1.Analysis of the treatment of distal humeral fractures by double plate fixation

Objective To investigate the internal fixation for distal humeral fractures and its outcome .Methods A total of 74 patients with distal humeral fractures were performed surgical operation between Jan 2006 and Jun 2009 .According to AO/ASIF classification ,the factures were classified as type A (n=26) ,B(n=34)and C(n=14) .Internal fixation was applied to the patients , who did functional exercises after operation ,on the basis of classification .The healing of the fractures and the function the joints was examined via follow-up and X-ray films 1 ,3 and 6 months after operation .Results According to the Jupiter elbow score ,40 ca-ses were excellent ,22 cases were good ,12 cases were fair and 0 case was poor ,and the good rate was 83% .Conclusion Internal fix-ation with double plate ,is an effective therapeutic approach to distal humeral fractures in adults .Influencing factors of the efficacy include severity of the initial trauma ,operative scheme ,and post-operation functional recover .

Objective To explore the feasibility of team-based learning (TBL) for practice teaching of the aerospace nutrition and food hygiene.Methods A total of 90 students were investigated,and were divided into the TBL group (n=46) and the TL group (n=44) by simple random sampling.After the experiment teaching,a test and questionnaire were used to compare the teaching effect of TBL group and TL group.SPSS 18.0 was used for date and the data was analyzed by Independent-Samples t test and Chi-square test.Results Students in TBL group achieved higher mean test scores (85.70 ± 3.47) compared with students (74.10±2.41) in TL group and the difference was statistically significant (t=8.703,P=0.000).The results of the questionnaire showed that,compared with the TL group,TBL group students more agreed with the positive impact of the teaching methods on their team work ability,interpersonal skills,communication ability,problem-solving and self-learning and other aspects,and the differences between two groups were statistically significant (P<0.05).In addition,the majority of the respondents in TBL group [93.5% (43/46)] were satisfied with TBL.Conclusions TBL teaching is better than the TL teaching,and it can significantly improve students' comprehension of knowledge and enhance their learning effect.

Dextroscope workstation is an advanced equipment developed in recent years and isused to make virtual reality model.We not only used it in surgical planning of thyroid disease,but also applied it in clinic teaching.Application of Dextroscope virtual reality system can help to improve students'capabilities of clinical thoughts and clinical techniques,stimulate their learning interest,enlarge the knowledge,shorten learning curve and release the conflict in practice.Dextroscope system will push forward the progress of education reform,so it is worth spreading.
4.Determination of Quinolones in Soils Using Solid Phase Extraction and High Performance Liquid Chromatography-Fluorimetric Detection

An analytical method for the simultaneous determination of four quinolones in soil was developed. Soil samples were extracted by a mixture of 50% magnesium nitrate and 10% ammonia(96∶ 4, V/V)with an ultrasonic-assisted extraction, then purified and concentrated by HLB cartridge, and eluted with acetonitrile and 0.067 mol/L phosphoric acid(5∶ 1, V/V). Using acetonitrile and 0.067 mol/L phosphoric acid(pH=2.5) as the mobile phase, these analytes were quantificated by HPLC(fluorimetric detector) at excitation and emission wavelength of 280 nm and 450 nm respectively. The detective limits for four quinolones in soil were from 0.58 to 0.03 g/kg. The recoveries were 60.4% to 99.3% for soil samples. The method was successfully applied to determine the quinolones in soil samples from vegetable fields. Four quinolone compounds were detected to a different extent with total amounts of quinolones ranged from 27.84 to 129.26 μg/kg.

Clinical data of 899 patients,who received colposcopic examination,were retrospectively analyzed.Both Reid's colposcopic index(RCI)score and biopsy were performed.The study showed that the value of RCI score was positively related to the severity of cervical lesions(F=2.043,P=0.03).The specificity of RCI for cervicitis,cervical intraepithelial neoplasia(CIN)Ⅰ were 98.8%,95.7% respectively;while sensitivity and negative predictive value for CIN Ⅱ,CIN Ⅲ were 77.7%,70.0%and 96.8%.97.4%,respectively.So RCI scoring can increase the diagnostic value of colposcope.
6.Effects of Shendan Jianyi Capsule on aldose reductase activity of kidney in diabetic rats

OBJECTIVE: To evaluate the effects of Shendan Jianyi Capsule (SDJYC) on aldose reductase (AR) activity of the kidney in diabetic rats and the significance of prevention and treatment of diabetic nephropathy with Chinese natural herbs. METHODS: Diabetic Wistar rats were treated with SDJYC and sorbinil respectively, and the AR activity in kidney, urine protein and the activity of Na(+)-K(+)-ATPase were tested. RESULTS: The AR activity in diabetic rats was decreased obviously after being treated with sorbinil or SDJYC. The activity of Na(+)-K(+)-ATPase in diabetic rats decreased obviously at the end of the eighth week, and increased after being treated with sorbinil and SDJYC. There was no difference between two treated groups after the treatment. It demonstrated that the inhibition of AR activity could improve the activity of Na(+)-K(+)-ATPase and SDJYC had the same effect. CONCLUSIONS: SDJYC exerts effects by inhibiting the activity of AR.
7.Relationship of PI3K/Akt signaling pathway and the activation of hepatic stellate cells and its role in radiation-induced hepatic fibrosis

Objective To investigate the relationship of PI3K/Akt signaling pathway with the activation of hepatic stellate cells (HSC) and its role in radiation-induced hepatic fibrosis.Methods HSC was treated with 6 MV X-ray irradiation (IR) together with the inhibitor of PI3K/Akt signaling pathway.The cells were divided into inhibitor group,10 Gy IR group,10 Gy + inhibitor group,20 Gy IR group,an 20 Gy + inhibitor group and blank control group.Then cell apoptosis rate was detected,the expression of transforming growth factor β1 (TGF-β1) in cell supernatant and the mRNA expressions of α-smooth muscle actin (α-SMA) and phosphorylation protein kinase B (p-Akt) were measured.Results Compared with the control group,the apoptosis rate of 10 and 20 Gy IR group increased with irradiation dose (t =8.43,11.63,P <0.05) but they were reduced by the inhibitor of PI3K/Akt (t =8.09,4.88,P <0.05).The expressions of TGF-β1,α-SMA,and p-Akt also increased with irradiation dose (t =6.91,7.80,9.28,P<0.05) but they were declined by this inhibitor for both 10 Gy IR (t =6.17,15.11,10.34,P<0.05) and 20 Gy IR (t =10.04,6.85,23.84,P<0.05).Conclusions X-ray irradiation could activate HSC through PI3K/Akt signaling pathway,which may further result in hepatic fibrosis.
8.Clinical analysis on microsurgery assisted by improved Paine point ventricular puncture in 67 patients with early intracranial aneurysm rupture

Objective To explore the microsurgery methods in patients with early intracranial aneurysm rupture. Methods Sixty-seven patients with early intracranial aneurysm rupture received the emergency microsurgery after releasing cerebrospinal fluid from improved Paine point via pterion craniotomy. Thirty-nine patients received surgery within 1 d after intracranial aneurysm rupture, and the other 28 patients were within 1-3 d. Results The therapeutic effect was evaluated by Glasgow outcome score (GOS) at discharge. Among the 45 patients with Hunt-Hess Ⅰ-Ⅲ grade, cure was in 41 cases, improved in 4 cases. Among the 22 patients with Hunt-HessⅣ-Ⅴgrade, cure was in 6 cases, improved in 6 cases, moderate disability in 4 cases, severe disability in 1 case, and death was in 5 cases. Conclusions The microsurgery of clipping aneurysm directly via pterion approach is still by far the most reliable treatment. And releasing cerebrospinal fluid from improved Paine points can effectively reduce the acute brain swelling of early operation, which will make it an effective adjuvant therapy to emergency microsurgery of clipping aneurysm.
9.Hyperfractionation radiotherapy combined with hepatic artery catheterization chemotherapy for unresectable primary liver cancer

Objective To compare the effect of hyperfractionation radiotherapy combined with hepatic artery catheterization chemotherapy and hepatic artery ligation (group A, 65 patients) with conventional fractionation radiation combined with hepatic artery catheterization chemotherapy and hepatic artery ligation (group B, 65 patients) for unresectable primary liver cancer.Methods A total of 130 stage Ⅱprimary liver patients confirmed by pathology and evaluated as unresectable by exploratory laparotomy were divided evenly into group A and group B by the sequence of exploration. The two groups were comparable in age, sex, tumor type and positive fetal protein (AFP). Group A patients were treated by hepatic artery chemotherapy (PDD, 10 mg/day) 6 days a week with hepatic artery ligation followed by hyperfractionation radiotherapy (250 cGy/f, bid) 3 days a week with the scheme alternated weekly. Group B patients were treated by conventional fractionation radiotherapy with the same scheme of chemotherapy as group A. The total dose of PDD and radiotherapy for both groups were 240 mg and 45Gy. Results The AFP level was reduced to half in 89.7% of patients in group A and 67.6% in group B. The 1 , 3 and 5 year survival rates were 90.8%, 63.1%, 23.1% for group A and 73.9%, 41.5% 9.2% for group B, the difference was statistically significant (P0.05). Conclusions Hyperfractionation radiotherapy combined with hepatic artery catheterization chemotherapy and hepatic artery ligation is an effective and reasonable therapeutic scheme for unresectable liver cancer. It can effectively relieve symptoms, reduce the tumor, increase second surgical resection rate and prolong the survival. At the same time, the operation is simple and relatively easy.

We put forward an image rendering system based on pipeline framework for processing and displaying medical images. Compared to original computer graphics algorithms divided into volume rendering and surface rendering, this framework can effectively comprehend methods of computer graphics and image processing, import some new concepts such as vertex buffer, pixel buffer and texture buffer. We implement Shaded Surface Display, Maximum Intensity Projection, Digitally Reconstructed Radiography, Multi planar Reformation, Curved Planar Reformation and Interactive Virtual Endoscopy in our new developed PACS image system.
