1.Laparoscopic Totally Extraperitoneal Hernia Repair with Non-Stapling of Mesh and Without Using A Balloon Dissection(Report of 32 Cases)

Objective To evaluate the clinical experience and skills of laparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a balloon dissection in order to spread and popularize this kind of operation.MethodsUnder general anesthesia,32 patients(42 sides)with inguinal hernias were repaired by laparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a dissection balloon from August 2005 to December 2007.ResultsAll of operations were successfully performed.The operative time was 60-120 minutes,the blood loss was 20-60 ml,and the length of postoperative hospitalization was 3 days.All cases were followed up for 8 to 18 months and found no recurrence.ConclusionLaparoscopic totally extraperitoneal hernia repair with non-stapling of mesh and without using a balloon dissection is feasible,reliable and effective,offering a low recurrence rate,while its price is higher than tension-free herniorrhaphy by traditional method.
2.EXPERIMENTAL OBSERVATION OF 75Se METABOLISM ON RATS FED ON CROPS FPOM ENDEMIC DISTRICTS OF KESHAN DISEASE

Observations were conducted on 11 kinds of tissues and organs(brown adipose tissue, thymus gland, blood, heart, lungs, muscle, bone, kidneys, spleen, liver, and intestine) in experimental animals, which were fed on crops from endemic areas of Keshan disease and non-endemic areas for 8-13 weeks. The absorption and distribution of 75Se (Na2 SeO3) in above tissue and organs were studied. Results revealed that the absorption of Na2-SeO3 in experimental group fed on crops from endemic area was higher than control group fed on crops from non-endemic area. The gradient of distribution of 75Se in various organs was regular: The highest absorption was in kidneys, the lesser was in liver, spleen and thyrnus, the least was in muscle, heart and bones.In one week observation, the contents of 75Se changed as time passed, but the fluctuating changes of the two groups were different.The regular differences of 75Se absorption as above, revealed a distinction between endemic and non-endemic crops. Under the influences of the crops on experimental animals, there appeared more or less a condition of selenium deficiency, which proved that the soil-water-food chain had apparently influenced the selenium metabolism of the organisms.
3.Nerve transfer to biceps muscle using a part of ulnar nerve for elbow flexion restoration in acute and delayed upper brachial plexus injury

[Objective]The purpose of this study was to describe mid report the result of the ulnar nerve transfer to biceps muscle to restore elbow flexion after acute and delayed upper brachial plexus injuries.[Methods]Two patients with acute brachial plexus injury (the time between the injury and the operation were six and eight months) and three patients with delayed brachial plexus injury(the time between the injury and the operation were from twevle to eighteen months) underwent nerve transfer using fascicles of the ulnar nerve to the motor branch of the biceis muscle. The average age of the patients was twenty eight and the mean follow-up periods were nine months after the surgery. Patients were evaluated with regard to reinnervation of the biceps, ulnar nerve function, elbow flexion strength, and grip strength.[Results]For the two acute patients, the first sign of biceps muscle contraction were observed within 1 week, the average time required for reinnervation of the biceps after nerve fascicle transfer was within six months. For the three delayed patients, the first sign of bicep muscle contraction was observed in about three month, and the average time required for reinnervation of the biceps was ten months.Hypoesthesia of the ulnar nerve was clinically abserved in three patients, but this symptom disappeared within month with no treatment.Compared with those delayed cases, the acute patients had faster and better recovery of their olbow flexion function.However, all patients achieved grade-3 or better elbow flexion strength according to the grading system of the Medical Research Council.[Conclusion]The author recommend this safe, simple and effective Oberlin procedure for brachial plexus injuries involving the C5、6 or C5~7 nerve roots.
4.Steroidogenic acute regulatory protein-related lipid transfer 4 (StarD4) promotes breast cancer cell proliferation and its mechanism.

Oncogene StarD4 had the function of promoting proliferation and metastasis of triple-negative breast cancer (TNBC), but its clinical value and molecular mechanism are unknown. This paper found that StarD4 was highly expressed in cancer tissues of TNBC patients, and higher expression level of StarD4 in TNBC patient resulted in poorer prognosis. Based on transcriptomics of MDA-MB-231 cell model, the results of bioinformatics analysis showed that down-regulated expression level of StarD4 led to overall downregulation of cholesterol-relative genes and significant enrichment of cancer mechanism and pathway. Further analysis and investigation verified that StarD4 might cross-promote the protein stability of receptor ITGA5 through the cholesterol pathway to enhance TNBC progression, which provides guidance for clinical application of TNBC diagnosis and treatment.
