1.Familial disorders of steroid 17?-hydroxylase deficiency

Objective To study and to improve the understanding of the etiology, diagnosis and treatment of familial congenital adrenal hyperplasia(CAH) due to 17? hydroxylase deficiency. Methods 2 rare cases of 17? hydroxylase deficiency in a family were studied with review of the literature. Results 2 cases were definitely diagnosed,one being male pseudohermaphroditism and the other female hypogonadism.Treatment consisted of replacement doses of glucocorticoid hormones and supplemented by estrogen therapy.The serum potassium became normal and the blood pressure was kept below 150/90 mm Hg. Conclusions 17? hydroxylase dificiency is a kind of hypertensive forms of CAH and is inherited as an autosomal recessive trait,its prevalence being rare.It is associated with hypogonadism or pseudohermaphroditism, hypertension or hypokalemia and with absent/decreased 17 OHCS and 17 KS.Hypoaldosteronism and positive dexamathasone inhibition test are the outstanding features.
2.64-detector Helical CT in Diagnosis of Coronary Heart Disease

Objective To investigate the accuracy of 64-detector helical CT in assessing stenoses of coronary arteries. Methods 64-detector helical CT angiography was performed in 128 patients with suspected coronary artery disease,and the results were compared with that of invasive coronary angiography. Results Among 128 patients, 1 724 of 1 920 coronary artery segments were assessable (90%). Overall sensitivity for classifying stenoses was 92%, specificity was 94%, positive predictive value was 82%, and negative predictive value was 96%.Conclusion 64-detector helical CT provides a high diagnostic accuracy in assessing coronary artery stenoses.
3.Recombinant human augmenter of liver regeneration may promote stromelysin-1 gene expression in experimental liver fibrosis

Objective To investigate the influence of recombinant human augmenter of liver rege neration (hALR) on stromelysin 1 gene expression in experimental liver fibrosis. Methods Two kinds of rat model of experimental liver fibrosis induced by CCl 4 and albumin were established and different dosages of recombinant human augmenter of liver regeneration were given during the process of model making. Total RNA of liver tissues was extracted and stromelysin 1 gene expression levels were measured by reverse transcription polymerase chain reaction (RT PCR). Results In both rat models of experimental liver fibrosis, stromelysin 1 gene expression levels in hALR treating group were significantly higher than those of model groups in different periods of model forming. Stromelysin 1 gene expression levels in high dose hALR treated group were significantly higher than those of low dose hALR treated group.Conclusion Recombinant human augmenter of liver regeneration may have effects of promoting gene expression of stromelysin 1 in experimental liver fibrosis.
4.Expression and significance of monocyte chemotactic protein 1 in early radiation-induced lung injury tissue of mice

Objective To study the expression and significance of monocyte chemotactic protein 1 (MCP-1) in the process of early radiation-induced lung injury in mice.Methods 40 mice were randomly divided into control group (10 cases) and post-irradiation group (30 cases).A large dose (15 Gy) irradiation on chest was performed in post-irradiation group.Every 10 mice in this group were killed on the 1st day (RT1d),the 10th day (RT10d),the 30th day (RT30d),respectively,and the specimens were collected.The expression of MCP-1 mRNA was detected by quantitative real-time PCR (RT-PCR),and MCP-1 and IL-6 proteins in BALF were detected by ELISA.The pathological changes of lung tissue were observed with HE staining.Results Under the microscope,lung tissue began to show inflammation on the 10th day and was more seriousness on the 30th day in post-irradiation group.Immunohistochemistry staining showed that the expression of MCP-1 in lung tissue irradiated was increased.RT-PCR results showed that the relative expressions of MCP-1 mRNA in control group,RT1d group,RT10d group and RT30d group were 7.63±0.77,6.81±0.75,5.75±0.73 and 4.61±0.75,respectively.The differences between RT1d group and control group,RT10d and RT1d group,and RT30d and RT10d were significantly different (all P < 0.05).ELISA results showed that the levels of MCP-1 and IL-6 proteins in post-irradiation group were significantly higher than those in the control group (P < 0.05).Conclusion The expression of MCP-1 in mice after irradiation is abnormal,and MCP-1 may participate in early radiation-induced lung injury.
5.Clinical application of MR susceptibility weighted imaging in intracranial hemorrhage

Objective To assess the value of susceptibility weighted imaging(SWI) in the diagnosis of intracranial hemorrhage. Methods Forty patients with intracranial hemorrhage underwent MRI scanning (GE Signa HDe 1.5 T), which included T2WI, T1WI, T2 * WI and SWI. Of them, DWI was conducted in 37 cases and enhanced MRI was conducted in 10 cases additionally. After post processing on the workstation, both magnitude and phase images of SWI were acquired for further analysis. The images of all sequences were scored from 1 to 3, according to their ability of depicting the lesions. Statistical analysis was conducted to compare the scores among these sequences. Results On SWI, the scores in detecting the lesions, their margin and adjacent veins were 2. 8, 2. 8, and 2. 8 respectively. The scores of those were 1.8, 1.7,and 0.0 on T1WI, 2.3, 2.0 and 0.0 on T2WI, 2.0, 2.1 and 0.2 on T2* WI, respectively. There was statistical difference between the scores on SWI and those on T1WI, T2 WI and T2 * WI ( P < 0. 01 ). The numbers of micro hemorrhagic lesions that could be observed on SWI, T1WI, T2WI, DWI and T2 *WI were 402, 55, 61, 84 and 188 respectively. There was statistical difference in showing micro hemorrhagic lesions between SWI and T1WI, T2WI, DWI, T2 * WI (P < 0. 01 ). Conclusion SWI is sensitive to visualize the hemorrhagic region, and has predominant advantage over conventional MR sequences including T2 * WI in detecting intracranial hemorrhage, especially cerebral microbleeding. According to the features of the paramagnetic and diamagnetic lesions, radiologists can differentiate hemorrhage and calcification with phase images.
6.Empirical Study on Qi-yin Deficiency Syndrome of Type 2 Diabetes Based on Structural Equation Model

Objective To verify the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information on type 2 diabetes mellitus.Methods Data of 185 cases of qi-yin deficiency syndrome of type 2 diabetes and 351 cases of other syndromes at TCM Hospital of Hongqiao District in Tianjin and Metabolic Disease Hospital of Tianjin Medical University were randomly collected. Etiology and diagnostic information of qi-yin deficiency syndrome were screened by the likelihood radio of forward step method by using Logistic regression analysis. On this basis, the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information was verified by using maximum likelihood estimation method of the structural equation model.Results The goodness of fit index of the model (GFI) was 0.909; root mean square residual (RMR) of that was 0.071; comparative fit index (CFI) of that was 0.942, which suggested that the fitting effect of the model was good. The reasonable interpretation of the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information was given. Conclusion From the aspect of single syndrome factors, the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information on type 2 diabetes mellitus is verified.
7.Strengthening Management of Hospital's Network by Means of Active Directory

Objective To solve the problems in the network management of the hospital's information construction.Methods The client workstations and the users of the hospital network were designed and organized by means of active directory.They are deployed and managed by means of setting the type of users and issuing the group policy.Results The application of active directory could manage the hospital's network uniformly,agilely and safely.Conclusion The management level is enhanced by means of active directory.It will have broad prospects in the hospital's network management.
8.Research of T-lymphocytes rDNA Transcriptional Activity Detection System

Objective To perform the image edge extraction by image segmentation in computer view. Methods The T- lymphocytes images were divided, distinguished and extracted automatically by gray histogram and gray changing distribution map overlap method. Results In the development of the peripheral blood T-lymphocytes rDNA transcriptional activity detection system, the processing method of stained T-lymphocytes images was researched to solve the problem of edge extraction. Conclusion The system is reliable, simple and convenient. The veracity and accuracy can meet requirements of clinical diagnosis.
9.The combination of dexmedetomidine and propofol for sedation in pediatric patients undergoing regional anesthesia

Objective To evaluate the effectiveness and safety of the combination of dexmedetomidine and propofol for sedation in pediatrics undergoing regional anesthesia.Methods Forty-eight children aged 2 -6 years old,scheduled for elective surgery under regional anesthesia,were randomly divided into the two groups.The dexme-detomidine plus propofol group(group DP,n=24)received an infusion over 10 minutes of dexmedetomidine 0.7μg/kg and propofol 1.0 to 2.5 mg/kg bolus for induction,then an infusion of dexmedetomidine 0.5μg·kg-1·h-1 and propofol 100-300μg·kg-1·min-1 for maintenance.The propofol group(group P,n=24)received a propofol 1.0 to 2.5mg/kg bolus for induction,then an infusion of propofol 150 -300μg·kg-1 ·min-1 for maintenance.If body movement was detected during surgery,a bolus of propofol 1 .0mg/kg was administered in both groups,and if there was another body movement within three minutes of the propofol bolus,a bolus of ketamine 1 .0mg/kg was adminis-tered.Hemodynamic data,respiratory rate,and oxygen saturation were recorded by researchers blinded to the study drugs.Recovery time and the primary outcome were evaluated by a modified Aldrete score.The occurrence of adverse events and the amount of supplemental ketamine used were also recorded.Results The consumption of propofol in the group DP was (149.00 ±24.93)μg·kg-1·min-1,and in the group P was (217.00 ±17.90)μg·kg-1· min-1.The propofol consumption was significant lower that in group DP(t=117.94,P=0.00).The needs for sup-plemental ketamine bolus in the group DP was 0.50(1.00)/case,and it was 2.00(1.00)/case in group P.The need for supplemental ketamine was less in group DP(Z=-4.48,P=0.00).Hypopnea occurred in 5 cases in group P, and 0 in group DP.The incidence of hypopnea was more in group P(χ2 =5.58,P=0.03).In group DP,the mean arterial pressure and heart rate at discharge were respectively (74.96 ±5.69)mmHg and (101.17 ±9.65)bpm,and their baseline values were (81.13 ±7.23)mmHg and (112.00 ±13.65)bpm.Both mean arterial pressure and heart rate in the group DP remained decreased at discharge(t=10.78,P=0.00;t=10.08,P=0.00;respectively).The discharge time of the group DP was (40.91 ±10.90)min and it was(39.67 ±14.95)min in group P.There was no significant difference in the discharge time(t=0.11,P=0.74).Conclusion The combination of dexmedetomidine and propofol can provide appropriate depth of sedation in pediatric patients undergoing regional anesthesia.
10.Susceptibility Evaluation of EGFR Targeted Small Molecule NXGH/NXGF for Lung Cancer Cell

Objective:To investigate the sensitivity of novel small molecule tyrosine kinase inhibitors NXGH and NXGF to 4 lung cancer cell lines with different EGFR expression and mutant status.Methods:Novel small molecule tyrosine kinase inhibitors NXGH and NXGF based on NXG structure were designed.Four lung cancer cell lines with different EGFR expression and mutation status:PC9(Exon 19 deletion mutation),H1975(L858R mutation combined T790M mutation),H358(wide EGFR expression) and H520(EGFR negative expression) were chosen.Inhibition ratio of NXGH and NXGF at different concentration (1.25,2.5,5.0,10,20,30,40,60,80 μmoL·L-1)against 4 lung cancer cell lines in 48 h were investigated by MTT method.IC50 and cell viability were calculated,and sensitivity between different cell lines were compared.Results:IC50 of PC9,H358,520 and H1975 cells incubated with NXGH were 0.675 μ moL·L-1,12.097 μmoL·L-1,11.368 μmoL·L-1 and 0.981 μmoL·L-1,respectively.IC50 of PC9 and H1975 were less than H358 and H520 when the concentration was 1.25,2.5 and 5 μmoL ·L-1 (P<0.05).IC50 of PC9,H358,H520 and H1975 cells incubated with NXGF were0.685 μmoL·L-1,4.265 μmoL·L-1,3.097 μmoL·L-1 and 0.331 μmoL·L-1,respectively.IC50 of PC9 and H1975 were less than H358 and H520 when the concentration was 1.25 and 5 μmoL·L-1 (P<0.05).Conclusion:The novel small molecule tyrosine kinase inhibitors NXGH and NXGF,which were designed and constructed in our laboratory successfully,had high affinity for lung cancer cells with different EGFR expression and mutation status.And they were more sensitive to EGFR mutant cell at low concentration as expected.
