LDM, a product of the combination of contemporary information network technology and the tradi tional medical mode, is a rapidly developing new undertaking of recent years. The transformation of the medical mode in the establishment of LDM has brought about a series of problerms, including quality control of LDM, delegation of responsibility in IDM, standardization and unification of LDM, and psychological problems of patients under the mode of LDM. These problems present issues of new contemplation and exploration for hospital managers.
2.Teaching of chronic renal failure in a reasonable application of proprietary Chinese medicines

Chronic kidney disease has become a global public health problem, and a new pandemic, which seriously endanger people's health and life. With the gradual increase in kidney disease treatment of traditional Chinese medicine, western medicine doctors to rational use of Chinese patent medicine is also worthy of attention and discussion. In this research, we discussed the reliability of traditional Chinese medicine in chronic renal failure with practicability, to the analysis of the current status of Chinese patent medicine in General hospital, and summarizes the effect of traditional Chinese medicine in the treatment of renal diseases and analyzed the disadvantages. Finally, from the clinical research, we concluded that the reasonable application of traditional Chinese medicine in chronic renal failure has a long way to go and we need to make unremitting efforts.

4.Qualitative Value of Enlarged Axillary Lymph Nodes by Breast Molybdenum Target Radiography

Objective To evaluate the value of breast molybdenum target radiography in evaluating the enlarged axillary lymph nodes in breast diseases.Methods 79 patients underwent breast molybdenum target radiography,including conventional laxial and oblique projection. Some of them were examine furtheremore in compress projection in axillary fossa. The X-ray findings were analysis. And radiologic-pathologic correlation were carried out.Results There were 46 cases of brease carcinoma, 20 cases of hyperplasia,7cases of fibroadenoma,2 cases of plasmacytic mammitis,2cases of lipoma and 2 cases of papilloma in 79 patients. In the 33 cases of breast benign lesion , the enlarged axillary lymph nodes were seen bilaterally in 12 cases, sum was 1 to 5 in each case, smaller than 2 cm in diameter in 31 cases and uneven density in 13 cases were showed. In the 46 cases of brease carcinoma, the enlarged lymph nodes were seen laterally in 43cases, sum was 1 to 4 in each cases, larger than 2 cm in diameter was showed in 17 cases, and all were even density. There were statistical differences in number, diameter and density of enlarged lymph nodes between two groups. Conclusion The enlarged axillary lymph nodes are of different X-ray features in breast benign lesion and malignant lesion,it is helpful in differential diagnosis of breast diseases.
5.The Diagnostic Value of Breast Galactography in Nipple Discharge Diseases

Objective To investigate the value of galactography in the diagnosis with nipple discharge.Methods 40 cases of galactography with nipple discharge proved by clinic and pathology were analyzed retrospectively.Results There were normal ducts in 7 cases, simple duct dilatation in 17 cases, breast duct dilatation accompanied chronic mastitis in 4 cases, breast duct dilatation accompanied hyperplasia in 2 cases, intraductal papilloma in 8 cases, ductal carcinoma in 1 case and abscess of the breast in 1 case.Conclusion Breast galactography plays important roles in diagnosis and differentiation of the breast disease with nipple discharge, and provides significant information for clinical department.
6.Predictive values of the expression of ERCC1 and TS for FOLFOX adjuvant chemotherapy in gastric cancer

Objective To study the expression of the excision repair cross complementing 1 ( ERCC1 ) and thymidylate synthase ( TS) in gastric cancer and to investigate the correlation between the ERCC 1 and TS ex-pression with the efficacy of FOLFOX adjuvant chemotherapy in gastric cancer patients .Methods A total of 73 gastric cancer patients received modified FOLFOX adjuvant chemotherapy .Real-time PCR was used to investi-gate the expression of ERCC 1 and TS in gastric cancer tissue .We also studied their correlation and association with prognosis .Results ERCC1 and TS expressed in gastric cancer tissues with the positive rate were 42 .5%(31/73)and 61.6%(45/73).There was no significant correlation between the ERCC 1 or TS expression and the clinical features .In this study ,the median of RFS and MST were 16 months and 27 months in patients with ER-CC1-negtive expression,9 months and 16months in patients with ERCC1-positive expression,the median of RFS and MST were significantly different in the two groups (P=0.000,P=0.002).There were no statistical differences on TS expression pattern in either median of RFS or MST (P=0.10,P=0.71).Conclusion The ex-pression of ERCC1 may be useful in prediction of the clinical outcome in gastric cancer patients who treated with FOLFOX7 adjuvant chemotherapy .Patients with ERCC1-negtive expression may benefit from the therapy .
7.Establishment of human-rhesus chimeric liver using adult bone marrow mesenchymal stem cells

BACKGROUND:Human-mammal chimeric liver chimera has been a vital significance for the proliferation and differentiation of bone marrow mesenchymal stem cells.
OBJECTIVE:To establish an animal model of human-rhesus chimeric liver using adult bone marrow mesenchymal stem cells.
METHODS:Adult bone marrow mesenchymal stem cells were isolated, purified and cultured for the sixth generation. The number of bone marrow mesenchymal stem cells was no less than 5×108. Bone marrow mesenchymal stem cells labeled with green fluorescent protein were transplanted into the liver of the embryo rhesus with pregnancy of 10 weeks under guided by type-B ultrasound. At the 1st and 3rd months of birth, the liver tissue of the infant rhesus was taken for biopsy. After routine pathological section, histological specimens were observed under fluorescence microscope to confirm if there were adult bone marrow mesenchymal stem cells positive for green fluorescent protein and their distribution, and detected by immunohistochemical staining to identify if human albumin expressed in the liver of infant rhesus.
RESULTS AND CONCLUSION:Fluorescence microscope observation indicated that at the 1st and 3rd months after birth, there were surviving bone marrow mesenchymal stem cells derived from human with green fluorescence in the liver of infant rhesus, and these cells migrated to form more concentrated distribution. The immunohistochemical results demonstrated that functional liver cells expressing human albumin were observed in the liver of infant rhesus at the 1st and 3rd months after birth, and their distribution was in accordance with bone marrow mesenchymal stem cells with green fluorescence. Human-rhesus chimeric liver can be established using adult bone marrow mesenchymal stem cells, which can generate functional liver cells in the liver of infant rhesus.
8.Inhibition of sibutramine on hERG potassium current

Objective To study the effects of sibutramine on the human-ether-a-go-go-related gene (hERG) current in HEK293 cell line that overexpresses hERG potassium channels. Methods HEK293 cell line that overexpresses hERG potassium channels was constructed. Whole cell patch clamp techniques were employed to investigate the effects of sibutratime on Iherg in the cell line. Results Sibutramine reduced hERG current in a concentration-dependent manner:the inhibition rate was (8.82?2.32)%,(26.7?5.78)%,(44.93?4.60)% and (85.22?3.37)% at the dose of 3,10,30 and 100 ?mol/L. IC50 value was 32.66 ?mol/L (n=8). Ⅰ-Ⅴ relationship was inhibited by sibutramine (3,10,30 and 100 ?mol/L) at membrane potentials from -10 mV to +60 mV (step) and from 0 mV to +60 mV (tail) (n=8). Conclusion Sibutramine significantly inhibited Iherg in HEK293 cell line that overexpresses hERG potassium channels in a concentration-dependent manner,which may be a major mechanism of causing QT interval prolongation.
9.The relationship between the degree of basilar artery stenosis and the short-term prognosis in patients with isolated pontine infarction

Objective:To investigate the relationship between the degree of basilar artery stenosis and the short-term prognosis in patients with isolated pontine infarction.Methods:One hundred and thirty-seven patients with isolated pontine infarction within 1 month after symptom onset admitted to our hospital from April 2016 to April 2018 were consecutively included.Based on modified Rankin scale(mRS)socres, patients were divided into the good outcome group(mRS score≤2)and the poor outcome group(mRS score>2). Venous blood samples were taken for biochemical testing on admission or the next day.Baseline National Institutes of Health Stroke Scale(NIHSS)scores and demographic data were recorded and compared between the two groups.The degree of basilar artery stenosis was assessed by magnetic resonance angiography(MRA), and subjects were divided into the non-stenosis, mild stenosis, middle stenosis and severe stenosis subgroups.Results:There were 108 patients in the good outcome group and 29 in the poor outcome group.The baseline NIHSS score(2.71±0.22 vs.7.10±0.59, t=6.99, P<0.01)and total cholesterol[(4.29±0.101)mmol/L vs.(4.76±0.17)mmol/L, t=2.21, P=0.03]were lower in the good outcome group than in the poor outcome group.The proportion of patients without stenosis was higher(76 or 70.4% vs.5 or 17.2%, χ2=26.70, P<0.01)and the proportion of patients with severe stenosis were lower(4 or 3.7% vs.7 or 24.1%, P=0.002)in the good outcome group than in the poor outcome group.Binary logistics regression analysis showed that baseline NIHSS score( OR=1.658, 95% CI: 1.327-2.071, P=0.000)and degree of basilar artery stenosis( OR=2.071, 95% CI: 1.159-3.701, P=0.014)were risk factors for the short-term prognosis. Conclusions:The degree of basilar artery stenosis is a risk factor for the short-term prognosis in patients with isolated pontine infarction, and patients with severer stenosis will have a poorer prognosis.
