1.Perioperative nursing care of congenital heart disease underwent minimally invasive occlusion surgery via subaxillay incision

Objective To explore the perioperative nursing method and effect of doing minimal invasive occlusion to children with congenital heart disease via subaxillary incision.Methods The data of 86 children patients with congenital heart disease underwent minimally invasive occlusion via right subaxillary incision were retrospectively analyzed and summarized nursing points including preoperative and postoperative mental nursing,postoperative postural care and sedative care,observing whether the occluder moved or detached and mechanical hemolysis,monitoring ECG and blood pressure,and preventing hemorrhage and thrombogenesis.Results One case of infracristal ventricular septal defect with frequent premature ventricular beat after occlusion was converted to thoracotomy surgery;one case of mixed atrial septal defect with heart rate increasing and dyspnea after 12 hours of occlusion was found occluder detached and immediately converted to remove the occluder and fix the atrial septal defect;The other children were successfully recovered and been discharged.Conclusions Full understanding and strengthened perioperative nursing care of complications of children with congenital heart disease after surgical minimally-invasive surgical closure is the key point to avoid the occurrence of fatal complications and ensure the children recover safely and smoothly.
2.Application of high quality nursing service in perioperative treatment of patients with coronary heart disease

Objective The study was to find the application effect of high quality nursing service in perioperative treatment of patients with coronary heart disease.Methods Eighty patients were randomly divided into the comfort nursing group and the routine nursing group with 40 patients in each group.The comfort nursing group was given high quality nursing service,and the routine nursing group received the conventional care.The effect in two groups was compared after rehabilitation.The statistical software of SPSS 17.0 was used to analyze the data.Results The degree of anxiety was lower,the number of patients with complication was less,the degree of satisfaction was higher and the time in hospital was less in the comfort nursing group.Conclusions The comfortable nursing care should be used in peri-operative treatment of coronary heart disease,which can make patients keep the best mental state to accept and cooperate with the surgical treatment.It also can improve the care quality and patients' satisfaction degree and shorten the hospital stay.
3.Perioperative nursing care of minimally invasive occlusion technique via the right subaxillary route for perimembranous ventricular septal defect

Objective To investigate the perioperative nursing Methods of minimally invasive occlusion technique via the right subaxillary route for perimembranous ventricular septal defect in children.Methods A total of 119 patients, who were successfully undergoing minimally invasive occlusion via the right subaxillary route for perimembranous ventricular septal defect from June 2013 to June 2016, were divided into inferior position group (82 cases, adjacent to inflow tract) and superior position group (37 cases, adjacent to outflow tract) according to location of perimembranous ventricular septal defect. Postoperatively complications in patients were compared between two groups. Patients with complications were given individually nursing interventions. Results There were 2 patients from superior position group with occluders displacement post operation. The difference was statistically significant between two groups (P<0.05). A total of 5 patients had extensive subcutaneous emphysema in right chest, 3 patients of which were from inferior position group and other 2 patients were from superior position group. There were 2 patients who had moderate pleural effusion with one for each group respectively. A number of 2 patients was found auscultation of cardiac murmur and one per each group. There was no difference between two groups in the rate of above complications. Occluder was extracted and surgical repair was perfomed in the children with occluders displaccment. Patients with complications were treated in time and then were cured and discharged. Follow-up for all patients ranged from 6.1 months to 24.9 months (13.4±5.0). There was no malignant arrhythmia, device dislocation, hemorrhage, thrombogenesis or other complications.Conclusions The cognition of characteristic of different position in perioperative perimembranous ventricular septal defect occlusion and targeted nursing measures are the key points to avoid lethal complications after surgery for patients and assure the safety of children.

5.Qualitative study on emotional experiences of the primary caregivers of patients with lower extremity deep vein thrombosis in Department of Neurosurgery

Objective To study the psychological experience of the primary caregivers of patients with lower extremity deep vein thrombosis in Department of Neurosurgery, and to provide the basis for the establishment of nursing intervention measures under the cultural background of China. Methods By using qualitative phenomenological research method, 13 caregivers of patients with lower extremity deep vein thrombosis in Department of Neurosurgery were semi structured interviewed from May to October in 2017, and data were analyzed by using Colaizzi method. Results In the end, 4 themes were generated. They were psychological response at the beginning of diagnosis, the pressure in the process of care, changes in social adaptation ability and coexistence of difficulties and hopes. Conclusions The mood of caregivers is directly related to the physical and mental health of patients with lower extremity deep vein thrombosis. When interviewing patients, medical staff should give more knowledge of disease and psychological counseling to their caregivers to reduce the uncertainty of disease, release the burden of primary caregivers and ultimately promote them recovery.
6.Diagnostic value of MRI retention enema cannula enhanced scanning in high complex anal fistula

Objective:To investigate the clinical diagnostic value of MRI retention enema cannula enhanced scanning in the high complex anal fistula.Methods:The clinical data of 60 anal fistula patients underwent surgery treatment from May 2020 to May 2022 in Affiliated Hospital of Shanxi University of Traditional Chinese Medicine were retrospectively analyzed. All patients underwent MRI plain scanning and enhanced scanning before operation. Compared with the surgical results, the difference between MRI plain scanning and enhanced scanning in the diagnosis of high complex anal fistula was compared.Results:All of the 60 patients successfully completed surgical treatment, and 58 cases internal orifices, 55 cases complex anal fistulas and 53 cases high anal fistulas were found intraoperatively. MRI plain scanning results showed 32 cases internal orifices, 46 cases complex anal fistulas and 42 cases high anal fistulas were found. MRI enhanced scanning results showed 54 cases internal orifices, 53 cases complex anal fistulas and 50 cases high anal fistulas were found. Based on surgical results, the coincidence rates of internal orifice, complex anal fistula and high anal fistula in MRI enhanced scanning were significantly higher than those in MRI plain scanning: 93.10% (54/58) vs. 55.17% (32/58), 96.36% (53/55) vs. 83.64% (46/55) and 94.34% (50/53) vs. 79.25% (42/53), and there were statistical differences ( χ2 = 21.76, 4.95 and 5.27; P<0.01 or <0.05). Conclusions:The MRI retention enema cannula enhanced scanning has obvious advantages in the diagnosis of high complex anal fistula, which provides scientific reference value for the diagnosis and operation of high complex anal fistula in clinic.

Outpatient humanistic care refered to providing a full process of caring medical services to outpatients. In order to standardize the human caring services for outpatients in medical institutions, promote the comprehensive service level of outpatient services, and improve the patient′s medical experience, Chinese Association for Life Care issued the group standard of Management Norms for Human caring of Outpatients in April 2023. This standard clarified the relevant terms and definitions of human caring for outpatients, specified the basic requirements for human caring, the humanistic quality and care responsibilities of outpatient staff, the outpatient care environment and facilities, the outpatient care process and measures, and quality management. It designed standardized and personalized full process care service norms, providing references for medical institutions at all levels to promote the development of human caring for outpatients.

OBJECTIVETo break immune tolerance to prion (PrP) proteins using DNA vaccines.
METHODSFour different human prion DNA vaccine candidates were constructed based on the pcDNA3.1 vector: PrP-WT expressing wild-type PrP, Ubiq-PrP expressing PrP fused to ubiquitin, PrP-LII expressing PrP fused to the lysosomal integral membrane protein type II lysosome-targeting signal, and PrP-ER expressing PrP locating the ER. Using a prime-boost strategy, three-doses of DNA vaccine were injected intramuscularly into Balb/c mice, followed by two doses of PrP protein. Two weeks after the last immunization, sera and spleens were collected and PrP-specific humoral and cellular immune responses evaluated by ELISA and ELISPOT tests.
RESULTSHigher levels of serum PrP antibodies were detected in mice vaccinated using the strategy of DNA priming followed by protein boosting. Of these, WT-PrP, Ubiq-PrP, and PrP-LII induced significantly higher humoral responses. ELISPOT tests showed markedly increased numbers of IFN-γ-secreting T cells in mice vaccinated using the strategy of DNA priming followed by protein boosting after stimulation with recombinant PrP23-90 and PrP23-231. PrP-ER induced the strongest T-cell response.
CONCLUSIONPrion vaccines can break tolerance to PrP proteins and induce PrP-specific humoral and cellular immune responses.

Objective To analyze the PrP protein expression and the clinical degrees and pathological grade of head and neck squamous cell carcinomas.Methods Immunohistochemistry technique was used to test the PrP protein expression in the 92 head and neck squamous cell carcinomas cases.SPSS software was used to analyze the relationship between the PrP protein and the clinical degrees and pathological grade of head and neck squamous cell carcinomas.Results In the 92 head and neck squamous cell carcinomas cases,the positive rate of PrP protein was different with the sites of the carcinomas,the clinical degree and the pathological grade.Conclusion High expression of PrP protein related with the occurrence and development of head and neck squamous cell carcinomas.
