1.Effects of Perioperative Parenteral Nutrition Under Different Osmotic Pressure on the Incidence of Phlebitis

Objective To explore ways to decrease peripheral vascular complications during parenteral nutrition through peripheral veins. Methods According to the support sequences of giving peripheral parenteral nutrition, ninty perioperative cases were randomly divided into low(20. 3 kPa·h-1 ),medium(27. 1 kPa·h-1 ) and high(33. 9 kPa·h-1 ) osmotic pressure rates(n=30 in each group). The infusion pump was used to control the rate of infusion. The total parenteral nutrient was infused through BD24G venous indwelling needles. The pain ratings at injection sites were evaluated after parenteral nutrition everyday,the incidences of phlebitis were observed three days later after removing the indwelling needles. ResultsUnder the low osmotic pressure,the pain was mild,while for the medium osmotic pressure,the pain became more obvious. The pain was aggravated obviously under high osmotic pressure. There were only two cases of over grade-II phlebitis under low osmotic pressure rate,9 cases under medium osmotic pressure rate and 14 cases under high osmotic pressure rate, in which a grade-Ⅲ phlebitis developed. Conclusion Decreasing the osmotic pressure rate through peripheral venous is an effective method to avoid or reduce the incidences of peripheral vascular complications with the peripheral parenteral nutrition support.
2.Application of functional-structural relationship between automated perimetry and retinal nerve fiber layer in the early diagnosis of open-angle glaucoma

Background One of the difficulties in identifying early glaucoma is the variability of perimetry performance.For this reason,a field defect have to be reproduced more than two consecutive examinations before it is confirmed.The relationship between visual field sensitivity and retinal nerve fiber layer (RNFL) thickness may give us a more efficient and objective assessment to the early diagnosis of glaucoma.Objective This diagnostic test was to evaluate the application of functional-structural relationship between automated perimetry and RNFL in the early diagnosis of open-angle glaucoma.Methods Sixty-four eyes of 64 patients with early open-angle glaucoma and 50 eyes of 50 normal volunteers with the matched age and gender were enrolled in Xiangya Hospital January to June 2007.Visual field examinations,including standard automated perimetry (SAP),short wave automated perimetry (SWAP),and imaging of scanning laser polarimetry with variable corneal compensation(GDxVCC),were performed on the subjects with the informed consent.The sensitivity and specificity of the joint or single measurement of GDxVCC,SAP and SWAP under the local diagnostic criteria,and overall diagnostic criteria were calculated.Results According to the overall diagnostic criteria,the sensitivity of GDxVCC,SWAP and SAP were 70%,63%,61% respectively,and the specificity were 84%,80%,78%,respectively.According to the local diagnostic criteria,the sensitivity of GDxVCC,SWAP and SAP were 78%,86%,78%,respectively,and the specificity were 54%,40%,50%,respectively.With the local diagnostic criteria,the sensitivity and specificity of serial test of GDxVCC,SWAP and SAP were 86% and 90% respectively,and the positive likelihood ratio and negative likelihood ratio were 8.59 and O.16,respectively.Conclusions The functional-structural relationship between automated perimetry and RNFL which can provide valuable individual diagnostic information for patients,and serial test of GDxVCC,SWAP and SAP can reduce the false positive rate and improve the diagnostic performance significantly.

This study was aimed to isolate and characterize of chemical constituents in a traditional Mongolian medicine HERBA CLEMATIS. Normal and reverse phase coloumn chromatography, gel filtration chromatography sephadex LH-20, and preparative HPLC were used for isolation and purification compounds from the water extraction of the arial part ofC. aethusaefoliaTurcz. The planar structures and spatial configurations of isolated compounds were identified by high resolution MS, 1D and 2D NMR, and other spectrographic methods. The chemical research on the Mongolian medicine results 6 compounds, dihydrodehydrodiconiferyl alcohol (1), syringaresinol (2), pinoresinol (3), epi-pinoresinol (4), lirioresinol B dimethyl ether (5) and loliolide (6). All the compounds were isolated from this plant for the first time.

OBJECTIVETo evaluate clinical outcomes of fixation for the treatment of radial head fracture with collapse of anterior articular surface.
METHODSFrom March 2006 to January 2013,17 patients with radial head fractures with collapse of anterior articular surface were analysed. According to the Mason classification, there were 12 cases with Mason type II fractures and 5 cases with Mason type III fractures. All the patients were treated with open reduction through posterolateral entrance of elbow joint and Herbert or titanium cannulated screw internal fixation.
RESULTSAll the patients were followed up, and the duration ranged from 6 to 18 months, with a mean of 11.3 months. According to the Broberg and Morrey score system, 2 patients got an excellent result, 12 good and 3 fair. There were no complications such as infection of elbow joint, nerve injury, non-union, traumatic osteoarthritis, heterotopic ossification and elbow instability. However, the postoperative activity range of elbow in the injuried side was less than that in the normal side.
CONCLUSIONRadial head fracture with collapse of anterior articular surface is easily misdiagnosed, and it can be treated with open reduction and internal fixation through posterolateral entrance.

8.Microsurgical treatment of lateral ventricular tumors

Objective To investigate the clinical characteristics,minimal invasive operation technique and perioperative management of lateral ventrieular tumors.Methods The clinical characteristics,image diagnosis,surgical approaches and postoperative management and surgical outcomes of 65 consecutive cases of lateral ventricular tumors were retrospectively analyzed.Results In our series,total resection was achieved in 54 cases,and subtotal resection was achieved in 11 cases.Lateral ventricular tumors were mostly found in male and in the left side.Headache caused by increased intracranial pressure was the most common clinical symptom.Ependymocytoma and astrocytic glioma are the most common pathologic diagnosis.Postoperative complications included fever (26 cases),hydrocephalus (9 cases),intraventricular hematoma (7 cases) of which 2 cases were evacuated by craniotomy,epilepsy (7 cases),wound infection (3 cases).Postoperative death was happened in 3 cases.Two of them died of respiratory failure due to postoperative epilepsy.Conclusion Early discovery of lateral ventricle tumors,meticulous operation,subtle micromanipulation and right postoperative treatment are the criticality to improve the rate of total resection of lateral ventricle tumors through microsurgical treatment and reduce postoperative complications and mortality.
10.The multi-slice CT perfusion imaging in evaluating the prevention and treatment by edaravone on lung ischemia-reperfusion injury after pulmonary thromboembolism

Objective To evaluate the multi-slice CT perfusion imaging in investigating whether edaravone can prevent and treat pulmonary thromboembolism ischemia-reperfusion injury(PTE-IRI).Methods Twenty mongrel canines were included.A Swan-Ganz catheter wag introduced into the right internal jugular vein using the Seldinger technique,and then was inserted into the pulmonary artery.Balloon occlusion of the right inferior lobe pulmonary artery for 4 h was followed by removing catheter and 4 h of reperfusion.Animals were divided into four groups of A(no edaravone during ischenmia and reperfusion),B(edaravone used only during ischemia),C(edaravone used during both ischemia and reperfusion)and D group(edaravone used only during reperfusion)(n=5 per group).Every group was divided into three time points including before ischemia,4 h after ischemia and 4 h after reperfusion.CT scan and CT perfusionwere performed at the three time points.The blood flow(BF),blood volume(BV)and mean transit time (MTT)of the bilateral inferior regional lung parenchyma were measured with the software of perfusion 3.Results CT examination showed pulmonary edema in the right inferior lung lobe at 4 h after reperfusion.(1)The BF and MTT of A,B,C and D group were[(259.4±15.7)ml·min-1·100 g-1,(293.7±7.9)ml·min-1·100 g-1,(379.4±14.5)ml·min-1·100 g-1,(382.5±16.6)ml·min-1·100 g-1]and[(3.1±0.2)s,(2.6±0.2)s,(2.2±0.1)s,(1.9±0.2)s]respectively at 4 h after reperfusion.The BF and MTT were statistically difierent(P<0.01)between groups(A and B,A and C,A and D,B and C,B and D)except between group C and D(the P value>0.05)at 4 h after reperfusion,but the BV was not statistically different between groups(P>0.05).(2)The BF[(397.2±19.2)ml·min-1·100 g-1and(259.4±15.7)ml·min-1·100 g-1in group A,(393.2±16.1)ml·min-1·100 g-1and(293.7±7.9)ml·min-1·100 g-1 in group B]and MTT[(1.8±0.1)8 and (3.1±0.2)s in group A,(1.8±0.2)s and(2.6±0.2)s in group B]were statistically different(P<0.01),but the BV[(12.0±0.9)ml/100 g and(12.2±1.0)ml/100 g in group A,(11.9±1.5)ml/100 g and(12.2±1.3)ml/100 g in group B]were not different(P>0.05)between groups before ischemia and 4 h after ischemia.The BF.MTT and BV were not statistically significant between before ischemia and4 h after reperfusion in group C and D(P>0.05).ConclusionsEdaravone can attenuate the degree of the PTE IRI.Multi-slice CT perfusion imaging can evaluate effect.
