1.Cryotherapy for retinopathy of incontinentia pigmenti.

A case with typical skin lesions of incontinentia pigmeti showed progressing retinal vascular abnormalities. So cryotherapy was done on avascular peripheral retina, and retinal vascular changes ceased. We propose that cryotherapy may be a good treatment method for progressing retinal vascular lesions of incontinentia pigmenti.

2.Clinical Aspects of Transient Myopia after Blunt Eye Trauma

Purpose:
To investigate the clinical aspects of transient myopia after blunt eye trauma.
Methods:
The medical records of 32 patients treated at our hospital for from 2020 to 2022 traumatic microhyphema were analyzed retrospectively. The visual acuity, intraocular pressure, anterior chamber cells, anterior chamber depth, axial length, lens thickness, and refractive index were analyzed immediately after injury and after treatment. Patients with additional diseases such as vitreous and retinal hemorrhage, traumatic retinal detachment, iridodialysis, or orbital fracture were excluded. Correlations between the myopic changes and the amount of myopic refractive change caused by trauma were analyzed.
Results:
The 32 patients included 24 (75%) males and 8 (25%) females. The average myopic change was -3.03 ± 0.92 (range -1.00 to -5.25) diopters (D). The average change in anterior chamber depth due to trauma was -0.22 ± 0.11 mm, the change in lens diameter was 0.20 ± 0.09 mm, and the change in axial length was -0.07 ± 0.05 mm. The average change in refractive index due to ciliary spasm was -1.63 ± 0.65 D. The correlations between the change in refractive index and anterior chamber depth (r = 0.475, p = 0.008), traumatic ciliary spasm (r = 0.649, p < 0.001), and lens thickness (r = -0.184, p = 0.330) were determined.
Conclusions
Factors such as ciliary spasm, change in the anterior chamber depth, anterior shift of the lens-iris diaphragm, and increased lens thickness affect the myopic changes caused by trauma. Of these, a shallower anterior chamber depth and ciliary spasm are highly correlated with the transient myopic changes.

5.Effect of machining precision of single ceramic restorations on the marginal and internal fit

Purpose:
The purpose of this study was to evaluate the machining precision and the marginal and internal fit of single restorations fabricated with three types of lithium disilicate ceramic blocks and to evaluate the correlation.
Materials and methods:
Single restorations were designed using a CAD software program. The crown designed model file was extracted from the CAD software program. Three types of lithium disilicate blocks (Rosetta; HASS, IPS e.max CAD; Ivoclar vivadent, VITA Suprinity; VITA) were milled using a milling machine. For the fabrication of the crown scanned model file, the intaglio surface of the restoration was digitized using a contact scanner. Then, using the three-dimensional inspection software (Geomagic control X; 3D Systems), the process of the overlap of the crown designed model and the scanned model and 3-dimensional analysis was conducted. In addition, the marginal and internal fit of the crowns was evaluated by a silicone replication method. The difference among three types of single ceramic crown was analyzed using a Kruskal-Wallis H test, and Spearman correlation analysis was performed to analyze the correlation between machining precision and fitness (α=.05)
Results:
There was a significant difference in the machining precision and the marginal and internal fit according to the type of ceramic block (P<.001). In addition, the machining precision and the marginal and internal fit were positively correlated (P<.001).
Conclusion
The marginal fit of crowns fabricated according to the types of ceramic blocks was within the clinically acceptable range (< 120 µm), so it can be regarded as appropriate machining precision applicable to all clinical as aspects in terms of the marginal fit.
6.Evaluating Accuracy according to the Evaluator and Equipment Using Electronic Apex Locators

Purpose:
Using two types of electronic apex locators, this study aimed to investigate the differences in accuracy according to the evaluator and equipment.
Materials and Methods:
Artificial teeth of the lower first premolars and two mandibular acrylic models (A and B) were used in this study. In the artificial teeth, the pulp chamber was opened and the access cavity was prepared.Using calibrated digital Vernier calipers, the distance from the top of the cavity and the root apex was measured to assess the actual distance between two artificial teeth. The evaluation was conducted by 20 dentists, and each evaluator repeated measurements for each electronic apex locator five times. The difference between the actual distance from the top of the cavity to the root apex and the distance measured using electronic measuring equipment was compared. For statistical analysis, the Friedman test the Mann–Whitney U-test were conducted and the differences between groups were analyzed (α=0.05).Result: As for the accuracy of measurement according to the two types of electronic apex locators, the value of the measurement error was 0.4753 mm in Dentaport ZX and 0.3321 mm in E-Cube Plus. Moreover, electronic apex locators Dentaport ZX and E-Cube Plus showed statistically significant differences (P<0.05). As for the difference in the accuracy of the two types of electronic apex locators according to the evaluator, the resulting values differed depending on the evaluator and showed a statistically significant difference (P<0.001).
Conclusion
Electronic apex locator E-Cube Plus showed higher accuracy than did Dentaport ZX. Nevertheless, both types of electronic apex locators showed 100% accuracy in finding the region within root apex ±0.5 mm zone. Fur-thermore, according to the evaluator, the two electronic apex locators showed different resulting values.

8.Parkinsonism in corticobasal syndrome may not be primarily due to presynaptic dopaminergic deficiency

The clinical features of corticobasal degeneration (CBD) are quite asymmetric. The severity of
clinical symptoms and dopamine transporter (DAT) bindings were less correlated compared to other
parkinsonisms, suggesting that presynaptic nigrostriatal dopaminergic dysfunction may not explain
extrapyramidal manifestations in CBD. Therefore we wanted to reexamine asymmetry and severity
between DAT imaging and clinical findings. We studied patients meeting the diagnostic criteria for
CBD based on clinical features. We collected their clinical information and imaging retrospectively.
Seven patients were enrolled and all had asymmetric rigidity, bradykinesia and unilateral limb dystonia.
These symptoms did not improve with levodopa. All patients showed symptoms bilaterally in the last
visit, but asymmetry of clinical symptoms was remarkable at the time of DAT imaging. The DAT
bindings were decreased in six subjects. However, one patient showed normal DAT binding. Four
patients had a more evident DAT reduction on the side contralateral to the more clinically affected
side, however, two patients had a more prominent reduction on the ipsilateral side. The symptoms
that we regard as parkinsonian features in CBD are not only explained by presynaptic dopaminergic
dysfunction. Our findings suggest that postsynaptic dopaminergic or nondopaminergic systems may
play a major role in parkinsonian symptoms in corticobasal syndrome.

10.Ureteral Penetration Caused by Drilling During Internal Pelvic Bone Fixation: Delayed Recognition.

A 49-year-old man was referred to our department with profuse serous fluid discharge from a Penrose drain after undergoing internal fixation with metal screws for multiple pelvic bone fractures. A definite ureteral penetration was identified that was orientated from the lateral to the medial aspect of the right distal ureter. The patient was surgically treated with excision of the 2-cm injured ureteral segment, end-to-end ureteroureterostomy, and double J ureteral stent placement. To our knowledge, a penetrating ureteral injury caused by bone drilling has not been reported previously in the published literature. This case shows that surgeons who do pelvic surgery, including orthopedic surgeons, should be familiar with the anatomical relationship of the ureter and its potential injuries.
