2.Experimental Study of the Bionic Technology for the Construction of Small-caliber Vascular Prostheses

Objective:To study the feasibility of using the bionic technology to construct small-caliber vascular prostheses with modified SIS.Methods: The vascular endothelial cells and smooth muscle cells were separated from canine saphenous artery,the cells blended with collagen gel,which were planted respectively on the SIS films,these films were rolled into the biologic three-layer prostheses around a 3mm diameter polyethylene tube;one-layer prostheses without these cells and collagen gel served as control.These 2 types of prostheses were implanted into the defect of bilateral canine femoral by anastomosis in 15 dogs.doppler colour ultrasonic,histology detection and electron microscope examination were done postoperatively.Results: By 12 weeks postoperatively,14 biologic vascular prostheses had kept well patency,the patency rate being 93.3%,the biologic structure like blood vessels had formed,the inner surface of the vessels had been covered with full endothelial cells,a lot of smooth muscle cells had been found in the media of vascular walls in regular line;the patency rate in control group was 60.0%,the endothelial cell coverage was incomplete.Conclusions: The bionic vascular prostheses showed potent blood compatibility,which could keep long-term patency in vivo.Curative effect of repairing the small-caliber vessel defect was well satisfactory.
3.Intracapsular reduction for comminuted distal radial fractures

Objective To explore the application and clinical outcome of intracapsular reduction for comminuted distal radial fractures.Methods From January 2003 to October 2005,37 cases of comminuted fractures of distal radius were treated in our hospital.They were categorized according to the AO classification,and underwent open intracapsular reduction,internal fixation with either locking com- pressing plate(LCP)or external fixator according to the types of fractures.The results of treatment were evaluated by functions and X-ray examination of involved wrists.Results All the 37 patients were fol- lowed up regularly.Satisfied synostosis by X-ray had been acquired.The wrist functions were evaluated by the Gartland and Werley system to find excellent result in 26 cases,good in eight,fair in two,and poor in one,with the excellent and good rate of 92%.Conclusion For the treatment of comminuted distal radial fractures,open intracapsular reduction can obtain fast fracture healing and recovery of wrist joint function.
4.Study on antithrombogenicity and surface heparinization of small intestinal submucosa

Objective To study surface heparinization of small intestinal submucosa(SIS)and an- tithrombegenicity of beparinized SIS films with plasma initiation technique for the engineering vascular scaffolds. Methods The SIS films were grafted with heparin by hypothermia plasma initiation technique.The blood com- patibility of the modified SIS films was assessed by observing blood coagulation time in vitro and the long term pa- tency of hepafinized SIS vascular scaffolds directly under the circulation of blood.Results The hypothermia plasma initiation method could attach heparin onto the SIS surface,The water contact angle of SIS films modified with heparin was decreased while the surface free energy and hydrophilicity increased.The prothrombin time(PT),ac- tivated partial thromboplastin time(APTT)and thrombin clotting Time(TT)of the SIS films modified with heparin were prolonged obviously.Small caliber engineering vascular scaffold made of heparinized SIS films kept the patency for six weeks.Conclusion Heparin can be attached to SIS films by hypothermia plasma initiation technique.The modified surfaces provide good and persistent antithrombogenicity,and possess potent blood compatibility,
5.Influence of different preservation solutions and placing time on survival rate of umbilical cord-derived mesenchymal stem cells

BACKGROUND:The relationship between placing time after stem cel preparation and cel survival is the basis of safety and effectiveness for the clinical application. OBJECTIVE: To observe effects of different preservation solutions and different storage time on survival rate of umbilical cord-derived mesenchymal stem cels, and to provide important evidence for identifying effectiveness of stem cels. METHODS:Umbilical cord-derived mesenchymal stem cels were selected to prepare stem cel preparation, which was preserved in physiological saline, medium, medium+physiological saline, physiological saline containing epidermal growth factor, and medium containing low molecular heparin calcium suspension. Cold closet was selected for imitating celular transport conditions. Samples were obtained at 0, 6, 12, 18, 24, 30, 36, 42 and 48 hours. Total cel number and cel survival rate were detected. RESULTS AND CONCLUSION:The difference in cel number and survival rate was not great within 24 hours in each group. Twenty-four hours later, total cel number and survival rate were better in the medium and medium containing low molecular heparin calcium groups than in the physiological saline containing epidermal growth factor, physiological saline, and medium+physiological saline groups. These findings suggest that after stem cel preparation, the cel survival rate can reach more than 90% within 24 hours under refrigerated transport conditions. Nutritional ingredients and proper pH value of preservation solution can make the cel survival rate increased greatly.
6.Phosphorylcholine coating enhances biocompatibility of expanded polytetrafluoroethylene used in polymeric prosthetic heart valves

BACKGROUND:Our preliminary study found that the monocusp valves made of ultramicropore expanded
polytetrafluoroethylene (ePTFE) revealed no significant thrombus, calcification, or degradation 20 weeks after implanted into the descending aorta and the left pulmonary artery in sheep, which verified the good property of ePTFE. However, the surface of ePTFE in the left pulmonary artery was covered with obvious neointima.
OBJECTIVE: To assess the biocompatibility of phosphorylcholine-coated ePTFE.
METHODS:ePTFE surface was modified by phosphorylcholine derivative. Then the changes of surface shape, tensile stress at yield and elasticity modulus, water contact angle, and protein absorption capacity of ePTFE after surface modification were observed. (1) Hemolytic test: the leaching solution of phosphorylcholine-coated ePTFE, leaching solution of uncoated ePTFE, normal saline, and distiled water were added to the diluted human blood, respectively. (2) Platelet count test: the phosphorylcholine-coated ePTFE, uncoated ePTFE, high density
polyethylene, and Zymosan A were added to the whole blood samples from healthy volunteers, respectively.
(3) Platelet activation test: the phosphorylcholine-coated ePTFE, uncoated ePTFE, γ-Globulins, and Zymosan A
were added to the whole blood samples from healthy volunteers, respectively.
RESULTS AND CONCLUSION: The mean micropore diameter of ePTFE was significantly decreased after
phosphorylcholine coating (P < 0.001). The hydrophilicity and the ability of suppressing protein adsorption were
significantly strengthened after phosphorylcholine coating (P < 0.001). Phosphorylcholine coating did not influence
ePTFE in biomechanical properties and hemolytic test. The platelet count test and platelet activation test demonstrated that phosphorylcholine coating significantly improved anti-thrombus function of ePTFE. So, phosphorylcholine coating can enhance anti-thrombus function, suppress protein adsorption, and improve biocompatibility of ePTFE.
7.Diagnosis of Breast Carcinoma by Multislice Spiral CT

Objective To explore the diagnostic value of multislice spiral CT (MSCT) for breast carcinoma.Methods One hundred and twenty consecutive suspicious cases of breast carcinoma diagnosed clinically and/or by imaging modalities were studied with MSCT prospectively. The dynamic contrast-enhanced serial scanning was done as follows:Scanning was triggered 35 s (early arterial phase) after beginning of contrast agent infusion at a speed of 3 mL/s by high pressure automatic injector, and would be repeated at the time point of 60 s and 150 s. Eventually, the CT diagnostic results were compared with the pathological consequences to calculate the diagnostic accuracy of MSCT for breast carcinoma. Results Eighty-nine patients with 100 foci of breast cancer from 120 suspicious cases were confirmed pathologically and their imaging data were analyzed. Among the 89 cases, 61 cases were invasive ductal carcinomas,12 ductal carcinomas in situ, 8 invasive lobular carcinomas, 6 medullary carcinomas and 2 inflammatory carcinomas. According to lesion morphologic appearance, lesion enhancement mode, time-density curve of lesion, and combination of all the above three parameters, the diagnostic accuracies were 78%, 82%, 84% and 92%, respectively. Conclusion MSCT has a great clinical value in diagnosing breast carcinoma.
8.The Relationship between Thyroid Hormone and Purine Metabolism and Body Weight in Patients with Type Ⅱ Diabetes and Normal Thyroid Function

Objective To investigate the relationship between thyroid hormone and uric acid (UA) and body mass index (BMI) in patients with type Ⅱ diabetes and normal thyroid function.Methods Total of 313 patients with type Ⅱ diabetes and normal thyroid function were selected.BMI,fasting blood glucose (FBG),the metabolism of blood lipid,thyroid hormones and UA indicators were examined and the correlations of thyroid hormone,BMI and UA were analyzed.Results (1) The patients were divided into two groups according to gender,and FT3,FT4,and UA of male were found to be significantly higher than those in female (P<0.01).TSH,SBP,HDL-C in female were significantly higher than those in male (P<0.01);(2) The patients were divided into three groups according to BMI Level.Thyroid-stimulating hormone (TSH),three iodine armour gland original glycine (TT3),free three iodine thyroid glycine (FT3),UA,and FBG in overweight and obesity groups were found to be higher than those in normal weight group (P < 0.05);(3) The patients were divided into two groups according to the TSH level.Serum uric acid,TT3,FT3,fasting insulin in the group with TSH above 2.5 uIU/L were found to be higher than those in the group with TSH under 2.5 uIU/L (P < 0.05);(4) Patients were divided into two groups according to the UA level.TSH,FT3 in male with high uric acid were found to be higher than those in male with normal uric acid (P < 0.05);TSH was in female with high uric acid was found to be higher than that in female with normal uric acid (P < 0.05).Conclusion Thyroid hormone in patients with type Ⅱ diabetes can be used to assess the body weight and uric acid,which is of great clinical importance.
9.Diagnosis and surgical treatment of cardiac tumors of the right ventricle

Objective To discuss the diagnosis and surgical treatment of the primary right ventricular tumors.Methods The results of surgical treatment of 7 patientss (5 males,2 females; median age,48 years,range,37 to 68 years) with primary right ventricular tumors hospitalized from Jan,1999 to May,2012 were analyzed.By preoperation echocardiography,cardiac myxoma was diagnosed in 2 cases,and cardiac space-occupying lesion was diagnosed in 5 cases.Median sternotomy were performed in all the 7 cases for biopsy or resection of tumor.when resection of tumor,extracorporeal circulation and cardiac arrest were performed.Results The 7 patients accounted for 4.96% of patients with cardiac tumors surgically treated in the corresponding period.Complete resection of tumor was performed in 5 cases,and only biopsy in 2 cases.The pathological diagnoses included myxoma in 2 cases,lipoma in 1 case,liomyoma in 1 case,undifferentiated sarcoma in 1 case,and mesenchymal sarcoma in 2 cases.There was no in-hospital death.During a follow-up of 1 ~ 51 months (median,38 months),the 3 cases of sarcoma died,and the other patients were good and showed no recurrence.Conclusion The incidence rate of primary right ventricular malignant tumor is relatively high.Echocardiography is the most important diagnosis method.The prognosis of right ventricular malignant tumor is poor.The results of surgical treatment of right ventricular benign tumor are satisfactory.
10.Exercise Preconditioning Improving the Pathological Cardiac Hypertrophy in Pressure Over-loaded Rats

Objective: To explore the effect of exercise preconditioning (EP) on pathological cardiac hypertrophy and heart failure (HF) in pressure over-loaded experimental rats.
Methods:A total of 60 SD rats at the age of 6 weeks were randomly divided into 3 groups, n=20 in each group. Sham-operation group, Transverse aortic constriction (TAC) group and EP + TAC group. The cardiac function and structure were evaluated by echocardiography, patholgical changes and HF biomarkers were examined for EP effect at 4 and 8 weeks after TAC.
Results:Compared with Sham-operation group, the cardiac function and structure had obvious changes in the other 2 groups. Compared with TAC group, the ejection fraction in EP+ TAC group increased 15%, the heart weight index and left ventricular weight index decrease 15.7%and 20%respectively at 8 weeks after TAC, all P<0.05. Compared with Sham-operation group, the mRNA and protein expressions of ANP and BNP increased in TAC group at 4 and 8 weeks after TAC, increased in EP+TAC group at 8 week after TAC. Compared with TAC group, the mRNA expressions of ANP and BNP in EP+TAC group decreased 47%and 62%at 4 weeks after TAC, decreased 44%and 28.1%at 8 weeks after TAC, all P<0.05;the protein expression of ANP and BNP in EP+TAC group decreased 22.3%and 48%at 4 weeks after TAC, decreased 21.5%and 38.3%at 8 weeks after TAC, all P<0.01.
Conclusion: EP may improve cardiac pathological hypertrophy in pressure over-loaded rats at the early stage, and delay the heart failure process.
