Bone morphogenetic protein-4(BMP4) is one of BMPs member,it not only can play an important role in cell proliferation and invasion,but also play an important role in cell differentiation,apoptosis and angiogenesis.A study showed BMP4 in malignant tumor can inhibit the growth of tumor cells,and also can induce tumor cell invasion,metastasis and epithelial mesenchymal transformation,and SMAD is the classic signaling pathways.BMP4 is expected to become a potential therapeutic targets and in evaluating the prognosis,immune therapy in malignant tumor.In this paper,the structure and function of BMP4 and its mechanism in tumor were reviewed.
2.Effect of lidocaine and bupivacaine on Na~+ current in the dorsal horn neurons of spinal cord

Objective The effect of lidocaine and bupivacaine on the Na+ current of dorsal horn neurons was observed to further evaluate the mechanism of local anesthetics . Methods The dorsal horn neurons of the SD neonates(0-7 d) were isolated acutely. Under the condition of holding voltage -80mV , and testing voltage -30mV with duration of 20 ms , the whole-cell patch-clamp technique was applied to recording the changes of voltage-gated Na+ currents following the administration of lidocaine or bupivacaine at 50-1000?mol/L.Results The voltage-gated Na+ currents ranged from 0.5-8nA peak amplitude , was inhibited by lidocaine and bupivacaine at clinical concentrations, the inhibitory degree was parallelly correlated with the concentration of local anesthetics(r=0.949 and 0.847 ,P

4.The role of the posterior elements of the spine in the classification of th oraco-lumbar burst fractures and spinal cord injuries

Objective To observe the role of the posterior e lements of the spine in the spinal cord injuries and to provide the basic evidence for proper evaluation of the se verity of spinal cord injuries and th e possibilities of functional recove ry.Methods Experimental study:The experiment al model of spinal burst fracture was established in the thoraco-lumbar column section which subjected high velocity vertical loadi ng in static pattern.The pressure changes in the vertebral canal and the severity of posterior elements of the spin e were observed,and then their relationship was studied.Clinical Study:The data were obtained from 127consecutive cases of thoraco-lumbar burst fracture associated with paraplegia.The relationship between t he severity of spinal cord injury,functional recovery and the posterior elements injuries was studied retrosp ec-tively.Results The experimental study showed that g reat pressure changes appeared in th e vertebral spec-imens without posterior elements in juries.On the contrary,the peak value of pressure was low in cases with posterior elements injuries.Clini cal results:Of 38cases(69.09%)with spinal cord injuries in 55patie nts without posterior elements injurie s,functional recovery was found in 24cases(63.16%).In another group,of 52cases(72.22%)with spinal cord injuries in 72patie nts with posterior elements injurie s,functional recovery was found in 20cases(38.46%).The statistic difference between t he two groups indicated that the severity of spinal cord injuries in t he patients without posterior eleme nts injuries was more severe than that with posterior elements injuries.Conclusion The spinal posterior elements play a n important role in spinal cord injuries.The severity of posterior elements injuries can be used as a sca le in the evaluation of the severity o f spinal cord injuries,functional re covery and improvement of Denis classification.[
5.Liver protection of prostaglandin E_1 in patients after hepatectomy

Objective To investigate the effect on and mechansm by which prostaglandin E_1(PGE_1)protects liver functions after hepatectomy.Methods In this study,82 cases undergoing hepatectomy were divided randomly into control group with conventional therapy(41 cases),and PGE_1 treatment group(41 cases)treated with liposomal prostaglandin E_1 in addition to conventional therapy.Postoperative hospital days,urinary volume and abdominal drainage volume were observed.Pre-and postoperative liver functions were measured.Results Postoperative hospital days(median time 22 days)in PGE_1 treatment group were significantly shorter than those(median time 26 days)in control group.The postoperative levels of alanine transaminase,aspartic transaminase,total bilirubin and albumen in PGE_1 treatment group recovered to preoperative levels more quickly,than control group.Postoperative urinary volume in patients of PGE_1 treatment group was significantly more than that in control group,while abdominal drainage volume was markedly less,although there was no significant difference in prothrombin time between the two groups.Conclusion In patients undergoing hepatectomy,PGE_1 is very useful and safe to protect and improve hepatic function,decreasing the level of bilirubin,preventing ascites,formation shortening hospital days,without causing prolongation of prothrombin time remarkably.
6.Clinical Characteristics of Acinetobacter baumannii Infections in Elderly Patients in Intensive Care Unit and Analysis of Drug Susceptibility

Objective To understand the clinical characteristics of Acinetobacter baumannii infections in elderly patients in intensive care unit (ICU)and investigate its drug susceptibility. Methods Bacterial identification was carried out by VITEK?32 automatic micro?analyzer(Bio?Merieux Company of French). Data processing was performed with WHONET5.6 software. Results Totally 259 strains of Acinetobacter baumannii were isolated from 220 infected elderly patients. The detection rate of Acinetobacter baumannii was 71.43%(185 strains),13.13%(34 strains), 6.18%(16 strains )and 9.26%(24 strains)from sputum,drainage fluid,blood and other specimens,respectively . Drug resistance of Acinetobacter baumanniito antibiotics was relatively high. Resistance to tigecycline of Acinetobacterbaumanniistrains was the lowest(0%?23.38%)while the sus?ceptibility was the highest(70.13%?80.85%)in all kinds of antibiotics in this study. Conclusion Acinetobacter baumannii varied in drug resis?tance to different antibiotics. It is necessary to choose susceptible antibiotics for clinical anti?infection treatment on the basis of in?vitro antibiotics sus?ceptibility testing for isolated strains from local regions.
7.Establishment of a nomogram model for predicting necrotizing enterocolitis in very preterm infants.

OBJECTIVES:
To investigate the risk factors for necrotizing enterocolitis (NEC) in very preterm infants and establish a nomogram model for predicting the risk of NEC.
METHODS:
A total of 752 very preterm infants who were hospitalized from January 2015 to December 2021 were enrolled as subjects, among whom 654 were born in 2015-2020 (development set) and 98 were born in 2021 (validation set). According to the presence or absence of NEC, the development set was divided into two groups: NEC (n=77) and non-NEC (n=577). A multivariate logistic regression analysis was used to investigate the independent risk factors for NEC in very preterm infants. R software was used to plot the nomogram model. The nomogram model was then validated by the data of the validation set. The receiver operating characteristic (ROC) curve, the Hosmer-Lemeshow goodness-of-fit test, and the calibration curve were used to evaluate the performance of the nomogram model, and the clinical decision curve was used to assess the clinical practicability of the model.
RESULTS:
The multivariate logistic regression analysis showed that neonatal asphyxia, sepsis, shock, hypoalbuminemia, severe anemia, and formula feeding were independent risk factors for NEC in very preterm infants (P<0.05). The ROC curve of the development set had an area under the curve (AUC) of 0.833 (95%CI: 0.715-0.952), and the ROC curve of the validation set had an AUC of 0.826 (95%CI: 0.797-0.862), suggesting that the nomogram model had a good discriminatory ability. The calibration curve analysis and the Hosmer-Lemeshow goodness-of-fit test showed good accuracy and consistency between the predicted value of the model and the actual value.
CONCLUSIONS
Neonatal asphyxia, sepsis, shock, hypoalbuminemia, severe anemia, and formula feeding are independent risk factors for NEC in very preterm infant. The nomogram model based on the multivariate logistic regression analysis provides a quantitative, simple, and intuitive tool for early assessment of the development of NEC in very preterm infants in clinical practice.

8.Gab2-Akt-ARK5 signaling pathway is associated with the invasion of glioma cells

Objective:This study aimed to investigate the effect and significance of a binding protein-2 (Gab2)-Akt-ARK5 signaling pathway on the invasion of glioma cells. Methods:Immunohistochemical methods were used to detect the expressions of Gab2 and ARK5 in 45 cases of glioma tissue. siRNA plasmid was used to transfect LN-229 cells, and western blot was performed to analyze the protein expressions of Gab2 and ARK5. In vitro Matrigel invasion assay was conducted to detect variations in the invasiveness of transfected cells. Western blot was also conducted to analyze the protein phosphorylation of Akt and ARK5 in the cells transfected with Gab2 plasmid. Results:Immunohistochemical assay revealed that the expressions of ARK5 and Gab2 in glioma cells were positively correlated, and both expressions were higher in high-grade glioma (WHO gradeⅢ,Ⅳ) than in low-grade glioma (WHO gradeⅠ,Ⅱ). LN-229 cells transfected with ARK5 plasmid, Gab2 plasmid, ARK5 and Gab2 plasmid, and control plasmid were named siARK5/LN-229, siGab2/LN-229, siARK5 and siGab2/LN-229, and SCR/LN-229, respectively. After transfection was performed, the protein expressions of ARK5 and Gab2 were respectively decreased in siARK5/LN-229 and siGab2/LN-229. The protein expressions of ARK5 and Gab2 in siARK5 and siGab2/LN-229 were also respectively decreased. After ARK5 or Gab2 was downregulated, the number of glioma cells, which invaded and penetrated Matrigel, was decreased (P<0.01). The number of glioma cells also decreased significantly after ARK5 and Gab2 were downregulated. The phosphorylation of Akt and ARK5 in siGab2/LN-229 cells was decreased after these cells were stimulated by insulin-like growth factor-1. Conclusion:The silencing of ARK5 or Gab2 impaired glioma cell invasiveness. The decreased protein expression of Gab2 inhibited the phosphorylation of Akt and ARK5. These results suggested that the Gab2-Akt-ARK5 signaling pathway could be relevantly involved in glioma cell invasion.
9.The survey on perceptions of patient safety culture among nurses in tertiary hospitals in Kunming

Objective To investigate perceptions of patient safety culture(PSC) among nurses in tertiary hospitals in Kunming. Methods Totally 2629 nurses were recruited by convenience sampling method from 7 tertiary hospi-tals in Kunming between July 2015 to January 2016. Hospital Survey on Patient Safety Culture (HSOPS) was used to assess PSC. Results The top three superior dimensions of PSC were Organizational Learning-Continuous Im-provement(86.60%),Teamwork within Hospital Units(83.34%) and Feedback and Communication about Error(75.98%). The last three inferior dimensions of PSC wereNonpunitive response to error(23.74%),Staffing(33.71%) and Teamwork across hospital units(49.45%). There were significant differences in certain items among nurses with different educational level,academic title,working hour per week(P<0.05). Conclusion Establishing nonpunitive re-sponse culture,reasonably arranging staffing and working hours,and encouraging nurses with high educational level and title to play their full role,are critical measures to improve patient safety culture in hospital.
