1.Long-term follow-up study of percutaneous transluminal septal myocardial ablation for hypertrophic obstructive cardiomyopath

36 mm?Hg. No permanent cardiac pacemaker was implanted. Transit trifascicular was seen in 3 cases, of whom, 2 disappeared with 1 hour after the procedure and, 1 trifascicular disappeared with 3 days. Permanent branch blocks were demonstrated in 36% patients. Conclusion PTSMA was feasible for HOCM with controllable complications. Precise position was the key point to prevent the alcohol leakage which will induce large area myocardial infarction and cardiac shock. Further study was needed to appreciate the more long-term follow-up.

3.Study of lesion characteristics and stenting for diseased grafts in older patients

Objective To determine the lesion characteristics and effect of intervention on diseased grafts in older patients. Methods Thirty-eight patients with angina after coronary artery bypass grafts (CABG) were enrolled into Nanjing First Hospital. Interventions were performed after multiple-view angiogram and formal consent from relatives received. Angiographic follow-up was conducted within 6 months. Cardiac events were recorded during the 6-month clinical follow-up. Results Of the 38 patients (36 males and 2 females), the average age was 63.2 years. Angina occurred 11.5 months after CABG. Angipgrams were performed 4.8 years after CABG. Diabetes was complicated in 10 cases and old myocardial infarction in 8, respectively. 4.Efficacy of Total Pelvic Floor Reconstruction with Pesh Match for Presby-pelvic Floor Dysfunction

Objective To compare the efficacy of total pelvic floor reconstruction with mesh patch and vaginal hysterectomy with anterior and posterior vaginal wall repair for presby-pelvic floor dysfunction patients.Methods We randomly chose 92 pelvic floor dysfunction inpatients of our hospital from September 2007 to February 2009 and divided them into 2 groups:total pelvic floor reconstruction group(n=70) underwent total pelvic floor reconstruction with mesh patch[12 patients of this group complicated with stress incontinence,4 patients received tension free vaginal tape-obturator(TVT-O) simultaneously];control group(n=22) received vaginal hysterectomy with anterior and posterior vaginal wall repair(3 patients of this group complicated with stress incontinence).A follow-up for 3 to 18 months after the surgery were taken in all the patients.Results In total pelvic floor reconstruction group,no recidivate case was found and the FOP scores and cure rate were 0 and 100% respectively.In control group,there were 7 recidivate cases,all of who were primary vagina anterior wall bulge patients.The cure rate of control group was 68%;For the patients complicated with stress incontinence,they were all cured by TVT-0 or by ascending the front patch in total pelvic floor reconstruction group,but 1 case was as before operation and 2 cases were even worse in control group.Conclusion Total pelvic floor reconstruction with vicarious materials had a definite efficacy for presby-pelvic floor dysfunction.
5.Effect of pre-interventional coronary remodeling on outcomes of percutaneous balloon angioplasty in patients with angina pectoris

70% were enrolled into the study. Intravascular ultrasound (IVUS) was used before procedure, immediately after the final inflation of balloon and at the time of 3 month follow up. Cross sectional area(CSA) at the site of lesions, proximal and distal reference segment(vessel CSA,lumen CSA, plaque CSA) and remodeling ration were calculated respectively. Percutaneous cutting balloon angioplasty and intravascular irradiation was adopted to treat restenotic lesions during 3 month follow up period. Results Thirty seven of 76 cases had positive remodeling .There was no difference in the age, gender and other clinical parameters between positive and non positive remodeling(39 cases) groups. OBJECTIVE:To analyse the correlation between cyclosporine A(CsA)and the biochemical indicator of renal transplantation patients.METHODS:The patients were divided into6groups according to the different time length after renal transplatataion:0y~1y,1y~2y,2y~3y,3y~4y,4y~5y and above5y;the blood samples were taken12h after oral administra?tion of CsA,the of concentration CsA in blood of each group was determined and the biochemical tests of which were taken;post-operation medication and the correlation between the blood biochemical index were statistically analyzed by Stata soft?ware.RESULTS:The dosage of CsA decreased gradually from group1to group6,and the concentration of CsA also de?creased.And the data of BUN in group1,5and6were higher than the upper limit of their standard data,the data of triglyc?eride(TG)in group1,2,3,5and6were higher than the upper limit of their stand data;there were significant differences in the data of TP、ALB、TB、DB、Ua、TC、TG、?-GT among the6groups,while the data of ALP、ALT、AST、Glu、BUN、Cr among the6groups have no notable difference.CONCLUSION:The administration of CsA after renal transplantation can change bio?chemical indicator in patients.

objective To detect CK19 mRNA and CEA mRNA in blood of the peripheral vein and to investigate the effect of ligating pulmonary vein firstly or ligating pulmonary artery firstly during surgical operation on haematogenous dissemination of malignant cells.Methods Fifty six non-small cell lung cancer patients were collected and random assigned to two groups before operation (ligating pulmonary vein firstly or ligating pulmonary artery firstly).The patients were accepted radical operation and their operations were put in practice by doctors of one team.Vein blood was collected one day before operation and one week after operation.CK19 mRNA and CEA mRNA in blood were detected by nested reverse transcriptase-polymerase chain reaction. Result The positive rate of expression of CK19 mRNA and CEA mRNA after operation is lower than that before operation.The positive rate of expression of CK19 mRNA and CEA mRNA in ligating pulmonary vein firstly after operation is lower than that in the other group. Conclusions Surgical operation have effects on the dissemination of malignant cell and ligating palmonary vein firstly during operation can reduce the dissemination of malignant cell.
9.Therapeutic effect of erythropoietin combined oral iron in patients with chronic heart failure accompanied by anemia

Objective To explore the therapeutic effect of erythropoietin (EPO)combined oral iron in patients with chronic congestive heart failure( CHF)accompanied by anemia. MethodsNinety six patients with CHF accompanied by anemia, whom were consecutively hospitalized from January 2007 to December 2009, were enrolled into this study. They were randomly divided into treatment group accepted routine anti-heart failure therapy combined EPO and oral iron, and control group solely accepted routine anti-heart failure therapy. After 6 months follow up, the changes of hemoglobin ( Hb ), cardiac function classification, left ventricular ejection fraction(LVEF) ,6-minute walking distance,readmission rate of CHF and cardiac death were compared between two groups. ResultsCompared with those before therapy, we found significant improvements of hemoglobin level ( [ 120. 12 ± 10. 42 ] g/L vs [ 86.40 ± 14. 30 ] g/L, P < 0. 01 ), cardiac function classification ( 2. 65 ± 0. 67 vs 3. 13 ±0. 61, P < 0. 01 ), LVEF ( [ 37.21 ± 4. 96 ]% vs [ 33. 92 ± 7. 28 ]%, P < 0. 01 ), 6-minute walking distance ( [ 443.52 ± 97. 39 ] mvs [ 379. 15 ± 59. 34 ] m, P < 0. 01 ) in treatment group after EPO combined oral iron administration. After 6 months follow up, we also found significant improvements of Hb level ( [ 120. 12 ±10. 42 ] g/L vs [ 86. 40 ± 14. 30 ] g/L, P < 0. 01 ), cardiac function classification ( 2.65 ± 0. 67 vs 2. 98 ± 0. 81,P<0.01),LVEF([37.21 ± 4.96]% vs [34.67 ±4.10]%,P < 0. 01),6-minute walking distance ( [443.52 ±97. 39 ] mvs [ 379. 15 ± 59. 34 ] m, P < 0. 01 ) in the comparison between treatment and control group. The readmission rate of CHF fell significantly in treatment group compared to control (20. 83% vs 39. 58% ,P < 0. 05 ). However, we found no significant difference in cardiac death rate ( 0% vs 4. 17%, P >0. 05). ConclusionTreatment of EPO combined oral iron could significantly improve the cardiac function,increase exercise tolerance,lower the readmission rate of CHF in patients with chronic congestive heart failure (CHF)accompanied by anemia.
10.Application of Whole-body MRI in Idiopathic Inlfammatory Myopathy

PurposeTo investigate whole-body magnetic resonance imaging in diagnosis of idiopathic inflammatory myopathy (IIM), and to guide clinical treatment.Materials and Methods Twenty-six patients clinically diagnosed IIM including 19 dermatomyositis and 7 polymyositis were analyzed retrospectively. Whole-body MRI was performed in all patients including whole-body coronal scan and axial scan of both sides of thigh. Short tau inversion recovery (STIR) sequences were used in the coronal scan and T1WI and fat-suppressed T2WI were performed in the axial scan. Ten healthy volunteers were recruited as control group. MRI findings were compared between the two groups.Results Whole body MRI showed that muscles were homogeneously low signal intensity in the control group. Imaging findings of the patients with IIM included muscle inflammation, fascitis, subcutaneous tissue edema, muscle atrophy and fatty infiltration. The most common distribution site of muscle inflammation of dermatomyositis patients was thigh (n=18), followed by gluteus (n=14), great psoas muscle (n=13), shoulder muscles (n=11), leg muscle (n=11), erector spinae (n=10) and neck muscles (n=9). Findings out of muscles included cavities in the both lungs, infarction in femurs and tibias, avascular necrosis of femoral head, adrenal mass and thyroid nodules. The most common distribution site of muscle inflammation of polymyositis was thigh (n=5), followed by gluteus (n=3), great psoas muscle (n=3), shoulder muscles (n=2), leg muscles (n=2), erector spinae (n=1) and neck muscles (n=1). Findings out of muscles included 1 case of infarction in femurs and tibias.Conclusion Whole-body MRI can not only comprehensively evaluate the activity of IIM but also detect complications such as bone infarction. It's a powerful tool for diagnosis, guiding treatment and monitoring of IIM.
