1.Clinicopathological features of Caroli’s disease: An analysis of 5 cases

ObjectiveTo investigate the clinical features, pathological features, differential diagnosis, and prognosis of Caroli’s disease (CD). MethodsA retrospective analysis was performed for the clinicopathological data of 5 parents with CD who were treated in Beijing Tsinghua Chang Gung Hospital from January 2014 to May 2019, and a literature review was also performed. ResultsThere were 3 male and 2 female patients, with a mean age of 20 years. All of them attended the hospital due to the presence of intermittent upper abdominal pain, and among these patients, 4 had pyrexia, 2 had splenomegaly and portal hypertension, 1 had gallstones, 4 had bilateral renal cysts, and 4 had abnormal hepatic function. Imaging examination showed varying degrees of cystic dilatation of the intrahepatic bile ducts which communicated with the biliary tree, and fibrous bands were observed in these cysts. Pathological examination showed intrahepatic bile duct dilatation with fibrous tissue proliferation and chronic inflammatory cell infiltration on the bile duct wall, and fibrovascular bundles were observed in the bile duct cavity. In addition, fibrosis and proliferation of the portal area and abnormal small bile ducts were observed in type II CD. ConclusionCD is often seen in children and adolescents, and its clinical manifestations lack specificity. A confirmed diagnosis of CD relies on imaging and pathological examinations. Histopathological features of this disease include cystic dilatation of the intrahepatic bile ducts communicating with the biliary tree and the bands consisting of fibrous tissue and vessels in these cysts.
2.Efficient coding of natural images.

Efficient coding theory proposes that sensory systems in the brain have been adapted to process natural scenes efficiently over the long history of evolution. Computational modeling the statistical regularities of natural images is therefore beneficial to our understanding of the mechanisms of visual information processing. In this paper, we briefly review the recent progress in using efficient coding approaches to study the encoding of natural images in the visual system.

3.Influencing factors of intraoperative blood transfusion in primary malignant osteosarcoma: a retrospective study

【Objective】 To provide reference for formulating preoperative blood preparation plan for malignant osteosarcoma scientifically and rationally under the persistent COVID-19 epidemic by studying the high-risk influencing factors related to intraoperative blood transfusion in patients with primary malignant osteosarcoma. 【Methods】 The general data, preoperative blood routine and coagulation parameter, clinicopathological record and surgical data of 120 patients with primary malignant osteosarcoma in Beijing Jishuitan Hospital from January 2020 to January 2021 were retrospectively analyzed by univariate analysis, multivariate regression analysis and Pearson correlation analysis to determine the high risk factors for intraoperative blood transfusion in patients with primary malignant osteosarcoma. 【Results】 The incidence of intraoperative and postoperative blood transfusion of malignant osteosarcoma patients were 48.33% (58/120) and 62.50% (75/120), with the average blood transfusion units at (1.36±1.14) U and (2.93±2.26) U, respectively. The univariate analysis showed that such factors as sex, preoperative Hct (%), preoperative Plt (×109/L), location of lesion, Huvos classification, incision length (cm), reconstruction length(cm) and intraoperative blood loss (mL) were significantly different (P<0.05). Multivariate logistic regression analysis showed that sex, location of lesion, Huvos classification, incision length (cm), reconstruction length(cm) and intraoperative blood loss (mL) were significantly different (P<0.05). 【Conclusion】 For primary malignant osteosarcoma with a high rate of intraoperative blood transfusion, early intervention should be carried out according to the high-risk factors of intraoperative blood transfusion, and preoperative blood preparation plan should be accurately formulated to effectively reduce the rate and units of blood transfusion in patients under the premise of surgery safety.
4.Application of surface electromyography in motor function evaluation of patients after elbow fracture surgery

ObjectiveTo investigate the features of surface electromyography (sEMG) signals of elbow and shoulder muscle groups during maximal isometric contraction of elbow joint, and flexion and extension exercises of elbow joint after elbow fracture surgery. MethodsFrom August, 2021 to April, 2022, 15 convalescent patients after elbow fracture surgery (patient group) and 11 healthy controls (control group) were collected. sEMG signals of biceps brachii, triceps brachii, brachioradialis brachii, upper trapezius, anterior deltoid, middle deltoid and posterior deltoid were recorded during maximal isometric contraction, and flexion and extension exercises of the elbow joint. Root mean square (RMS) value, co-synergy contraction ratio (CSR), co-activation radio (CR), and target muscle activation percentage during flexion and extension were calculated. ResultsIn flexion and extension of elbow joint during maximal isometric contraction, the maximum strength of biceps brachii and triceps brachii were lower in the patient group than in the control group (|t| > 4.109, P < 0.01), and the RMS value of triceps brachii was lower in the patient group than in the control group (t = -7.695, P < 0.001). During maximal isometric extension of elbow joint, the CR of biceps brachii and brachioradialis brachii were more in the the patient group than in the control group (t > 2.326, P < 0.05); during maximal isometric flexion of elbow joint, the CSR of upper trapezius was more in the patient group than in the control group (t = 2.232, P < 0.05). During the extension exercise, the activation level of triceps brachii was more in the patient group than in the control group (t = 3.336, P < 0.05); during flexion exercise, the activation level of biceps brachii and triceps brachii was more in the patient group than in the control group (t >2.339, P < 0.05). ConclusionThere is abnormal contraction pattern of elbow and shoulder muscle groups in patients with elbow fracture after operation, which includes obvious co-contraction pattern of elbow flexor muscle group, insufficient activation of elbow extensor muscle and the compensatory contraction pattern of perishoulder muscle group. sEMG has great potential in quantitative evaluation of motor function after elbow fracture.
5.Regulatory effect of curcumin on renal apoptosis and its mechanism in overtraining rats.

OBJECTIVE:
To study the effects and mechanisms of curcumin alleviating oxidative stress induced by overtraining and inhibiting renal apoptosis in rats.
METHODS:
Male Wistar rats of 7 weeks old were divided into control group (C group, 12), overtraining group (OM group, 11), curcumin + overtraining group (COM group, 14). Group C did not undergo any exercise intervention. Rats in OM group and COM group underwent 8-week incremental load swimming training. During the training, the COM group was treated with curcumin at the dose of 200 mg/(kg·d) in the volume as 5 ml/kg by intragastric administration, and the other groups was treated with an equal volume of 0.5% carboxymethylcellulose. Twenty-four hours after the last training, renal histopathological changes were observed by light microscopy, related biochemical indicators in blood and renal tissue were detected.
RESULTS:
The results showed that after 8 weeks of incremental load swimming training, the renal tissue structure of group C was normal under light microscope; histopathological changes were observed in OM group; COM group was significantly relieved compared with OM group. Compared with group C, serum levels of corticosterone (Cor), creatinine (Cr) and blood urea nitrogen (BUN) in OM group were increased (<0.01), serum level of testosterone (T) was lower (<0.01); the expression of nuclear factor erythroid 2-related factor 2 (Nrf2) was not changed significantly (>0.05), while the expression of heme oxygenase-1 (HO-1) was decreased (<0.05), total antioxidant capacity (T-AOC) and superoxide dismutase (SOD) activity were decreased (<0.01), malondialdehyde (MDA) concentration was increased (<0.01); the renal apoptosis was increased (<0.01), the expression of anti-apoptotic B cell lymphoma-2 protein (Bcl-2) was decreased (<0.01), and the expression of proapoptotic Bcl-2 associated X protein (Bax) was increased (<0.01). Compared with the OM group, Cor level was decreased (<0.01) in the COM group, T level was increased (<0.01), Cr and BUN levels were lower (<0.05); the expression of Nrf2 and HO-1 were increased (<0.05), T-AOC and SOD activity were increased (<0.01), MDA concentration was decreased (<0.05); the renal apoptosis was decreased (<0.05), the expression of Bcl-2 was increased (<0.05), and the expression of Bax was decreased (<0.01). The trend of testosterone/corticosterone ratio between groups was consistent with testosterone change, and the change trend of Bcl-2/Bax ratio was consistent with the change of Bcl-2.
CONCLUSIONS
The 8-week incremental load swimming training triggered excessive training in rats, aggravated oxidative stress and accelerated renal apoptosis, leading to pathological changes and dysfunction of kidney. Curcumin can up-regulate expression of Nrf2 and HO-1, effectively alleviates oxidative stress induced by overtraining, thereby increasing Bcl-2 expression, decreasing Bax expression, inhibiting renal apoptosis and protecting renal tissue structure and function properly.

6.Current situation analysis and suggestions on clinical pharmacy undergraduate internship education in China

OBJECTIVE To provide a reference for the improvement of the clinical pharmacy undergraduate internship education in China. METHODS The status of clinical pharmacy undergraduate internship education in China was briefly summarized, and suggestions on its future development based on advanced methods of domestic and abroad were provided. RESULTS & CONCLUSIONS In China, undergraduate students of clinical pharmacy were required to undergo hospital internships for no less than 42 weeks. The internship took place in both pharmacy and clinical departments in hospitals. Students’ performance was usually assessed at the end of the internship. The mainstream teaching patterns included on-site teaching, simulation teaching, case-based discussion teaching, medical and pharmaceutical joint teaching, team-based teaching, academic exchange, and so on. The form of assessment had also shifted from final evaluations to more dynamic formats. The clinical pharmacy undergraduate internship education in China can adopt teaching models such as the layered teaching approach, establish pre-internship assessment or training, and encourage interns to participate in social pharmacy practice and directly interact with patients, etc., so as to improve the effect of internship.
7.Monitoring and evaluation of environmental radioactivity levels in Beijing, China, 2015—2022

Objective To investigate the present situation of radiation environment in Beijing and provide data support for monitoring and management of radiation environment in Beijing. Methods The γ radiation dose rate of environmental surface as well as the radioactive levels of aerosol, sediment, surface water, and soil samples were monitored in Beijing from 2015 to 2022. The radioactive levels of various environmental elements in this area were presented using statistical charts and statistical tables, and the related issues were discussed. Results The automatic monitoring and cumulative monitoring results of γ radiation dose rate were in the range of local natural background fluctuation. The radioactive levels of beryllium-7, lead-210, and polonium-210 in aerosols were low in summer and high in winter. The radioactive levels of total α, total β, and beryllium-7 in sediments were low in winter and high in summer. The activity concentration of radionuclide potassium-40 in aerosols and sediments did not change significantly with seasons. The total α and total β radioactivity levels of surface water fluctuated within the range of background. These values of reservoir water were within the limits specified in the Hygienic Standard for Drinking Water (GB 5749—2006). Natural radionuclides in soil were within the range of natural background fluctuation, and artificial radionuclides in soil were within the range of normal fluctuation over the years. Conclusion From 2015 to 2022, the environmental surface γ radiation dose rate as well as aerosol, sediment, surface water, and soil radioactivity levels in Beijing were generally low, all of which fluctuated within the environmental background.
8.Effect of Kansui Radix and Glycyrrhizae Radix et Rhizoma in Gansui Banxiatang on CYP450 Enzyme in Ascites Rats

ObjectiveTo explore the relationship between Kansui Radix and Glycyrrhizae Radix et Rhizoma in the Gansui Banxiatang based on cytochrome P450 (CYP450) enzyme and ascites rats. MethodA total of 150 Wistar rats were randomized into blank group (distilled water, ig), model group (distilled water, ig), Gansui Banxiatang group (5.68 g·kg-1·d-1, ig), Gansui Banxiatang without Kansui Radix group (5.57 g·kg-1·d-1, ig), Gansui Banxiatang without Glycyrrhizae Radix et Rhizoma group (4.01 g·kg-1·d-1, ig), and Gansui Banxiatang without Kansui Radix And Glycyrrhizae Radix et Rhizoma group (3.90 g·kg-1·d-1, ig), with 25 rats in each group. The Gansui Banxiatang was composed of 1.1 g Kansui Radix, 9 g Pinelliae Rhizoma Praeparatum, 15 g Paeoniae Radix Alba, 16.7 g Glycyrrhizae Radix et Rhizoma praeparata cum melle, and 15 g honey. The rats, except for the blank group, were injected (ip) with Walker-256 cells to induce cancerous ascites. The administration lasted 7 days for each group. The concentration of probe drugs was detected by high performance liquid chromatography (HPLC) and based on the metabolic rates of the drugs, the CYP450 enzyme activity was yielded. The CYP450 enzyme gene expression was determined by Real-time PCR, and CYP450 enzyme protein expression by Western blot. ResultCompared with the blank group, the model group showed high activity of CYP1A2 and CYP2C9, low activity of CYP2E1, CYP2C19, and CYP3A4, low mRNA expression of CYP2E1, CYP2C19, CYP2C9, and CYP3A4, and low protein expression of CYP2E1 and CYP3A4 (P<0.05). Compared with the model group, Gansui Banxiatang group displayed low activity of CYP2C9 and high expression of CYP3A4 mRNA (P<0.01), and Gansui Banxiatang without Kansui Radix group demonstrated low activity of CYP1A2, high activity of CYP2E1 and CYP3A4, high mRNA expression of CYP1A2, CYP2E1, CYP2C19, and CYP3A4, and high protein expression of CYP2E1 and CYP3A4 (P<0.05). In comparison with the model group, the Gansui Banxiatang without Glycyrrhizae Radix Et Rhizoma group showed low activity of CYP2C9, high activity of CYP3A4, and high mRNA expression of CYP2E1 and CYP2C19 (P<0.05), and Gansui Banxiatang without Kansui Radix and Glycyrrhizae Radix Et Rhizoma group had low activity of CYP2C9, high activity of CYP3A4, and high CYP3A4 mRNA expression (P<0.01). The activity of CYP2E1, CYP2C9, and CYP3A4 was higher, and the mRNA expression of CYP1A2, CYP2C19, and CYP3A4 and CYP3A4 protein expression were higher in the Gansui Banxiatang without Kansui Radix group than in the Gansui Banxiatang group (P<0.05). The activity of CYP2C9 and CYP3A4, and CYP2C19 mRNA were higher and mRNA expression of CYP3A4 was lower in the Gansui Banxiatang without Glycyrrhizae Radix Et Rhizoma group than in the Gansui Banxiatang group (P<0.05). The activity of CYP2C9 and CYP3A4 in the Gansui Banxiatang without Kansui Radix and Glycyrrhizae Radix et Rhizoma group was higher than that in the Gansui Banxiatang group (P<0.01). ConclusionThe four prescriptions all showed certain efficacy in the perspective of CYP450 enzyme, among which Gansui Banxiatang without Kansui Radix had the best effect and was better than Gansui Banxiatang. The Kansui Radix and Glycyrrhizae Radix et Rhizoma did not show antagonism in Gansui Banxiatang, but reduced the effect. Thus, in this study, the medicinal pair may show mutual inhibition.
9.Effect of juhua jueming powder in preventing high-risk corneal transplantation immune rejection in rats.

OBJECTIVETo study the inhibitory effect of Juhua Jueming powder (JJP) on high-risk corneal transplantation immune rejection in rats.
METHODSThe high-risk corneal transplantation immune rejection rat model was established by inducing corneal neoangiogenesis by suture method and the penetrating transplantation. Model rats were divided into two groups, the treated group and the control group, they were administered with JJP 0.1 g (dissolved in 2 mL water) and normal saline respectively via gastric infusion every day after transplantation. The survival status of the allograft, histopathology, local and systemic immune status in the recipients were observed using immunofluorescence histochemistry and flow cytometry.
RESULTSThe survival time of the allograft in the treated group (14.50 +/- 3.55 days) was significantly longer than that in the control group (8.25 +/- 0.71 days, P < 0.01). Levels of Fas, FasL expressions in iris were stronger, and the percentage of CD4 CD FOXP3 positive cells in peripheral blood was less (5.11 +/- 3. 92% vs. 14.81 +/- 2.58%) in the control group than those in the treated group respectively. The concentration of IL-2 was lower while that of IL-10 was higher in aqueous humor of the treated group than those of the control group, respectively.
CONCLUSIONJJP has certain effect for preventing high-risk corneal transplantation immune rejection in rat model.

By summarising Professor WANG Xianbo's clinical experience in treating hepatitis B virus associated acute-on-chronic liver failure (HBV-ACLF), it is believed that HBV-ACLF is a syndrome of root-cause deficiency and manifestation excess, with spleen deficiency as the root cause and dampness-heat-toxicity-blood stasis as the manifestation, and the therapeutic methods proposed as “detoxification and cooling of the blood to promote circulation of the internal organs, and strengthening spleen and resolving dampness to take care of the middle energizer”. In the treatment of HBV-ACLF, for syndrome of stasis-heat-toxicity mass, it was common to use the Jiedu Liangxue Formula (解毒凉血方) by detoxifying and cooling blood; For syndrome of dampness-heat-toxicity mass, it was common to use in the Jiedu Liangxue Lishi Formula (解毒凉血利湿方) by detoxifying and cooling the blood, strengthening the spleen and resolving dampness; For syndrome of spleen-deficiency and dampness-heat, it was common to use in the Jiedu Liangxue Jianpi Formula (解毒凉血健脾方, also known as Zhonggan No.2 Formula) by strengthening the spleen and reple-nishing qi, clearing heat and resolving dampness.
