1.Analysis on distribution and drug-resistance of clinical pathogenic bacteria in a large scale general hospital in Yunnan province

Objective To investigate the distribution and drug resistance situation of clinical isolated bacteria in our hospital to provide the basis for rational selection of antibacterial drugs in clinic .Methods The pathogenic bacteria isolated from the clinical sepcimens in our hospital from January 2012 to December 2013 were performed the idenitification and drug susceptibility test by a‐dopting the the bioMerieux VITEK‐2 compact automatic bacterial analyzer .The detection results were judged according to the standard of the Clinical and Laboratory Standard Institute (CLSI) .The WHONET5 .6 software was used for conducting the statisti‐cal analysis .Results 2 274 strains were isolated during 2012-2013 ,in which 1986 strains (72 .4% ) were Gram‐negative bacteria , 675 strains(24 .6% )were Gram‐positive bacteria and 94 ccases(3 .0 % ) were fungi .The top 3 of Gram‐negative bacteria were Esch‐erichia coli(694 strains ,25 .3% ) ,Klebsiella pneumoniae (303 strains ,11 .0% ) and Pseudomonas aeruginosa (266 strains ,9 .7 % ) respectivley .The top 3 of Gram‐‐positive bacteria were Staphylococcus aureus (184 strians ,.6 .7% ) ,coagulase negative staphylococ‐cus(146 strains ,5 .3% )and enterococcus faecium (119 strians ,4 .3% ) .The respiratory tract infection ,urinary tract infection and blood infection were predominant .The detection rate of methicillin‐resistant Staphylococcus aureus (MRSA) was 45 .7% ,while which of methicillin‐resistant coagulase negative Staphylococcus (MRCNS) was 84 .6% .MRSA showed the multiple resistance to fluoroquinolones and aminoglycosides antibcatieral drugs ,the resistance rate> 90 .00% ,but resistance rate macrolide antibiotic was<50 .0% ;no vancomycin‐and linezolid‐resistant Staphylococcus was found ;the resistance rate of enterococcus faecium to vancomy‐cin was 1 .7% and no linezolid‐resistant enterococcus faecium was detected .The detection rates of extended‐spectrum β‐lactamase‐producing K .pneumoniae and E .coli were 65 .2% and 39 .9% respectively ,and which of E .coli and K .pneumoniae to imipenem were 0% and 6 .3% respectively .The resistance rate of Acinetobacter baumannii to commonly used antibacterial drugs was more than 50% .The resistant rate of pseudomonas aeruginosa to common antipseudomonal drugs was <40 .0% .Conclusion The drug resistance phenomena of pathogenic bacteria isolated from our hospital is relatively universal ,especially multi‐drug resistant non‐fer‐mentative bacteria ;the enzyme‐producing mechanism leads to increase the detection rate of multi‐antibacterial resistant Enterobacte‐riaceae bacteria ,which causes the enormous difficulty for clinical anti‐infection therapy .Conducting the bacterial drug‐resistance mo‐nitoring has an important significance for guiding clinically rational drug use ,reducing the nosocomial infection rate and controlling the bacterial drug resistance .
2.A Case-controlled Stody of Personality Dysfunction among High School Students

Objective:To explore the effect of parental rearing behavior and related factors of personality dysfunction in adolescents Method:By means of stratified cluster sampling, 9574 students from 25 representative senior high schools in urban and rural areas of Beijing were investigated by PDQ-4 (personality diagnostic questionnaire-4), EMBU and general information questionnaire A case control study based on community was carried out afterwards The cases were the students whose total PDQ-4 score was 42 (means?1 645s) or more The controls were students whose total PDQ-4 score was 25 (means) or less Cases of personality dysfunction (n=531) were individually matched with 1062 controls by age and gender in ratio of 1:2 The single variable and multivariate conditional logistic regression analyses were performed to screen out the risk factors of personality dysfunction Result: The single variable conditional logistic regression analysis revealed that parental rejection, overprotection, favoring subject, rural area, type of school, poor parental relationship and single parent family increased risk of personality dysfunction, while parental emotional warmth was a protective factor The multivariate conditional logistic regression analysis gave the same results with OR=11 43 for parental rejection, OR=7 68 for overprotection, OR=13 94 for rural area, OR=4 15 for poor parental relationship, and OR=0 50 for parental emotional warmth There was no association between personality dysfunction and nationality, family income, single child family or level of parental education Conclusion:Personality dysfunction in adolescents is closely related to family environment
3.Washing Effect of Surgical Instruments after Three Different Methods to Keep Moist

OBJECTIVE To study the washing effect of different methods to keep the surgical instruments moist. METHODS The instruments which were selected from the routine surgical instruments within a week were washed after three different methods to keep moist. The washing effect was compared by eyes, without equipment and with 5 times magnifying glass. RESULTS The multienzymatic detergent was the best one to keep moist, the next was the water, and the chlorine releasing agent was the worst. CONCLUSIONS Hospitals should select the suitable method to keep moist by considering the actuality.

5.Relationship of p63 and CD44 expressions with age and body sites

Objective To clarify the relationship of p63 and CD44 expressions with age and ultraviolet irradiation in the process of skin aging.Methods Full-thickness skin samples were obtained from the normal skin at different sites,including 44 sun-exposed sites(face,neck,dorsal aspect of the forearm),46 sun-protected sites(abdomen,thigh),and 31 semi-exposed sites(back,flexor aspect of the forearm,upper arm,leg),of 121 volunteers(63 males and 58 females).Of all the volunteers,55 were 10 t0 30 vears of age (young),41 were 31 to 50 years of age(adult),and 25 were 51 to 79 years of age(old).Immunohistochemisty using antibodies to p63 and CD44 was applied to detect the expression of p63 and CD44 in these specimens.Imaging analysis software was utilized to calculate the percentage of p63-expressing cells in basal layer as well as the expression level of p63 and CD44.Results In stratum basale,the percentage of p63-expressing cells decreased with age(r=-0.218,P<0.05),but showed no significant difference among sun-exposed skin,semi-exposed skin and sun-protected skin(P>0.05).The correlation between the expression intensity of epidermal p63 and age was negative in sun-exposed sites(r=-0.389,P=0.010),positive in sun-protected sites(r=0.341,P<0.05).but non-significant in semi-exposed sites(P>0.05).With regard to the expression of epidermal p63,no significant difference was observed among sun-exposed sites,sun-protected sites and semi-exposed sites in volunteers aged from 10 to 30 years.while in those aged from 31 to 50 years and from 51 to 79 years.sun-protected sites was significantly higher than semi-exposed sites followed by sun-exposed sites(all P<0.05).There was a weak correlation between the expression intensity of epidermal CD44 and age(r=-0.083,P<0.05),but no significant difierence was noted among the three kinds of sites(P>0.05).Conclusions In stratum basale,the percentage of p63.expressing cells and the expression intensity Of CD44 negatively correlate with age,but have no correlation with ultraviolet irradiation.In individuals aged from 31 to 50 years and from 51 t0 79 vears.the expression of epidermal p63 decreases with the accumulation of sun-exposure.

Objective To examine the effect of labouring in water on analgesia of vaginal delivery.Methods From June 2009 to February 2011,38 women who had volunteered to deliver in water in the obstetrical department were set as the observation group,another 70 women who chose vaginal delivery were named as the control group in the corresponding period.The items of labor pain,satisfaction with the birth experience,length of labor and neonatal outcomes were compared.Results The pain level of the observation group after labouring in water decreased compared with that before immersion in water.The delivery course was not influenced with a high rate of vaginal delivery.Conclusions Labouring in water can alleviate delivery pain,increase rate of vaginal delivery with no influence on delivery course and outcome of mothers and infants.It is a safe and effective analgesia method which should be widely applied.

AIM: To investigate the functions of GR in the course of hepatic secondary injury after severe multiple injury. METHODS: Rat model was produced by adopting severe thoracic impact injury accompanied with mono-side femur fracture, and glucocorticoid receptor was blocked before severe multiple injury. Hepatic macropathology and alterations under light microscope were examined. Maximal binding volume of glucocorticoid receptor (GR) in hepatic tissue was assayed by radio-ligand binding assay and protein content was assayed by Western blot. Objective To analyze the factors related to the occurrence of complications in performing CT-guided percutaneous lung biopsy with 18 G-Bard Magnum needle. Methods CT-guided percutaneous lung biopsy with 18 G-Bard Magnum needle was carried out in a total of 58 patients. The postoperative complications were recorded, and the related factors causing complication were analyzed. Results Successful puncturing was achieved in all patients with a technical success rate of 100%. Postoperative complications included pulmonary hemorrhage (n=11,19.0%), a little amount of bleeding in needle tract (n=7,12.1%), hemoptysis (n=3,5.2%), hemothorax (n=1,1.7%), and pneumothorax (n=10,17.2%). Chi-square test showed that the occurrence of pulmonary hemorrhage bore a close relationship to the lesion’s diameter, the distance between the lesion and the chest wall, the lesion’s location and times of puncturing (P<0.05). The occurrence of pneumothorax was closely correlated with the age, the distance between the lesion and the chest wall, the presence of perifocal emphysema, the lesion’s location and times of puncturing (P<0.05). Univariate analysis indicated that the postoperative complications were liable to occur in the patients whose imaging examination showed perifocal emphysema and lung hilar lesion, and who had more than two independent risk factors (P<0.05). Conclusion CT-guided percutaneous lung biopsy with 18 G-Bard Magnum needle is an accurate and safe technique with relatively higher positive diagnosis rate, but this technique should be carefully used in patients who has perifocal emphysema, or lung hilar lesion, or more than two independent risk factors.(J Intervent Radiol, 2015, 24:792-796)
