1.Regulatory effect of Zn~(2+) on P2X receptor-mediated,ATP-induced currents in different autonomic ganglion neurons in rats

Objective:To compare the effects of Zn~(2+)onthe P2X receptor-mediated,ATP-induced currents in neurons separated from rat superior cervical ganglion(SCG),nodose ganglion(NG),and otic ganglion(OTG).Methods: Whole-cell patch clamp recording technique was used to study the regulatory effects of Zn~(2+) on ATP/??-me ATP-induced currents in the above 3 ganlglion neurons.Results: All SCG neurons responded to ATP with a sustained current,while no neurons responded to ??-me ATP;Zn~(2+) potentiated ATP-induced sustained currents to(1 442?34)% of the original value.All NG neurons responded to ATP and ??-me ATP with a similar sustained current;coapplication of Zn~(2+)(10 ?mol/L) potentiated their responses to(180?12)% and(262?28)%,respectively.All OTG neurons responded to both ATP and ??-me ATP with a sustained current.Coapplication of Zn~(2+)(10 ? mol/L) did not significantly potentiate the sustained currents induced by 10 ?mol/L ATP,but when ATP was at 30 ?mol/L,Zn~(2+)(10-100 ?mol/L) inhibited ATP-induced sustained currents in a dose dependent manner.If TNP-ATP(100 nmol/L) was first used to inhibit ATP-induced current to(26?2)% of the original value,Zn~(2+) at 10 ?mol/L potentiated the inhibited current to(127?9)% of its original value.Coapplication of Zn~(2+)(10 ?mol/L) potentiated ??-me ATP-induced currents to(146?5)% of the control.Zn~(2+)(300 ?mol/L) had no effect on ?_(on) and ?_(off) of ATP-and ??-me ATP-induced(30 ?mol/L) currents in OTG neurons.Conclusion:(1) Zn~(2+) is an allosteric modulator of P2X_(2) and P2X_(2/3) receptors in SCG and NG neurons and can potentiate the currents they induced.(2)The predominant receptor subtypes in OTG appear to be homomeric P2X_(2/3) and a little P2X_(2).Zn~(2+) has an inhibitory effect on the ATP-induced currents in OTG neurons,suggesting some novel members of the P2X purinoceptor exist in these neurons.
2.Diagnosis and surgical management of acute gangrenous cholecystitis early after endoscopic sphincterotomy and endoscopic retrograde cholangiopancreatography

Objective To discuss the diagnosis and surgical management of acute gangrenous cholecystitis (AGC) early after EST and ERCP. Methods Clinical and pathological data of 1468 cases receiving EST and ERCP in our hospital from 2005 to 2007 were retrospectively analyzed. Results AGC occurredin 16 cases and the incidence was 1.09%. Its main manifestations were pain in the right upper abdomen with local pertonitis and fever 1 to 3 d after operation, blood WBC exceeding 15.0× 109/L in 15 cases (93.8%), gallbladder tumefaction and double layer structure by B mode ultrasound, the diagnostic accuracy before operations was 81.3 %. We performed cholecystectomy in 10 cases, cholecystectomy and choledochotomy for common bile duct exploration and stone removal and T-tube drainage in 2, partial cholecystectomy in 3, cholecystostomy in 1. Pathological examination showedthat it was AGC in all the patients. Conclusion Latency in 1 to 3 d, local pertonitis, high blood WBC and B mode ultrasound are main managements in diagnosis of AGC and it should be diagnosed carefully distinguished from perforation and severe acute pencreatitis. It is suggested to undergoing early surgi-cal management once diagnosed definitely.
3.Repeated injection of mitoxantrone containing thermosensitive liposomes in rat induced ABC phenomenon.

To investigate whether accelerated blood clearance (ABC) phenomenon could be induced after repeated injection of mitoxantrone thermosensitive liposomes, LC-MS/MS and enzyme linked immunosorbent assay (ELISA) were used to measure the concentration of mitoxantrone and the anti-polyethylene glycol (PEG) IgM levels in rat plasma, separately. The drug was rapidly cleared away after the second administration. The anti-PEG IgM was detected after the first dose which was neutralized quickly after the second dose. It is proved that repeated administration of mitoxantrone thermosensitive liposomes in rat caused the ABC phenomenon.

7.Nebulized glucocorticoid combined with olfactory training in the treatment of postviral olfactory dysfunction

OBJECTIVETo investigate the efficacy and safety of nebulizing glucocorticoid combined with olfactory training in the treatment of postviral olfactory dysfunction.METHODSPatients with postviral olfactory dysfunction were recruited in this study. All patients underwent T&T olfactory testing, sinonasal computer tomgraphy scanning, as well as magnetic resonance scanning of the olfactory pathway. Nebulizing glucocorticoid (pulmicort repulse) was inhaled once daily at the starting dose of 2 mg tapered to 1 mg after two weeks combined with olfactory training for 4 weeks. T&T olfactory testing were repeated after 4-week treatment.RESULTS Twenty four patients received teatment, with a mean age of 54 years old(range 37 to 81 years old), a mean olfactory dysfunction course of 2.20 months(range, 0.25-9 months). Of whom, 21 were anosmia, 3 were hyposmia. After teatment, complete recovery were achieved in 4 patients(16.7%), obvious improvement in 9 (37.5%), improvement in 5 (20.8%), no improvement in 6 (25.0%). No side effect and untoward effect were found.CONCLUSIONThe primmary outcomes suggest the efficacy and safety of nebulizing glucocorticoid combined with olfactory training in the treatment of postviral olfactory dysfunction.
8.Application of bulbocavernosus reflex in differential diagnosis of patients with Parkinson's disease and multiple system atrophy

Objective To assess the value of bulbocavernosus reflex (BCR) in the differential diagnosis of Parkinson' s disease (PD) and multiple system atrophy (MSA).Methods BCR was compared in 55 patients with PD,20 patients with MSA,and 50 healthy controls,who were enrolled from the Department of Neurology,the First Affiliated Hospital of Wenzhou Medical University from May 2013 to April 2014.The clinical features of autonomic nerves dysfunction were also recorded.Results Among all related autonomic symptoms,the occurrence rate of constipation,urinary urgency and frequency in patients with MSA was higher than those with PD.The elicit percent of BCR in patients with PD was 93%,higher than those with MSA (70%).The average latency of BCR in patients with MSA was longer than those with PD (tmale left =16.275,tmale right =14.269,tfemale left =5.954,tfemmale right =5.905,all P < 0.05).The degree of BCR amplitude decreasing in three groups was MSA > PD > healthy controls.There was statistically significant difference among three groups (Fmale left =75.73,Fmale right =73.13,Ffemale left =72.70,Ffemale right =59.44,all P < 0.05).The area under receiver operating characteristic curve (ROC) in differential diagnosis of PD and MSA of the average latency of BCR in male and female was 0.947 and 0.948.The area under ROC curve in differential diagnosis of PD and MSA of the average amplitude of BCR in male and female was 0.886 and 0.920.The ROC curve showed the average latency of BCR in male of 44.80 ms with a sensitivity of 95% and a specificity of 84%,and in female of 61.35 ms with a sensitivity of 86% and a specificity of 88% ; the average amplitude of BCR in male of 0.37 mV with a sensitivity of 96% and a specificity of 68%,and in female of 0.36 mV with a sensitivity of 98% and a specificity of 76%,which were critical cutoff values in differential diagnosis of PD and MSA with the best sensitivity and specificity.Conclusion The latency and amplitude of BCR test helps to increase the accuracy in the differential diagnosis of PD and MSA.

10.Novel cationic liposome loading siRNA inhibits the expression of hepatitis B virus HBx gene.

In order to solve the problem of selection and in vivo delivery problem in siRNA treatment, hepatitis B virus (HBV) HBx gene which could be targeted by siRNA was studied. The siRNA expression plasmid which specific inhibits HBx expression was obtained by in vitro selection via a dual-luciferase plasmid including HBx-Fluc fusion protein expression domain. The selected siRNA expression plasmid was then encapsulated in PEG-modified cationic liposome, which was devoted into pharmacodynamic studies at both cellular and animal level. The results illustrated that the cationic liposome which encapsulated siRNA expression plasmid could effectively inhibit HBx gene expression both in vitro and in vivo.
