1.Efficacy and safety of triple drug fixed-dose combination of Telmisartan, Amlodipine and Hydrochlorothiazide in the management of hypertension.

Hypertension is a major health problem in India. Different clinical studies have reported that reducing the blood pressure can substantially decrease cardiovascular risk and all cause mortality. This study was conducted to evaluate the efficacy and safety of triple drug fixed dose combination of Telmisartan 40 mg, Amlodipine 5 mg and Hydrochlorthiazide 12.5mg. 41 hypertensive patients having systolic blood pressure ≥ 160 mmHg and diastolic blood pressure ≥100 mmHg who were uncontrolled on dual drug therapy with Telmisartan-Amlodipine or Telmisartan-Hydrochlorothiazide combinations were enrolled in this study. The treatment period was of 120 days and patients were administered once daily fixed dose combination of Telmisartan 40 mg, Amlodipine 5 mg and Hydrochlorthiazide 12.5mg. Patients were evaluated on 15th, 30th, 60th and 120th days of treatment. There was statistically significant (p<0.0001) decrease in systolic and diastolic blood pressure from baseline to 15th, 30th, 60th and 120th days of treatment. At the end of the study period of 120 days 95.6% & 94.4% patients of age group >60 years and <60 years achieved the JNC VIII recommended target goal respectively. This triple drug fixed dose combination of Telmisartan, Amlodipine and hydrochlorothiazide was found to be effective and safe option for the optimal management of hypertension without any safety concern.

Pim-1 belongs to a family of serine/threonine protein kinases that are highly conserved through evolution in multicellular organisms. Originally identified from moloney murine leukemia virus (MuLV)-induced T-cell lymphomas in mice, Pim-1 kinase is involved in the control of cell growth, differentiation and apoptosis. Expression of Pim-1 kinase can be stimulated by a variety of growth factors and regulated at four different levels: transcriptional, post-transcriptional, translational and post-translational. Several signal transduction pathways may be associated with the regulation of Pim-1's expression; accumulating data support that the expression of Pim-1 protein is mediated through activation of JAK/STATs. Recent studies of Pim family kinases indicate that Pim-1 kinase plays important roles outside of the hematopoietic system as well.

3.Incremental Role of ¹⁸FDG PET/CT in Assessment of Testicular Viability

Testicular torsion is a common differential diagnosis of acute scrotal pain along with acute epididymo-orchitis, which may lead to testicular non-viability. Doppler ultrasound and testicular scintigraphy are two routinely used modalities for the assessment of testicular viability. However, in some cases, these investigations may prove inadequate in differentiating between the two entities with widely differing management. Here, we present a case of a 52-year-old male with questionable viability of testis, who was investigated initially using testicular scintigraphy and was further subjected to a regional 18F-FDG PET/CT scan, in view of inconclusive findings with the conventional modalities.
