1.Clinical Outcomes of a Refractive, Aspheric, Bifocal Intraocular Lens Imparting 1.5 Diopters Adding Power

Purpose:
We evaluated the 3-month, postoperative clinical outcomes of patients implanted with refractive, aspheric, bifocal, Lentis Comfort (LS313-MF15, Oculentis) intraocular lenses (IOLs).
Methods:
Eighty-two eyes of 51 cataract patients underwent phacoemulsification and were implanted with LentisComfort IOLs. The uncorrected distant visual acuity (UCDVA), the 80-cm uncorrected intermediate visual acuity (UCIVA), the 40-cm uncorrected near visual acuity (UCNVA), and refractive values were evaluated 1 and 3 months postoperatively. At the latter time, defocus curves were drawn, contrast sensitivity tested, and satisfaction surveyed.
Results:
At the 3-month postoperative follow-up, the mean spherical equivalent was -0.12 ± 0.21 D; and the mean LogMAR UCDVA, 80-cm UCIVA, and 40-cm UCNVA were 0.02 ± 0.03, 0.13 ± 0.07, and 0.42 ± 0.10 respectively. The defocus curves revealed visual acuities in excess of 0.2 LogMAR at points between the distant and intermediate targets. The mean contrast sensitivity fell in the range of 90% of a normal population under both photopic and mesopic conditions. In terms of satisfaction, 71.8% of patients were satisfied or very satisfied, but about 20% reported dysphotopsia.
Conclusions
Implantation of the refractive, aspheric, bifocal, intraocular, LentisComfort in patients with cataracts improved both the distant and intermediate visual acuities by 3 months after surgery and the visual quality was comparable to that afforded by other IOLs both objectively and subjectively.
2.Repair for Orbital Bone Fracture with Supramid(R) Plate.

We analyzed the clinical manifestations and results of 29 patients with orbital bone fracture who were repaired with Supramid(R) plate. The time interval between the day of injury and the operation averaged 84 days. Only 7 patients(25%) were operated within two weeks after trauma. The site of fractures were the orbital floor in 11 patients, the medial wall in 7 patients, both the medial and inferior wall in 10 patients, and the three walls of the roof, the medial, and the infereior wall in one patIent. During the average follow-up period of 9.4 months, there were significant improvement of symptoms and signs in patients on whom operations were performed within two weeks. Twenty-five percent of patients who were operated more than 2 weeks after recelvmg trauma showed no improvement of diplopia and imbalance of extraocular muscles. One patient showed optic nerve damage as postoperative complication, which was improved to the preoperative visual acuity three months after operation. No other complications such as implant extrusion or infection developed.

3.Cryotherapy for retinopathy of incontinentia pigmenti.

A case with typical skin lesions of incontinentia pigmeti showed progressing retinal vascular abnormalities. So cryotherapy was done on avascular peripheral retina, and retinal vascular changes ceased. We propose that cryotherapy may be a good treatment method for progressing retinal vascular lesions of incontinentia pigmenti.

4.Changes in the Corneal Curvature and Recurrence Rate following Pterygium Surgery with Relation to Pterygium Size and Morphology.

PURPOSE: We examined changes in the corneal curvature and recurrence rate following pterygium surgery with relation to pterygium size and morphology. METHODS: One hundred eyes with primary nasal pterygia were graded as atrophic(Group 1), intermediate (Group 2), or fleshy(Group 3) according to morphology and grouped as small area group or large area group according to an assessment of pterygium length from and along the limbus. Corneal curvature was measured with autokeratometer at 3 days, 1 week, 1 month, and 6months preoperatively and postopera-tively as well. Recurrence was evaluated at the same periods. RESULTS: Each group had no significant difference(p>0.05) to preoperative corneal curvature when pterygia were divided according to morphology. Large area group had significantly smaller corneal curvature than the preoperative one(p<0.05), but small area group didn't(p>0.05). The angle of astigmatism had the tendency of against-the-rule at 3 days postoperatively(163.3degrees), more strikingly in small area group(159.1degrees). The recurrence rate was 10% totally with no significant difference among each group(p>0.05). CONCLUSIONS: The magnitude of the corneal curvature following pterygium surgery was smaller than the preoperative one and the change of angle had the tendency of against-the-rule astigmatism. Surgically induced changes in the corneal curvature and recurrence rate related to morphology had no significant difference, but preoperative corneal curvature and postoperative changes were related to pterygium size.

5.Trabeculectomy and the use of Low-dose 5-Fluorouracil.

5-Fluorouacil (5-FU) 5mg/day was injected subconjunctivally from the third day after trabeculectomy in 8 eyes of 7 patients who had poor prognostic glaucomas such as previously unsuccessful trabeculectomy, aphakic glaucoma, glaucoma after retinal reattachment surgery, and inflammatory glaucoma. The eyes were injected once or multiple times no more than eigth doses in total. Each dose contained 5 mg of 5-FU so patients were given a minimum dosage of 5 mg or an additive maximum dosage of 40 mg. The follow-up period ranged from 2 months to 9 months and averaged 6 months. All eyes showed intraocular pressure below 21 mmHg with or without anti glaucoma medications with the mean intraocular pressure reduced by 16.5 +/- 9.4mmHg from 30.8mmHg preoperatively to 14.3mmHg postoperatively. The postoperative visual acuity was decreased in one eye and remained unchanged in 7 eyes. Diffuse filtering blebs were seen in 7 eyes and an encapsulated bleb was encountered in one eye. The average number of antiglaucoma medications was decreased from 3 preoperativdy to 1.4 postoperatively. Postoperative complications were corneal epithelial detect and conjunctival wound leak in one eye and conjunctival wound leaks alone in two other eyes.

6.A Case of Corneal Ulcer Caused by Combined Infection of Stenotrophomonas Maltophilia and Aspergillus Fumigatus.

Stenotrophomonas maltophilia keratitis is rare and none of it has been reported to be combined with Aspergillus keratitis.I have experienced a corneal ulcer caused by combined infection of S.maltophilia and A. fumigatus.S. maltophilia was grown on culture and a brownish pigmented anterior chamber mass containing filamentous fungi was attached to corneal ulcer area endothelium without the usual filamentous fungal keratitis findings such as feathery edge, satellite lesions, endothelial plaques, etc. A spergillusfumigatus was grown on culture of abscess which developed along the superior incision for removal of anterior chamber mass.

Most loci associated with type 2 diabetes mellitus (T2DM) discovered to date are within noncoding regions of unknown functional significance. By contrast, exonic regions have advantages for biological interpretation. We analyzed the association of exome array data from 14,026 Koreans to identify susceptible exonic loci for T2DM. We used genotype information of 50,543 variants using the Illumina exome array platform. In total, 7 loci were significant with a Bonferroni adjusted We found exonic loci having a susceptibility for T2DM. We found that such genetic information is advantageous for predicting T2DM in a subgroup of obese individuals.

9.The Relation Between Clinical Manifestations of Dry Eye Patients and Their BUTs.

Although the measurement of tear film break-up time (BUT) is the most frequently used diagnostic test for dry eye syndrome, there have been no reports for the relationship between clinical manifestations of dry eye patients and the degree of their BUTs. To investigate the relationship, we analyzed retrospectively clinical manifestations of 466 dry eye patients and categorized them into two groups in terms of BUT, the group of BUT <5 seconds and BUT >or=5 seconds, in various clinical manifestations. The two groups were compared by X2-test and the results were as follows. The two groups did not show statistically significant difference in sex, age, duration of symptoms, degree of corneal erosion, and time from the beginning of management to symptom improvement. Therefore we thought that there was no statistically significant correlation beteween the above-mentioned clinical manifestations of this study patients and their BUT's. There were more patients whose BUT were below 5 seconds than those having BUT above 5 seconds in group complaining foreign body sensation as a main symptom and in group having associated chronic conjunctivitis. 10.Clinical Outcomes of Intraocular Lenses Produced in the Republic of Korea

PURPOSE:
To evaluate the clinical outcomes of aspheric hydrophilic acrylic EYELIKE® (Koryo Eyetech Co., Seoul, Korea) intraocular lenses (IOLs).
METHODS:
We conducted a retrospective study of 176 consecutive eyes of 160 patients who underwent cataract surgery with implantation of aspheric EYELIKE® IOLs. The following preoperative measurements were collected: slit-lamp examination, uncorrected distance visual acuity (UCDVA), corrected distance visual acuity (CDVA), intraocular pressure (IOP), automatic and manual keratometry, fundus examination, corneal topography, and specular microscopy. The UCDVA, CDVA, IOP, refraction, and slit-lamp examination were evaluated postoperatively at 1 week and at 1, 3, and 6 months. Specular microscopy was also postoperatively conducted at 1 month.
RESULTS:
At the 6-month postoperative visit, the mean UCDVA was 0.15 ± 0.23, and the mean CDVA was 0.03 ± 0.07 logarithm of minimal angle of resolution (logMAR). No statistically significant differences were observed among the four groups (postoperative 1 week, postoperative 1 month, postoperative 3 months, and postoperative 6 months) in the manifest spherical equivalent. In total, 74% of IOL Master700 and 54% of A-scan measurements were within ± 0.5 diopters (D) difference from the target refraction, and 95% and 77%, respectively, were within ± 1.0 D from the target refraction. With regard to complications, one case of CME and posterior capsule opacification with decreased visual acuity was detected at month 3 postoperatively; in this case, the CDVA recovered to 0 logMAR with conventional treatment and yttrium aluminium garnetlaser posterior capsulotomy after postoperative 6 months.
CONCLUSIONS
Domestically produced EYELIKE® IOLs had high refraction predictability and stability.
