1.Prospect of evidence-based medicine on non-surgical treatment of hepatocellular carcinoma

Non-surgical method plays a very important role in the treatment of hepatocellular carcinoma(HCC). The related evidence-based medical research findings such as the therapeutic effect differences in different technical methods, materials of ablation therapy and interventional therapy, and in the survival benefits of differential anti-tumor drugs etc, have attracted more and more attention.
2.Effects of different target concentrations of remifentanil when combined with propofol on BIS in patients undergoing neurosurgery

Objective To investigate the effects of different target concentrations of remifentanil when combined with propofol on BIS in patients undergoing neurosurgery. Methods Fifteen ASA Ⅰ or Ⅱ patients, aged 18-64 yr, weighing 50-85 kg, scheduled for intracranial surgery, were involved in this study. The patients received propofol by target controlled infusion (TCI) at a target effect site concentration (Ce) of 3μg/ml. TCI of remifentanil was started after TCI of propofol reached the preset Ce and Ce of remifentanil was increased step by step from 2 ng/ml to 3, 4, 5, 6, 7 and 8 ng/ml. Invasive BP, MAP, HR and BIS were continuously monitored and recorded when remifentanil reached the each preset Ce. Vecuronium 0.1 mg/kg was injected iv when Ce of remifentanil reached 5 ng/ml, and then the patients were intubated and mechanically ventilated 3 min later. The changes in BP, MAP, HR and BIS were recorded. When HR < 50 bpm and/or MAP < 60 mm Hg, TCI of remifentanil was stopped and iv atropine or ephedrine was given. Results BIS was significantly decreased after TCI of remifentanil when propofol reached 3 μg/ ml compared with the baseline value ( P < 0.05 or 0.01). BIS was significantly decreased when Ce of remifentanil ≥6 ng/ml compared with that when propofol reached 3 μg/ml ( P < 0.05 or 0.01) .Conclusion Low-concentration of remifentanil when combined with propofol has no effect on BIS, but when Ce of remifentanil≥ 6 ng/ml, BIS is obviously decreased.
3.Molecular biomarkers related prognosis in hepatocellular carcinoma

The occurence and progress of hepatocellular carcinoma(HCC)is a complex network inter-action process connected by multiple signaling pathways involving genes,cells and various active molecules. The effects and functions of biological molecular markers also vary among different pathways. Moreover,wheth-er the molecular markers have prediction effects for the prognosis of HCC remains unclear,and studies investing this issue are still insufficient. The current studies reveal that molecular markers involves in multiple pathways including gene expression,gene regulation,cell cycle regulation,cancer stem cell proliferation and differentia-tion. These processes are intimately associated with the occurrence and prognosis of HCC. The relevant gene or molecular that may indicate the prognosis of HCC includes p53 gene,long non-coding RNA,cell cycle regula-tors,liver cancer stem cell surface-associated molecules,etc.
4.Cerebral ischemic tolerance mediated by toll like receptor signal transduction pathway

Aim Cerebral ischemic tolerance can be induced by a variety of preconditioning stimuli,such as transient global and focal ischemia,hypoxia,and administration of lipopolysaccharide(LPS),proinflammatory cytokines,anesthetics.Tolerance can be established with two forms: a rapid form in which the trigger induces tolerance to ischemia within minutes and a delayed form in which development of protection takes several hours or days.During ischemia,inflammatory reaction was initiated by toll-like receptors(TLRs) that recognize host-derived molecules released from injured tissues and cells,which exacerbates ischemic injury.In the delayed form of tolerance,the preconditioning stimuli first triggers the TLRs inflammatory pathway,leading not only to inflammation but also to simultaneous upregulation of feedback inhibitors(such as anti-inflammatory cytokines,decoy receptors,and signaling inhibitors),which reduced the inflammatory response to subsequent severe ischemia.The rapid tolerance is achieved by direct interference with membrane fluidity,causing change of lipid rafts leading to inhibition of TLRs signaling pathways.The comprehension of mechanism of cerebral ischemic tolerance highlights new avenues for future prevention and treatment of ischemic brain injury
5.Clinical Observation on 80 Cases of Functional Dyspepsia Treated with Dahuang Jianzhong Decoction

Objective:To observe the therapeutic effect of Dahuang Jianzhong Decoction on functional dyspepsia(FD).Methods:120 cases of FD were divided into four syndrome types:deficiency-cold of the spleen and stomach,stagnation of the liver-qi,insufficiency of stomach-yin and accumulation of damp-heat,and they were treated for 4 weeks.The treatment group(n=80) were treated with Dahuang Jianzhong Decoction,and the control group(n=40) with Motilium.Results:The total effective rate for the clinical symptoms was 90% in the treatment group and 72.5% in the control group,with a significant difference between the two groups(P
6.Blood-saving efficacy of acute hypervolemic hemodilution-hemostatics-intraoperative blood salvage in patients undergoing orthopedic surgery

Objective To evaluate the blood-saving efficacy of acute hypervolemic hemodilution (AHH)-hemostatics-intraoperative blood salvage (IOBS) in patients undergoing orthopedic surgery. Methods One hundred and twenty ASA Ⅰ or Ⅱ patients of both sexes, aged 18-64 yr, scheduled for elective orthopedic surgery under general anesthesia with an expected blood loss of 800 ml or more, were randomly divided into 4 groups ( n = 30 each) : AHH + IOBS + hemostatics group; IOBS + hemostatics group; AHH + IOBS group; AHH + bemostatics group. AHH was induced with 6% hydroxyethyl starch 130/0.4 15 ml/kg infused iv at a rate of 40 ml/min immediately after tracheal intubation until the time of immediately before skin incision, IOBS was performed immediately before skin incision. Intravenous hemocoagulase 2 kU and im hemocoagulase 1 kU were injected 10 min before skin incision. The total volume of fluid intake and output, HR, MAP and CVP were recorded during the operation. Vein blood samples were taken for determination of Hb, Hct, platelet counts (Plt), prothrombin time (PT), activated partial thromboplastin time (APTT), and fibrinogen concentration (Fib) .Results CVP was significantly lower in IOBS+ hemostatics group than in AHH + IOBS + hemestaties group ( P < 0.05), while no significant difference in CVP was found between AHH + IOBS and AHH + IOBS + hemostatics group and between AHH + hemostatics and AHH +IOBS + hemostaties group (P > 0.05). The volume of blood loss was significantly higher in AHH + IOBS group, and the allogenic blood transfusion volume was significantly higher, while the percentage of the patients without allogeneie blood transfusion and without FFP transfusion lower in AHH + hemostatics group than in AHH + IOBS + hemostatics group ( P < 0.05 or 0.01 ), but there were no significant differences in the above parameters between IOBS + hemostatics group and AHH + IOBS + hemostatics group ( P > 0.05). Hb, Hct, Plt and Fib were significantly higher in IOBS + hemostatics group than in AHH + IOBS + hemostafics group( P < 0.05), but there were no significant differences in the above parameters between AHH + IOBS and AHH +IOBS + hemostatics group and between AHH + hemostatics and AHH + IOBS + hemestatlcs group (P > 0.05 ). Conclusion The blood-saving efficacy of AHH-bemostatics-IOBS is good in patients undergoing orthopedic surgery and it is a safe technique.
7.Erythropoietin ameliorate experimental allergic encephalomyelitis

Objective:To investigate the beneficial effect of erythropoietin(EPO)on experimental allergic encephalomyelitis(EAE).Methods: The severity of EAE mice with or without EPO therapy were evaluated through clinical symptoms score and histological observation.Mononuclear cells from spleens,lymph nodes, brains and lumber spinal cords were applied to flow cytometry to detect ratios of CD4~+T subgroup, scales of apoptotic and dead cells, levels of inflammatory cytokines.LDH release of peripheral mononuclear cells were examined after incubating with EPO at different concentrations.Results: EPO ameliorated clinical symptoms and infiltration of pathological inflammatory cells in EAE mice(P<0.05); Ratio of CD4~+T subgroup, levels of IL-17 and IFN-γ in EAE mice with EPO therapy were significantly lower than those of EAE control group(P<0.05).No statistical significant differences existed on scales of dead cells, levels of IL-10 and CD4~+ CD25~+ T cells (P>0.05).EPO at the concentrstions of 1-1 000 U/ml had no effect on LDH release of peripheral mononuclear cells(P>0.05).Conclusion: EPO play the role of neuroprotecion in EAE mice by the way of decreasing infiltrating inflammatory cells and lowering the levels of IL-17 and IFN-γ.
8.Diagnostic Value of Enlargement of Anterior Space of the Right Branch of Portal Vein in the Early Cirrhosis at Multi-detector Row CT

Objective To evaluate the diagnostic value of enlargement of the anterior space of the right branch of portal vein in early cirrhosis.Methods 37 cases with early cirrhosis and 42 cases with type B hepatitis proved by pathology as well as 40 health cases underwent MDCT examinations.The width of the anterior space of right branch of portal vein(RBPV)and the diameters of RBPV in each group were measured on CT imaging,then the results were analysed comparatively.Results The width of anterior space of RBPV and the diameters of RBPV were(5.5±2.4)mm and(11.84±1.91)mm in health group,(13.6±5.8)mm and(12.36±61.97)mm in early cirrhosis group,(5.7±3.1)mm and(12.13±1.89)mm in type B hepatitis group,respectively.There were significant differences in the width of the anterior space of RBPV between three groups(P<0.0001).However,there were not significant differences in the width of RBPV among these three groups(P>0.05).While,there were significant differences of the width of the anterior space of RBPV and the diameters of the RBPV between normal control and type B hepatitis(P>0.05).Conclusion CT sign of the anterior space of RBPV increased is of important value in diagnosis of early cirrhosis.
9.Theoretical modeling and experimental research on direct compaction characteristics of multi-component pharmaceutical powders based on the Kawakita equation.

Base on the Kawakita powder compression equation, a general theoretical model for predicting the compression characteristics of multi-components pharmaceutical powders with different mass ratios was developed. The uniaxial flat-face compression tests of powder lactose, starch and microcrystalline cellulose were carried out, separately. Therefore, the Kawakita equation parameters of the powder materials were obtained. The uniaxial flat-face compression tests of the powder mixtures of lactose, starch, microcrystalline cellulose and sodium stearyl fumarate with five mass ratios were conducted, through which, the correlation between mixture density and loading pressure and the Kawakita equation curves were obtained. Finally, the theoretical prediction values were compared with experimental results. The analysis showed that the errors in predicting mixture densities were less than 5.0% and the errors of Kawakita vertical coordinate were within 4.6%, which indicated that the theoretical model could be used to predict the direct compaction characteristics of multi-component pharmaceutical powders.

Objective To understand the mechanism of extracts of Hedysarum ploybotrys hand Ma22 on Schwann cell,we studied the effects of the extracts on protein tyrosine kinase (PTKs) in vitro Methods Getting sciatic nerve by operation from SD rats and cultured them in PRMI 1640 medium with 15% embryo bovine serum and with Hedysarum Ploybotrys Hand Ma22 at rate of 1∶1 600,1∶3 200,1∶6 400,1∶12 800 or with nerve growth factor (NGF) NGF as positive control and normal medium as negative one After 48,72 and 96 hours cultures,the sciatic nerve been captured separately and used to examine the activity of PTKs by [? 32 P] ATP incorporation test using protein tyrosine kinase assay system Result In normal culture condition,there are no active PTKs Cultured with NGF and Hedysarum Ploybotrys Hand Ma22,the activity of PTKs increases notable The peak of NGF group appears at 48th hours and 96th hours were 0 0636 and 0 1548 respectively. Hedysarum Ploybotrys Hand Ma22 at 48th hours were 0 4460,0 3326 and 0 4169 at concintration of 1∶1 600,1∶3 200,1∶6 400, and decrease after it.
