1.Clinical characteristics and high risk factors of diabetes mellitus following renal transplantation

Objective To investigate the clinical characteristics and risk factors for postrenal transplant diabetes mellitus (PTDM). Methods 512 consecutive kidney recipients were included in this study from 1994.1 to 1997. 5.Biochemical data, body weight changes, drug history, genetic markers such as HLA phenotypes, anti-virus antibodies were studied in these patients.Results The mean age and the dose of hormone used 6 months after operation in the diabetes mellitus group were older and higher than in the nondiabetes groups respectively. PTDM often occurred half a year following renal transplantation. And no patients with PTDM were obese. Most PTDM patients had a higher blood glucose level, but few of them had ketoacidosis. The occurrence of HLA-B15 and the positive rate of anti-gland virus antibody were significantly higher in PTDM group. Conclusion Both genetic factors such as HLA type antigens and environmental factors such as the amounts of steroid and cyclosporine A used, infections of some kinds of viruses, and also stress during transplantation may play an important role in the causes of PTDM.
2.Dithothreitol (DTT) treating lymphocytes highly sensitized antibodies in renal pretransplantation patients

72 sera of highly sensitized antibody patients were detected by 35 samples of undonor lymphocytes before and after the dithothreitol (DTT) treating.The results showed that 72 sera reacted completely to B lymphocytes in both 4℃ and 37℃,but incompletely to T lymphocytes.After removal of the transplant kidney due to rejection,the sera antibody levels of T or B lymphocytes were higher than those of the patients'sera after blood transfusion or pregnancy.The serum T or B antibody levels of pregnancy were the lowest.In DTT-treated sera of highly sensitized patients,the antibodies were mixed types of IgG and IgM.
3.Peripheral blood skin-homing CD8+ T cells in patients with atopic dermatitis

Objective To quantify the percentage of CD8+ T cells and their expressions of cytotoxic molecules and homing-related chemokine receptors in peripheral blood from patients with atopic dermatitis (AD).Methods Peripheral blood was obtained from 15 patients with AD and 14 healthy controls.Flow cytometric analysis was performed to determine the percentages of CD8+ T cells and CD8+CLA+ T cells in the peripheral blood samples,as well as the expression levels of cytotoxic molecules and homing-related chemokine receptors on these cells.Differences in these parameters were analyzed using t test,and relationship between these parameters was evaluated using Pearson correlation coefficient.Results No significant difference was observed between the patients with AD and healthy controls in the percentage of CD8+ T cells,the expressions of perforin,granzyme B,CCR10,CCR6 or FasL on CD8+ T cells,or the expressions of CCR4,CCR10,CXCR6 or FasL on CLA+CD8+ T cells (all P > 0.05).A significant increase was noted in the percentage of CLA+CD8+ T cells (3.80% ± 1.46% vs.2.18% ± 0.85%,t =3.636,P < 0.01) and expression rates of CCR4 on CD8+ T cells (13.86% ± 4.42% vs.9.50% ± 2.14%,t =3.738,P < 0.01) as well as perforin and granzyme B on CLA+CD8+ T cells (74.27% ± 15.94% vs.57.20% ± 14.64%,t =2.998,P < 0.01; 70.90% ± 13.85% vs.56.41% ± 11.00%,t =3.104,P < 0.01) in the patients with AD compared with the healthy controls.Conclusions The proportion of CLA+CD8+ T cells is increased with enhanced expressions of cytotoxic molecules such as perforin and granzyme B in peripheral blood of patients with AD,which may contribute to the pathogenesis of AD.
4.The potential effects of Lactobacillus acidophilus on the prevention and treatment of neonatal mice infected with human rotavirus

Objective To investigate the potential effect of Lactobacillus acidophilus (L. acidophilus) on prevention and treatment of neonatal mice infected with human rotavirus (HRV). Methods Sixty 4-day-old kunming mice were randomly divided into control group. HRV infected group, L. acidophilus pretreated group (treated before HRV infection ) and L. acidophilus treated group(treated after HRV infection). The manifestation and pathological changes in small intestine of neonatal mice were observed. The HRV antigen in the feces and intestines were measured by ELISA and fluorescent-focus assay, respectively. Results The severity and duration of diarrhea as well as mortality in L. acidophilus pretreated group and treated group were lower than those in HRV infected group. The duration of HRV-antigens shedding following infection was considerably prolonged in HRV infected group compared to that in L. acidophilus pretreated group and treated group. Furthermore, decreased expression of HRV antigen and little pathological changes in intestinal mucosa were found in L. acidophilus pretreated group and treated group when compared with HRV infected group. Conclusion L. acidophilus may be used as an alternative approach for the prevention and treatment of neonatal mice infected with HRV.
5.Effects of xuefu zhuyu decoction on functions of vascular endothelium in patients with unstable angina pectoris

OBJECTIVE: To evaluate the effects of Xuefu Zhuyu Decoction (XFZYD) on functions of vascular endothelium in patients with unstable angina pectoris (UAP). METHODS: Fifty patients with UAP were randomly divided into two groups: XFZYD-treated group with 32 cases and control group with 18 cases. The patients in the control group were given regular therapy, while the patients in the XFZYD-treated group were given XFZYD additionally on the basis of regular therapy. All patients in both groups were treated for 8 weeks. Then the contents of serum endothelin (ET), nitric oxide (NO), soluble vascular cell adhesive molecule-1 (sVCAM-1) and soluble intercellular adhesion molecule-1 (sICAM-1) were examined by radioimmunoassay, enzymic method and enzyme-linked immunosorbent assay (ELISA) respectively. RESULTS: The total response rate in the XFZYD-treated group was 93.75%, which was significantly higher than 66.67% in the control group (P<0.05). The contents of serum ET, sVCAM-1 and sICAM-1 in the XFZYD-treated group were much lower than those in the control group (P<0.01), while the content of serum NO was obviously higher than that in the control group (P<0.01). CONCLUSION: XFZYD can improve the functions of vascular endothelium by lowering the levels of endothelium-derived contracting substances, enhancing the levels of endothelium-derived relaxing substances, and reducing the cell adhesions, and hence to raise the therapeutic effects on UAP.
6.Comparative Study on Allograft Functions of HLA Antibody in Patients Who Received Pairs of Allograft from the Same Donor

Objective To study the influence on allograft function of HLA antibody in patients who received pairs of allograft from the same donor.Methods In Beijing Friendship Hospital.HLA antibodies were tested from October,2008 to April 2009 in patients.Recently (October,2013-February,2014),renal functions(serum creatinine/urea nitrogen)were studied in 226 patients who received transplant from 113 donors.LATM10x5,One Lambdas used for Panel reactive antibody screen-ing.Results 41 pairs of renal for male,21 pairs of renal for female and 51 pairs of renal for both male and female.PRA posi-tive in 26 patients (only 4 pairs of renal for patients were positive),11 recipients (HLA II antibody positive in only 1 pair of renal for patients)and 36 recipients (only 5 patients produced antibody)in 226 patients,HLA antibody positive in 73 pa-tients,in which renal function lost or decreased in 64 patients.HLA antibody negative in 153 recipients,in which renal func-tion lost or decreased in 4 patients.There was significant difference between the two group (χ2=160.70,P<0.001).Con-clusion HLA antibody is a important factor influence renal function and long term survival.
7.Influence of HLA typing on renal allograft function in patients receiving kidney from same donor

Objective To investigate the influences of HLA mismatching on renal function in the kidney transplant patients re-ceiving pairs of allograft from the same donor.Methods 171 pairs of renal transplant patients receiving the kidneys from the same donors were investigated.They were admitted in our hospital before 2008.Their human leukocyte antigens (HLA)were typed with the commercial polymerase chain reaction (PCR)-sequence-specific primers (SSP)HLA typing kit (One Lambda,Inc.,USA;and GTI Diagnostics,USA).The serum creatinine (SCr)and blood urea nitrogen(BUN)were measured in the clinical laboratory of our hospital.Results Among 171 pairs of renal transplant patients,there were 162 recipients with HLA mismatch≤4,in which the re-nal function was remained stable in 107 recipients and lost or decreased in 55 patients.There were 180 recipients with HLA mis-match >4,in which the renal function was stayed normal in 84 recipients and lost or decreased in 96 patients.The difference in in-fluencing the renal function between the HLA mismatch≤4 and HLA mismatch>4 had statistical significance (χ2 =12.22,P <0.05).Conclusion Excellent HLA typing match has important significance for renal long term survival.
8.Fever,poor response, convulsions, and hepatomegaly

Objective To investigate the differential diagnosis of Reye syndrome and the characteristics of primary carnitine deficiency,and to provide diagnostic strategy for similar cases.Methods There was a case presented with fever,poor response,convulsions and hepatomegaly hospitalized in Wuhan Children's Hospital,and the clinical manifestations were described,the physical examination was comprehensively conducted,the auxiliary examination results were recorded,some pediatric specialists from ICU,neurology department,genetic metabolic department,digestive system department were invited to discuss the case.The treatment was adjusted according to the suggested opinions;the treatment effects and the final diagnosis were tracked.Results The primary diagnosis of the case was central nervous system infection or toxic encephalopathy at the time of admission,but Reye syndrome could not be excluded.Although the cerebrospinal fluid test and brain MRI examination detected nothing abnormal,liver function suggested alanine aminotrans ferase ALT increase,blood sugar decrease,the liver volume increase,which was detected by liver ultrasound.Blood amino acids examination revealed the carnitine level decreased,and it was confirmed as primary carnitine deficiency in the end.L-carnitine was used to treat the disease,and its effect was good.Conclusions Great importance should be attached to children with onset age,physical check-up,and multidisciplinary cooperation.Use monism to explain the illness and the auxiliary inspection as far as possible,so that it can get early diagnosis and treatment,and the outcome is good.
9.Value of MRI diffusion weighted imaging in localization of prostate cancer with whole-mount step section pathology

ObjectiveTo evaluate the value of MRI diffusion weighted imaging in localization of prostate cancer with whole-mount step section pathology.MethodsWe treated 36 patients using laparoscopic radical prostatectomy from Oct. 2009 to Jun. 2010. Patients who did not have an MRI/DWI examination or a surgical history of prostate and previous hormonal therapy were excluded, leaving 19 patients in our study. We analyzed the MRI and DWI collected before radical prostetectomy surgey and the post-surgery step section pathology made by the whole mount technique. The prostatic gland was divided into six sections. Two doctors read the MRI/DWI without knowing the patients' information and scored, using a 5 point scale, for each section. The tissue was graded according to the following scale: 1-definitely no cancer, 2-probably no cancer, 3-possible cancer, 4-probable cancer and 5-definite cancer. When the average score ≥4 the region was assumed to be the prostate cancer region by MRI or MRI/DWI.ResultsIn 19 patients with 114 regions, there were 48 (42%) prostate cancer regions approved by whole-mount step section pathologically.The number of prostate cancer regions was 15 (39%), 21 (55%) and 12 (32%) in base, mid and apex parts respectively. The sensitivity and specificity of the MRI was 62.5% and 69.7%. When considering DWI, the specificity and sensitivity was elevated to 79.1% and 83.3%. As for the apex and mid parts, the sensitivities of MRI were a little bit low (46.7% and 66.7% ). But the sensitivities of localization of prostate cancer for the apex and mid of prostate were elevate to 73.3% and 85.7% respectively when DWI was also used.ConclusionsWith whole-mount step-section pathology, the combination of MRI and DWI can improve the sensitivity of MRI for localized diagnosis in prostate cancer, especially in apex and mid parts of the prostate.
10.The significance of CK-19 mRNA and CEA mRNA in the circulating vein blood of non-small cell lung cancer

objective To detect CK19 mRNA and CEA mRNA in blood of the peripheral vein and to investigate the effect of ligating pulmonary vein firstly or ligating pulmonary artery firstly during surgical operation on haematogenous dissemination of malignant cells.Methods Fifty six non-small cell lung cancer patients were collected and random assigned to two groups before operation (ligating pulmonary vein firstly or ligating pulmonary artery firstly).The patients were accepted radical operation and their operations were put in practice by doctors of one team.Vein blood was collected one day before operation and one week after operation.CK19 mRNA and CEA mRNA in blood were detected by nested reverse transcriptase-polymerase chain reaction. Result The positive rate of expression of CK19 mRNA and CEA mRNA after operation is lower than that before operation.The positive rate of expression of CK19 mRNA and CEA mRNA in ligating pulmonary vein firstly after operation is lower than that in the other group. Conclusions Surgical operation have effects on the dissemination of malignant cell and ligating palmonary vein firstly during operation can reduce the dissemination of malignant cell.
