2.Two cases of trichofolliculoma.

Trichofolliculoma is an adnexal tumor of hair follicle oriyin, considered to be of intermediate differentiation between a hair follicle nevus, a simple hyperolasia of the hair follicle, and trichoepithelioma, Which usually lacks mature hair follicles. Grossly the tumor usually appears on the head or neck as a 0.5 to 1.0cm solitary white to pearly papule, Many lesions show a cental pore, and occasionally woolly or cottony hairs may protrude from this pore. Histologically, trichofolliculomas are characterized by a central dilated follicle filled with keratin or hair. Radiating from this primary hair follicle are numerous secndary hair follicles of varying degrees of maturation. An organized fibro-vascular stroma surrounds these epithelial structure, giving it a well-defined nodular appearance. We presented here two case of trichofolliculoma with typical clnical and histopathological findings, one in a 65 year-old female and the other a 56 year-old males.

4.Acute Gastric Volvulus due to Diaphragmatic Eventration.

We present the case report of a 72-year-old female in whom diaphragmatic eventration and secondary gastric volvulus developed 10 years after a left partial pneumonectomy for a bronchiectasis. Eventration of the diaphragm is defined as an abnormal elevation of an intact diaphragm. The abnormally wide subdiaphragmatic space provides the potential for a gastric volvulus, which results from the strong negative intrathoracic pulling force created by the potential subphrenic space and paradoxical movement of the diaphragm. Unless this strong negative force is first eliminated, gastropexy alone will lead to recurrence. Obliteration of the subphrenic space by colonic displacement is a simple and effective way of abolishing this negative subdiaphragmatic pulling force. Treatment of gastric volvulus requires immediate surgical repair to prevent subsequent necrosis and perforation, with surgical correction of the underlying anatomic abnormality being the treatment of choice for gastric volvulus. We experienced a case of gastric volvulus due to diaphragmatic eventration who was treated with colonic displacement.

5.Analysis of Associsted Intra-articular Lesions in Popliteal Cyst under Arthroscope in Adults.

PURPOSE: The authors tried to analyze the distribution of associated intra-articular lesions using the arthroscope in excision of popliteal cyst observed in adults. MATERIALS AND METHODS: The authors examined intra-articular lesions identified by arthroscopic examination on 113 adult patients(117 cases) who were diagnosed as having popliteal cyst that didn't respond to the conservative treatment. And for the operation, intra-articular examination using arthroscope was performed in supine position and the authors performed one-stage excision of cyst without position change of patients. RESULTS: In 104 cases(88.9%) of total 117 cases, combined intra-articular lesions were identified, Among these, most cases were 72 cases(69.2%) of degenerative chondral lesion and 31 cases(29.8%) of meniscus tear. Communication between cyst and joint was observed in 63 cases(53.8%). There was neither postoperative complication nor recurrence of cyst during out-patient follow-up periods. CONCLUSIONs: In cases of adult patient who have popliteal cyst that doesn't respond to the conservative treatment, possible associated intra-articular lesions should be considered., and arthroscopic examination and proper treatment far those associated lesions are thought to lower the recurrence rate.

6.Fluoroscopic Landmark to Optimize the View of the Foramen Ovale Using the Neighboring Structure

Radiofrequency thermocoagulation (RFT) via trans-oval approach is effective treatment modalities for trigeminal neuralgia. Clear identification of the foramen ovale (FO) is the most important initial step for successful percutaneous procedures. The purpose of this study was to verify whether the medial end of the temporomandibular joint and the distance between the mandibular angle and the occipital inner line can be used as a fluoroscopic landmark to determine the proper angulation of the C-arm. Analysis of previously saved fluoroscopic FO images during RFT was performed. As a fluoroscopic landmark to guide a proper lateral and axial oblique rotation of the C-arm, the position of the medial end of the temporomandibular joint and the vertical distance between the mandibular angle and the occipital inner line were evaluated. When the FO was located at the most central point within the H-figure, the medial end of TMJ was always located within the mandibular notch. The maximum view of the FO was defined when the FO showed maximal short axis to long axis ratio. The FO showed the best view when the vertical distance between the mandibular angle and occipital inner line was 20.04 (5.1) mm. This study demonstrated that the medial end of the temporomandibular joint and the vertical distance between the mandibular angle and occipital inner line as a fluoroscopic landmark for obtaining the optimal view of the FO.
7.Factors Influencing Angular Deformity of Tibia Fracture below Isthmus Treated with Interlocking Nails.

This study analyzed the factors causing the angular deformity as the tibia fracture below isthmus were treated with interlocking nails. For this purpose, we observed 70 cases of tibia fractures in the Gil General Hospital as subjects. Of the 70 cases under the treatment, 32(45.7%) showed the angulation of over one degree, and 11(15.7%) showed that of 5-10 degrees in any plane. According to the outcomes of this studies, the degree of angulation seemed to have close relation to the length of fracture line. Also, if the fracture line reached to the isthmus, angulation was apt to be caused. Even though most angulated cases had concomitant fibular fracture, fibular fracture, itself, seemed to have no direct relation to angulation. In consequence, the intramedullary interlocking nailing needs special carefulness in the case of breakage of cortical buttress such as long fracture line, butterfly fragment and fracture including isthmus.

8.The Effect of Knee Joint Function after ACL Reconstruction with Accelerated Rehabilitation.

We studied 30 patients who participated in accelerated rehabilitation program after ACL reconstruction to evaluate the effects on knee joint function prospectively. This program emphasizes early full hyperextension, early weight bearing as tolerated, and closed-chain quadriceps functional activities with rapid return to sports. Timing of the return to occupation, activity level, manual knee test, range of motion, thigh circumference, Lysholm knee score and KT-2000 arthrometer measurement were checked at the latest follow-up which is at least more than 1 year. The result were as follows: 1. The mean time for patients to return to occupation was 7.5 weeks (2 weeks-6 months). 2. Range of motion of the knee were not limited in 25 cases (83%) at 1 year after reconstruction. 3. The mean Lysholm score was 92.7. 4. The mean manual maximum KT-2000 arthrometer score was 2.0mm. The results of this study show that patients who had an ACL reconstruction with an autogenous central 1/3 patellar tendon graft and followed an accelerated rehabilitation program obtained longterm stability, achieved full range of motion, had a low complication rate, and were able to return to full sporting activities predictably.

9.Correlation of EGFR and c-erbB-2 Gene Amplifications and Expression of COX-2 Protein in Invasive Ductal Carcinomas of the Breast.

PURPOSE: Abnormalities of epidermal growth factor receptor (EGFR) and c-erbB-2 have been actively investigated in breast cancer. Cyclooxygenase-2 (COX-2) seems to be involved in critical steps of cancer onset and progression. The purpose of this study is to determine the correlation between the gene ampilfications of the EGFR and c-erbB-2 and expression of the COX-2. METHODS: Using Chromogenic in situ Hybridization (CISH) the gene amplification of the EGFR and c-erbB-2 were studies on paraffin sections of 47 invasive ductal carcinomas. The expression of COX-2 was studied immunohistochemically (IHC). RESULTS: Of the 47 invasive duct carcinomas, gene amplifications of the EGFR and c-erbB-2 by CISH were observed in 21 cases (44.7%) and 14 cases (29.8%), respectively. The gene amplification of EGFR was significantly correlated with tumor size, nodal metastasis, and stage. The gene amplification of c-erbB-2 showed a statistically significant correlation with tumor size and stage. The protein of COX-2 was expressed by IHC in 30 cases (63.8%). The expression of the COX-2 protein showed a statistically significant correlation with tumor size, nodal metastasis, and stage. The gene amplifications of the EGFR and c-erbB-2 between expression of COX-2 protein showed a statistically significant correlation. CONCLUSION: The gene amplification of the EGFR and c- erbB-2 may be correlated with expression of COX-2 protein in the breast cancer.

10.Complete Binocular Blindness as the First Manifestation of HIV-Related Cryptococcal Meningitis.

Ocular complications of HIV-related cryptococcal meningitis are reasonably common, but complete binocular blindness as the first manifestation of HIV is extremely rare. A 58-year-old man presented with binocular blindness. He experienced blurred vision for 3 days before the blindness. Mild pleocytosis was present in the cerebrospinal fluid, from which Cryptococcus neoformans was cultured. Serology revealed positivity for HIV antibody. He was treated with antifungal and antiretroviral therapy. This case indicates that HIV-related cryptococcal meningitis should be taken into consideration when determining the cause of unexpected sudden binocular blindness.
