1.Influence of sequential enteral nutrition on nutritional status and prognosis in patients with acute cerebral infarction

Objective To explore the influence of the sequential enteral nutrition(SEN) on nutritional status and prognosis in patients with acute cerebral infarction(ACI).Methods 80 ACI patients complicated with dysphagia were divided into SEN group and control group(40 cases in each group).In the first three days patients in SEN group were treated with nasal feeding short-peptide enteral nutrition[84~126(kJ/kg.d) ],than,which would be replaced with nasal feeding integral-protein enteral nutrition from the forth day.Patients in the control group were provided with ordinary fluid food by nasal feeding.Nutrition support for both groups was at last more than 10 d.In the 1 d,7 d and 14 d after treatment,data of both groups concerned with triceps skin fold(TSF),mean upper arm muscle circumference of triceps(MAMC),serum albumen(Alb),serum prealbum(PAB),heamoglobin(Hb),incidence of infectious complications,National Institutes of Health Stroke Scale(NIHSS) score and Barthel index(BI) were examined.Results Nutritional indexes(TSF,MAMC,Alb,PAB,Hb) in the SEN group were significantly higher than those in the control group(P
2.Observations on the Therapeutic Effect of Acupoint Catgut Embedding on Breast Cystic Hyperplasia of Blood Stasis-phlegm Pattern

Objective To investigate the clinical efficacy of acupoint catgut embedding in treating breast cystic hyperplasia of blood stasis-phlegm pattern.Methods One hundred and three patients with breast cystic hyperplasia of blood stasis-phlegm pattern were randomly allocated to a treatment group of 53 cases and a control group of 50 cases. The treatment group received acupoint catgut embedding and the control group took Rupi Sanjie granules. The symptom/sign scores and the qi stagnation transformation score were recorded and sex hormone levels were measured in the two groups before and after 12 weeks of treatment. The clinical therapeutic effects were compared between the two groups.Results The cure rate and the total efficacy rate were 50.9% and 92.5%, respectively, in the treatment group and 46.0% and 90.0%, respectively, in the control group. There were no statistically significant differences in the cure rate and the total efficacy rate between the two groups (P>0.05). There were statistically significant post-treatment differences in the symptom/sign scores (pain, mass hardness, mass range, mass size and accompanying symptoms scores) between the two groups (P<0.05). At the three-month and six-month follow-ups, there were statistically significant differences in the pain, mass range and accompanying symptoms scores as compared with at the end of treatment in the treatment group, and between the treatment and control groups (P<0.05). There was a statistically significant pre-/post-treatment difference in the qi stagnation transformation score in the two groups (P<0.01,P<0.05). There was a statistically significant difference in the qi stagnation transformation score at the three-month and six-month follow-ups compared with at the end of treatment in the treatment group (P<0.05). There was a statistically significant difference in the qi stagnation transformation score between the treatment and control groups at the end of treatment and at the three-month and six-month follow-ups (P<0.05). There was a statistically significant pre-/post-treatment difference in the E2 content in the two groups (P<0.05). There were statistically significant pre-/post-treatment differences in the P and PRL contents in the treatment groups (P<0.05).Conclusion Acupoint catgut embedding is an effective way to treat breast cystic hyperplasia of blood stasis-phlegm pattern.
3.Assessment of efifcacy of GnRH analogs by LHRH stimulation test in children with central precocious puberty

Objective To evaluate and monitor the efifcacy of GnRH analogs (GnRHa) therapy. Methods Thirty girls with central precocious puberty diagnosied by LHRH stimulation test were treated with GnRHa for 6-24 months. The LHRH stimula-tion test were performed again at 3 months after initiation of therapy and then every 6 months during treatment. The relationship of peark LH and clinical suppressing pubertal (including Turner stage, bone age, grwoth speed) were compared. The monitor effect of peak LH to efifcacy of GnRHa were eveluated. Results Ninety LHRH stimulation tests were performed, only 7 cases were found to have clinical pubertal development. After 6 months treatment, the base LH level of thirty girls (0.48±0.20) IU/L was signiifcantly lower than that before the treatment (0.75±0.35 IU/L) (P=0.000). The correlation coefifcient between base LH and peak LH was 0.62. The best correlation between clinical suppressing pubertal and LHRH stimulation test was achieved when peak LH was less than 2 IU/L (85.7%sensitivity, 100%speciifcity). Conclusions Base LH value can be used in preliminary as-sessment of the efifcacy of GnRHa therapy for girls with central precocious puberty. The peak LH less than 2 IU/L can be as the indicator of treatment efifcacy.

5.In vitro selection of DNA aptamers to Bacillus anthracis spores by SELEX

Objective:To obtain oligonucleotide aptamers which can specifically bind to Bacillus anthracis spores by in vitro selection protocol-SELEX (system evolution of ligands by exponential enrichment).Methods:An in vitro synthesized 78 mer random DNA library (≤1014-15types of different DNAs ) was subjected to 15 rounds of selection using SELEX method against spores of B.anthracis vaccine strain A.16R. Binding of the aptamers to spores was visualized by biotin-streptavidin-horseradish peroxidase system.Results:PCR amplification band pattern of the first round selection was different from that of the ninth round. The binding assay demonstrated that D absorbance at 450 nm of the fifteenth round pool increased 9 times as compared with that of the first round , and the D absorbance increased with the increment of aptamers′ quantity binding to spores. Conclusions: A set of aptamers with considerable binding affinity to B.anthracis spores were successfully selected from the initial random DNA pool.
6.Effect of xiaoyu tablet on blood flow parameters and morphology of carotid artery in atherosclerotic patients.

OBJECTIVETo study the effect of Xiaoyu Tablet (XYT) on blood flow parameters and morphology of carotid artery in atherosclerotic patients.
METHODSUsing color Doppler ultrasonographic technique to examine the blood flow parameters and intimal thickness of carotid artery in 20 patients of carotid atherosclerosis after 24 weeks treatment of XYT, and compared with those in 10 patients treated with gastrodine lipid-lowering tablet.
RESULTSAfter 24 weeks treatment, blood flow parameters of carotid artery were obviously improved and intimal thickness of common carotid arteries in both side was markedly decreased. XYT showed an effect better than that of gastrodine lipid-lowering tablet.
CONCLUSIONXYT is effective in increasing blood flow of cervical and cerebral arteries.

7.Bone regeneration on hard palate after palatal surgery on cleft palate and the relative factors.

OBJECTIVEAttending to observe the bony healing on hard palate after palatal repair, and to discuss the factors affecting on it.
METHODS52 patients with repaired cleft palate over 5 years postoperatively were examined, the CT scan of head was taken. The incidence of the bone regeneration among the patients examined was calculated, the position and quality of bone tissue were measured according to the CT images. After all, analysis was applied to evaluate the factors affecting on the bone tissue formation.
RESULTSFormation of bone bridge was found in the 37 cleft gaps out of 52 patients (71%), the ratio of the sex among the patients who had bone bridge was 1:1, there was no obvious difference between unilateral and bilateral cleft palate. Considering the operation age with the bone formation, the highest percentage of bone bridge formation fell into the group of 4-7 years old, and the most occurring region were in premolar and anterior part of molar area.
CONCLUSIONThere could be bony healing(regeneration bone tissue RBT) after palatal repair on cleft palate patients. The operation age could be an important affecting factor to RBT, but the sex and the clinical type of cleft palate make no difference on the bone tissue formation.

8.The application of scene simulation teaching on cardiopulmonary resuscitation training for the practice students in emergency department

Objective To compare scene simulation teaching method with traditional methods on training in cardiopulmonary resuscitation (CPR) for the practice students. Methods A convenience sample of 62 nursing students in the emergency department of our hospital in 2014 were recruited as the observation group,and 75 practice students in 2013 were recruited as the control group. The observation group used the scene simulation teaching method for students and the control group used the traditional methods. The students′theoretical knowledge, operation skill of CPR and total score of core capability were compared between two groups. Results The theoretical knowledge, operation skill of CPR and total score of core capability in the observation group were (85.23±6.36), (86.90±4.85), (217.98±6.06), significantly higher than those of the control group, which were (75.36±7.77), (82.38±8.84), (209.33±8.91), t= 8.02, 3.60 and 6.50, P<0.01. Conclusions The scene simulation teaching method is an effective form of emergency department training in CPR, help to improve students practice ability and the ability of emergency cardiopulmonary resuscitation.
9.An updated role of astragalosideⅣin heart failure

Heart failure has become a global public health problem that seriously threatens human health. Due to "multi-target" effect, traditional Chinese medicine has unique advantages in the treat-ment of heart failure. Astragaloside Ⅳ is one of the main active ingredients of astragalus membrana-ceus.It has the functions of protecting the heart and neovascularization,anti-inflammation,anti-oxida-tion, regulating energy metabolism, neuroprotection and anti-cancer effects. This article reviews the recent progresses of astragaloside Ⅳ in the treatment of heart failure.Data show that astragaloside Ⅳcan inhibit heart fibrosis,attenuate excessively activation of renin-angiotensin system,increase energy metabolism, promote positive inotropic action, inhibit myocardial cell apoptosis, etc, which are all involved in the protective role of astragaloside Ⅳ in rodents or cellular models of heart failure.Astragalo-side Ⅳ may be a potential active ingredient in traditional Chinese medicine for the treatment of heart failure.
10.Expression of GABAR1 and NMDAR2B in aged rat cerebral temporal lopes after isoflurane inhalation

Objective To investigate the effects of different concentration and inhalation duration time of isoflurane on cognitive performance and the expression of GABAR1 and NMDAR2B in aged SD rat cerebral temporal lopes.Methods Aged male SD rats (9 months) were randomly divided into control group (n=10) and test group (n=80).The control group received air at room tempreture.Test groups were divided into four groups: group S1 (1.5%-2 h),group S2 (2.5%-2 h),group S3 (1.5%-4 h),group S4 (2.5%-4 h)according to isoflurane concentration and inhalation duration time.Every group was equally divided into two groups and Morris water maze test was performed day 1 and day 7 after isoflurane inhalation.Then the right temporal lobe was gathered and the mRNA transcription and protein expression of GABAR1 and NMDAR2B were detected by RT-PCR and Immunofluorescence technique.Results One day after isoflurane inhalation, accompanied with increased isoflurane concentration and inhalation duration, the spatial memory ability of every test group decreased continually, and the mRNA transcription and protein expression of GABAR1 increased and the mRNA transcription and protein expression of NMDAR2B decreased compared with control group (P<0.01).Seven days after isoflurane inhalation, the spatial memory ability of group S4 decreased, the mRNA transcription and protein expression of both GABAR1 increased, the mRNA transcription and protein expression of NMDAR2B decreased compared with control group and the other test groups (P<0.01).There was no significant difference between the control group and groups S1, S2, S3.Conclusion Continuous inhalation of isoflurane has great effects on spatial memory ability.And impaired spatial memory by isoflurane inhalation of high concentration with long duration is present in a long time.Thoses are related with the mRNA transcription and protein expressions of GABAR1 and NMDAR2B in cerebral temporal lope.
