1.Clinical research on temozolomide chemotherapy for adult diffuse low-grade gliomas

Objective:To compare the therapy effects of chemotherapy and non-chemotherapy groups and to determine the value of chemotherapy for adult diffuse low-grade gliomas. Methods:We reviewed and analyzed 64 patients with adult diffuse low-grade glio-mas admitted to the Beijing Tiantan Hospital from May 2005 to April 2008. Among the patients, 18 received 4 to 12 cycles of temozolo-mide chemotherapy after operation and radiotherapy, whereas the other 46 cases only received radiotherapy. The progression-free survival of the two groups was compared. Results:The median progression-free survival of the chemotherapy and the control groups were 67.8 and 51.3 months, respectively. The results of the comparison implied a significant difference (χ2=8.741, P=0.003). Conclu-sion:Chemotherapy can improve the progression-free survival of the patients with adult diffuse low-grade gliomas, and thus, can be recommended as its effective therapy.
2.Feasibility of Hydration Treatment During Perioperative Period of Interventional Therapy for Gerontic Patients with Coronary Heart Disease Complicated by Chronic Renal Insufficiency

Objective To investigate the safety and efficacy of protective measures(hydration treatment)during perioperative period of interventional therapy for renal function of gerontic patients with coronary artery disease(CAD)complicated by chronic renal insufficiency(CRI).Methods The relation between hydration therapies and the renal function of 168 CAD patients(≥60 years old)complicated by CRI were analyzed.These patients were divided into three groups:in the group A,patients underwent selective coronary angiography(SCA)with conventional treatment during interventional perioperative period(IPP);in the group B,patients underwent SCA and treated with the NaCl plus low dose NaHCO3 during IPP;in the group C,patients underwent percutaneous coronary intervention(PCI),and meanwhile treated with the same dose NaCl and NaHCO3 as the group B during IPP.Results Coronary interventions were successful in all 168 patients.Contrast medium associated nephropathy(CAN)was found in 30 patients,and incidences of the CAN were 17(27.4%)in the group A,3(7.5%)in the group B and 10(15.2%)in the group C,respectively.The accompanying diseases and characteristics of the patients also effected the incidence of CAN:the CAN incidence of patients with hyperlipidemia was significantly higher than that with normal cholesterol(P<0.001);patients with diabetes was significantly higher than that with normal serum glucose(P<0.001);patients with stenosis of renal artery was significantly higher than that with normal renal artery(P<0.001);patients with heart failure(grade Ⅲ~Ⅳ)was significantly higher than that with the grade Ⅰ~Ⅱ(P<0.001).Conclusion The hydration treatment with the NaCl plus low dose NaHCO3 during IPP can protect renal function and increase tolerance for gernotic CAD patients complicated by CRI during IPP,and improve prognosis.
3.Endovascular treatment of a rare type of aortic arch aneurysm derived from the fourth aortic arch

Objective:To report a rare type of aortic arch aneurysm. Methods: Three cases of aortic arch aneurysm derived from the fourth aortic arch were retrospectively analyzed. The pathogenesis and treatment of this type of aortic arch aneurysm were investigated. Results:Most of the aneurysm body was located in the Z2 zone, which was the stem from the fourth aortic arch in the embryonic development period. All of the 3 cases could not be explained by common etiology. We speculated that the cause might be developmental anomaly of the fourth aortic arch. All the 3 aortic arch aneurysms were totally ex-cluded with a covered stent. The technical success rate was 100%. Endoleak of typeⅠwas seen in one case, which was resolved in a later open surgery. During the follow-up, no type of complications was found. Conclusion:To the best of our knowledge, this is the first report of this type of aortic arch aneurysm. The cause may be developmental anomaly of the fourth aortic arch. Endovascular treatment of this type of aortic arch aneurysm is feasible.
4.Myocardial expression of Caspase-12 and GRP78 in cardiac arrest and beating heart mitral valve replacement

Objective To observe the expression of Caspase-12 and GRP78 of endoplasmic reticulum stress (ERS) in cardiac arrest and beating heart mitral valve replacement Methods Thirty patients with rheumatic heart disease mitral stenosis were randomly divided into beating heart group (BH,n=15) and cardiac arrest group(CA, n = 15). Both groups accepted MVR by beating heart surgery and cardiac arrest surgery under cardiopulmonary bypass (CPB) respectively. Right atrial myocardial tissues were collected at prior the start of CPB (T0), after aortic cross-clamping 30 minutes (BH group 30 minutes after CPB, T1) and stitched right atrium (T2) respectively. The method of reverse transcriptase polymerase chain reaction (RT-PCR) was applied to detect the expression level of Caspase-12 and GRP78 in two groups and positive staining of Caspase-12 and GRP78 of myocardial tissue slices in both groups was observed by immunohistochemical method. Results The expression of Caspase-12 in CA group heightened at T1and significantly increased at T2 (P < 0.05) but the expression of Caspase-12 in BH group had increased in T2 only (P < 0.05). Caspase-12 in CA group expressed higher than that in BH group at T1 and T2. The expression of GRP78 had increased at T1 in two groups but it in CA group expressed higher than that inBH group at T2. The number of positive staining of Caspase-12 and GRP78 in CA group was higher than that in BH group at T2. Conclusion MVR of beating heart can reduce the reaction of ERS to enhance the myocardial protection under CPB.
5.Genetic analysis and SSR mapping on an new stem stripe rust resistance gene YrY206 in Aegilops tauschii.

A wheat stripe rust resistance gene was screened out from Aegilops tauschii which is relative genera of wheat species, broadening the genetic basis of the anti-disease character of wheat species. By hybridizing diversed Ae. Tauschii species, which is either resistant or susceptible to wheat stripe rust, a dominant wheat stripe rust resistance gene was detected from Ae. Tauschii (Coss.) Schmal Y206. The novel gene was temporarily designated as YrY206. By bulk segregation analysis, four microsatellite markers Wmc11a, Xgwm71c, Xgwm161 and Xgwm183 were found to be linked to YrY206 with genetic distances of 4.0, 3.3, 1.5 and 9.3 cM, respectively. According to the locations of the linked markers, the resistance gene was located on chromosome 3DS. Based on the chromosomal location and the resistance pattern of the gene, YrY206 should be a novel stripe rust resistance gene.

6.Clinical research of targeted drug combined with chemotherapy for primary central nervous system lymphoma

Objective To explore the effect of new therapy of high-dose methotrexate (HD-MTX,3 g/m2) combined with rituximab on primary central nervous system lymphoma.Methods Thirty-seven patients with primary central nervous system lymphoma,admitted to our hospital from January 2006 to January 2013,were chosen in our study; targeted therapy group (n=9) accepted chemotherapy scheme (HD-MTX +Rituximab 375 mg/m2) for 2-6 cycles (4 weeks a cycle),and the traditional treatment group adopted whole brain radiotherapy plus HD-MTX for 2-6 cycles.Progressive free survival (PFS) was compared between the two groups.Results Complete response was achieved in 5 patients of targeted therapy group,partial response in 2,stable disease in 1 and progressive disease in 1.Complete response was achieved in 12 patients of traditional treatment group,partial response in 11,stable disease in 4 and progressive disease in 2.The median PFS in the targeted therapy group was 28 months while that in the traditional treatment group was 11 months,with significant difference (R=0.823,P=0.021).Conclusion The scheme of HD-MTX combined with Rituximab for primary central nervous system lymphoma relieves the side-effect of the traditional therapy and gains satisfied results,which can be recommended as an effective therapy for primary central nervous system lymphoma.
7.Clinical research of targeted drugs for recurrent malignant gliomas

Objective To compare with the therapy effect of bevacizumab or bevacizumab combined with dose-dense temozolomide on recurrent malignant gliomas.Methods Thirty-eight patients with recurrent malignant glioma,admitted to our hospital from June 2008 to June 2012,were chosen in our study.Among them,26 patients accepted bevacizumab therapy (single drug group) and 12 accepted bevacizumab combined with dose-dense temozolomide therapy (unite drugs group).Then,the clinical data,progressive free survival (PFS) and adverse effects were retrospectively analyzed.Results Single drug group:the median PFS was 21.8 weeks,PFS-6 months was 37.8%;united drug group:the median progressive free survival was 34.7 weeks,PFS-6 months was 52.7%;significant differences were noted between the two groups (x2=8.161,P=0.023) Conclusion Bevacizumab can prolong the PFS of patients with recurrent malignant glioma;the therapy effect of bevacizumab and dose-dense temozolomide is better than single drug,which can be recommended as an effective therapy for recurrent malignant glioma.

Sepsis is a complex syndrome caused by multiple pathogens and involves multiple organ failure, particularly spleen dysfunction. In 2017, the worldwide incidence was 48.9 million sepsis cases and 11 million sepsis-related deaths were reported (Rudd et al., 2020). Inflammation, oxidative stress, and apoptosis are the most common pathologies seen in sepsis. Liensinine (LIE) is a bisbenzylisoquinoline-type alkaloid extracted from the seed embryo of Nelumbo nucifera. Lotus seed hearts have high content of LIE which mainly has antihypertensive and antiarrhythmic pharmacological effects. It can exert anti-carcinogenic activity by regulating cell, inflammation, and apoptosis signaling pathways (Manogaran et al., 2019). However, its protective effect from sepsis-induced spleen damage is unknown. In this research, we established a mouse sepsis model induced by lipopolysaccharide (LPS) and investigated the protective effects of LIE on sepsis spleen injury in terms of inflammatory response, oxidative stress, and apoptosis.
