1.Current clinical research of intraperitoneal perfusion chemotherapy for gastric cancer

Gastric cancer is the first in morbidity and mortality among cancers in China. The presence of peritoneal dissemination is the most common cause of recurrence after radical surgery, and has become a pivotal to the curative resection. Whether intraperitoneal chemotherapy can prevent peritoneal dissemination,lymph node metastasis, eradicate invisible seeding tumor cells, reduce peritoneal metastasis, improve survival and quality of life is always under investigation. This review focus on the theory, clinical applications and problems of intraperitoneal chemotherapy for gastric cancer.
2.Progress of clinical research of neoadjuvant chemotherapy for breast cancer

Breast cancer is a systemic disease, only through multidisciplinary treatment including surgery, radiotherapy, chemotherapy, endocrine, can we enhance its effectiveness. In recent years, neoadjuvant chemotherapy for breast cancer has aroused great interest in tumor academia. We hope that this therapy can not only improve breast conservation rate, but also extend the overall survival through a series of clinical studies. This review presents the research progress of neoadjuvant chemotherapy for breast cancer.
3.Isolation and Identification of Canine Parvovirus Serotype 2a and Its VP2 Protein Expression in Transgenic Tobacco

A strain of canine parvovirus (CPV) was isolated from feces of an ill puppy in an animal hospital in Wuhan, China. It was designated as CPV/WH02/06. This isolate was identified as serotype CPV-2a by the hemagglutination test, CPV Ag detection strip, electron microscopy, and PCR. The vp2 gene was cloned and sequenced and assigned GenBank accession number EU377537. A 1242 bp segment of the 5' region of the vp2 gene was cloned and inserted into the binary vector pBI121 and used for Agrobacterium-mediated tobacco transformation. Transgenic tobacco plants were selected on MS medium supplemented with 100 μg/mL kanamycin and 100 μg/mL timentin. Integration of the vp2 gene into the tobacco genome was confirmed by PCR using T1 progeny plants, and the expression of the VP2 protein was confirmed by Western blotting.
4.The safety and efficacy of temporary cardiac pacing by bedside to interfere the torsades de points in patients with acquired long QT syndrome

Objective To evaluate the safety and efficacy of the temporary bedside cardiac pacing in controlling torsades de points (TdP) in patients with acquired long QT syndrome (LQTS). Methods Twelve patients with acquired LQTS were enrolled from April 2003 to August 2007 consecutively and their clinical data were analyzed. Bedside cardiac pacing was adopted when other methods couldn't terminate the repeated TdP. Results Twelve patients successfully experienced the temporary bedside cardiac pacing via femoral venous. The average time spent in bedside cardiac pacing was about (10.5±2.4) min. After cardiac pacing the interval of QT and QTc were shortened [ (0.42±0.03 ) svs (0.52±0.06) s, P < 0.05; (0.43± 0.04 ) s vs (0.53±0.05 ) s, P <0.05 ]. The TdP occurred (4.6±1.2 ) times per day before cardiac pacing and it didn't reoccur any more after bedside cardiac pacing. The average time for cardiac pacing was(3.8±1.4) d. When the patients were discharged, the interval of QT and QTe were (0.41±0.02) s and (0.42±0.05) s respectively, there were significant differences compared with that before cardiac pacing(P< 0.05). During 1 year follow-up, the patients didn't experience TdP any more, and the interval of QT and QTe were (0.41± 0.06) s and (0.42±0.05) s respectively. Conclusion The immediate bedside cardiac pacing is a safe and effective way to control the repeated TdP.
5.Optimizing expression of recombinant human monocyte chemoattractant protein-1 in E.coli

Objective: To optimize the expression of recombinant human monocyte chemoattractant protein (rhuMCP-1) in E.coli DE3. Methods:With NBS-MICROS 15 L T.DR fermentor, pGEX-IN/huMCP-1 was constructed by our laboratory. Four parameters including pH,temperature,agitation rate and concentration of IPTG were studied by orthogonal experimental design. Results: It was found that the expression level was greatly affected by the amount of dissolved oxygen. This indicated that the agitation rate and ventilation amount were the most important parameters during fermentation. Examined by SDS-PAGE and gel scanning, the expression level of total protein was over 40% when agitation rate was 300 r/min and ventilation amount was 10 L/min. Conclusion: A method for high-level expression of huMCP-1 on pilot-scale is established, and it will be useful for large-scale industrial production of target protein.
6.Research of Adp and IGF-1 on elderly patients with chronic heart failure by catechin

Objective To analysis the role of adiponectin, insulin like growth factor -1 in elderly chronic heart failure.Methods 92 elderly patients with chronic heart failure were selected and divided into control group and experiment group.46 cases of the control group were treated with conventional treatment including strong heart, diuresis, vasodilator,lowering blood pressure, the experimental group was on the basis of routine treatment with catechin, 50 mg/kg orally, 1 times a day.2 weeks as a course, 1 course of treatment.Adiponectin, insulin like growth factor -1,BNP,LVEDD, LVESD and LVEF were compared before and after treatment.Results After treatment, the total effective rate of observation group was 93.48%, the control group total effective rate was 76.09%.The total effective rate of observation group was significantly higher than that of the control group (χ2 =5.39, P <0.05 ).After treatment, BNP, Adp in two groups decreased, IGF-1 increased, compared with control group, BNP, Adp level of the experiment group were lower, IGF-1 was higher(P<0.05),and LVEDD and LVESD of the experiment were lower, LVEF was higher, (P<0.05). Conclusion Catechins can decrease the serum levels of adiponectin, elevate insulin like growth factor-1, improve the degree of heart failure, is worthy of clinical application.
7.A novel pore-forming hemolysin protein in Leptospira species

Hemolysins of Leptospira interragans have been shown to be the virulence factor in the pathogenesis of leptospirosis and 10 potential hemolysin genes were charecterized by genomic annotation of L.interrogans serovar.Lai strain 56601. In the present study, the LA0202 gene supposed to encode one of the new potential hemolysin was cloned and the protein encoded was purified. The purified protein was shown to have highly hemolytic activity as demonstrated on the sheep blood agar plate. It was also confirmed that the LA0202 protein-mediated hemolysis on sheep erythrocytes was osmotically protected by PEG6000. Meanwhile, this protein could induce pore formation on sheep erythrocytes and cause damages on the membrane of human L-02 liver cells. In addition, it could induce apoptosis of human L-02 liver cells after treatment of cells with this protein for 24 hours. It is evident that LA0202 protein acting as a pore-formong hemolysin can induce cytotoxic damage on mammalian cells.
8.The immune responses induced by a recombinant DNA vaccine for leptospirosis:pVAX1/LipL21-LipL32

Two highly conserved and with abundant quantity of lipoproteins in outer membrane of pathogenic species,but not in saprophytic species of leptospira ,LipL32 and LipL21,were selected to construct the fusion gene DNA vaccine pVAX1/LipL21-LipL32,and its ability to induce immune responses in BALB/c mice inoculated with this recombinant DNA vaccine was investigated in the present study.Expression of the fusion-protein LiPL21-LiPL32 was demonstrated in HEK293 cells following transfection with the fusion gene DNA vaccine and the immune responses induced after intramuscular inoculation with this DNA vaccine in BALB/c mice was then evaluated by microscopic agglutination test (MAT),meanwhile the ELISA assay was used to detect the cytokines induced.It was demonstrated that significant level of specific antibodies agglutinating antigens of Leptospira interrogans could be detected by MAT after DNA vaccine inoculation.The production of cytokines IL-10 and TNF-β in mice inoculated with DNA vaccine pVAX1/LipL21-LipL32 was significantly increased in comparison with that of the group inoculated with pVAX1 alone.These results indicate that the recombinant DNA vaccine pVAX1/LipL21-LipL32 may be of potential value to design and develop new generation of vaccines against leptospirosis.
9.Clinical analysis of 14 cases with nasal respiratory enithelial adenomatoid hamartom.

OBJECTIVE:
To describe and analyze the clinical features of nasal respiratory epithelial adenomatoid hamartom and improve the levels of diagnosis and treatment.
METHOD:
Fourteen cases of nasal respiratory epithelial adenomatoid hamartom confirmed by pathology were collected and analyzed.
RESULT:
In 14 cases, primary complaint of Hyposmia(or Anosmia)and nasal obstruction were occured in 13 and 11 cases respectively. Four cases had past history of endoscopic sinus surgery because of the diagnosis of nasal polyps. Polypoid neoplasms could be seen in the bilateral olfactory clefts. Sinus CT showed soft tissue shadows in bilateral nasal cavity and mucous membrane thickening in different sinus. Endoscopic sinus surgery were utilized to eliminate focus in all cases. All cases weren t recurred after 2-20 months' following-up visitings.
CONCLUSION
Nasal respiratory epithelial adenomatoid hamartom is so rare that clinical and pathological doctors have limited knowledge of it. It has its own characteristics from the clinical symptoms, signs to sinus CT although they are nonspecific. So we should improve the understanding about it to avoid misdiagnosis or missed diagnosis.

10.CHANGES OF NAILFOLD MICROCIRCULATION IN PATIENTS OF TYPE II DIABETES MELLITUS WITH DIABETIC RETINOPATHY

Objective.To study the changes of microcirculation in patients with diabetic retinopathy(DR).Methods.Examination were performed in 153 cases of type Ⅱ diabetes mellitus, among them,72 cases were male,81 cases were female,mean age 57.0±10.0 years, mean disease course 8.2±7.5 years.All cases were examined fundi by ophthalmologist,urinary albumin excretion rate (UAE)in 24 hours was measured by radioimmunoassay.Moreover,we examined the blood glucose,blood pressure,blood viscosity and observed the changes of naifold microcirculation.Results.It was found that there were more evident disturbance of microcirculation,markedly slowed velocity of blood flow(P<0.05),significantly increased aggregation of blood cells(P<0.05)and exudation around the loop(P<0.05)in the group with DR,compared with the group without DR.Conclusion.It was more evident disturbance of nailfold microcirculation in patients with diabetic retinopathy.
