2.Clinical efficacy of ALA-PDT in the treatment of condyloma acuminatum

Objective To evaluate the clinical efficacy of 5-aminolevulinic acid-induced photodynamic therapy (ALA-PDT) in the treatment of condyloma acuminatum. Methods Thirty-six cases of condyloma acuminatum were treated with ALA-PDT. Results After the treatment, 33 (91.67%) patients were cured, 35 (97.22%) improved. Relapse occurred in 3 (8.33%) patients. Side effects were noted in 2 (5.56%) patients. Conclusions ALA-PDT is a relatively simple, effective and safe treatment with a lower recurrence rate for condyloma acuminatum, especially for small skin lesions or lesions at specific area. However, the high expense required for multiple treatments with ALA-PDT may restrict its application.
3.Expression autophagy-related gene pULK and PI3KC3 and their correlation with human non-small-cell carcinoma

Objective To examine expression levels of autophagy gene pULK and PI3KC3 and to explore their correla?tion with non-small cell lung cancer (NSCLC). Methods A total of 77 samples of surgical resection from NSCLC speci? mens (including 31 cases of squamous cell carcinoma, 31 cases of adenocarcinoma and 15 cases of large cell undifferentiated carcinoma) and 21 samples of same normal lung tissue were randomly selected. Expressions of pULK and PI3KC3 in lung tissues were assessed by immunohistochemistry and Western blot. All dates were analyzed using SPSS13.0 statistical pack?age. Results Immunohistochemistry indicated that pULK and PI3KC3 localized into the cytoplasm. The expression levels of pULK and PI3KC3 are significantly lower in patient with NSCLC than those in peri-tumor tissue (35.1%vs 81.0%, 40.3%vs 76.2%respectively, P<0.01) . Immunohistochemistry and Western bolt analysis confirmed that pULK and PI3KC3 ex?pressions were significantly down-regulated (P<0.01) in patients with low grade cellular differentiation, metastasis of lymph node, or stageⅢandⅣ. And expression levels of pULK and PI3KC3 in NSCLC did not differ significantly with ages, gen?der, tumor size and pathological type. Correlation analysis showed that the expression of PI3KC3 was positively correlated with pULK. Conclusion pULK and PI3KC3 expressions were lower in NSCLC than those in normal lung tissue group. The expression levels of pULK and PI3KC3 in NSCLC were correlated with patient's clinical stage, differentiation grade, lymph node metastasis but were unrelated with age, gender, histological type and size.

AIM To investigate the preventive effects of taurine on the left ventricle hypertrophy in renovascular hypertensive rats. METHODS Two-kidney 1-clip (2K1C) rats were used to establish the model of renovascular hypertension. The myocardial local aldosterone(Ald) and AngⅡconcentration (MDC,MAC) were measured by radioimmunoassay. Expression of oncogene c-fos mRNA was analyzed by means of in situ hybridization. The myocardial collagen concentration (MCC) was measured by biochemical method. The myocardial tissue structure was observed under the microscope ,and the myocardial fiber diamension (MFD) was measured with micrometer. 5.Study on Extraction Technology of Polysaccharides from Gossypium herbaceum L. by Pectinase Hydrolysis

OBJECTIVE:To optimize the extraction technology of polysaccharides from Gossypium herbaceum L. by pectinase hydrolysis. METHODS:Polysaccharides were extracted from G. herbaceum L. by pectinase hydrolysis combined with traditional hot water extraction. With the extraction rate of polysaccharides as the evaluated index,single factor test and orthogonal design were applied to investigate the effects of 4 factors including enzymolysis temperature,enzymolysis time,the amount of enzyme and pH value on polysaccharides extraction rate,and verification tests were conducted. RESULTS:The optimal extraction technolo-gy was as follows as enzymolysis temperature of 40 ℃,enzymolysis time of 150 min,enzyme accounting for 2.0%,pH value for enzymolysis of 4.6. Under the above conditions,the average extraction rate of polysaccharides from G. herbaceum L. was 2.474%(RSD=3.34%,n=5). CONCLUSIONS:Pectinase hydrolysis is an effective method to extract polysaccharides from G. herbace-um L.. The optimal extraction technology is reasonable and feasible.
6.HPLC simultaneous determination of contents of 5 saponin constituents in Ophiopogonis Radix.

This research is to establish an HPLC method for the simultaneous determination of ophiopogonin D, ophiopogonin D', ophiopogonin C, deacetylophiopojaponin A and ophiogenin-3-O-α-L-rhamnosyl-(1-->2)-β-D-glucoside in Ophiopogonis Radix. HPLC-ELSD analysis was performed on a Kromasil 100-5 C₁₈ column (4.6 mm x 250 mm, 5 µm), with the mobile phase of acetonitrile (A) -water (B) in gradient elution mode (0-45 min, 35%-55% A), at a flow rate of 1 mL · min⁻¹. The column temperature was 35 °C and the drift tube temperature was 100 °C in a gas flow rate of 3.0 L · min⁻¹. The result showed that baseline of all the 5 constituents was well separated, and every constituent had wide linearity range and good linear relation (r > 0.999). The recovery rate was between 95.75% and 103.1%. The new established method for simultaneous determination of saponin constituents in Ophiopogonis Radix was sensitive and has good, repeatability. It could be applied to quality evaluation of Ophiopogonis Radix.

7.Ischemic J wave before and post percutaneous coronary intervention in patients with coronary artery disease.

OBJECTIVETo explore the change of electrocardiographic ischemic J wave in patients with coronary heart disease (CHD) underwent percutaneous coronary intervention (PCI).
METHODSCHD patients (CHD, n = 162) underwent PCI in the hospital were divided into unstable angina pectoris (angina pectoris group, n = 108) and acute myocardial infarction group (AMI group, n = 54), 12-lead electrocardiogram was recorded within 24 hours before and after PCI. Patients were further grouped into left anterior descending artery group and right or circumflex coronary artery stenosis group. The incidence, amplitude and time course of J waves were measured and analyzed.
RESULTS(1) The highest incidence of J wave appeared in II, III, aVF leads pre- and post-PCI in CHD groups (33.95%, 22.84%, 26.54% and 30.86%, 19.75%, 23.46%). Higher incidence of J wave in AMI group was evidenced in V(5), II, aVF, III, V(3) leads before PCI (33.33%, 27.78%, 16.67%, 12.96%, 11.11%) and in V(5), II, aVF, aVL, III leads after PCI (22.22%, 22.22%, 14.81%, 12.96%, 9.26%). (2) Before PCI, higher incidence of J wave was found in V(5), V(6) leads (25.78%) in left anterior descending artery group and in II, III, aVF leads (35.37%) in right or circumflex coronary artery stenosis group. After PCI, the higher incidence of J wave in left anterior descending artery group and right or circumflex coronary artery stenosis group was evidenced in II, III, aVF leads (21.09% and 35.37%, respectively). (3) The ischemic J wave' amplitude and time course in II, III, aVF, V(3), V(4) leads were significantly reduced post PCI in CHD patients.
CONCLUSIONSHigher incidence of J wave was found in II, III, aVF in CHD patients. PCI is associated with reduced amplitude and time course of J wave suggesting PCI could improve ischemic J waves in CHD patients.

9.Retrospective analysis of the value of arterial blood lactate level and its clearance rate on the prognosis of septic shock patients

Objective To explore the prognostic value of arterial blood lactate (Lac) levels and lactate clearance rate (LCR) in the patients with septic shock.Methods A retrospective study was conducted.Clinical data of 94 septic patients admitted in the Department of Critical Care Medicine in Subei People's Hospital from January 2011 to June 2014 were analyzed.The arterial blood Lac levels at the moment of diagnosis of septic shock (incipient value,0 hour) and early-stage after treatment (3,6 and 24 hours) were reviewed,and individual LCR was calculated at 3,6,24 hours for each patient.According to the outcome in intensive care unit (ICU),patients were divided into survival group (n =48) and death group (n =46).The Lac and LCR at different time points in two groups were analyzed,and the relationships between them and outcome were analyzed.The receiver-operating characteristic (ROC) curve was plotted to assess the value of Lac and LCR at different time points for predicting the outcome.Results Lac level after treatment in survival group was significantly lower than incipient value,but there was no obvious change in death group.Compared with death group,early Lac levels (mmol/L) in survival group were significantly reduced (0 hour:3.80 ± 2.14 vs.5.75±3.21,3 hours:2.05± 1.04 vs.5.03±2.53,6 hours:1.80±0.77 vs.4.40±2.02,24 hours:1.35±0.43 vs.4.90 ± 2.72,P < 0.05 or P < 0.01),the LCR was significantly increased [3 hours:50.00 (72.35)% vs.13.51 (20.67)%,6 hours:41.43 (58.42)% vs.22.00 (22.31)%,24 hours:58.73 (29.94)% vs.18.92 (47.28)%,P < 0.05 or P < 0.01].The Lac levels at all time points were positively correlated with the outcome,and 6-hour and 24-hour LCR were negatively correlated with the outcome.According to the incipient Lac level,patients were divided into low Lac group (Lac < 2 mmol/L),mild Lac group (Lac 2-3 mmol/L) and high Lac group (Lac ≥ 4 mmol/L).The mortality in low Lac group,mild Lac group,high Lac group was gradually increased [23.07% (6/26),50.00% (8/16),61.54% (32/52),x2=10.270,P =0.006].ROC curves demonstrated that the area under ROC curve (AUC) of 24-hour Lac was the largest,0.944,and it was more sensitive and specific in the prognosis evaluation (100% and 78.3%,respectively).According to the cut-off value of 24-hour Lac as 2.35 mmol/L,patients were divided into high Lac and low Lac groups,and mortality rate in high Lac group was significantly higher than that in low Lac group [100.0% (36/36) vs.17.24% (10/58),x2=30.441,P =0.000].The AUC of 24-hour LCR was the largest,0.865,and it was more sensitive and specific for the prognosis evaluation (83.3% and 91.3%,respectively).According to the cut-off value of 24-hour LCR as 36.8%,patients were divided into high LCR group and low LCR group,and mortality rate in low LCR group was significantly higher than that in high LCR group [84.00% (42/50) vs.9.09% (4/44),x2=26.278,P =0.000].Conclusion Early high Lac in patients with septic shock prompts a poor prognosis,and 24-hour Lac levels and LCR are indicators of assessment of clinical therapeutic effect and prognosis of patients with septic shock.
10.Staged pendulous-prostatic anastomotic urethroplasty followed by reconstruction of anterior urethra for male complex post-traumatic posterior urethral stricture

Objective To describe a novel surgical technique for male long-segment urethral stric- ture after pelvic trauma using the intact and pedieled pendulous urethra to replace the bulbar and membra- nous urethra,and then reconstructing anterior urethra.Methods Three patients with long-segment post- traumatic bulbar and membranous urethral strictures with short left pendulous urethras who had undergone several failed previous surgeries were treated with staged pendulous-prostatic anastomotic urethroplasty fol- lowed by reconstruction of the anterior urethra.This procedure was divided into 3 stages.The first-stage sur- gery was mobilization of anterior urethra down to the coronary sulcus and then re-routing the prostatic urethra followed by pendulous-prostatic anastomotic urethroplasty with transposition of penis to perineum.The sec- ond-stage surgery was transecting the anterior urethra at the site of coronary sulcus 6 months later when it was re-vaseularized,then straightening the penis and performing urethroperineostomy.The third-stage surgery was reconstruction of anterior urethra 6 months later.Results Case 1 reported satisfactory voiding postopera- tively.Retrograde urethrography showed that the urethra was patent with no post-voiding residual urine (PVR),and bilateral vesicoureteral reflux almost disappeared.The Qmax was 18.8ml/s,and 18ml/s after the third stage surgery and at 2-year follow-up.Case 2 also had satisfactory voiding.A 22F urethral catheter could smoothly pass through the urethra,and Qmax was 19.5 ml/s with no PVR at 2-year follow-up.Case 3 underwent the first stage surgery through perineal and pubic routes.The urethrorectal and urethroperineal fis- tulas were excised and repaired simultaneously.After operation the fistulas healed,but the stenostomia resul- ting from wound infection needed further treatment.Conclusions This procedure is effective for men with complex long-segment post-traumatic bulbar and membranous urethral strictures,especially for those undergo- ing failed previous surgical treatment.
