Objective To improve the management of the complications in patients with advanced prostate cancer. Methods Bone pain, anemia, urinary tract obstruction, hematuria and edema in the lower extremities and scrotum of patients with prostate cancer were managed with analgesics(24 cases), bisphosphonates(14 cases), isotope strontium-89(6 cases), external beam irradiation(9 cases), blood transfusion(9 cases), TURP(7 cases), superpubic cystostomy(8 case), percutaneous nephrostomy(4 case) and cysto-irrigation(6 case). Results 16 patients were alived and 10 were died of respiratory or circulation failure . The patients were relieved from pain and the other complications for several months after the palliative measures had been taken. Conclusions Bone pain, anemia, urinary tract obstruction and hematuria are common complications of advanced prostate cancer. They may have great impact on the life quality of patients. Diverse therapeutic modalities should be employed to palliate these disabling symptoms.

Objective To study in vitro anti-HIV activity of an extract of herb medicines,SanJiangDan,and the possible mechanism.Methods The main active ingredient of SanJiangDan was extracted by distillation.Three subtypes of HIV pseudovirus (B subtype,C subtype,CRF01_AE subtype) were used to evaluate the anti-HIV activity of SanJiangDan extract in vitro.The possible mechanism was evaluated through analyzing the effects of SanJiangDan extract on the expression of surface receptors and cytokines by T cells.The cytotoxicity of SanJiangDan extract was detected by using four different sources of cell lines including epithelial cells Caco-2 cells,TZM cells,Huh7 cells derived from liver cells and lymphocyte Jurkat-T cells.Results SanJiangDan extract effectively inhibited the infection of HIV pseudoviruses at concentrations of 1.6 mg/ml and 0.16 mg/ml.The inhibition rates were 30.9％,36.6％ and 65.0％ for B subtype,C subtype and CRF01_AE subtype respectively at the concentration of 0.16 mg/ml.As the concentration increased to 1.6 mg/ml,the inhibition rates increased to 96.4％ (B subtype),97.4％ (C subtype) and 99.5％ (CRF01_AE subtypes),but no toxicity to host cells was detected.Moreover,SanJiangDan extract inhibited the expression of HIV surface receptors including CD4,CXCR4 and CCR5 on TZM-bl cells,but enhanced IL-2 production.Conclusion SanJiangDan extract could inhibit HIV pseudovirus infection without causing cytotoxicity to host cells in vitro.The possible mechanism might be associated with the reduced expression of CD4,CXCR4 and CCR5 and enhanced secretion of IL-2 as well.

Objective To investigate the clinical value of serum VEGF levels in the diagnosis and treatment of colon cancer.Methods Serum VEGF was detected by ELISA,and CEA and CA199 concentration were detected by CLEIA in 66 patients with colon cancer,55 patients with colon benign diseases and 50 health persons.The value of individual and joint detection for VEGF in colon cancer was evaluated.Analysis had been done on relationships between serum VEGF and pathology,treatment effects and prognosis.Results The levels of serum VEGF in colon cancer group [(318.5±148.6) ng/L] were significantly higher than those in control group [(125.7±49.4) ng/L] and benign colon diseases [(136.9±52.6) ng/L] (t =8.830,8.805,all P ＜ 0.01).There was a positive correlation between serum levels of VEGF and depth of tumor size,tumor invasion,lymph node metastasis and TNM stage (P ＜ 0.01).The susceptibilities of VEGF,CEA,CA199 were 61％,45 ％,53 ％.The sensitivity of detection was improved to 86 ％ when the combined detection of VEGF,CEA and CA199 (x2 =11.237,P ＜ 0.01).The serum levels of VEGF in patients with colon cancer was significantly decreased after treatment in the 3,7,10 day compared with that before operation [(272.3±88.1),(236.8±77.4),(173.1±59.9) vs (318.5±148.6) ng/L,t=2.173,P ＜ 0.05; t =3.961,P ＜ 0.01; t=7.464,P ＜ 0.01],respectively.Conclusion The VEGF was related to the onset and progression and metastasis of colon cancer.It has clinical significancy for diagnosis of colon cancer and judgment of curative effect and prognosis.

Objective To evaluate the safety and clinical efficacy of finasteride in treating patients with benign prostatic hyperplasia (BPH) during a 14-year period in a hospital.Methods Forty-one patients with BPH receiving finasteride 5 mg daily for the treatment from December 1994 to Febrary 2009 were included in the study. The base line and the end of study data of nocturia, prostate volume, serum creatine, complete blood count and serum prostatic specific antigen (PSA) were recorded. The acute urinary retention, surgical treatment and drug adverse reaction (prostate cancer or breast cancer) during the observation periods were recorded as well.ResultsAll the 41 cases took finasteride regularly for long-term medical therapy of BPH. At the end of this study, the average age of patients was (87.9±5.4) years old and the average duration of treatment was (141.1±27.1) months. The numbers of nocturia were 1.8±1.5 and 3.2±1.3 pre- and post- treatment, respectively (t= -4.52,P<0. 05). Before and at the end of the study, the prostate volumes were (44.9±26.6) ml and (42.8±31.3) ml, respectively(t=0. 33,P>0.05). Stratified study showed that, compared with the baseline data, the prostate volume was increased by 17.3 ％ in patients with prostate volume <25 ml(t= -0. 88 ,P>0. 05) ; the prostate volume was decreased by 17.2％ in patients with prostate volume of 25-40 ml(t=2.59,P<0.05); the prostate volumes were (63.3±28. 9) ml and (62.6±36.5) ml pre- and post-treatment in patients with prostate volume > 40 ml, and there was no significant change(t= 0.07, P>0. 05). Before and after the treatment, the serum creatine levels were (96.8±18. 6) mol/L and (86.45±32. 3) mol/L, respectively(t= 1.79, P>0. 05) ; the white blood cell counts were (6.4±1.5) × 109 L and (6.0±1.7) ×109 L, respectively (t= 1.13,P>0. 05) ; and the PSA levels were (1.2±2.0) μg/L and (1.4±1.7) μg/L, respectively (t=-0. 49,P>0. 05). Three cases (7.3％) occurred acute urinary retention. There was no prostate cancer and breast cancer case, and no new adverse event occurred during long-time use of finasteride. Conclusions This retrospective study has demonstrated that the clinical progress of BPH can be controlled effectively by long-term administration of finasteride.

Objective To observe the immunologic effect of lactoferrin on repeated respiratory infection(IRRI) in children.Methods Ninety-eight cases of IRRI were divided into two groups randomly.The control group (48 cases)were treated with routine therapy.The treatment group(50 cases) were treated with lactoferrin based on routine therapy for 2-3 months.T cell subgroup,immunoglobulin and complements were determined before and after treatment.Results Total effective rates in treatment group and control group were 86% and 22.9% respectively.The therapeutic efficacy in treatment group was significantly higher than that in control group(P

BACKGROUND:Our preliminary study found that the monocusp valves made of ultramicropore expanded
polytetrafluoroethylene (ePTFE) revealed no significant thrombus, calcification, or degradation 20 weeks after implanted into the descending aorta and the left pulmonary artery in sheep, which verified the good property of ePTFE. However, the surface of ePTFE in the left pulmonary artery was covered with obvious neointima.
OBJECTIVE: To assess the biocompatibility of phosphorylcholine-coated ePTFE.
METHODS:ePTFE surface was modified by phosphorylcholine derivative. Then the changes of surface shape, tensile stress at yield and elasticity modulus, water contact angle, and protein absorption capacity of ePTFE after surface modification were observed. (1) Hemolytic test: the leaching solution of phosphorylcholine-coated ePTFE, leaching solution of uncoated ePTFE, normal saline, and distiled water were added to the diluted human blood, respectively. (2) Platelet count test: the phosphorylcholine-coated ePTFE, uncoated ePTFE, high density
polyethylene, and Zymosan A were added to the whole blood samples from healthy volunteers, respectively.
(3) Platelet activation test: the phosphorylcholine-coated ePTFE, uncoated ePTFE, γ-Globulins, and Zymosan A
were added to the whole blood samples from healthy volunteers, respectively.
RESULTS AND CONCLUSION: The mean micropore diameter of ePTFE was significantly decreased after
phosphorylcholine coating (P < 0.001). The hydrophilicity and the ability of suppressing protein adsorption were
significantly strengthened after phosphorylcholine coating (P < 0.001). Phosphorylcholine coating did not influence
ePTFE in biomechanical properties and hemolytic test. The platelet count test and platelet activation test demonstrated that phosphorylcholine coating significantly improved anti-thrombus function of ePTFE. So, phosphorylcholine coating can enhance anti-thrombus function, suppress protein adsorption, and improve biocompatibility of ePTFE.

Objective To evaluate valvular functionality after transcatheter pulmonary valve replacement in sheep using a novel polymeric prosthetic pulmonary valve.Methods In this study,we designed a novel polymeric trileaflet transcatheter pulmonary valve with a balloon-expandable stent,and the valve leaflet was made of 0.1 mm expanded polytetrafluoroethylene (ePTFE).We chose bovine pericardium valve as control.Pulmonary valve stents were implanted in situ by right ventricular apical approach in 8 healthy sheep(6 for polymeric valve and 2 for bovine pericardium valve) weighing an average of(22.8 ± 2.2) kg.Angiography was performed after implantation to assess immediate valvular function.Color Doppler echocardiography and 64-row computed tomography were used to assess valvular function 4 weeks after implantation.Results Implantation was successful in 8 sheep.Angiography at implantation showed one polymeric valve was located below the ideal position and most of the stent was in the outflow tract of right ventricle.While,all the other prosthetic valves demonstrated orthotopic position and exhibited normal open and close functionality.Echocardiography 4 weeks after implantation showed all the prosthetic valves exhibited normal functionality and no significant insufficiency.The peak-peak transvalvular pressure gradient of the polymeric valves was (18.8 ± 6.0) mmHg,while that of two bovine pericardium valves were 9 mmHg and 20 mmHg.CT 4 weeks after implantation demonstrated orthotopic position of the stents except the above-mentioned one and all the stents had no deformation.Conclusion The success rate of transcatheter pulmonary valve replacement by right ventricular apical approach is satisfactory.The early valvular functionality of the novel ePTFE pulmonary valve after transcatheter pulmonary valve replacement in sheep is good.

Objective To investigate rates and clinical associates of combined antipsychotic therapy in hospitalized patients with schizophrenia during 1986 ～ 2006. Methods Hospital charts of schizophrenic inpatients who discharged from six psychiatric hospitals in Tangshan in 1986,1996,2001, and 2006 were reviewed. Demographic and clinical character and prescription information at time of discharged of patients' was coded by trained investigators with a special designed form. Results The rates of combination antipsychotic therapy in the four years mentioned above were 13.2%, 18. 1% ,32.7%, and 28.9% ,respectively and the difference was statistically significant( x2 = 76.431, P ＜ 0.01; linear by linear x2 = 25. 542, P ＜ 0.01 ). Combined antipsychotic therapy in schizophrenic inpatients in Tangshan region show a increasing trend. There is significant difference of rate of combined antipsychotic therapy between hospitals. Combined antipsychotic therapy were associated with duration of illness,numbers of prior psychiatric hospital admission,hospital from which patients discharged,and discharged year.
