Objective To summarize the clinical manifestation ,the main pathogenic bacteria distribution and drug resistance of neonatal community acquired sepsis late onset in our hospital .Methods Retrospectively analyse the clinical material of 122 cases (41 premature cases and 81 cases of full term) with neonatal community acquired sepsis late onset ,which were clinically diagnosed , from January 2009 to December 2012 in our hospital .Results The main clinical manifestation of neonatal community acquired sep-sis late onset was poor response(64 .7% ) ,repellent milk(57 .4% ) ,temperature changes(61 .5% ) ,and the respiratory tract and um-bilical region were the main infection ways .42 cases were checked out with pathogen in the 122 cases ,blood culture positive rate was 34 .4% ,and there was no statistically differences between the premature and the full term infant group ,In the 42 cases ,there were 29 cases with staphylococcus ,including 10 cases of staphylococcus aureus ,14 cases of coagulase negative staphylococcus and 5 cases of enterococcus ;and there were 10 cases are checked out with e .coli .All of the coccus detected were resistant to penicillin and erythromycin ,but sensitive to vancomycin ,teicoplanin ,linezolid .The e .coli was sensitive to amikacin ,imipenem ,meropenem ,and al-so had a higher sensitivity to cefazolin ,ceftriaxone ,cefepime ,cefoperazone and nitrofurantoin .Conclusion Blood culture positive rate is not high in neonatal community acquired sepsis late onset ,and its′clinical manifestations are nonspecific .The main pathogenic bacteria is coagulase negative staphylococcus ,staphylococcus aureus ,followed by escherichia coli .

Objective:The significant role of long non-coding RNAs (lncRNAs) in early diagnosis and predicting prognosis has been recognized in various cancers recently.However,the prognostic value of HOXA transcript at the distal tip (HOTTIP),a vital lncRNA in tumorigenesis,remains unclear.In this study,we evaluated its prognostic value by analyzing the correlation of HOTTIP expression with overall survival (OS),lymph node metastasis (LNM) and distant metastasis (DM) in different cancer types by meta-analysis.Methods:We performed a systematic search in PUBMED,MEDLINE,Web of Science and Cochrane Library update to November of 2016.A total of 604 patients from 6 studies were included in final analysis and went through a quantitative meta-analysis by Review manager 5.3.Results:We demonstrated that high expression of HOTTIP had a significant correlation with poor OS (hazard ratio [HR] =2.37,95％ confidence interval [CI] =1.81-3.10,p＜0.001),high LNM rate (odds ratio [OR]=2.29,95％CI=1.54-3.40,p＜0.001) as well as more DM occurrence (OR=3.30,95％CI=1.78-6.12,p＜0.001).Conclusion:Our results indicated that long non-coding RNA HOTTIP may serve as a potential prognostic biomarker in cancer progression.

Objective To analyze the effects of self-ligating bracket on enlarging the maxillary alveolar arch and to investigate the the non-extraction treatment programs.Methods All patients were used heat activated basic and precise theory for the orthodontist in order to make a plan fordeveloping nickel-titanium wire,then assessed by occlusalplane analysis method.The maxillary model andcephalometric radiographs were collected and measured before the treatment.The data of the resultswereanalyzed.Results After the treatment,the increase of the width of the dental arch was related to the /FMA,crowding degree and canines initial position.The increase of arch length was related to the ∠ANB,crowding degree,malocclusion classification,canines initial position andthe second molar eruption.Conclusion The essential factors include the crowding degree of arch,∠FMA,∠ANB,the angle of the anterior teeth,malocclusion classification,canines initial position andthe second molar eruptioncan affect the result of non-extraction quick self-ligating bracket appliance treatment on enlarging the maxillary alveolar.

Objective To study the effect of surgery combined with 125I radioactive particles and chemotherapy in advanced gastric antrum cancer. Methods The data of 45 patients with advanced gastric antrum cancer were collected. The nutritional status, quality of life and survival time of patients in surgery combined with 125I radioactive particles and chemotherapy group (group A, 23 cases) and chemotherapy group (group B, 22 cases) were compared. Results The average period that patients canˊt eat through the mouth in group A was 10.22 months, it was 6.32 months in group B, and the difference between group A and group B was statistically significant (t= 3.765, P= 0.001). There were statistically differences in quality of life scores between the two groups at 3rd month and 9th month [3rd months: (73.48 ±19.21) scores vs (59.09 ± 24.86) scores, t= 2.178, P= 0.035; 9th month: (42.17±14.55) scores vs (32.05±21.10) scores, t= 3.43, P=0.001]. There were statistically differences in nutritional status between the two groups at 3rd month and 9th month (χ2 = 9.846, P= 0.020; χ2 = 7.713, P= 0.049). At 9th months, 7 patients died in group A and 13 patients died in group B, and there was a difference between the two groups (χ2= 3.964, P= 0.046). The overall survival rates of 12 months in two groups had not difference (P= 0.119). Conclusion The treatment of surgery combined with 125I radioactive particles and chemotherapy can improve the nutritional status and quality of life in patients with advanced gastric antrum cancer, and extend life appropriately in one year.

Objective To study arterial distribution of rectus abdominis musculocutaneous flap and to evaluate whether it can be divided into several units for reconstruction. Methods The arteries of the rectus abdominis musculocutaneous flap were studied on 60 sides of cadavers by dissection and angiography. Results The superior epigastric artery (SEA) and the inferior epigastric artery (IEA) continued in a longitudinal direction. Most of their branches took on a typical spiral configuration and communicated with each other within muscle above the level of umbilicus. Many perforating arteries penetrated through the anterior rectus sheath to get to the overlying skin, but the highest concentration of major perforators were in the paraumbilical area. The inferior epigastric artery was more significant than superior epigastric artery in supplying the skin of the musculocutaneous flap. Based on thefstudies of the vascular anatomy of muscles, we could classify arterial distribution into 3 types: type Ⅰ (SEA 26.5 %, IEA 34.6%) revealed a single main intramuscular artery: type Ⅱ (SEA 64.7 %, IEA 48.1%) had two major intramuscular branches; type Ⅲ (SEA 8.8%, IEA 17.3 %) revealed three intramuscular branches. Our anatomic studies showed that the superior and inferior epigastric artery bifurcated or divided into more than two main branches in the majority of cases (SEA 73.5%, IEA 65.4%). Conclusion The rectus abdominis musculocutaneous flap could often be divided into several regions for breast construction which is based on the distribution of each branch of the artery.

Objective To investigate the role of protein kinase C (PKC) in induction of vascular endothelial growth factor (VEGF) secretion by isoflurane in primary cultured rat cardiomyocytes. Methods Primary cultured neonatal rat cardiomyocytes were randomly divided into 6 groups ( n = 6 each): control group (group C), 3 different concentration isoflurane groups (group Ⅰ1-3 ), PKC inhibitor calphostin C group (group P), and PKC inhibitor + isoflurane group (group PI). The cells were exposed to 0.7%, 1.4% and 2.1% isoflurane for6 h in group Ⅰ1-3 respectivly. Calphostin C was added to the culture medium with a final concentration of 50 nmol/L in group P. Calphostin C was added to the culture medium with a final concentration of 50 nmol/L, then the cells were exposed to 1.4% isoflurane for 6 h in group PI. VEGF concentrations and expression of PKC isoforms were determined by ELISA and Western blot respectively. Results Compared with group C, the VEGF concentration was significantly increased in group Ⅰ2 and Ⅰ3, and PKCε expression was down-regulated in the cytoplasm while upregulated in the cytomembrane in group Ⅰ2 ( P ＜ 0.01 ), but no significant change was found in the parameters mentioned above in group Ⅰ2 ( P ＞ 0.05). PKCα, PKCδ and PKCζ expression was significantly higher in the cytoplasm than in the cytomembrane in group C and Ⅰ2. VEGF concentrations were gradually increased with the increase in isoflurane concentrations ( P ＜ 0.05). VEGF concentrations were significantly lower in group PI than in Ⅰ2 ( P ＜0.05) .Conclusion Isoflurane induces VEGF secretion in primary cultured rat cardiomyocytes through translocation of PKCε from the cytoplasm to the cytomembrane, suggesting that it is a mechanism of the cardioprotective effects of isoflurane.

Objective To explore any changes in surface electromyogram (sEMG) signals from the lumboabdominal muscles during exercise on stable and unstable surfaces. Methods sEMG signals from the lumbo-abdominal muscles of 33 healthy young persons [18 male and 12 female; average age (26.5 ± 4.3 ) years] were measured with the FlexComp Infiniti apparatus.Each subject performed 5 exercises on and off a Swiss ball:sit,bridge,bridge with both knees flexed,reverse bridge as well as press-up. Results ①In bridging there was a significant increase in the activation of the erector spinae during exercise on the ball compared with on the stable surface.②Bridging with both knees flexed gave a significant increase in activations of the erector spinae,the external obliques and the transverses abdominus/internal obliques during exercise on the unstable surface compared with the stable surface.③During reverse bridging there was a significant increase in activation of the erector spinae and rectus abdominus during exercise on the unstable surface compared with the stable surface.④During press-ups there was a significant increase in activation of the rectus abdominus,the external obliques and the transverses abdominus/internal obliques during exercise on the ball compared with the stable surface. Conclusion The unstable surface provides better training stimulus for the activation of the lumbo-abdominal muscles.

Objective To compare the long-term outcomes in patients with newly diagnosed stage T1G3 bladder cancer treated with bladder preserving approach and intravesical instillation or im-mediate cystectomy.Methods of 113 patients with a median age of 64 years (range 27 to 88) diag-nosed with T1G3 bladder cancer from January 1993 to February 2007,81 cases were treated by tran-sureteral resection with additional intravesieal instillation and 32 were treated with immediate cystecto-my.Differences between the 2 groups in 5-year overall survival and tumor specific survival were calcu-lated using the Kaplan-Meier survival function and analyzed by the log rank test.Results of 81 pa-tients treated with organ preserving approach and postoperative intravesical instillation,53 patients developed local recurrence and 21 patients underwent deferred cysteetomy in a median 64 (range 6-140) months follow-up.The overall and tumor specific survival at 5 years was 64.2% (52/81) and 77.8%(63/81),and in those who had deferred cystectomy it was 61.9% (13/21) and 76.2% (16/21),respectively.Of the 32 patients treated with immediate cystectomy,the 5-year overall and tumor specific survival was 59.4%(19/32) and 75.0%(24/32) within a median follow-up of 62(range 4-141)months.There was no statistical difference of the 5-year overall and tumor specific survival be-tween patients treated with bladder preserving approach or immediate cystectomy.Conclusion Blad-der preserving approach and immediate eystectomy might have similar 5-year overall and tumor specific survival for primary T1G3 bladder cancers.

Objective To investigate the diagnosis and surgical treatment of intracaval venous tumors. Methods Clinical data of 6 cases were retrospectively analyzed, including signs and symptoms diagnostic means such as type-B ultrasound, CTA, MRA, surgical procedures and prognosis. Results All six cases received type-B ultrasonic examination, final definite diagnosis was achieved by CTA exam in 2 cases and through MRA in 4 cases. Heart involvement was found in 3 cases. All patients underwent a surgery. According to the extent of the tumor,3 cases had thoraco-abdominal incision,3 cases with extracorporeal circulation and right atrium opening. All of the tumors were completely resected. Pathological exam revealed that 4 cases were of leiomyomatosis and 2 cases were of leiomyosarcoma. One case with leiomyosarcoma died of liver disfunction postoperatively.The other 5 cases recovered without major complications. An average 51 months of follow-up found no recurrence. Conclusions CT and MRI are the mainstay for the diagnosis,and MRI can provide clear anatomy image to the surgeons, help choose the surgical procedures. The one-stage operation is effective. During the operation, the main branches of the vena cava system should be detected, and the attachment of the tumor should be found and removed thoroughly to prevent the recurrence of the tumor. When the attachment point is lower than the iliac vein level, ligation of the involved iliac vein should be mandatory.
