Histone methyltransferase modulates heterochromatin formation,genomic imprinting and genetic transcription by regulating the combination of histones and DNA.As encoding genes of histone methyltransferase,mixed-lineage leukemia (MLL) genes are found to have close relationship with tumors.Recently,MLL2,a member of this family,has been found highly expressed anomalously in breast cancer,colorectal cancer and lymphoma.Whether MLL2 participates in the progression of cancer,the time phase of its participation and its specific role are still remained to further study.
2.Effects of red light flashing therapeutic instrument combined with distance visual training through reading red visual acuity chart on myopic amblyopia in children

AIM: To explore the effects of red light flashing therapeutic instrument combined with distance visual training through reading red visual acuity chart on myopic amblyopia in children.
METHODS: Totally 112 cases of children patients with myopic amblyopia treated in our hospital were assigned to the control group and the observation group. The control group was given short covering method and short-range visual function training, and the observation group was given red light flashing therapeutic instrument combined with distance visual training through reading red visual acuity chart on the basis of the above treatment. The clinical efficacy, refractive dynamic changes, the BCVA and amblyopia recurrence rate were compared between the two groups.
RESULTS: The total clinical effective rate in the observation group was significantly higher than that in the control group(P<0.05). Before and after treatment, the myopic diopter was no significant difference in the observation group(P>0.05), the control group gradually increased myopic diopter with time(P<0.05). The myopic refraction, the change value of average annual refraction and the recurrence rate of amblyopia after 1y of treatment in the observation group was lower than that in the control group(P<0.05). The BCVA after 1y of treatment was significantly higher than that after 6mo of treatment(P<0.05).
CONCLUSION: Red light flashing therapeutic instrument combined with distance visual training through reading red visual acuity chart for children patients with myopic amblyopia can effectively reduce the change value of average annual refraction, correct the best visual acuity, and reduce the risk of amblyopia recurrence.
3.Comparison of Diagnostic Performance Between PI-RADS v2.1 and PI-RADS v2 for Prostate Cancer: A Meta-analysis

Objective To compare the diagnostic performance of PI-RADS v2.1 and PI-RADS v2 in the detection of clinically significant prostate cancer(csPCa) by Meta-analysis. Methods The major biomedical databases were searched (CNKI, CBM, Medline, and Embase) with the keywords "PIRADS v2.1" or "PI-RADS v2.1". The Quality Assessment of Diagnostic Accuracy Studies Tool v2 (QUADAS-2) was used to evaluate literature quality. Meta-analysis was performed using STATA17.0 and ReMan5.4 software. Forest plots were used to represent the sensitivity and specificity of PI-RADS v2.1 and PI-RADS v2 for each study. Sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and diagnostic odds ratio were combined, and diagnostic performance was evaluated using asummary receiver operating characteristic curve (SROC). Subgroup analysis was performed on three covariables: tumor location, threshold, and the nationality of authors. Results A total of 12 studies were included, involving 3 158 patients and 3 243 lesions. Forall zones and the whole gland, PI-RADS v2.1 had a larger area under the SROC curve (AUC) for csPCa performance, compared with PI-RADS v2. Subgroup analysis: PI-RADS v2.1 also had a larger area under the SROC (AUC) to detect transitional zone csPCa. Different diagnostic thresholds: when a score of 4 was used for the threshold, PI-RADS v2.1 had the maximum area under SROC (AUC) for csPCa performance detection. Author nationality: Researches of PI-RADS v2.1 in Chinese authors had the largest area under the SROC (AUC) in detecting csPCa performance. Conclusion Compared with PI-RADS v2, the diagnostic performance of PI-RADS v2.1 in detecting csPCa is not obviously improved and overall specificity is still low.
4.Clinicopathological features and prognosis of immunoglobulin A nephropathy after renal transplantation

Objective:To summarize the relationship between the clinicopathological features and prognosis of immunoglobulin A nephropathy (IgAN) after renal transplantation.Methods:A total of 34 patients with IgAN after renal transplantation confirmed by renal biopsy were enrolled. And another 34 patients with primary IgAN confirmed by initial renal biopsy were adopted as controls. Clinical and pathological features of two groups were compared to explore the relationship between clinicopathological features and prognosis of allograft IgAN.Results:As compared with primary IgAN group, renal function in allograft IgAN group included serum creatinine [(158.5±75.9) vs (84.8±26.8) umol/L], urea nitrogen [(9.7±6.1) vs (5.2±1.4) mmol/L], uric acid [(406.7±87.8) vs (359.0±92.6) umol/L], estimated glomerular filtration rate {(57.4±25.4) vs (91.2±28.6) [ml/(min·1.73m 2)]}. All were statistically significantly higher ( P<0.05) while other parameters showed no differences. Pathologically, the proportion of T1 type (50.0% vs 17.6%) of renal tubular atrophy/interstitial fibrosis was significantly higher in allograft IgAN group than control group ( P<0.05). Furthermore, univariate and multivariate Logistic regression analyses were performed between various pathological parameters and prognosis in allograft IgAN patients. It indicated that the degree of mesangial hyperplasia of patients with transplanted IgAN had a significantly negative impact on the prognosis. Conclusions:The clinicopathological features of patients with allograft IgAN show no difference from those of patients with primary IgAN. And among patients with allograft IgAN, those with severe mesangial hyperplasia often have a worse prognosis.
5.Electromechanical Effects of Bone Remodeling Based on Edge Smoothed Finite Element Method

Objective Aiming at solving the problem of poor accuracy for numerical solution of traditional finite element method (FEM) in numerical analysis on piezoelectric effects of bone remodelling, a model with an edge-based smoothed FEM (ES-FEM) was proposed. Methods The bone model was discretized by triangular elements, and the smoothing domain was constructed based on edges of the existing mesh element. Based on gradient smoothing technique, the smoothed strain gradient and the smoothed electric field gradient were obtained, and the discrete equations of the system were constructed under the framework of smoothed Galerkin weakform. Results The changes of bone mineral density (BMD) and the distributions of electric potential under piezoelectric effects in the process of bone remodelling were reflected by using the above model. Compared with FEM, ES-FEM could improve the accuracy of simulation result for bone remodelling to a certain extent. Conclusions The proposed ES-FEM can simulate the process of bone remodelling more accurately. The accurate prediction for piezoelectric effect of bone reconstruction by this method provides an effective theoretical basis for clinical research of bone diseases.
6.The predictive role of the expression ratio of androgen receptor splice variant-7 and androgen receptor in the prognosis of prostate cancer treated by castration therapy

Objective To evaluate the prognostic value of the ratio of AR and AR-V7 expression in prostate cancer treated by castration therapy.Methods Immunohistochemical staining was performed in biopsy specimen of 136 prostate cancer patients received hormone therapy in Tongji Hospital of Huazhong University of Science and Technology from January 2010 to December 2015.Expression was determined using modified H score method.Patients aged from 53-96,the median age was 71,median tPSA value at diagnosed was 110.00 ng/ml(2.61-4 003.4 ng/ml),median fPSA value was 14.62 ng/ml(0.12-640.19 ng/ml),median PSA density was 1.15 ng/(ml · cm3) [0.02-62.63 ng/(ml · cm3)].Among these,88 (64.7%)patients were diagnosed Gleason score≥8,39(28.7%) patients with Gleason score 7,while 7 (6.6%) patients Gleason score <7.There were 54(39.7%) patients diagnosed T4 stage,57(41.9%)patients T3 stage and 25 (18.4%) patients Tx stage;62 (45.6%) patients were diagnosed N 1 stage,46 (33.8%) N0 stage and 28(20.6%) patients Nx stage;97(71.3%) patients were diagnosed M1 stage,30(22.1%) M0 stage,9 (6.6%) patients Mx stage.Cause-specific Cox regression and Kaplan-Meier Analysis were used to analyze the prognosis risk.Results The median follow-up time was 44 months,ranged 15-71 months.During the surveillance,the disease progression-free survival time ranged from 5-59 month,median 19 months.The overall survival time ranged from 12-61 months,median 31 months.Among these,79(58.1%) patients were AR positive and 26(19.1%) patients were AR-V7 positive,while AR and AR-V7 expression had no significant correlation (Spearman-test r =0.042,P =0.629).The AR-V7 positive patients showed significantly lower CRPC progression free survival (10.8 months vs.25.0 months,P < 0.001) and much lower overall survival (20.3 months vs.42.8 months,P < 0.001).The high AR-V7/AR expression ratio group showed significantly lower CRPC progression free survival (12.0 months vs.24.8 months,P < 0.001) and much lower overall survival (22.5 months vs.42.8 months,P < 0.001).In univariate Cox regression analyses,Gleason score at diagnosis,T stage,tPSA,PSA density and high AR-V7/AR expression ratio could predict the prognosis of hormonal therapy.While in multivariate Cox regression analyses,T stage(HR =2.597,95% CI 1.351-4.995,P =0.004) and high AR-V7/AR expression ratio(HR =5.788,95% CI 2.530-13.242,P < 0.001) could effectively and independently predict the prognosis of hormonal therapy.Conclusion High AR-V7/AR HS ratio is the independent predictor of the prognosis of prostate cancer hormone therapy.AR-V7 positive and high AR-V7/AR HS ratio patients may have shorter PFS and overall survival time than AR-V7 negative and low AR-V7/AR HS ratio patients.
7.Value of MELD 3.0, MELD, and MELD-Na scores in assessing the short-term prognosis of patients with acute-on-chronic liver failure: A comparative study

ObjectiveTo investigate the value of MELD 3.0, MELD, and MELD-Na scores in assessing the 90-day prognosis of patients with acute-on-chronic liver failure (ACLF) through a comparative study. MethodsA retrospective analysis was performed for the clinical data of 605 patients with ACLF who were treated in Tianjin Third Central Hospital, The Fifth Medical Center of Chinese PLA General Hospital, and Beijing YouAn Hospital from November 2012 to June 2019, and according to the 90-day follow-up results after admission, they were divided into survival group with 392 patients and death group with 213 patients. The receiver operating characteristic (ROC) curve, the area under the ROC curve (AUC), net reclassification improvement (NRI), integrated discrimination improvement (IDI), and decision curve analysis (DCA) curve were used to investigate the value of MELD 3.0, MELD, and MELD-Na scores at baseline, day 3, week 1, and week 2 in predicting the prognosis of the disease. ResultsAt day 3 and week 1, MELD 3.0 score had an AUC of 0.775 and 0.808, respectively, with a better AUC than MELD score (P<0.05). At day 3, week 1, and week 2, MELD 3.0 score showed an NRI of 0.125, 0.100, and 0.081, respectively, compared with MELD in predicting the prognosis of ACLF patients, as well as an NRI of 0.093, 0.140, and 0.204, respectively, compared with MELD-Na score in predicting prognosis. At baseline, day 3, week 1, and week 2, MELD 3.0 showed an IDI of 0.011, 0.025, 0.017, and 0.013, respectively, compared with MELD in predicting the prognosis of ACLF patients. At day 3 and week 2, MELD 3.0 showed an IDI of 0.027 and 0.038, respectively, compared with MELD-Na in predicting the prognosis of ACLF patients. All the above NRIs and IDIs were >0, indicating a positive improvement (all P<0.05). DCA curves showed that MELD 3.0 was superior to MELD at day 3 and was significantly superior to MELD-Na at week 2. There was no significant difference in the ability of the three scores in predicting the prognosis of ACLF patients with different types, and there was also no significant difference in the ability of the three scores in predicting the prognosis of ACLF patients with the etiology of HBV infection, alcohol, or HBV infection combined with alcohol, while MELD 3.0 was superior to MELD for ACLF patients with other etiologies (P<0.05). ConclusionMELD 3.0 score is better than MELD and MELD-Na scores in predicting the 90-day survival of patients with ACLF, but with limited superiority.
