Objective To seek a time-saving,economical and safe method on cleaning and disinfecting endotraeheat tubes,to improve nursing work efficiency. Methods The endotracheal tubes were divided into four groups, which were sterilized by Xiaojiajing solution,3% hydrogen peroxide solution, rapid multi-enzyme cleaner in advanced and regular method respectively. Each group include 30 tubes. The germ contaminations and flora distribution of endotracheal tubes,as well as cost and time of each method were analized before and after disinfection. Results Before sterilization, the germ contaminations were insignificantly different between four groups(P >0.05).The sterile effects of the four groups were statistically insignificant(P > 0.05).However,the time-consuming with Xiaojiajing solution,3% hydrogen peroxide solution and rapid multi-enzyme cleaner was obviously shorter compared with the regular method(P < 0.05),Xiaojiajing solution group cost the least. Conclusions The methods with xiaojiajing solution, hydrogen peroxide solution and rapid multi-enzyme cleaner can shorten the time compared with the regular method in different degree.Considering the costeffectiveness, rapid multi-enzyme cleaner has a good effect but expensive, hydrogen peroxide solution was relatively longer but cheaper and Xiaojiajing solution was cheapest method overall, which is worthy to be nsedf in the hospital.

Objective:To obtain a high specificity and high affinity anti-PcrV protein monoclonal antibody which can be used for the treatment of Pseudomonas aeruginosa infected. Methods: The PcrV gene was amplified by PCR using P.aeruginosa PAO1 genome DNA as the template. The expression vector(pET-28a-PcrV) was constructed and transformed into E.coli BL21(DE3).The re-combinant PcrV protein was expressed by IPTG induction and purified by Ni2+affinity chromatography. The specific binders of PcrV were screened by phage display. The genes encoding VH and VL were amplified respectively by PCR using the plasmid of positive clone as the template. Then the recombinant expression vectors were constructed and transfected into 293E cells.Monoclonal antibody were purified by the Protein A affinity resin from the culture supernatants. The affinity of antibody was detected by ELISA and the function of YG5 was verified in murine pneumonia model caused by P.aeruginosa.Results: Recombinant PcrV protein was expressed and purified.A full human monoclonal antibody(named as YG5) against PcrV was obtained by phage display. The results of ELISA showed that YG5 had a high affinity with EC50=61 ng/ml.Furthermore,it was found that YG5 could protect mice from infection caused by P.aeruginosa.Conclusion:Our findings present a novel human monoclonal antibody YG5 against PcrV,which inhibits the infection casued by P.aeruginosa and may be a potential drug for treatment of P.aeruginosa infection.

Either cetuximab or bevacizumab can improve the survival of patients with metastastic colorectal cancer (mCRC) if administered combided with cytotoxic agents. However, the effect of two or more target agents in combination is uncertain in these patients. Here, we reported a patient with mCRC successfully treated by a combination of target agents after the failure of chemotherapy. The patient received palliative resection of primary tumor followed by 9 cycles of postoperative XELOX regimen, cytokine-induced killer cell (CIK)-based biotherapy, traditional Chinese medicine, particle implantation in the lung metastatic lesions. The tumor progressed 20 months after the standard treatments. Then, the regimen cetuximab, bevacizumab and cefitinib was applied. During the treatment with targeted agents, grade IV acne-like rash and relatively severe parionychia of the toes occurred. Both of them recovered smoothly. The PET-CT reexamination at 40 days after the target treatment showed that the metabolism of mediastinal lymph nodes basically recovered to a normal level. The combination of multiple targeted agents obtained a progression-free survival(PFS) of 11 months and the patient with a good quality of life during this period.

Objective:To investigate the effect of post pyloric feeding on hospital-acquired pneumonia and mortality in patients with critical respiratory disease. Methods:A retrospective survey of 41 cases of post pylorus feeding (post pylorus feeding group,n =41) in respiratory intensive care unit (RICU) was carried out,and 41 patients who received nasogastric feeding (nasogastric feeding group,n =41) were investigated in the same RICU.The incidence of ventilator-associated pneumonia (VAP),the incidence of hospital-acquired pneumonia (HAP,excluding VAP) and the in-hospital mortality of RICU were observed and compared between the two groups.Vomiting, aspiration, gastric retention, diarrhea, and the times of stopping enteral feeding were observed and compared as well. Results:The incidence of HAP in the post pylorus feeding group was 2.4%,which was significantly lower than that in the nasogastric feeding group (22%,P < 0.05).The incidences of VAP in the post pylorus feeding group and nasogastric tube feeding group were 16.7% and 8.7% individually, which was no significant difference between the two groups (P > 0.05).The mortality in RICU in the post pyloric feeding group was 36.6%,which was similar to that in the nasogastric feeding group (41.5%) (P > 0.05).There was no difference in terms of vomiting, aspiration, gastric retention, diarrhea, and the times of stopping enteral nutrition. Conclusion:Compared with nasogastric feeding, post pyloric feeding reduces the incidence of HAP in critical respiratory patients.Nevertheless, post pyloric feeding fails to affect the incidence of VAP and in-hospital mortality in RICU.

Cerebrospinal fluid morphology examination is an important method of diagnosing central nervous system diseases, but manual microscopy has shortcomings such as low efficiency, long staff training period, and poor homogeneity of test results. In recent years, the application of artificial intelligence in the medical field has developed rapidly, providing new technical means for cerebrospinal fluid morphology examination. In the future, AI-assisted morphological examination of cerebrospinal fluid will not only realize digitalization and networking, but also improve the level and efficiency of intelligent diagnosis of cerebrospinal fluid morphology, which has a broad application prospect in the intelligent assisted diagnosis of cerebrospinal fluid.

The chemical consituents from the stems and leaves of Psychotria serpens were separated and purified by column chromatographies with silica gel, ODS, Sephadex LH-20 and PR-HPLC. The structures of the isolated compounds were identified on the basis of physicochemical properties and spectroscopic analyses, as well as comparisons with the data reported in literature. 18 compounds were isolated from the 90% ethanol extract of the stems and leaves of P. serpens, which were identified as chrysin(1), acacetin(2), genkwanin(3), chrysoeriol(4), rhamnocitrin(5), isorhamnetin(6), tricin(7), jaceosidin(8), dillenetin(9), kumatakenin(10), ayanin(11), isosakuranetin(12), eriodictyol(13), homoeriodictyol(14), taxifolin(15), pomonic acid(16), fupenzic acid(17) and euscaphic acid(18). All compounds were isolated from the genus Psychotria for the first time.
