1.Low-grade Uterine Endometrial Stromal Sarcoma Resembling Ovarian Sex-Cord Tumor : A case report.

A case of low-grade endometrial stromal sarcoma resembling ovarian sex-cord tumor in the uterus of a 43-year-old woman is described. This tumor belongs to the group II category of uterine tumors resembling ovarian sex-cord tumor described by Clement and Scully, and the epithelial-like elements show prominent smooth muscle differentiation, proved by immunoreactivity for desmin and actin. The patient did not receive any adjuvant therapy; she is alive and well without recurrence 8 months postoperatively.

2.Glaucomatocyclitic Crisis.

The glaucomatocyclitic crisis was first described by Posner and Schlossman in 1948, and belongs to the classification of secondary open angle glaucoma. It is unilateral and recurrent. and characterized by minimal inflammatory signs and symptoms. The inflammation may be confined to the trabecular mesh work. The facility of outflow reduced while attacks last from a few hours to over 2 weeks. Etiopathogenic nature of glaucomatocyclitic crisis is not clear but recently the role of allergy is suggested in this disease entity. A 35 year old man was found to have a glaucomatocyclitic crisis in his right eye that was treated with systemic diamox and topical corticosteroid.

3.A Case of Dilated Cardiomyopathy Associated with Pheochromocytoma.

The diagnosis of dilated cardiomyopathy associated with pheochromocytoma was made in a 24 year old male on the basis of symptoms and signs, measurements of metabolites of catecholamine,echocardiography and radionuclide ventriculography were preformed and demonstrated. The presence of a tumor on both adrenal glands without definite distant metastasis was demonstrated by abdominal ultrasonography, CT scanning and 131I-MIBC scintigraphy. Surgical removal was performed after proper preoperative preparation with a-adrenergic blocker. During the surgery, neither significant arrhythmia nor severe change of blood pressure was observed. After surgery, cardiac function of the patient improved slowly and progressively.

5.Detection of methicillin or vancomycin-resistant Staphylococcus aureus from dental hospital.

The purpose of this study was to obtain the basic information for the improvement of dental environment by investigating the presence of methicillin- or vancomycin-resistant Staphylococcus aureus (MRSA or VRSA) isolated from dental health care workers (DHCWs) and environment of the Chosun University Dental Hospital (CUDH) and a private dental clinic (control group). Staphylococcus aureus (S. aureus) was isolated from anterior nares of 42 DHCWs and 38 sites, unit chairs, x-ray devices, computers, etc., at 10 departments of the CUDH and 20 DHCWs and 11 sites at the private dental clinic. S. aureus was isolated on mannitol salt agar plate and confirmed by PCR with S. aureus species-specific primer. Antimicrobial susceptibility test of clinical isolates of S. aureus against several antibiotics including methicillin (oxacillin) was performed by investigating minimum inhibitory concentration (MIC) using broth microdilution assay. In addition, PCR was performed to detect the methicillin- or vancomycin-resistant gene. The data showed that one strain of S. aureus was isolated from DHCWs of the CUDH and three strains of S. aureus was isolated from 3 samples of the private dental clinic, respectively. All of the isolates from the CUDH and the private dental clinic had resistance to penicillin G, amoxicillin and vancomycin and susceptibility to oxacillin and ciprofloxacin. The S. aureus strains were already obtained the resistance to penicillin G and amoxicillin. These results suggest that two dental clinics were under relatively safe environment.

8.Long-term Follow-up Results of Anterior Cervical Interbody Fusion with and without Cervical Plate.

Eighty-four patients whose cervical lesions not associated with any trauma or tumor were operated for anterior cervical interbody fusion. The patients were separated into two groups, one group operated with a cervical plate and the other group, without the cervical plate. The mean follow-up duration in anterior cervical interbody fusion with cervical plate cases was 16 months(from 6 months to 30 months), without cervical plate 35 months(from 6 months to 84 months). Retrospective analysis of the two groups were done in order to compare the incidence of postoperative complications, clinical outcome, and radiological changes. The most notable postoperative complications of the group operated without the cervical plate were migration of the bone graft(3.3%) and angulation(3.3%), whereas of the group operated with the cervical plate were screw loosening(4.2%) and infection of the operative wound(4.2%). Comparative analysis of the postoperative clinical outcome and changes in radiological measurement of the intervertebral heights and angles showed no statistically significant difference between the two groups. The results of this study suggest that whether or not the cervical plate should be used for anterior cervical interbody fusions requires a decision made carefully and meticulously by the operator, so that the possibility of any postoperative complications can be minimized.

9.Surgical Treatment of Syringomyelia Secondary to Tuberculous Meningitis: Report of 4 Cases.

The authors report our recent experience with 4 cases of syringomyelia that had developed secondary to tuberculous meningitis. Three cases of syringomyelia were treated by syringoperitoneal shunt and 1 case, by syringosubarachnoid shunt. Postoperatively, upper and lower extremity pain that had persisted in all the patients was relieved in all the patients, but improvement in limb sensation was noted in only 1 case. Improvement in limb motor power was noted in 3 cases. The clinical presentation, surgical consideration, and pathogenesis of syringomyelia is discussed together with a review of the literature.

10.Uterine Arterial Embolization for the Treatment of Leiomyomas Accompanying with Adenomyosis.

OBJECTIVE: The purpose of this study was to make a guideline of uterine artery embolization for the treatment of uterine leiomyomas accompanying with adenomyosis in Korea. MATERIALS AND METHODS: We performed the retrospective study for 37 women who had uterine leiomyomas accompanying with adenomyosis. Bilateral uterine artery embolization was performed in 37 patients (age range 25-65) during 17 months with pain, hypermenorrhea, urinary frequency etc due to leiomyomas. Ultrasound imaging was performed before the procedure and at mean 6.9 months after the procedure. RESULTS: All procedures were technically successful. Mean clinical follow-up was 12.8 months. Minor complication occurred in 82% patients after the procedure. After imaging follow-up (mean, 6.9 months postprocedure), median uterine volume decreased 34.4%, and dominant myoma volume decreased 86%. There was no statistical difference in uterine volume reduction and dominant myoma size reduction whether occluding agents was polyvinyl alcohol, polyvinyl alcohol plus gelfoam, and gelfoam, and whether ultrasound measured Resistance Index value before the procedure was low or high. CONCLUSION: Primary candidates for uterine artery embolization include those with symptomatic uterine leiomyomas who no longer desire fertility but wish to avoid surgery or are poor surgical risks. To our study, uterine volume reduction and dominant myoma size reduction in patients who had adenomyosis were similar to previous other studies in patients who had not adenomyosis. Therefore adenomyosis should not be considered as a contraindication for uterine artery embolization. Because there is little data about subsequent reproductive potential after this procedure, it should not be routinely advocated for infertile women. Further investigation is warranted for occluding agents and Resistance Index.
