1.Effect of Bezafibrate(Bezalip(R))in Patients with Hyperlipidemia.

41 patients with hyperlipidemia(Serum cholesterol > or =260mg% or triglyceride> or =200mg%) were treated for 12 weeks each with either Bezafibrate(200mg t.i.d.)(n=23) or placebo(n=18) in a randomized trial. Compared with placebo, Bezafibrate was more effective in lowing total cholesterol(287+/-63 to 244+/-32 mg%, P<0.01), LDL-cholesterol(183+/-39 to 153 +/-25mg%, P<0.01) and the LDL/HDL ratio(4.7+/-2.9 to 3.4+/-1.3, P<0.05). There was also a marked reduction in serum triglyceride(307+/-295 to 218+/-167mg%, P<0.05) as well as a significant increase in HDL-cholesterol(43+/-12 to 48+/-12mg%, P<0.05). Compliance or tolerance to both Bezafibrate and placebo was good. No significant side effects except one case, who complained of mild indigestion, were observed. Therefore, Bezafibrate seemed to be effective in lowing serum total cholesterol, LDL-cholestrol, triglyceride and in increasing in HDL-cholesterol without significant side effect.

2.Subcutaneous Metastasis at the PTBD Catheter Insertion Site in Bile-duct Carcinomas: Report of two cases.

Percutaneous transhepatic biliary drainage (PTBD) is the basis for most biliary interventional pro cedures, and the application of preoperative PTBD to patients who have malignant obstructive jaundice has a significant effect on overall morbidity, mortality, and patient survival by allowing selective application of the most appropriate therapeutic modality. There are few case reports on subcutaneous metastasis of bile-duct cancer. We recently observed a subcutaneous metastasis at the catheter insertion site after PTBD in two patients with bile-duct cancer. Although tumor-cell seeding along the catheter tract is a very rare complication, we think that PTBD requires more attention when a curative resection is planned.

3.Hematopoietic Recovery of Peripheral Blood Stem Cells Stored at 4degrees C.

PURPOSE: Cryopreservation has been the standard method of storing hematopoietic cells for the past 20 years, but this prdegrees Cedure is laborious and expensive. So, we evaluated the hematopoietic recovery of stored PBSCs at 4degrees C for a variable storage period MATERIALS AND METHODS: Eight leukapheresis products were kept unprdegrees Cessed at 4degrees C for 96 hours. To evaluate the effect of storage period on the hematopoietic recovery of PBSCs, assays for viability of mononuclear cells (MNCs), CFU-GM colony counts and CD34 cell counts were performed every 24 hours after PBSC collection. We tried to compare hematopoetic recovery of stored PBSCs at 4degrees C with that of cryopreserved PBSCs by using repeated measures ANOVA. RESULTS: Viability of MNCs, CFU-GM colony counts and CD34 cell counts were monitored at 24 hour, 48 hour, 72 hour and 96 hour after PBSC collection. Data are expressed as percentage of baseline value and shown as mean s.d.; MNCs viability (96+/-2%, 94+/-2%, 92+/-2%, 88+/- 3%), CFU-GM colony counts (87+/-10%, 79+/-11%, 65+/-13%, 56+/-15%), and CD34 cell counts (93+/-13%, 93+/-12%, 88+/-14%, 85+/-19%). After storing PBSCs at 4degrees C for 96 hours, viability of MNCs and CFU-GM colony counts were significantly reduced (p<0.05) except CD34 cell concentration (p>0.05). Prdegrees Cedures of controlled-rate freezing and thawing resulted in a notable loss of viability (77+/-9%) and CFU-GM colony count (71+/-29%). CFU-GM colony counts of 72 hour-stored PBSCs at 4degrees C was similar to those of cryopreserved PBSCs. CONCLUSION: If G-CSF mobilized PBSCs are stored at 4degrees C in less than 72 hours after collection, those hematopoietic recovery would be comparable to that of cryopreserved stem cells which are achieved by the rate-control freezer.

4.The Clinical Experiences of Ultrasound-Assisted Lipo plasty.

Suction-assisted lipoplasty is now flrequently-performed pre cedure in plastic surgery, but it has several drawback including bleeding contour irregularity due to its traumatic nature. The recently introduced ultrasound-assisted lipoplasty (UAL) technique uses ultrasonic energy which has specificity in lower density tissue like fat tissue. Therefore, UAL can minimize these compli cations. We performed UAL in 21 patients from OCt. 1988 to Jun 1999. There were 17 females and 4 males, and patient age ranged from 18 to 52 years(average 40 years). A total of 58 areas were operated on for an averge of 28 areas per patient. We used a fivestage technique consisting of tumescent infiltration, ultrasonund treatment, emulsion suction, endermology, and postperative pressuregarment application. Total volume (fluid and fat) removed ranged from 200 to 4,050 cc(averge 1,750 cc) per patient and the lipocrit within the aspirate was 4-8%, which was significantly lower compared with traditional liposuction Residual emulsion was evacuated by endermology and pressuregarment was applied to all patients for postoperative 2-3 months. Pestoperative complications were seroma, induration, and paresthesia, but all these problems resolved spontaneously within a month. One patient required secondary UAL for correction of excess fat at the medial knee. We believe that UAL is a safe and excellent technology in liposuction because of reduced surgical bleeding and good contral of body contour.

5.Comparative Study of the Manual and Stapled Anastomosis in Patients Undergoing a Billroth I Gastrectomy for Carcinoma of the Stomach.

BACKGROUND : The purpose of this study was to determine whether the Billroth I method using an EEA stapler is safe, reliable, and easy-to-use for treatment of gastric cancer compared with a distal gastrectomy with conventional manual anastomosis. METHODS : A prospective comparative study was performed between forty patients with EEA stapling and thirty patients with manual suturing during gastroduodenostomies performed during the past three years with respect to operation time, diameter of anastomosis, postoperative clinical course, and post operative complications. RESULTS : The operation time was significantly shortened by about 50 minutes on the average, in the cases where the EEA stapler was used (p<0.001). On postoperative hypotonic duodenography, the diameter of the anastomosis was significantly larger in the cases where the EEA stapler was used (p< 0.001). However, the postoperative clinical course showed no difference between the two groups. Complications, such as anastomotic stenosis and anastomotic leak age were noted only in the cases where manual suturing was used. CONCLUSIONS : A gastroduodenostomy using the EEA stapler might be a faster, simpler and safer pro cedure compared with the gastroduodenostomy using the conventional manual anastomosis.

6.A Case of Bezafibrate-induced Myopathy.

One case of bezafibrate-induced myopathy was experienced and presente with a brief review of literatures. A 80-year-old woman with diabetes mellitus developed generalized myalgia and chest tightness 2 days before admission. The patient was treated with bezafibrate (800mg/day) for the previous 3 months. Serum creatine kinase, LDH and AST were markedly increased. Muscle biopsy revealed type 2 fiber atrophy. Withdrawal of the drug under the impression of bezafibrate-induced myopathy was followed by rapid clinical improvement.

7.Assessment of Right Ventricular Function During Orthotopic Liver Transplantation.

BACKGROUND: Patients undergoing orthotopic liver transplantation(OLT) may develop significant hemodynamic instability. This study was performed to assess the role of right ventricular function in the hemodynamic alteration during 23 cases of OLT. METHODS: A thermodilution ejection fraction catheter was used to measure ejection fraction(EFrv), allowing for calculation of right ventricular(RV) end-diastolic volume index(EDVIrv), end-systolic volume index(ESVIrv), stroke volume index(SVIrv). RV stroke work index(SWIrv), maximum elastance (Emaxrv) and performance index(PIrv) as the functions of contractility were also calculated. Those RV hemodynamic measures were taken during preanhepatic phase(stage I), anhepatic phase (stage II), after reperfusion of the grafted liver and postanhepatic phase (stage III). All of measures in each surgical stage were statistically analyzed for their differences by repeated measured ANOVA. And correlation between changes from baseline of RV hemodynamic variables was determined by polynomial regression analysis. RESULTS: EFrv, SVIrv, SWIrv appeared to be well preserved throughout the prdegrees Cedure during stage I, II, III and much higher 5 min after reperfusion. No correlation was observed between right atrial pressure(Pra) and EDVIrv. There were significant correlation between EDVIrv and SVIrv, SWIrv and EFrv, Emaxrv and EFrv. CONCLUSION: RV function was well preserved during uncomplicated OLT using venovenous bypass. EDVIrv were more reliable determinants of RV preload than Pra for assessing RV contractility under conditions of this operation.

8.A Clinical Study on the Effect of Bezafibrate in Hyperlipidemia.

The lipid lowering effects of bezafibrate were evaluated in 18 patients with hyperlipidemia, i.e. over 200mg/dl of cholesterol and/or of cholesterol and/or triglyceride before treatment. The materials were 18 patients(male:8, female:10);7 with diabetes mellitus, 8 with congestive cardiomyopathy, 1 with parkinsonism, The serum lipid concents were measured with enzyme method before and 6 weeks after the treatment with daily 600mg of bezafibrate administered per oral and the differences were observed. 1) The level of serum cholesterol was reduced from 262.8mg/dl(+/-58.9SD) before treatment to 191.3gm/dl(+/-31.3SD) after 6 weeks treatment(p<03.005). 2) The level of serum triglyceride was reduced from 231.8mg/dl(+/-104SD) before treatment to 144.4mg/dl(+/-51SD) after 6 weeks treatment(p<0.005). 3) The level of serum HDL increased from 47.2mg/dl(+/-19.7SD) before treatment to 61.3mg/dl(+/-13.9SD) after 6 weeks treatment(p<0.005). 4) The level of serum LDL was reduced from 169.3mg/dl(+/-52.4SD) before treatment to 101.2mg/dl(+/-29.5SD) after 6 weeks treatment(p<0.05). 5) HDL/Cholesterol ratio was increased from 17.8%(+/-5.4SD) before treatment to 32.6%(+/-7.9SD) after 6 weeks treatment(p<0.005). 6) HDL/LDL ratio was increased from 29.2%(+/-10.8SD) before treatment to 67.2%(+/-30.8SD) after 6 weeks treatment(<0.005). The side effects of bezafibrate such as G-I trouble, myositis, hypersensitivity, elevation of BUN and creatinine were not observed.

9.A Case of Bezafibrate Induced Rhabdomyolysis

The case is presented a 49-year-old man had several year history with chronic renal failure with hyperlipidemia due to diabetes mellitus. Treatment of hyperlipidemia was started by oral bezafibrate intake 600 mg per day. Several days later, patient noticed muscle weakness and myalgia. The serum CK, LDH, AST levels were remarkably elevated, myoglobulinuria was also noticed The symptoms of the patient were resolved after the drug was discontinued, thus the diagnosis was established as having bezafibrate induced rhabdomyolysis. On the basis of the above description, bezafibrate may induce muscle damage if dose is excess over the renal capacity. Extreme caution is warranted when the patient is placed on bezafibrate and has renal dysfunction.Strict dose adjustment is necessary in taking account of renal function to avoid muscle damage including rhabdomyolysis.

10.Long Term Surgical Efficacy of Endonasal Dacryo cys torhinostomy.

Primary success rate of endonasal dacryocystorhinostomy has been reported lower than that of conventional cedure. Gradual stenosis of mucosal ostium would progress until 6 to 10 months following operation and a longterm follow-up should be necessary. Endonasal dacryocystorhinostomy was performed in 100 patients[118 eyes]between July 1993 and March 1998 at the Department of Ophthalmology, Yeungnam University College of Medicine. After the follow-up period of 1 to 4 years[average 16.2 months], longterm success rate, postoperative omplications, and factors to affect success rate were studied. The success rate of primary operation was 89.83%, and the success rate after secondary revision of the ostium was up to 99.15%. Granulation tissue formation was the most common postoperative complication, and other complications included protrusion of silicone tube, membranous obstruction, partial obstruction of common canaliculus, canaliculitis and prolapse of orbital fat. Factors to obtain good success rate were careful and acurate surgical technique to achieve large mucosal ostium, postoperative management, antibiotics and steroid medication, etc. Long-term surgical efficacy of endonasal dacryocystorhinostomy was similar to skin approach and secondary success rate following revision was very satisfactory.
