To study parameters influencing the prognosis of patients suffering from gallbladder carcinoma.MethodsA retrospective clinical analysis was made on 56 cases of gallbladder carcinoma, admitted to our hospital between 1995～2001.ResultsThere were 20 males and 36 femals. Thirty four patients(61%) complicated with gallstons.Abdominal pain(47/56,84%) and jaundice(23/56,41%) were the major complaints.Ultrasonography,CT scan and CA 19 9 measurement were helpful for preoperative diagnosis. Thirty cases(including 5 Nevin stage Ⅱ cases,3 stage Ⅲ cases,4 stage Ⅳ cases,and 18 stage Ⅴ cases) received radical or extended radical chelecystectomy, 11 cases with Nevin stage Ⅴ underwent palliative resection and 15 cases of Nevin stage Ⅴ were subjected to bypass procedure and/ or biopsy.The statistical analysis showed staging of Nevin,surgical procedure,gallstone,adjuvant radiotherapy and chemotherapy significantly affected the survival.Conclusions Early diagnosis, radical resection, active and comprehensive adjuvant radiotherapy with/or chemotherapy help to improve the prognosis of gallbladder carcinoma.

Objective Implementing the scientific research performance evaluation indicator system constructed to evaluate the situation of scientific research performance conducted by two basic Departments during the 12th Five-Year period in a general hospital.To understand the practical applicability of the scientific evaluation index system,and also propose comments and suggestions based on the comparison between assessment score and actual research situation in the departments.Methods Based on the database of scientific research management information platform,collect related research data from two basic departments of the hospital in latest five years.Using SPSS software to conduct statistical analysis on the total score of the first and two indicators,as well as the specific indicators of the three indicators,according to the performance evaluation index system and the weight built by the hospital.Results The index system can basically reflect the general information of scientific research input and output in the department,however,some indicators cannot truly reflect the index importance and the differences between the index weight is not significant,so it is necessary to make amendment based on demonstration.Conclusions Hospital should dynamically update the index system to make them match the developing strategy of the hospital,also important to combine them with research stimulation to guarantee more scientific management and better service to the research development of hospitals.

Objective The number of awarded provincial-level research projects and their budget had been declining year by year.We analyzed the data of provincial-level research projects in a general hospital in last ten years to be able to describe the distribution of research projects,the subject crossing situation,the distributions of subject and age structure,degree,title of the project leaders.We tried to find out the problems of them and to provide evidence for policy making.Methods Based on the scientific research management software,the data of the provincial-level research projects can be collected from 2005 to 2014.To Analyze the distributions of subject and structure of age,degree,professional title of the projects leaders by SPSS software.Results In general,the number and funds of provincial-level projects were unstable in the nearly ten years.Many of them focused mainly on basic departments and key subject.Young and middle-aged professionals were the backbone of the hospital team and talents with a high degree and title were the main force in the successful provincial-level projects.Conclusions According to the distribution trend of the projects,the hospital should deliver a carefully targeted set of actions.According to the distribution trend of the subject departments,the hospital should strengthen cooperation among departments and promote the integration of subjects.Strengthen the construction of talent team,pay attention to the cultivation of young talents.Strengthen scientific research management,To provide services for provincial level projects management.

Objective To investigate the possibility of performing epicanthoplasty with same stage small incision blepharoplasty. Methods At the same stage of designing small incision blepharo-plasty, modified "Z" plasty without flap transposition was designed. The upper lid incisive line of epi-canthoplasty was bided in the double eyelid fold, while the lower eyelid incision located at the edge of eyelid. Results A series of 56 patients were treated with this method. Satisfactory esthctical result of double eyelid fold and inner canthus shape was achieved. Conclusion This modified method including epicanthoplasty combined with same stage small incision blepharoplasty has the advantages of minimal scar formation, easy to design perform and in accordance with physiology.

Scientific and technological ability is one of the core competitiveness of the hospital.The hospital medical researchers are the main body of technological innovation activities.The establishment of scientific and effective incentive mechanism is an important way to stimulate the potential innovation of researchers and to promote the development of the hospital.In this article,we summarized the common problems on current research incentive mechanism in hospitals and proposed the corresponding strategies,in order to improve the research incentive mechanism.

Objective Investigating and analyzing problems and influencing factors in the process of hospital-level research project management,proposing effective and feasible recommendations,and to prove a reference for hospitals administrative management development.Methods The research analyzed factors including researchers,the departments and implementation.Results Since 2005,the total funding to support research projects in the hospital-level project reached 6.345 million yuan,the pass rate in middle-term examination was 84.94％ and was 71.81％ in final examination,with the majority of them were senior researchers.Conclusions Problems founded in this study includes the participate rate was low inyoung and middle-aged researchers,the quality of hospital-level research is also low,lack of research time,and the research outcome is not satisfied.We suggest strengthen the management of the research process,improve the mechanism of scientific rewards and punishments,integrate the disciplines resources,and improve the quality of hospital-level research project.

Objective To investigate the expression and the effects of tissue inhibitor of metalloproteinases-1 (TIMP-1) on lungs of rats with sepsis.Methods Forty Sprague-Dawley (SD) rats were randomly divided into two groups,namely sham group (n =8) and sepsis model group (n =32).The rats of model group were modeled by cecal ligation and puncture (CLP),and were further divided into four subgroups as per the time after modeling,namely 6 h (n =8),12 h (n =8),24 h (n =8),48 h (n =8)subgroups.Blood and lung samples were taken 6 h,12 h,24 h and 48 h after modeling.The histological changes in lungs of the rats were observed under light microscope.Expressions of TIMP-1 mRNA,Bax mRNA and Bcl-2 mRNA in lungs were measured by RT-PCR.The immunohistochemistry was used to label the CD18 in lungs during different phases of sepsis.The data were processed by t test.Results Compared with sham group,the lung tissues of rats in model group were injured to a certain extent after CLP.The expression of TIMP-1 mRNA and the number of CD18 positive cells increased at the same time (P ＜ 0.01),and peaked 24 hours later (P ＜ 0.01).While the expression of Bax mRNA in model group decreased markedly 12-48 hours after modeling (P ＜ 0.01-0.05),and reached minimum 48 hours later (P ＜ 0.01).The expression of Bcl-2 mRNA in model group changed unnoticeable.The positive correlation between variations in number of CD18 positive cells and expression of TIMP-1 mRNA was found in model group (r =0.426,P ＜ 0.01).Conclusions The increase in expression of TIMP-1 mRNA in lungs is closely associated with the lung injury of sepsis.The mechanism of lung injury is likely attributed to the preservation of inflammatory cells from apoptosis,and the persistent inflammation response causes tissue damage,leading to organ dysfunction.

Objective To identify the miR-217 targeted gene ANLN by experiment.Methods Bioinformatic algorithms were used to predict the potential targets of miR-217.Then,ANLN binding with miR217 and mutant ANLN (mutANLN) sequence were designed and synthesized,and their amplified fragments were inserted into plasmid psiCHECK-2,and recombinant plasmid psiCHECK-2-ANLN and psiCHECK-2-mutANLN were reconstructed.The two recombinant plasmids were co-transfected into pancreatic cancer cell line PANC1 with miR-217,miR-217 inhibitor,NC,NC inhibitor by liposome,respectively.Dual luciferase reporter system was used to determine the luciferase activity,and Western blot was used to measure the expression of ANLN protein.Results The luciferase activities of psiCHECK-2-ANLN,psiCHECK-2-ANLN +miR-217,psiCHECK-2ANLN + miR-217 inhibitor,psiCHECK-2ANLN + NC,psiCHECK-2-ANLN + NC inhibitor were 2.221 ± 0.188,0.769 ± 0.061,3.764 ± 0.371,2.265 ± 0.201,2.242 ± 0.018,and the difference among these groups was statistically significant (F =77.405,P ＜0.001),but the difference among psiCHECK-2ANLN group,psiCHECK-2-ANLN + NC group and psiCHECK-2-ANLN + NC inhibitor group was not statistically significant.However,luciferase activities of psiCHECK-2-ANLN + miR-217 group were significantly decreased when compared with other 3 groups,and luciferase activity of psiCHECK-2-ANLN +miR-217 inhibitor group were significantly increased when compared with other 4 groups (all P ＜0.001).Luciferase activities of groups transfected with psiCHECK-2-mutANLN was not significantly different (P =0.053).The expression of ANLN protein in PANC1 with psiCHECK-2-ANLN + miR-217 co-transfection was significantly down-regulated when compared with that with psiCHECK-2-ANLN transfection alone.Conclusions ANLN is one of the direct target genes of miR-217 in PANC1 cells.

Objective The subject number and funds of cooperative projects had been an important indicator of the strength of scientific research in hospital.This article analyzed the data of cooperative projects of a general top three hospital in recent tenyears to provide reference for policy-making of scientificresearchmanagement department.Data collected and analyzed including subject distribution,the disciplines crossing situation,background information on principle investigators.Methods Based on the scientific research management software,the data of the cooperative projects can be collected from 2006 to 2015.The distributions of subjects and the age,education,and title of PIs were analyzed by SPSS software.Results In general,the project numbers and fundingof cooperative projects were unstable in past ten years.Many focused mainly on basic departments and key subjects.There was no obvious advantage in the integration of subjects.Thecooperative projects of national science and technology supporting plan played an important role in national level projects.And the science and technology supporting plan of Beijing city played an important role in province-level projects.Many of the PIs of cooperative projects were talents with higher education,higher professional title and senior in their career stage.Conclusions Administrators of the hospital should pay attention to the management of cooperative projects.Strengthen cooperation among departments and promote the integration of subjects.Improve the construction of human resource,pay attention to the cultivation of young talents.Establish effective incentive mechanism.

OBJECTIVETo study the application of three-dimensional visualization technology for laparoscopic resection of cystic carcinoma in the pancreatic body and tail.

METHODSSix cases of cystic carcinoma in the pancreatic body and tail treated between Nov, 2009 and Mar, 2011 were retrospectively analyzed. The original image data of 64-slice spiral CT were collected and using adaptive region growing algorithm, the serial CT images were segmented and automatically extracted to obtain the 3-dimensional reconstruction images with customized image manipulation software. The specific surgical approach (the trocar position) and surgical procedure were planned based on the reconstructed mode.

RESULTSThe reconstructed 3-dimensional model clearly displayed cystic carcinoma in the pancreatic body and tail and the adjacent organs, showing distinct relationship between the cystoma and the splenic artery and vein. All the patients successfully underwent laparoscopic resection of the pancreatic body and tail without perioperative death. The spleen was preserved in 5 cases and removed in 1 case due to mucinous cystadenocarcinoma. The overall rate of pancreatic fistulae was 33.3% without incidences of postoperative hemorrhage. The average hospital stay of the patients was 12 days.

CONCLUSIONThree-dimensional reconstruction based on pancreatic CT data provides valuable assistance for laparoscopic resection of cystic carcinoma in the pancreatic body and tail.
