TC gene was screened from cDNA library of adult Clonorchis sinensis, cloned, expressed and purified. The purified protein of TC gene will be of importance for further research on the biological function of the gene.
7.Application of breathing pattern parameters to extraction of REM sleep information

Objective To extract breathing pattern parameters during sleep and get the varying law of NREM and REM sleep stages. Method A newly designed respiratory inductive plethysmography (RIP) and a polysomnography (PSG) are utilized to record whole-night-sleep data simultaneously. The breathing pattern parameters obtained by RIP are dealt with according to the results of sleep stages and sleep apnea by PSG. Then the rule found out and summarized from the experiment is applied to distinguish REM sleep. Conclusion RC/VT can be used as an effective parameter to differentiate NREM and REM sleep. Using this parameter, the results of RIP totally accord with the results of PSG.
8.Acetabulum revision alone in 11 patients following total hip arthroplasty

Eleven patients underwent acetabulum revision using modified anterolateral approach due to acetabulum loosening but femoral prosthesis in sound fixation following total hip arthroplasty in Department of Joint Surgery,Tianjin Hospital between August 2002 and February 2005. During the mean follow-up of 4.2 years,all 11 patients gained primary stable fixation and good function. One patient underwent another revision due to infection; one developed femoral prosthesis loosening and underwent femoral revision. X-ray of the other patients showed no loosening or dislocation or radiotransparent region surrounding the prosthesis. One patient developed lateral femoral cutaneous nerve injury and relieved 6 months later.
9.Periprosthetic fractures during primary total hip arthroplasty for developmental dysplasia of the hip in 9 cases

From February 2002 to May 2007,9 cases(9 hips) of periprosthetic fractures during the primary total hip arthroplasty for developmental dysplasia of the hip were selected,including 1 male and 8 females aged 52-69 years.In the primary total hip arthroplasty,femoral prosthesis with the same size as intramedullary reamer and acetabular prosthesis with 2 mm diameter larger than acetabular reamer were used.The pain,function,range of motion and degree of deformity were accessed using the Harris score.The patients were followed for 1-6 years.Among the 9 patients with periprosthetic fractures,4 acetabular fractures were stable which were treated by additional augmentation screws,and 1 fracture was unstable which was treated by structural bone grafting and additional screws.And there was one Vancouver type AG fracture which was treated by cerclage wire,3 Vancouver B1 fractures which were fixed by locking compression plate(LCP) or cerclage wire,and one Vancouver type C fracture which was also fixed by LCP.No component loosening or migration was found in the postoperative X-ray.The mean Harris score was 87.2 at the final follow-up.The results of the study show that during the primary total hip arthroplasty in patients with developmental dysplasia of the hip,the oversize of the acetabular component should be controlled to equal to or less than 2 mm.For patients with severe osteoporosis,acetabular components with the same size to reamer are recommended combined with additional screws,or cemented cup.When an acetebular fracture is found during total hip arthroplasty,we could use additional screws or bone grafting.Once a periprosthetic femoral fracture occurs during the operation,methods of fixation should be selected based on the type of the fracture and stability of the prosthesis.
10.Clinical application of the combined the anterior malleolus flap and anterior tibia flap

Objective To investigate the clinical application of the combined the anterior malleolus flap and anterior tibia flap. Methods Based on the dissection of the perforating branches of anterior tibial artery on the middle and inferior section, the combined the anterior malleolus flap and anterior tibia flap was designed to repair the necrotic skin of anterior foot for 5 patients. The sizes of the flaps ranged from 17 cm×10 cm-10 cm× 5 cm. And the area of the flap was from tibial tuberosity(upper bound) to the line between internal malleolus and external malleolus (lower bound), and from the median line of one side of leg to the other side. Results Postoperatively, all flaps survived, and the primary healing of transplanted skin in donor site was achieved. The texture of flaps were excellent, the phenomenon of abrasion did not happen, and the clinical therapeutic efficacy was satisfactory after a follow up of 2-24 months. Conclusion It's a good method that combined the anterior malleohls flap and anterior tibia flap, which not only could enlarge the area of the flap but also has reliable blood supply, in repair of large size skin defect of anterior foot.
