2.The influence analysis of public health emergencies on mood fluctuations of depression and related factors in the elderly in Shanghai

Objective:To analysis the impact of public health emergencies on mood fluctuations of depression and the related factors in the elderly in Shanghai, in order to provide a new evidence for early psychological intervention.Methods:A cross-sectional survey was conducted among 983 elderly in the Department of Geriatrics of Xinhua Hospital Affiliated to Shanghai Jiaotong University and two communities in Changning and Hongkou Districts of Shanghai by using a self-compiled social life questionnaire and Geriatric Depression Scale(GDS-30)during corona virus disease(COVID)-19 outbreak from 1 February to 15 February 2020.The mood fluctuations of depression and the related influencing factors were analyzed by comparing the current GDS-30 scores with the scores within previous six months.Results:Of 983 questionnaires, 867 were valid.The incidence of mild fluctuations of depression was increased from 20.9%(181/867)to 27.2%(235/867)during the public health emergencies( P<0.05), with 3 cases of newly emerged severe depression.The GDS-30 scores were higher during the public health emergencies than before the emergencies(9.88±3.85 vs.7.67±3.54, P<0.05). The four risk factors inducing fluctuations of depression in the elderly were the number of coexisted chronic diseases ≥2( P=0.036), the lack of visits or communication from families( P=0.015), the family members exposing a risk to COVID-19( P<0.01), and the daily viewing of epidemic news more than 4 h( P=0.023). Linear regression analysis showed that cough, sore throat, dizziness, sleep disturbance and dyspnea were significantly related to the aggravation of depression(based on the increase of GDS-30 score)( P<0.05). Conclusions:Public health emergencies may exacerbate depression in the elderly.They are more likely to experience the aggravation of depression when they suffer from multiple chronic diseases, lack the child visits or communications, have the family members exposing a risk to COVID-19 and pay excessive attention to the epidemic.
3.Practice on the case-based teaching innovation of Science of Meridians, Collaterals and Acupoints.

The case-based teaching method which is applied to the teaching of Science of Meridians, Collaterals and Acupoints is discussed in this paper, in which the typical cases such as the growth of the acupuncture-moxibustion eminent physicians, the application of acupoints by the eminent physicians and the experiences in the acupoint combination are integrated. The students are instructed to launch the clinical practical activity through establishing the clinical base of Science of Meridians, Collaterals and Acupoints, in association with the true cases. It is proved that the case-based teaching method can promote the training of the manipulation techniques of the students and the inheritance of the experiences of eminent physicians in higher education.

4.Values of glycosylated hemoglobin in screening for patients with prediabetic state in Guangzhou region

Objective To study the values of glycosylated hemoglobin in screening for patients with prediabetic state in Guang-zhou region .Methods 525 Guangzhou people who had accepted health examination were enrolled and were subjected to oral glucose tolerance test(OGTT) .BIO-RAD D-10 automatic glycosylated hemoglobin analyzer was employed to detect their glycosylated he-moglobin A1c(GHbA1c) .OGTT results were served as diagnostic criteria ,Receiver operator characteristic (ROC) curve analysis was performed to obtain the optimal threshold of GHbA1c in diagnosing impaired glucose regulation (IGR) .Results The optimal threshold of GHbA1c in diagnosing IGR was 5 .95% .The sensitivities of GHbA1c≥5 .95% and GHbA1c≥5 .7% in diagnosing IGR were 53 .3% and 84 .8% ,respectively ,while their specificities were 72 .8% and 31 .0% ,respectively .The difference of sensitivity between GHbA1c≥5 .95% combined with FPG≥5 .6 mmol/L and GHbA1c≥5 .7% alone in diagnosing IGR showed no statistical significance(P= 0 .406) ,while the specificity increased obviously (P= 0 .000) .Conclusion The criteria of GHbA1c≥5 .7% can be used for prediabetic state screening but not for diagnosis .GHbA1c≥5 .95% combined with FPG≥5 .6 mmol/L can be used effectively for prediabetic state screening in Guangzhou people .
5.Impact of third-party bone marrow mesenchymal stem cells on allogenic skin transplantation.

OBJECTIVETo investigate the effect of the third-party bone marrow-derived mesenchymal stem cells (BMSCs) on the allogeneic skin transplantation.
METHODS40 female C57BL/6 mice and 50 male BALB/C mice were respectively used as donors and recipients of skin transplantation. 50 BALB/C mice were divided randomly into 5 groups: Blank control group, Cyclophosphamide group BMSCs group, Cyclophosphamide + BMSCs group and CM-DiI staining group, with 10 mice in each group. Before skin transplantation, high-dose abdominal injection of Cyclophosphamide (200 mg/kg, 2 d) was performed in recipient mice. On the transplantation day, a bonus of 1 x 10(5) BMSCs of the SD rat (SD-BMSCs) were injected through the tail vein. The observation of skin grafts, mixed lymphocyte culture (MLC), HE staining, the observation of CM-DiI-labeled SD-BMSCs and FACS were used.
RESULTSThe skin graft survival time was significantly prolonged in the Cyclophosphamide + BMSCs group, as compared with the blank control group, the Cyclophosphamide group, the BMSCs group respectively. When BMSC and lymphocyte mixed at the ratio of 1:1 and 1:10, rat BMSCs inhibited T lymphocyte proliferation. More angiogenesis and less lymphocyte infiltration were found in the experimental group than them in other groups. Red fluorescent cells were found in CM-DiI staining group under long-term observation. The SD-BMSCs can he detected by flow cytometry in the cell group and the Cyclophosphamide + BMSCs group.
CONCLUSIONSBMSCs can survive in the heterogeneous recipient body; the third-party BMSCs transplantation can prolong skin graft survival time; BMSCs can inhibit T lymphocyte activation and proliferation.

6.Comparison of the clinical effects of intravenous or oral iron on renal anemia in patients with maintenance hemodialysis effects

Objective To compare the therapeutic effects and adverse reactions between intravenous and oral iron,and the effects of erythropoietin (EPO)in patients with chronic renal failure.Methods Between November,2007 and October,2008,48 patients with renal anemia were randomized into 2 groups:intravenous group and oral group,24 in each.The duration of administration lasted for 10 weeks.All patients received regular EPO,with the dose of 100U·kg~(-1),per week. If the patient's hemoglobin (Hb) was more than 100 g·L~(-1),then the dose of EPO decreased by 50%.If the patient's Hb was more than 120 g·L~(-1),then the dose decreased by 75%.The levels of Hb,hematoerit,serum ferritin,adverse reaction and dose of EPO before and 4 weeks,8 weeks and 10 weeks after the administration were observed.Results The levels of Hb,Hematocrit (Hct)and ferritin(SF)in both groups after administration increased compared with before administration;and there was significant difference between the intravenous group and the oral group(P<0.01).The dosages of EPO in both groups after administration were lower than before administration,whereas the dosage in the intravenous group decreased significantly(P<0.05).In the intravenous group,the adverse reactions were significantly lower and the effect of EPO was significantly higher than in the oral group.Conclusion The intravenous iron can not only supply the iron the patients with renal anemia needed,but also increase the effect of EPO and reduce the dosage of EPO.The adverse reactions of intravenous iron are lower and it is safe and convenient.
7.The Inhibition of Medicine Composition Extracts from Keliu Pills on Human Lung Cancer Cells

OBJECTIVE To study the inhibitory effects of five medicine composition extracts from Keliu Pills on human lung adenocarcinoma A549 cells and streamline the prescription.METHODS MTT assay was used to investigate antiprolifera-tive effects of five extracts(total astragalosides,supercritical fluid extracts of cantharis,alcohol extract of insect,total alka-loids from tonkinensis and aqueous extract of astragalus and tonkinensis)on A549 cells.Five factors two levels orthogonal ex-periment design was used to determine the different antiproliferative effects of five extracts against A549 cells and to explore the interaction between each extract.RESULTS Cytotoxic experiment results showed that IC50 values of supercritical fluid ex-tracts of cantharis,alcohol extract of insect drugs,total astragalosides and total alkaloids from tonkinensis were 0.330,1.041, 1.961,1.041 mg/mL,respectively.The inhibitory rates of supercritical fluid extracts of cantharis,alcohol extract of insect drugs reached the peak at 48 h,with a concentration-effect and time-effect relationship.Orthogonal experiment showed that al-cohol extract of insect drugs played a main role in antiproliferation against A549 cells.Supercritical fluid extracts of cantharis and total astragalosides followed.Analysis of variance showed that interaction effects were found between the total astragalo-sides and alcohol extract of insect drugs,as well as supercritical fluid extracts of cantharis and alcohol extract of insect drugs. CONCLUSION The alcohol extract of insect drugs,supercritical fluid extracts of cantharis and total astragalosides are neces-sary for Keliu Pills,which can be streamlined for the compatibility of these three extracts.
8.Supportive effects of human aorta-gonad-mesonephros-derived stromal cells on umbilical cord blood LTC-IC.

The objective of this study was to explore the supportive effects of human aorta-gonad-mesonephros (AGM)-derived stromal cells on human umbilical cord blood long-term culture-initiating cells (LTC-IC). A co-culture system was established with human AGM stromal cells and umbilical cord blood CD34(+) cells. Different stromal cells derived from human AGM region (hAGM S1-S5) were plated on 24-well plates as feeder cells. CD34(+) cells were positively selected from human umbilical cord blood through immunomagnetic bead selection method, seeded on the feeder cells, and co-cultured for 8 weeks. The hematopoietic cells were collected at 5, 6, 7 and 8 weeks for CFC analysis. Frequencies of LTC-IC in umbilical cord blood CD34(+) cells after co-culture with AGM stromal cells were detected through limiting dilute analysis (LDA). The results showed that there was no any hematopoietic CFC in the feeder cell-free culture system after 5 weeks of co-culture. However, in AGM feeder cells culture systems, there were still CFCs after 5 weeks of co-culture, which indicated that human AGM stromal cells could maintain LTC-IC in vitro. In groups of hAGM feeders, hAGMS3 and S4 had better supportive effects than other AGM groups (P < 0.05). The absolute number of LTC-IC in hAGM S3 and S4 culture systems got expansion up to (176 +/- 46)% and (187 +/- 52)% respectively without significant difference between hAGMS3 and S4 (P > 0.05). It is concluded that human AGM stromal cells S1-S5 support the maintenance of umbilical blood LTC-IC in vitro, while hAGMS3 and S4 cells have better effects on maintaining LTC-IC and expansion of LTC-IC.

9.Analysis on preparation and characterization of asiaticoside-loaded flexible nanoliposomes.

Asiaticoside is a compound extracted from traditional Chinese medicine Centella asiatica, and mainly used in wound healing and scar repair in clinical, with notable efficacy. However, its poor transdermal absorption and short action time restrict its wide application. In this experiment, the reserve-phase-extrusion-lyophilization method was conducted to prepare the lyophilized asiaticoside-loaded flexible nanoliposomes (LAFL). Its characteristics including electron microscope structure, particle size, Zeta potential, entrapment rate, drug-loading rate, stability and drug release were determined with the intelligent transdermal absorption instrument. LAFL were white spheroids, with pH, particle size and zeta potential of 7. 03, 70. 14 nm and - 36. 5 mV, respectively. The average entrapment rate of the 3 batch samples were 31. 43% , and the average asiaticoside content in 1 mg lyophilized simple was 0. 134 mg. The results indicated that LAFL have good physicochemical properties and pharmaceutical characteristics, with an improved transdermal performance.

OBJECTIVE:
To investigate the current status of antibiotic use for very and extremely low birth weight (VLBW/ELBW) infants in neonatal intensive care units (NICUs) of Hunan Province.
METHODS:
The use of antibiotics was investigated in multiple level 3 NICUs of Hunan Province for VLBW and ELBW infants born between January, 2017 and December, 2017.
RESULTS:
The clinical data of 1 442 VLBW/ELBW infants were collected from 24 NICUs in 2017. The median antibiotic use duration was 17 days (range: 0-86 days), accounting for 53.0% of the total length of hospital stay. The highest duration of antibiotic use was up to 91.4% of the total length of hospital stay, with the lowest at 14.6%. In 16 out of 24 NICUs, the antibiotic use duration was accounted for more than 50.0% of the hospitalization days. There were 113 cases with positive bacterial culture grown in blood or cerebrospinal fluid, making the positive rate of overall bacterial culture as 7.84%. The positive rate of bacterial culture in different NICUs was significantly different from 0% to 14.9%. The common isolated bacterial pathogens Klebsiella pneumoniae was 29 cases (25.7%); Escherichia coli 12 cases (10.6%); Staphylococcus aureus 3 cases (2.7%). The most commonly used antibiotics were third-generation of cephalosporins, accounting for 41.00% of the total antibiotics, followed by penicillins, accounting for 32.10%, and followed by carbapenems, accounting for 13.15%. The proportion of antibiotic use time was negatively correlated with birth weight Z-score and the change in weight Z-score between birth and hospital discharge (r=-0.095, -0.151 respectively, P<0.01), positively correlated with death/withdrawal of care (r=0.196, P<0.01).
CONCLUSIONS
Antibiotics used for VLBW/ELBW infants in NICUs of Hunan Province are obviously prolonged in many NICUs. The proportion of routine use of third-generation of cephalosporins and carbapenems antibiotics is high among the NICUs.
