1.Preparation and measurement of magnetic adriamycin-verapamil-albumin nanoparticles

Objective To prepare magnetic adriamycin-verapamil-albumin nanoparticles(MAVANs) and measure the physical property of the MAVANs,such as magnetism,drug loading(DL)and size of nanoparticles in order to direct its further experimental study on animals and its application in clinical practice.Methods Commercial cottonseed oil was refined,then mixed with adriamycin,verapamil,human albumin and Fe3O4 magnetic powder according to appropriate proportion,and MAVANs was prepared by using a heat denaturation technique.The size and shape of nanoparticles were determined by electron transmission micro-photography.The magnetic response and structure of the MAVANs were analyzed by upside-down microscope.The standard curve was plotted.The drug loading of verapamil in the MAVANs was quantified by high performance liguid chromatography.The drug loading of adriamycin was quantified by a standard spectrophotonuorometry.Results The nanoparticles featured homogenous size,regular and spherical shape,and good magnetic response.The mean diameter of nanoparticles was 0.5?m.The loading amount of verapamil was 0.8%.The drug loading of adriamycin was 1.25%.Conclusions The heat denaturation technique is feasible to prepare the nanoparticles which can load two kinds of drug with good physical property.The process is simple and the testing condition is easy to control.
2.THE PROTECTIVE EFFECTS OF SOYBEAN ISOFLAVONE ON GENETIC TOXICITY INDUCED BY DI-N-BUTUL PHTHALATE IN MICE

Objective To investigate the protective effects of soybean isoflavone(SI) on genetic toxicity induced by di-n-butul phthalate(DBP) in mice.Method(1) Micronucleus test:40 male 7 w old Kunming mice were randomized into 4 groups:High and low dose SI intervention groups,DBP model group,and solvent control group.SI intervention groups were given different doses of SI(50,100mg/kg) for 30 d,meanwhile,the DBP group and solvent group were given 0.5% sodium carboxymethyl cellulose.Then all groups were treated by 0.5g/kg DBP for 5d except solvent group.Mice were sacrificed 6 hour after last treatment,and then counting micronucleated cells in bone marrow.(2) Sperm malformation test:40 male 6w old Kunming mice were grouped and treated the same as micronucleus test.Mice were sacrificed at 35 day after the first treatment,and then sperm quantity,motility,viability and abnormality rate were calculated.Result Micronucleus rate and sperm abnormality rate of SI intervention group were lower than DBP model group,while sperm motility and viability were higher than DBP model group.Conclusion SI can relieve the genetic toxicity induced by DBP in mice.
3.Signaling mechanisms involved in the priming effects of lipopolysaccharide on Staphylococcus aureus-induced nitric oxide production in macrophages

AIM: To investigate the signaling mechanisms involved in the priming effects of lipopolysaccharide(LPS) on heat killed Staphylococcus aureus(HKSa)-induced nitric oxide(NO) production in macrophages.METHODS: Murine macrophage RAW264.7 was used in the experiment.Griess reagent was used to measure the content of nitrite in culture medium.Real-time PCR and Western blot was utilized to examine the mRNA and protein levels of toll-like receptor 2(TLR2),respectively.Dual luciferase reporter assay was used to assess the transcriptional activity of nuclear factor of activated T cells(NF-AT).RESULTS: The RAW264.7 cells pretreated with LPS for 24 h significantly enhanced NO production induced by HKSa,suggesting that LPS primed the macrophages and increased the reactivity of the cells to HKSa.LPS increased the mRNA and protein levels of TLR2 in a dose-dependent manner in RAW264.7 cells.The RAW264.7 cells pretreated with LPS enhanced NO production induced by peptidoglycan,one of the specific ligand of TLR2.The priming effect of LPS on HKSa-induced NO production was partly blocked by TLR2 neutralizing antibody.LPS significantly enhanced the transcriptional activity of NF-AT,which was inhibited by BAPTA/AM(a cell-permeable cytosolic calcium chelator) and cyclosporine A(CsA,an inhibitor for calcineurin).Both BAPTA/AM and CsA inhibited the priming effect of LPS on HKSa-induced NO production in RAW264.7 cells.CONCLUSION: The present study confirms the priming effect of LPS on the reactivity of RAW264.7 cells to HKSa.Pattern recognition receptor TLR2 and calcium/calcineurin/NF-AT signaling pathway may be involved in the priming process initiated by LPS.

In recent years,studies have shown that inflammation is an important factor in secondaryischemic injury.The exploration of the related mechanisms of the occurrence of inflammation has become ahot spot in the field of cerebral ischemia.Toll-like receptor 4 (TLR4) signaling pathway is one of theclassical inflammatory pathways.Studies have shown that TLR4 is involved in the early inflammatoryresponse after cerebral ischemic injury and the late nerve repair.This article reviews the research progress ofthis signaling pathway in cerebral ischemia injury,so as to seek a therapeutic target against inflammatoryinjury caused by cerebral ischemia through the analysis of its potential research direction.
5.Study on the Predictive Value of Serum miR-411-5p and miR-485-5p Levels Combined with Ultrasound Blood Flow Indicators for Perinatal Outcomes in Preeclampsia Pregnant Women

Objective To analyze the predictive value of serum micro RNA(miR)-411-5p and micro RNA(miR)-485-5p levels in preeclampsia(PE)pregnant women combined with ultrasound blood flow indicators for perinatal outcomes.Methods A total of 88 pregnant women with PE who were enrolled in the obstetric card and underwent cesarean section in Beijing Nuclear Industry Hospital from January 2020 to December 2021 were selected as the study subjects(PE group).In addition,90 normal pregnant women who delivered by cesarean section due to other reasons in the same period were regarded as the control group.The differences of pulsitility index(PI)and resistance index(RI)were compared between the two groups.The serum levels of miR-411-5p and miR-485-5p were measured and compared between the two groups by real time fluorescent quantitative PCR(qRT-PCR).According to the different perinatal outcomes of PE patients,they were grouped into good outcome group and poor outcome group.The differences of PI,RI,miR-411-5p and miR-485-5p between the two groups were compared.The predictive effect of PI,RI,miR-411-5p,miR-485-5p and combined detection on the adverse perinatal outcome of PE patients was analyzed by the receiver operating characteristic(ROC)curve method.Results The RI(0.79±0.08)and PI values(1.82±0.08)of pregnant women in the PE group were higher than those in the control group(0.66±0.06,1.38±0.15),the serum levels of miR-411-5p(0.32±0.09)and miR-485-5p(0.26±0.03)were lower than those in the control group(1.01±0.08,1.02±0.09),and the differences were statistically significant(t=12.283,24.339,54.091,75.231,all P<0.001).The RI(0.83±0.08)and PI(1.86±0.09)values of PE patients in the poor outcome group were higher than those in the good outcome group(0.70±0.07,1.71±0.07),the serum levels of miR-411-5p(0.27±0.02)and miR-485-5p(0.24±0.02)were lower than those in the good outcome group(0.45±0.04,0.31±0.04),and the differences were statistically significant(t=11.545,12.428,37.840,14.716,all P<0.001).The areas under the curve of ultrasound blood flow index RI and PI to predict the adverse perinatal outcome of PE patients were 0.838(sensitivity was 90.8%,specificity was 65.2%),and 0.758(sensitivity was 50.8%,specificity was 91.3%),respectively.The areas under the curve of serum miR-411-5p and miR-485-5p in predicting adverse perinatal outcome of PE patients were 0.830(sensitivity was 90.8%,specificity was 73.9%),and 0.769(sensitivity was 95.4%,specificity was 61.9%),respectively.The area under the curve of the four combined tests to predict the adverse perinatal outcome of PE patients was 0.976(sensitivity was 98.5%,specificity was 91.3%).Conclusion The levels of miR-411-5p and miR-485-5p in serum of PE pregnant women were decreased,and the combination of miR-411-5p and miR-485-5p and ultrasound blood flow indicators PI and RI may predict the perinatal fetal outcome of PE pregnant women.
6.Effects of clinical nursing pathway in emergency thoracic trauma patients

Objective:To explore the effect of clinical nursing pathway in emergency thoracic trauma patients.Methods:From January to June 2021, convenience sampling was used to select 116 emergency thoracic trauma patients hospitalized in Fuyang People's Hospital as the research object. The emergency admission patients from January to March 2021 were in the control group, and the emergency admission patients from April to June 2021 were in the experimental group, with 58 cases in each group. The control group was given emergency routine nursing, while the experimental group received the clinical nursing pathway on the basis of the control group. After the patients were transferred to the ward, the emergency treatment and nursing risk events were compared between the two groups.Results:The emergency treatment time of the experimental group was shorter than that of the control group, and the pain score was lower than that of the control group, and the differences were statistically significant ( P<0.01) . The incidence of nursing risk events in the experimental group was lower than that in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Implementing the clinical nursing pathway can help shorten the emergency treatment time of emergency thoracic trauma patients, relieve the patient's pain, and reduce the occurrence of nursing risk events.
7.Status and influencing factors of compliance with immunosuppressive drugs in patients with allogeneic hematopoietic stem cell transplantation

Objective:To understand the current status of the compliance of immunosuppressive drugs in patients with allogeneic hematopoietic stem cell transplantation (allo-HSCT) and to explore the influencing factors of their compliance.Methods:This study was a cross-sectional study. Using the convenient sampling method, a total of 238 allo-HSCT patients who were followed up in the Transplant Center of Taizhou Hospital of Zhejiang Province, Zhejiang Provincial Hospital of Traditional Chinese Medicine and the First Affiliated Hospital of Wenzhou Medical University from March 2020 to February 2021 were selected as the research objects. The self-designed general information questionnaire, Basel Assessment Scale for Immunosuppressive Drug Compliance, Chinese version of Medication Belief-Specific Scale, General Self-Efficacy Scale, Perceived Social Support Scale and Family Care Index Scale were used for on-site investigation of patients. Pearson correlation analysis was used to analyze the correlation between medication compliance and medication belief, self-efficacy, social support and family care of allo-HSCT patients. Multiple linear regression analysis was used to investigate the influencing factors of immunosuppressive medication compliance in allo-HSCT patients.Results:A total of 238 questionnaires were distributed in this study, and 228 valid questionnaires were finally recovered. The medication compliance score of allo-HSCT patients was (4.80±1.18) points. And 111 patients (48.68%, 111/228) had poor medication compliance, among which the incidence of not taking medication on time was the highest, accounting for 44.30% (101/238) . Multiple linear regression analysis showed that average monthly family income, rejection, medication belief, social support and family care were the influencing factors of medication compliance ( P< 0.05) . Conclusions:The immunosuppressant medication compliance of allo-HSCT patients is at a medium and low level. Average monthly family income, rejection reaction, medication belief, social support and family care are the main factors affecting the immunosuppressant medication compliance of allo-HSCT patients. Medical staff should formulate targeted intervention measures according to the influencing factors of medication compliance to improve the immunosuppressive medication compliance of allo-HSCT patients.
8.A qualitative research on the causes of non-adherence to pulmonary rehabilitation exercise in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective:To understand the reasons for non-adherence of pulmonary rehabilitation exercise in patients with acute exacerbation of COPD, and in order to provide theoretical and practical basis for improving patient adherence.Methods:Using qualitative phenomenological research methods, eighteen patients of non-adherence with exercise were conducted deeply semi-structured interviews, and the Colaizzi analysis was used for data analysis and extracting themes.Results:Seven themes were extracted: effect of subjective and objective symptoms, limited by comorbidity, negative mood disturbance, lack of perception effect, material environment in ward, cultural environment in ward and inappropriate form of exercise.Conclusions:Health care providers should value these obstacles and implement targeted measures, in order to achieve the goal of improving exercise adherence, so that more patients would complete pulmonary rehabilitation exercise.

Premature ovarian insufficiency(POI),also known as"ovarian insufficiency",has an incidence of 1%~5%.The incidence has been on the rise in recent years,seriously affecting women's physical and mental health and quality of life.At present,the cause and mechanisms of POI are still unclear,and the method and applications of model construction are also confusing.Most models have some shortcomings in pertinence and stability.The limitations greatly limit research into the clinical diagnosis and treatment of POI.This paper summarizes and discusses the etiology and pathogenesis of POI and the construction of POI animal models to provide a comprehensive reference for those studying POI.
10.Effect of multidisciplinary collaboration management on the self?management efficacy of patients with ileostomy irritant dermatitis

Objective To investigate the effects of the multidisciplinary cooperative management intervention model on the self‐management efficacy of patients with ileostomy irritant dermatitis. Methods Among the patients with irritative dermatitis in the ileostomy section of the Anorectal Surgery and Ostomy Clinic of Hwamei Hospital, the Chinese Academy of Sciences, 72 patients, who visited the clinic from August 2016 to August 2018, met the inclusion criteria and were finally included in the study. According to the random number table method, 36 patients each were assigned to the control and observation groups respectively. There were no significant differences in age, sex and education between the two cohorts. The control groups were compared. The control group was treated with routine nursing methods. The observation group underwent multidisciplinary nursing intervention that was based on the routine measures of the control group. The Chinese version of the cancer self‐management efficacy was used as the questionnaire survey that was administered before and after treatment. The results were compared and analyzed between the patient groups. The independent sample t‐test was used to compare the results between the groups, and the paired sample t‐test was used for intra‐group comparisons. Results After intervention, the total scores of self‐management efficacy and positive attitude, self‐decompression and self‐decision scores were higher in the observation group than in the control group (t=4.192, 3.095, 4.123, 2.267, P<0.05). The scores of self‐administration efficacy and each dimension of the two groups were higher than those before intervention, and the difference was statistically significant (P<0.01). After the intervention, 3 patients (10.0%) in the observation group with recurrent dermatitis and 13 patients (43.3%) in the control group. The recurrence rate of the observation group was significantly lower than that of the control group (χ2= 8.357, P<0.05). Conclusion Multidisciplinary cooperative management can improve the self‐management efficacy of patients with irritating dermatitis and reduce the incidence of this condition.
